INTRODUCTION The pathophysiology and mechanism of in-stent restenosis (ISR) after implantation of second-generation drug-eluting stents (DESs) are not fully clear. We compared the morphological characteristics of ISR between first-and second-generation DESs using frequency domain optical coherence tomography (OCT). METHODS Patients who underwent follow-up coronary angiography (CAG) after first-generation (CYPHER™ and TAXUS™) and second-generation (Nobori®, PROMUS Element™, Resolute Integrity and XIENCE) DES implantations were examined. ISR was defined as lesions of over 50% diameter stenosis at follow-up CAG. Frequency domain OCT was performed at the time of revascularisation of ISR. Tissue morphology was assessed at minimum lumen area. OCT images of DESs at both early (≤ 1 year) and late (> 1 year) phase follow-up were compared. RESULTS On qualitative OCT assessment, the ratios of homogeneous, layered, heterogeneous without-attenuation and heterogeneous with-attenuation morphologies were 57.1%, 17.1%, 20.0% and 5.7%, respectively, for second-generation DES ISR (n = 35), and 16.7%, 25.0%, 25.0% and 33.3%, respectively, for first-generation DES ISR (n = 36). At late phase follow-up, homogeneous morphology was significantly more common for second-generation DES ISR compared to first-generation DES ISR (first-generation: 8.0% vs. second-generation: 50.0%; p < 0.01) while heterogeneous with-attenuation morphology was significantly more common for first-generation DES ISR (first-generation: 44.0% vs. second-generation: 5.6%; p < 0.01). CONCLUSION Homogeneous tissue morphology was more frequently found for second-generation than first-generation DES ISR, especially in the late phase. This suggested that neointimal hyperplasia was the main mechanism in second-generation DES ISR, and that the neointima was stabilised, much like in bare metal stent implantation.

肺高血圧症患者70例(男性15例、女性55例、平均62.1±14.6歳)を対象に、右室流出路駆出パルスドプラ波形から2相性波を抽出し、血行動態との関連について検討した。さらに、肺血管抵抗の推定式を作成した。全患者で経胸壁心エコー検査を実施し、また右心カテーテル検査により血行動態を評価した。右室流出路駆出ドプラ法による2相性波形の解析で早期波(第1波)をP1、後期波(第2波)をP2とした。P1とP2の持続時間をP1D(秒)、P2D(秒)とし、最大血流速度(PFV)をP1PFV(m/sec)、P2PFV(m/sec)と定義した。上記のパラメータ(P1D、P2D、P1PFV、P2PFV)が右心カテーテル検査による血行動態計測値と相関するかどうかについて検討した。P1Dと肺血管抵抗との間(r=-0.542)およびP2PFVと肺血管抵抗との間(r=-0.513)に有意な負の相関を認めた(いずれもP<0.0001)。肺血管抵抗の新たな推定式として、RVR=26-77xP1D-14xP2PFVを作成した。この推定式による血管抵抗値は右心カテーテル検査による肺血管抵抗値と有意に相関した(r=0.649,P<0.0001)。

【はじめに・目的】2013年の国民生活基礎調査（厚生労働省）による要介護原因をみると，転倒・骨折は12.2％を占め，特に女性では15.4％と認知症に次いで第2位となっている．転倒・骨折には骨粗鬆症やサルコペニアが大きく関与することが推察され，特に骨粗鬆症を有する高齢女性では転倒による骨折リスクが大きい．2005年における我が国の骨粗鬆症患者数は1280万人とされており，サルコペニア有病率は7.5％～8.2％と推測され，これらの有病率は高齢者の増加に伴い今後も増えていくことが予想される．骨粗鬆症とサルコペニア対策は，我が国において喫緊の課題となっており，骨粗鬆症とサルコペニアの関連性を調べた研究はみられるものの，骨粗鬆症と握力，歩行速度，筋量をそれぞれ検討した研究は限られている． そこで今回，地域在住高齢女性における骨粗鬆症と握力，歩行速度，筋量との関係を明らかにすることを目的とした．【方法】垂水研究2017に参加した地域在住高齢者380名のうち，主要データに欠損の無い高齢女性266名を分析対象とした．研究デザインは横断研究であり，調査項目として年齢，骨粗鬆症の有無などの基本情報を聴取し，握力，歩行速度，四肢骨格筋量（生体電気インピーダンス法）を測定した．握力は18㎏，歩行速度は1.0m/s，補正四肢骨格筋量（ASMI，四肢骨格筋量㎏/身長m2）は5.7kg/m2を基準とし，それぞれの値を下回った場合を握力低下，歩行速度低下，筋量低下と定義した．これらの低下の有無と骨粗鬆症の有無の割合をχ2検定で比較した．また，握力低下，歩行速度低下，筋量低下のそれぞれを従属変数とし，独立変数を骨粗鬆症の有無，共変量に年齢を投入したロジスティック回帰分析を行った．【結果】骨粗鬆症の有病率は27.3％で，握力低下は28.6％，歩行速度低下は4.9％，筋量低下は50.0％が該当し，骨粗鬆症のある者では有意に筋量低下に該当する者の割合が高かった（p＝0.013）．ロジスティック回帰分析においても，筋量低下は骨粗鬆症と有意に関連しており（OR 2.0，95％CI 1.2-3.5，p＝0.014），年齢で調整したモデルにおいても筋量低下と骨粗鬆症との関連性は有意であった（OR 1.9，95％CI 1.1-3.4，p＝0.028）．【結論】骨粗鬆症を有する地域在住高齢女性において，筋力や歩行速度の低下は認められないものの，筋量が低下している可能性が示唆された．筋量の低下は，将来的に筋力や運動機能の低下を招き，転倒・骨折へと繋がっていく可能性が高い．骨粗鬆症を有する高齢女性においては，筋力や運動機能の評価だけではなく筋量を測定することにより，将来の運動機能低下や転倒・骨折予防のための介入の必要性を把握することが可能と思われる．今回は，問診のみで骨粗鬆症の有無を分類したため，骨粗鬆症の診断を受けていない対象者のなかにも骨密度が低下している者が含まれる可能性がある．今後は骨密度を測定し身体機能や筋量との関係性を検証する必要がある．【倫理的配慮，説明と同意】本研究は鹿児島大学大学院医歯学総合研究科疫学研究等倫理委員会の承認を受けて行った．ヘルシンキ宣言に基づき，参加者に文書と口頭にて説明し同意を得た．

【はじめに・目的】サルコペニアは加齢に伴う筋量や筋力の低下を特徴とする症候群であり，その改善や予防の対策は理学療法の重要な対象となる。近年，高齢者の認知・精神機能がサルコペニアに影響することが注目されているが，軽度認知機能低下（MCI）と抑うつ傾向について高齢者のサルコペニアとの関連性は未だ明らかではない。本研究は，横断的な調査を行い，MCIおよび抑うつ傾向とサルコペニアとの関連性を検討することを目的とした。 【方法】垂水研究2017に参加した地域在住高齢者380名のうち，認知症やうつ病の既往歴がある者，測定項目に欠損がある者を除く65歳以上の350 名（女性263名)を分析対象とした。Body mass index（BMI)，四肢骨格筋量 (生体電気インピーダンス法)，握力，通常歩行速度，National Center for Geriatrics and Gerontology functional assessment tool（NCGG-FAT)，Geriatric depression scale 15 （GDS-15)により，身体・精神・認知機能を評価し，病歴や教育年数などの基本属性を聴取した。また，骨格筋量はAppendicular Skeletal Muscle Mass Index (ASMI) (kg/m2) を算出し，Asian Working Group for Sarcopenia のアルゴリズムに従い，ASMI，歩行速度，握力の参照値からサルコペニアを定義した。NCGG-FATで年齢と教育歴を考慮した標準値より1.5SD以上の低下を認めた者をMCIとし，GDS-15が6点以上の者を抑うつ傾向とした。統計解析は，サルコペニア該当者と非該当者の2群に分け，カイ二乗検定およびロジスティック回帰分析を行い，MCIおよび抑うつ傾向とサルコペニアとの関係を調べた。有意水準は5% 未満とした。 【結果】サルコペニア該当者77名（22%），非該当者273名（78%）であった。サルコペニアの有無を従属変数として，年齢，教育年数，BMI，MCI，抑うつ傾向を独立変数としたロジスティック回帰分析（強制投入法）の結果，MCI（OR 2.72，95%CI 1.48-4.99，p=0.001）と抑うつ傾向（OR 2.54，95%CI 1.19-5.43，p=0.016）のいずれともに，サルコペニアと有意に関連していた。 【結論】本研究による横断的な分析の結果，MCIと抑うつ傾向はいずれともサルコペニアと関連があることが示唆された。この結果は，サルコペニアの予防戦略を検討する上で，認知機能や抑うつの評価を行う重要性を示していると考える。今後は，これらの関連性が将来的なサルコペニアの発生，要介護状態にどのように影響するのか縦断的に調査する必要がある。 【倫理的配慮，説明と同意】本研究は，鹿児島大学大学院医歯学総合研究科疫学研究等倫理委員会の承認を得て実施した。発表に関連し，開示すべき利益相反関係にある企業などはない。また，対象者には書面と口頭にて研究の目的・趣旨を十分に説明し，研究に対する参加の同意を得た。

Background: The independent role of uric acid (UA) as a risk factor for atrial fibrillation (AF) has not been fully elucidated. Methods and Results: We studied 111,566 subjects (53,416 men; 58,150 women) who underwent annual health check-ups. We divided them by sex into tertile of baseline UA. To investigate the predictive power of UA for new-onset AF, we performed Cox proportional hazard analysis including UA tertiles, body mass index, creatinine, smoking and drinking status, and presence of hypertension, diabetes, and dyslipidemia. During 4.1 years, 467 men (0.87%) and 180 women (0.31%) had AF (P<0.001). Cut-off points for tertiles of UA were as follows: women, ≤3.9, 4.0–4.8, and ≥4.9 mg/dL; men, ≤5.4, 5.5–6.4, and ≥6.5 mg/dL. Hazard ratio (HR) for third to first tertile was 1.74 (95% CI: 1.15–2.70; P=0.008), whereas there were no differences between tertiles in men. Rate of new-onset AF was significantly higher in the group with initially increased UA (ΔUA ≥0.3mg/dL) than that with unchanged UA (ΔUA, −0.2 or +0.2 mg/dL) in the third tertile of baseline UA in both sexes. Conclusions: Higher baseline UA was significantly associated with higher AF incidence in women. Initial increase in UA was significantly associated with AF incidence when baseline UA was ≥6.5 mg/dL in men, and ≥4.9 mg/dL in women.

Thymidine phosphorylase (TP) is a rate-limiting enzyme in the thymidine catabolic pathway. TP is identical to platelet-derived endothelial cell growth factor and contributes to tumour angiogenesis. TP induces the generation of reactive oxygen species (ROS) and enhances the expression of oxidative stress-responsive genes, such as interleukin (IL)-8. However, the mechanism underlying ROS induction by TP remains unclear. In the present study, we demonstrated that TP promotes NADPH oxidase-derived ROS signalling in cancer cells. NADPH oxidase inhibition using apocynin or small interfering RNAs (siRNAs) abrogated the induction of IL-8 and ROS in TP-expressing cancer cells. Meanwhile, thymidine catabolism induced by TP increased the levels of NADPH and intermediates of the pentose phosphate pathway (PPP). Both siRNA knockdown of glucose 6-phosphate dehydrogenase (G6PD), a rate-limiting enzyme in PPP, and a G6PD inhibitor, dihydroepiandrosterone, reduced TP-induced ROS production. siRNA downregulation of 2-deoxy-D-ribose 5-phosphate (DR5P) aldolase, which is needed for DR5P to enter glycolysis, also suppressed the induction of NADPH and IL-8 in TP-expressing cells. These results suggested that TP-mediated thymidine catabolism increases the intracellular NADPH level via the PPP, which enhances the production of ROS by NADPH oxidase and activates its downstream signalling.

症例は74歳、男性。腰痛の精査にて近医より当院整形外科に紹介受診となり精査入院中、低血糖と見当識障害を契機とした全身精査にて、三尖弁に直径20×16mmの疣腫を認め、三尖弁感染性心内膜炎と診断した。直ちにペニシリンG2400万単位を開始した。その後、腰椎においても、初診時にはみられなかった骨破壊像が出現し、化膿性脊椎炎と診断された。三尖弁の疣腫は手術適応とされるサイズであったが、腰椎の感染巣の存在下での開心術は術後感染リスクが高いと判断された。一方、腰椎の骨破壊は進行性で、保存的加療での改善は困難と判断され、腰椎後方固定術を施行された。第79病日にて抗生剤投与終了したが、感染の再燃徴候なく、三尖弁の疣腫にも変化を認めず、開心術は施行せず経過観察の方針となった。(著者抄録)

定期検診を受診した一般集団被験者111566例(男性53416例、女性58150例、平均53.8±15.6歳)を対象に、尿酸値と心房細動の新規発症の関連を検討した。心不全、冠動脈疾患、甲状腺機能亢進症の患者は除外した。ベースラインの血清尿酸値は男性で5.9±1.3mg/dL、女性で4.4±1.0mg/dLであった。男性および女性被験者を尿酸値の3分位値でそれぞれ3群に分類した。追跡期間中央値は4.1年で、心房細動は女性で180例(0.31%)、男性で467例(0.87%)発生した。血清尿酸値で三分位に分けた心房細動の発症率は、女性では最低値の第1三分位群が0.20%、第2三分位群が0.31%、第3三分位群が0.49%であった。男性では有意な群間差はなかった。Cox比例ハザードモデルを用いた解析で、女性における第1三分位群を基準とした第3三分位群の心房細動発症の調整ハザード比は1.74(95%信頼区間1.15～2.70、P=0.008)であった。男性および女性の第3三分位群ではベースラインに比べて尿酸値が0.3mg/dL以上増加した被験者における心房細動の発症率が有意に高かった。

Myocardial ischemic events after non-cardiac surgery is still a serious problem, especially in older, high-risk patients. However, the prevalence and risk factors of blood pressure (BP) abnormalities, which may possibly lead to myocardial ischemic attack, have not been reported. Our aim is to elucidate predictive factors of postoperative BP abnormalities following a minor-to-moderate surgery, employing preoperative left ventricular diastolic function. Patients who underwent cardiac echocardiogram examination and received oral and maxillo-facial surgery under general anesthesia were enrolled. The echocardiographic parameters of diastolic function were compared between patients who had postoperative BP abnormalities (hypertension-systolic blood pressure [SBP] ≥170 mmHg-or hypotension-SBP < 80 mmHg-episode) that required therapeutic interventions until 7 days after surgery and those who had no BP abnormalities. Of the 173 patients analyzed, 25 (14.4%) had BP abnormalities. BP abnormalities patients were older, having a larger proportion of diabetes mellitus, lower E/A ratio and e’, and larger E/e’ and left atrial dimension than those without BP abnormalities. Subanalyses revealed that the independent risk factors responsible for hypertension episodes (14 patients) were the mean e’ (odd ratio [OR]: 0.434; 95% confidence interval [CI]: 0.229-0.824), diabetes mellitus (OR: 5.018; 95% CI: 1.030-24.436), SBP at hospitalization (OR: 1.099; 95% CI: 1.036-1.165), and operation time (hour; OR: 1.326; 95%CI: 1.109-1.586), while hypotension episodes (11 patients) were associated solely with operation time (OR: 1.206; 95% CI: 1.046-1.391). In conclusion, left ventricular diastolic dysfunction, increased insulin resistance, boosted SBP at hospitalization, and prolonged operation should be taken into consideration as risk factors of postoperative BP abnormalities, especially hypertension, following minor-to-moderate surgery.
