Background and objectives: There is insufficient epidemiological knowledge of hypouricemia. In this study, we aimed to describe the distribution and characteristics of Japanese subjects with hypouricemia. Materials and Methods: Data from subjects who underwent routine health checkups from January 2001 to December 2015 were analyzed in this cross-sectional study. A total of 246,923 individuals, which included 111,117 men and 135,806 women, met the study criteria. The participants were divided into quartiles according to their serum uric acid (SUA) levels. We subdivided the subjects with hypouricemia, which was defined as SUA level ≤ 2.0 mg/dL, into two groups and compared their characteristics, including their cardiovascular risks. Results: The hypouricemia rates were 0.46% overall, 0.21% for the men and 0.66% for the women (P < 0.001). The number of the subjects with hypouricemia showed two distributions at SUA levels of 0.4-1.1 mg/dL (lower hypouricemia group), which included a peak at 0.7-0.8 mg/dL, and at SUA levels of 1.4-2.0 mg/dL (higher hypouricemia group). The men in the higher hypouricemia group had lower body mass indexes (BMI) and triglyceride (TG) levels and had higher fasting blood glucose levels than those in the lower hypouricemia group. The women in the higher hypouricemia group were younger; had lower BMI, total protein, TG, total cholesterol and low-density lipoprotein cholesterol levels; and had higher estimated glomerular filtration rates levels compared to those in the lower hypouricemia group. Conclusions: The characteristics of the individuals in the lower and higher hypouricemia groups differed significantly, indicating different pathophysiologies within each group.

Midventricular obstructive hypertrophic cardiomyopathy (MVOHCM) is a rare form of hypertrophic cardiomyopathy (HCM). An 80-year-old man was administered bisoprolol and warfarin therapies as treatment for MVOHCM with an apex aneurysm due to myocardial damage and intra-aneurysmal thrombus not complicated by atrial fibrillation. The pressure gradient in the midventricle successfully improved from 53.9 to 21.8 mmHg, and the intra-aneurysmal thrombus disappeared.

本研究は、独居高齢者に焦点をあてて、心の理論と社会参加の関連について検討した。鹿児島県垂水市における垂水研究2017に参加した地域在住高齢者のうち独居者102人を対象とした。調査項目は、心の理論、年齢、性別、認知機能、抑うつ状態、社会参加、外出頻度、毎日の他者との会話の有無とした。心の理論の一次的信念課題はサリーとアン課題とし、同課題に「アンが宝物を移す様子を窓からサリーが見ている」という内容を加え「アンは、戻ってきたサリーがどこを探すと思うか」と質問し、二次的信念課題とした。一次的・二次的信念課題それぞれで、正解と不正解の2群に分けて単変量解析を行い、有意差を認めた項目を独立変数、正解・不正解を従属変数としたロジスティック回帰分析を行った。その結果、二次的信念課題において、社会参加が関連因子として抽出された。独居高齢者において、心の理論を保つために社会参加を支援することの重要性が示唆された。(著者抄録)

症例は80歳男性で、めまいを主訴に来院した。頭部MRIで橋に高信号域を認め、脳梗塞と診断し抗血小板薬を投与したところ、めまいの改善を得た。心房細動を伴う心原性脳梗塞の鑑別目的に心電図を持続的にモニタリングした結果、無症候性の非持続性心室頻拍(NSVT)が検出されたため、当科紹介となった。血液検査ではD-ダイマー5.70μg/mL、血漿中B型ナトリウム利尿ペプチド(BNP)525.7pg/dL、胸部X線では肺うっ血はなく、心エコー検査で非対称性の左室肥大を認めた。また、ドップラー心エコーで拡張早期における心尖部瘤から左室流出路にかけて奇異性血流がみられ、心室中部における加速流は収縮期に2.00m/s、拡張期に3.67m/sのピーク血流を呈していた。以上の所見から心室中部閉塞性肥大型心筋症と診断し、抗凝固療法としてヘパリンとワルファリンを開始した。さらに、NSVTと心室収縮(PVC)の予防、心室中部の圧較差減少を得るため経口ビソプロロールを開始した。投与3ヵ月後、心室中部の圧較差は減少し、BNP値の低下、NSVTの消失、PVCの低下、心尖部瘤における血栓消失が認められた。

Aim: The present cross-sectional study examined the associations of social frailty status with loss of muscle mass and weakness among community-dwelling older adults. Methods: Data from 353 older adults (74.8% women) who had participated in a community-based health check survey (Tarumizu Study) were analyzed. Social frailty was defined using responses to five questions (going out less frequently, rarely visiting friends, feeling unhelpful to friends or family, living alone and not talking with someone every day). Participants with two or more components were considered socially frail. We assessed appendicular skeletal muscle mass using bioelectrical impedance analysis and calculated appendicular skeletal muscle index. Dominant handgrip strength was assessed. Loss of skeletal muscle mass (appendicular skeletal muscle index <7.0 kg/m <sup>2</sup> for men, <5.7 kg/m <sup>2</sup> for women) and muscle weakness (handgrip strength <26 kg for men, <18 kg for women) were determined based on the Asian Working Group for Sarcopenia criteria. Results: The prevalence of social frailty was 14.7%. A higher prevalence of muscle weakness and loss of skeletal muscle mass in participants with social frailty was shown than in those without (muscle weakness 44.2% vs 23.6%, P ≤ 0.05; loss of skeletal muscle mass 59.6% vs 46.2%, P = 0.07). Social frailty was independently associated with muscle weakness (odds ratio 2.04, 95% confidence interval 1.06–3.95), but not with loss of skeletal muscle mass (odds ratio 1.47, 95% confidence interval 0.78–2.76) after adjusting for covariates. Conclusions: Social frailty status could be associated with muscle weakness, though not a loss of skeletal muscle mass. Geriatr Gerontol Int 2019; 19: 76–80.

＜文献概要＞現在使用されている主な降圧薬は,カルシウム拮抗薬,レニン・アンジオテンシン系阻害薬(アンジオテンシン変換酵素[ACE]阻害薬,アンジオテンシンII受容体拮抗薬[ARB],直接的レニン阻害薬),利尿薬,β遮断薬などが知られている。しかし,最近は配合剤を除き,新規高血圧治療薬は上市されていない。現在,高血圧に対する開発中の薬剤は,非ステロイド型ミネラルコルチコイド受容体(MR)拮抗薬とアンジオテンシンIIを標的とした高血圧治療ワクチンがある。欧米で心不全治療薬として使用されているARBとネプリライシン(NEP)阻害剤との合剤においても降圧効果の報告があり,本邦でも臨床試験が進められている。

地域在住高齢者の社会的フレイル状態と筋量喪失および筋力低下の関連を調べるため、横断研究を実施した。2017年に垂水市で実施された地域健診に参加した高齢者353名(女性264名、平均75.4±6.5歳)のデータを用いた。社会的フレイルは、外出頻度の低さ、友人訪問がめったにないこと、友人・家族に対する無力感、独居、毎日の会話がないことのうち2つ以上を満たす場合とした。生体電気インピーダンス法を用いて四肢の骨格筋量を評価し、四肢の骨格筋量指数を算出した。さらに、利き手の握力を測定した。社会的フレイルの発生は14.7%に認められた。社会的フレイルの参加者は、社会的フレイルのない参加者と比較して筋力低下および骨格筋量喪失の割合が高いことが示された。共変量の影響を補正後、社会的フレイルは筋力低下と独立して関連したが、骨格筋量との独立した関連は認められなかった。
