The elderly population is increasing because of increasing life expectancy, and the prevalence of frailty increases with age. Frailty commonly coexists with cardiovascular diseases (CVDs), such as coronary artery disease (CAD), heart failure (HF), aortic stenosis (AS), and atrial fibrillation (AF). Frail patients who undergo revascularization for CAD have higher complication rates; those with HF have a high prevalence of poor outcomes, and those with AF are vulnerable to increased stroke incidence. Moreover, frailty and asymptomatic severe AS were independent factors for mortality. The presence of frailty can lead to poor clinical outcomes, and frailty has been identified as a risk factor for mortality. Thus, the identification of frail patients who are at higher risks of disability and adverse clinical outcomes is important. In this review, the relationship between frailty and CVD is appraised and optimal treatments for frail patients are discussed.

Coronavirus disease-2019 (COVID-19), caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), has affected more than seven million people worldwide, contributing to 0.4 million deaths as of June 2020. The fact that the virus uses angiotensin-converting enzyme (ACE)-2 as the cell entry receptor and that hypertension as well as cardiovascular disorders frequently coexist with COVID-19 have generated considerable discussion on the management of patients with hypertension. In addition, the COVID-19 pandemic necessitates the development of and adaptation to a “New Normal” lifestyle, which will have a profound impact not only on communicable diseases but also on noncommunicable diseases, including hypertension. Summarizing what is known and what requires further investigation in this field may help to address the challenges we face. In the present review, we critically evaluate the existing evidence for the epidemiological association between COVID-19 and hypertension. We also summarize the current knowledge regarding the pathophysiology of SARS-CoV-2 infection with an emphasis on ACE2, the cardiovascular system, and the kidney. Finally, we review evidence on the use of antihypertensive medication, namely, ACE inhibitors and angiotensin receptor blockers, in patients with COVID-19.

日本で承認されているアンジオテンシンII受容体拮抗薬(ARB)の相対的有効性を評価するため、プラセボ対照/比較無作為化試験(RCT)の文献を対象とするネットワークメタアナリシス(NMA)を実施した。PubMedおよび医中誌を用いて、ARB単独療法を受けた原発性高血圧症患者のRCTで、1995年1月1日～2018年8月31日に発表された文献を検索した。固定効果モデル(FEM)と変量効果モデルを用いてNMAを実施した。同定したRCT 763件のうち77件が適格基準に合致した。そのうち37件がベースラインからの収縮期血圧(SBP)と拡張期血圧(DBP)の平均変化を報告していた。FEMでは参照値としてのアジルサルタンと比較した各薬剤の作用を示した。SBP/DBPにおけるベースラインからの平均変化は、カンデサルタンシレキセチルが3.8/2.6mmHg、イルベサルタンが4.8/3.7mmHg、ロサルタンカリウムが3.0/1.9mmHg、オルメサルタンメドキソミルが3.2/2.7mmHg、テルミサルタンが3.2/2.0mmHg、バルサルタンが3.1/2.4mmHgであった。アジルサルタンは、SBPとDBPの低下において他のARBよりも優れた有効性を示した。

Previous reports have found evidence that the lung uptake of iodine-123-metaiodobenzylguanidine (123I-MIBG) represents pulmonary vascular endothelial function. Therefore, it was believed that the reduced lung uptake of 123I-MIBG in patients with pulmonary artery hypertension may indicate poor pulmonary vascular endothelial function in those patients. In our previous report, we analyzed the lung uptake of 123I-MIBG in patients with pulmonary hypertension, and demonstrated that it is lower in patients with pulmonary arterial hypertension (PAH) than in those with chronic thromboembolic pulmonary hypertension and controls, suggesting that reduced uptake of 123I-MIBG in patients with PAH indicates endothelial dysfunction of the pulmonary artery. In the current report, we describe a 46-year-old woman diagnosed with scleroderma whose lung uptake of 123I-MIBG was decreased on admission, but she was not diagnosed with pulmonary artery hypertension at that time because her pulmonary artery pressure during right heart catheterization was not elevated. However, she was diagnosed with borderline PAH 2 years later. The lung uptake of 123I-MIBG was reduced before a reduction in %DLCO was observed. This report suggests that the lung uptake of 123I-MIBG may be useful for the early diagnosis of pulmonary artery hypertension.

軽度肥満中年者において減量による血圧上昇抑制効果に性差がみられるかどうか検討した。軽度肥満(BMI 25.0～29.9kg/m2)を呈する40～49歳の中年者を対象に、3年後と10年後の健診データを評価した。傾向スコアマッチングにより3年後にBMIが1.0kg/m2以上減少した体重減少群(WL群)392名(男性232名、女性160名)と体重非減少群(非WL群)392名(男性232名、女性160名)に分類し、3～10年後の血圧を比較し、10年後における高血圧の有病率について検討した。その結果、男性では3年後の拡張期血圧(DBP)はWL群の方が非WL群より有意に低値であり、収縮期血圧(SBP)もWL群の方が有意に低かった。10年後ではDBP、SBPともに有意な群間差は認められなかった。女性では10年後のSBPはWL群の方が非WL群より有意に低く、DBPに有意差はみられなかった。また、10年後の高血圧の発症率に関して、男性ではWL群と非WL群との間に有意差はなかったが、女性ではWL群の方が有意に低値を示していた。減量による血圧上昇抑制効果に性差が認められ、女性の方が減量の効果が大きいことが明らかになった。
