カテーテルアブレーション(CA)後の医原性心房中隔欠損(iASD)の発症率と予測因子を検討した。2005年5月～2018年6月に当院で心房細動(AF)に対して初回CAを施行された639例(男性489例、女性150例、平均60.2±10.7歳)を対象に後ろ向き研究を行った。全例に対して術前経食道心エコー(プレTEE)、術前経胸部心エコー(プレTTE)、術後TTE(ポストTTE)を実施し、CAから6ヵ月後のiASDの発症率を評価した。プレTTEにおける平均左房径(LAD)は41.0±6.5mm、平均左室駆出率(LVEF)は64.6±10.3%、左室拡張末期径は30.1±5.8mmであり、高周波CAを行った453例のうち複雑性分裂心房電位(CFAE)アブレーションが258例、CFAEアブレーション+肺静脈隔離術(PVI)が109例、PVI単独が86例、クライオアブレーションが44例であった。ポストTTEでのiASDの検出率は59.6%(497例中296例)、6ヵ月後TTEでは4.6%(23例)であり、年齢、BMI、AFのタイプ、BNP値、プレTTE上のLAD、左房容積、LVEF、右室収縮期圧(RVSP)に関してiASD群と非iASD群との間に有意差はなかった。一方、iASDの発症率は高周波アブレーション群が3.3%、クライオアブレーション群が18.2%とクライオアブレーション群の方が有意に高く、高周波アブレーション群では2シース法の方が1シース法よりiASDの発症率が有意に高値を示していた。単変量解析ではiASD発症の有意な関連因子としてシース径、単一穿刺における2シースの使用が抽出された。

経皮的冠動脈インターベンション(PCI)を受けた安定型冠動脈疾患(CAD)患者における学術研究コンソーシアム高出血リスクスコア(ARC-HBR)と臨床アウトカムとの関連について検討した。2017年1月～2020年12月にPCIを施行された安定型CAD患者328例をARC-HBRスコア1点未満の低値群143例(男性119例、女性24例、平均65.1±8.7歳)、1～2点の中等値群97例(男性66例、女性31例、平均72.4±9.5歳)、2点以上の高値群88例(男性67例、女性21例、平均75.7±9.5歳)に分類し、主要評価項目は全死亡、非致死性心筋梗塞、非致死性脳卒中の複合としての主要心血管有害事象(MACE)の発症とした。観察期間中央値983日においてMACEの発症を44例に認め、17例に大出血イベント、11例に虚血性イベントがみられた。Kaplan-Meier曲線解析では、ARC-HBRスコアが増すのに伴い主要評価項目の発症率は高くなっており、このうち約75%が全死亡であり、非心血管死が2/3を占めていた。Cox比例ハザード回帰モデルを用いた多変量解析では、ARC-HBRスコア2点以上と1～2点が主要評価項目の発症と有意に関連していた。時間依存性ROC曲線解析では、ARC-HBRのAUCはThrombolysis in Myocardial Infarction Risk Score for Secondary Prevention(TRS2°P)より有意に高く、Coronary Revascularization Demonstrating Outcome Study in Kyoto(CREDO-Kyoto)血栓症リスクスコアとほぼ同等であった。ARC-HBRスコアはPCI施行患者にみられるMACE発症の層別化に有用と考えられた。

Background: We hypothesized that the beneficial effects of sodium-glucose cotransporter-2 (SGLT2) inhibitors on diastolic function might depend on baseline left ventricular (LV) systolic function. Methods: To investigate the effects of SGLT2 inhibitors on LV diastolic function in patients with type 2 diabetes mellitus (T2DM), we conducted a post-hoc sub-study of the PROTECT trial, stratifying the data according to LV ejection fraction (LVEF) at baseline. After excluding patients without echocardiographic data at baseline or 24 months into the PROTECT trial, 31 and 38 patients with T2DM from the full analysis dataset of the PROTECT trial who received ipragliflozin or no SGLT2 inhibitor (control), respectively, were included. The primary endpoint was a comparison of the changes in echocardiographic parameters and N-terminal pro-brain natriuretic peptide levels from baseline to 24 months between the two groups stratified according to baseline LVEF. Results: Differences in diastolic functional parameters (e' and E/e') were noted between the two groups. Among the subgroups defined according to median LVEF values, those with higher LVEF (≥60 %) who received ipragliflozin appeared to have a higher e' and lower E/e' than did those who received the standard of care with no SGLT2 inhibitor, indicating longitudinal improvements between baseline and follow up (p = 0.001 and 0.016, respectively). Conclusions: Ipragliflozin generally improved LV diastolic function in patients with type 2 diabetes, the extent of this improvement might appear to vary with LV systolic function.

日本の39施設の2型糖尿病患者を対象にPROTECT試験のサブ解析として、左室拡張機能に対するイプラグリフロジンの効果をベースラインの左室駆出率(LVEF)で層別化して検討した。ベースラインおよび24ヵ月後の心エコー検査データが得られなかった患者を除外後、イプラグリフロジン群31例、対照群38例について解析した。24ヵ月後の拡張機能パラメータ(e'、E/e')の変化について、ベースラインのLVEFが中央値(60%)以上の患者においては、イプラグリフロジン群は対照群に比べてe'が有意に高値で、E/e'が有意に低値であったが、ベースラインのLVEFが中央値未満の患者では、e'およびE/e'に有意な群間差を認めなかった。2型糖尿病患者において、イプラグリフロジンは全般的に左室拡張機能を改善したが、改善の程度は左室収縮機能によって異なると考えられた。
