Background: Mitochondria are dynamic organelles whose morphology can change through fusion and fission. The role of these mitochondrial dynamics in cardiomyocytes has not been evaluated in patients with acute heart failure (AHF). Methods and results: Two hundred seventy-three AHF patients and 374 control patients who attended a cardiovascular outpatient clinic were prospectively analyzed. Blood samples were collected within 15 min of admission from AHF patients (AHF group) and control patients who visited a daily cardiovascular outpatient clinic (control group). Mitochondrial dynamics-related RNAs were compared between the two groups, and long-term prognosis (1000-day mortality) was evaluated across RNA quartiles. The miR-140-3p and miR-485-3p levels in the AHF group (median 6.73, range 2.75–14.55; median 0.45, range 0.13–1.86) were significantly (p = 0.002 and p = 0.049) lower than those in the control group (median 8.61, range 3.37–34.12; median 0.78, range 0.15–3.52), respectively. Cardiac biomarkers, including N-terminal pro-BNP, heart-type fatty acid-binding protein, and high-sensitivity troponin, were significantly higher in the low-miR-140-3p group than in the middle- and high-miR-140-3p groups. Kaplan–Meier curve analysis revealed that all-cause mortality within 1000 days was significantly higher in the low-140-3p group than in the middle- and high-miR-140-3p groups. Cox regression analysis revealed that the low-140-3p group was an independent predictor of 1000-day mortality (hazard ratio 1.567; 95 % confidence interval 1.004–2.444; p = 0.048). Conclusion: Impaired mitochondrial dynamics were observed in the acute phase of AHF. In particular, a mitochondrial fission-related miRNA (miR-140-3p) was independently associated with adverse outcomes.

Background: Cardiac amyloidosis is associated with poor prognosis. Biopsy-induced severe mitral regurgitation (MR) secondary to chordal rupture is a rare but critical complication, and its optimal management remains uncertain. Case Summary: A 78-year-old man with transthyretin cardiac amyloidosis developed acute severe MR after left ventricular myocardial biopsy, which caused rupture of the chordae tendineae. He presented with NYHA functional class IV heart failure. Given his condition, transcatheter edge-to-edge mitral valve repair (M-TEER) was prioritized. A MitraClip was successfully deployed, reducing the severe MR to mild without complications, and his symptoms markedly improved. Discussion: This is to our knowledge the first reported case of biopsy-induced MR in cardiac amyloidosis successfully managed using M-TEER. This case demonstrates the safety and efficacy of M-TEER in frail patients with iatrogenic mechanical complications. Take-Home Message: M-TEER may be a life-saving therapy for severe iatrogenic MR in patients with advanced cardiac amyloidosis.

Background: Psychological frailty (PsF), encompassing cognitive and emotional vulnerabilities, remains understudied despite its potential impact on health outcomes in older adults. This cross-sectional study aimed to explore the characteristics of diverse aspects of PsF, including cognitive frailty (CF), lower cognitive function, and affective frailty (AF), and to examine their associations with health-related outcomes in community-dwelling older adults. Methods: Data were analysed from 420 community-dwelling older adults participating in the Tarumizu Study in Japan, excluding those exhibiting physical or social frailty. Participants were categorised into Robust, Lower cognitive function, CF, AF, and Multi-domain PsF groups. AF was assessed using five items from the Geriatric Depression Scale, focusing on emotional vulnerability. Health outcomes included self-rated health, subjective memory complaints (SMC), and functional ability of daily living. Associations were analysed using adjusted multiple regression models controlling for covariates. Results: PsF was present in 39.5% of the cohort (lower cognitive function: 18.1%, CF: 7.6%, AF: 11.0%, Multi-domain PsF: 2.9%). In the adjusted models, only AF showed a significant positive association with SMC (adjusted risk ratio = 1.62, 95% CI: 1.06–2.42, p = 0.023). No significant associations were found between any PsF domain and subjective health or daily functional ability in the adjusted models. Conclusions: AF, characterised by emotional vulnerability, was uniquely associated with SMC, suggesting emotional factors may play a more central role than cognitive decline in shaping perceptions of memory problems. These findings highlight the heterogeneous nature of PsF and underscore the need for nuanced assessments to inform targeted prevention and intervention strategies.

Introduction: Frailty in older adults is a multidimensional condition that affects healthy life expectancy. Engagement in meaningful activities (MA) is thought to support healthy aging, but the specific relationship between satisfaction with MA and frailty remains underexplored. This study investigated the longitudinal association between satisfaction with MA and frailty (physical, social, and cognitive domains) in community-dwelling older adults. Method: A total of 231 community-dwelling older adults who completed a 2-year follow-up were analyzed. Frailty in each domain was assessed at baseline and at 1- and 2-years. Participants’ MA and satisfaction were obtained through face-to-face interviews. Mixed-effects logistic regression models were used to examine the longitudinal associations between satisfaction with MA and frailty status trajectories. Results: At follow-up, changes in physical frailty status were observed in most participants. Robust status was commonly maintained in the social and cognitive frailty domain. Participants’ MA primarily involved leisure, sports, and domestic activities. Mixed-effects logistic regression analysis showed that satisfaction with MA was significantly associated with physical frailty, even after adjusting for potential covariates (odds ratio 0.68; 95% confidence interval 0.45, 0.99, p = 0.045). However, this association was not significant for social or cognitive frailty. Conclusion: Satisfaction with MA was longitudinally associated with physical frailty, but not with social or cognitive frailty.

Pulmonary arterial hypertension (PAH) is a rare disease characterized by a progressive increase in pulmonary arterial pressure, leading to right heart failure. In clinical practice, reduced exercise tolerance is commonly observed in patients with PAH. Several studies have identified skeletal muscle abnormalities and muscle weakness as factors contributing to impaired exercise capacity in these patients. Recently, peak expiratory flow rate (PEFR) has been shown to correlate with skeletal muscle mass. Given that PEFR can be easily and noninvasively measured using respiratory function tests, we investigated the relationship between PEFR and prognosis in patients with PAH. We enrolled consecutive untreated patients diagnosed with PAH at Kagoshima University Hospital between July 2005 and July 2024. A total of 85 patients were included and divided into the preserved PEFR group and the reduced PEFR group. There were no significant differences in hemodynamic parameters between the two groups; however, the 6-minute walk distance was significantly shorter (p = 0.0062) in the reduced PEFR group. Kaplan–Meier analysis revealed that the cumulative event-free rate was significantly lower in the reduced PEFR group (Log-rank p = 0.0048). Reduced PEFR was independently associated with poorer outcomes after adjusting for age, plus each of right atrial pressure, cardiac index, and pulmonary vascular resistance. In conclusion, PEFR, a method for measuring skeletal muscle, was associated with poor prognosis in patients with PAH when assessed at the time of diagnosis. PEFR can be easily and repeatedly measured, making it potentially useful for prognostic prediction and exercise rehabilitation in these patients. (Figure presented.)

This subgroup analysis of the randomized, open-label, parallel-group EXCITE-HT study explored the antihypertensive efficacy and safety of esaxerenone vs trichlormethiazide in patients with type 2 diabetes mellitus (T2DM), stratified by baseline antihypertensive agent (angiotensin receptor blocker [ARB] or calcium channel blocker [CCB]) and urinary albumin-to-creatinine ratio (UACR; <30 or ≥30 mg/gCr). Using thresholds consistent with those used in the main study to interpret the difference in systolic/diastolic blood pressure (SBP/DBP), the between-group difference in least squares mean change (95% confidence interval [CI]) in morning home SBP/DBP at the end of treatment was −2.5 (−4.8, −0.2)/ − 0.7 (−2.0, 0.6) mmHg. Trends were consistent across all subgroups. The geometric mean UACR significantly decreased from baseline to Week 12 in the overall population, ARB subgroup (except for esaxerenone-treated patients), CCB subgroup, and both UACR subgroups. The overall incidence of serum potassium ≥5.5 mEq/L was 2.5% with esaxerenone and 0.9% with trichlormethiazide, with no cases of serum potassium ≥6.0 mEq/L. In this patient population, esaxerenone had a favorable safety profile, achieved blood pressure lowering similar to trichlormethiazide, and elicited a reduction of kidney damage (based on UACR), regardless of baseline antihypertensive agent or UACR. Mitochondrial dynamics-related RNAs during hospitalization for acute heart failure (AHF) were rarely evaluated in various points. In total, 234 patients who visited the emergency room for AHF were retrospectively evaluated. Blood samples were collected within 15 min of admission (day 1), after 48–72 h, and between days 7 and 21. Low miR-140-3p during hospitalization was defined as the level being categorized as Q1 more than once (on days 1, 3 and/or 14), and normal-140-3p during hospitalization as the level never being categorized as Q1. The median miR-140-3p levels were significantly decreased on days 3 and 14 (2.53 [1.06–6.42] and 3.65 [1.41–9.05], respectively) in comparison to the value on day 1 (6.71 [2.66–14.0]). Kaplan–Meier curves indicated that all-cause mortality within 1000 days was significantly higher in the low-miR-140-3p group than in the other-miR-140-3p group on days 1 and 14. Moreover, the survival rate was significantly lower and the rate of HF events was significantly higher in the low-miR-140-3p group than in the normal-miR-140-3p group. The miRNA levels of patients further decreased during treatment for AHF. Low levels of mitochondrial fission-related miRNAs during AHF treatment were independently associated with an increased risk of long-term adverse outcomes.

Mitochondria are dynamic organelles that can change their morphology. The role of these mitochondrial dynamics in cardiomyocytes remains obscure in patients with heart failure (HF). Endomyocardial biopsies were performed consecutively in 127 HF patients, and mitochondrial morphology data were obtained from 111 patients by electron microscopy. The patients were divided into three groups according to mitochondrial area quartiles (fission [Q1, area ≤ 0.119 μm2, n = 27], normal [Q2/Q3, 0.120 μm2 ≤ area ≤ 0.178 μm2, n = 55], and fusion [Q4, area ≥ 0.179 μm2, n = 28]). In the fission group, the serum N-terminal pro-brain natriuretic peptide and B-type natriuretic peptide (BNP) levels were significantly higher, and patients with HF and a reduced left ventricular ejection fraction were more common, than in the other groups. A multivariate logistic regression model showed that diabetes mellitus was independently associated with placement in the fission group (odds ratio: 2.835, 95%confidence interval [CI]: 1.037–7.752). A Kaplan–Meier curve analysis showed that the prognosis was significantly poorer in the fission group than in the other groups, and a multivariate Cox regression model revealed fission to be an independent predictor of 1500-day mortality (hazard ratio: 4.365, 95%CI: 1.198–15.909). The circulating levels of miR-140-5p (≥2500) were independently associated with the presence of mitochondrial fission (OR: 3.622, 95%CI: 1.260–10.413). Excessive mitochondrial fission was observed in patients with severe HF status, and was independently associated with adverse outcomes in HF patients. Circulating mitochondrial dynamics-related miRNA levels might be of use in detecting mitochondrial fission in the cardiomyocytes of HF patients. (Figure presented.)
