Atrial fibrillation (AF) is the most common cardiac arrhythmia in the world and has a high risk of thromboembolism. The most effective approach, catheter ablation, requires evaluation by electrocardiography. The aim of our study was to investigate novel clinical markers that predict restoration of sinus rhythm (SR) after catheter ablation. Seventy-eight consecutive patients with AF underwent catheter ablation and were separated into 2 groups: restored SR and recurrent AF. The levels of 4 blood proteins (serum or plasma) and 3 mature microRNAs (miRNAs) and their primary miRNAs (pri-miRNAs) in serum were measured before and after ablation, and the associations between each parameter were analyzed statistically. Soluble thrombomodulin (s-TM) and plasminogen activator inhibitor-1 (PAI-1) levels increased above baseline after ablation in both the restored SR (s-TM 11.55 [2.92] vs 13.75 [3.38], P <.001; PAI-1 25.74 [15.25] vs 37.79 [19.56], P <.001) and recurrent AF (s-TM 10.28 [2.78] vs 11.67 [3.37], P <.001; PAI-1 26.16 [15.70] vs 40.74 [22.55], P <.001) groups. Levels of C-reactive protein and asymmetric dimethylarginine were not significantly changed. Pri-miR-126 levels significantly decreased after ablation in the recurrent AF group, but the other miRNAs and pri-miRNAs did not. The measurement of s-TM and pri-miR-126 in blood was a useful tool to reflect the condition of AF patients with catheter ablation.

Background: This study was designed to investigate the relationship between right ventricular wall stress (RVWS) and plasma B-type natriuretic peptide (BNP) levels in patients with pulmonary hypertension (PH). Methods and Results: The 57 consecutive PH patients and 8 control subjects were enrolled. Right heart catheterization (RHC), echocardiography, and BNP measurements were performed, and RVWS and left ventricular wall stress (LVWS) were calculated with the formula based on Laplace’s law. Systolic RVWS and end-diastolic RVWS were higher in PH patients compared with controls (systolic RVWS: 77±41 vs. 17±5 kdynes/cm2 (P<0.0001), end-diastolic RVWS: 15±12 vs. 8±2 kdynes/cm2 (P<0.0005)). Univariate analyses showed that logBNP at baseline correlated with systolic RVWS (r=0.58, P<0.0001) and end-diastolic RVWS (r=0.61, P<0.0001). We performed multivariate regression analysis and determined that end-diastolic RVWS was an independent determinant of logBNP in patients with PH. In addition, change in plasma BNP levels after treatment correlated with change in systolic RVWS (r=0.70, P<0.0001) and change in end-diastolic RVWS (r=0.68, P<0.0001). Conclusions: Both systolic and end-diastolic RVWS were elevated in patients with PH, and correlated with the symptoms of PH. End-diastolic RVWS was an independent determinant of plasma BNP levels in PH patients.

肺高血圧症(PH)患者における右室壁応力(RVWS)の臨床的意義を調べ、RVWSと脳性ナトリウム利尿ペプチド(BNP)との相関を明らかにした。右心カテーテル法(RHC)で肺高血圧症(PH)と診断された患者57例(男性13例、女性44例、平均60±15歳)と対照8名(男性4名、女性4名、平均65±8歳)から血液検体を採取し、血漿BNPを測定した。心エコーを行い、RHCを実施して血行動態を調査して、RVWSを計算した。収縮期RVWSはPH群が77±41kdynes/cm2、対照群が17±5kdynes/cm2、拡張終期RVWSは15±12kdynes/cm2と8±2kdynes/cm2で、PH群の方が高かった。単変量解析ではベースラインでのlogBNPは収縮期RVWS、拡張終期RVWSと関連した。多変量回帰分析では拡張終期RVWSがPH群のlogBNPの独立決定因子であった。治療後の血漿BNPにおける変化は収縮期RVWSと拡張終期RVWSにおける変化と関連した。

This cross-sectional study investigated the association between chronic pain and physical frailty in community-dwelling older adults. We analyzed data obtained from 323 older adults (women: 74.6%) who participated in a community-based health check survey (the Tarumizu Study, 2017). Physical frailty was defined in terms of five parameters (exhaustion, slowness, weakness, low physical activity, and weight loss). We assessed the prevalence of chronic low back and knee pain using questionnaires. Participants whose pain had lasted ≥two months were considered to have chronic pain. Among all participants, 138 (42.7%) had chronic pain, and 171 (53.0%) were categorized as having physical frailty or pre-frailty. Logistic regression analysis showed that chronic pain was significantly associated with the group combining frailty and pre-frailty (odds ratio 1.68, 95% confidence interval 1.03–2.76, p = 0.040) after adjustment for age, sex, body mass index, score on the 15-item Geriatric Depression Scale, and medications. Comparing the proportions of chronic pain among participants who responded to the sub-items, exhaustion (yes: 65.9%, no: 39.4%) demonstrated a significant association (p < 0.001). Chronic pain could be associated with the group combining frailty and pre-frailty and is particularly associated with exhaustion in community-dwelling older adults. Therefore, there is a need for early intervention and consideration of the role of exhaustion when devising interventions for physical frailty in older individuals with chronic pain.

This cross-sectional study investigated the association between osteoporosis, grip strength, and skeletal muscle mass in community-dwelling older women. Data obtained from 265 older women who participated in a community-based health check survey (Tarumizu Study) were analyzed. Face-to-face interviews with participants revealed their history of osteoporosis. Appendicular skeletal muscle mass was assessed through bioelectrical impedance analysis, and appendicular skeletal muscle index was calculated. Dominant grip strength was also assessed. Loss of skeletal muscle mass (appendicular skeletal muscle mass < 5.7 kg/m2) and muscle weakness (grip strength < 18 kg) were determined based on criteria for sarcopenia put forth by the Asian Working Group for Sarcopenia. The prevalence rates of osteoporosis, muscle weakness, and loss of skeletal muscle mass were 27.2%, 28.7%, and 50.2%, respectively. Loss of skeletal muscle mass was more prevalent in participants with osteoporosis than in those without (65.3% vs. 44.6%, p < 0.01). The association between osteoporosis and muscle strength was not significant (30.6% vs. 28.0%, p = 0.68). After covariate adjustment, loss of skeletal muscle mass was found to be independently associated with osteoporosis (odds ratio 2.56, 95% confidence interval 1.33–4.91). In sum, osteoporosis was found to be associated with loss of skeletal muscle mass, but not with muscle weakness in community-dwelling older women.

脳卒中急性期以外は、発作時の急性期の降圧治療についての異なる治療戦略を評価するRCTはない。標的器官が降圧以外の具体的な治療介入が必要であるか、安全に治療するために降圧にかける時間および降圧の程度、降圧治療薬の種類について検討する必要がある。通常、速やかに治療が必要であることから、静脈注射薬を使用する。本ガイドラインに記載されている薬剤で、わが国での使用可能なものとしては、Ca拮抗薬のニカルジピンと硝酸薬のニトログリセリンが中心となるものの、その他の機序の薬剤も、各々の臨床現場において使用されている。高血圧緊急症を有する患者の生存率は改善しているが、これらの患者のリスクは高く、緊急症の状態から改善した場合は、二次性高血圧症についてスクリーニングし、経口薬開始後も専門的なフォローアップをおこなうことが推奨される。(著者抄録)
