Background: Proton pump inhibitors (PPIs) are known to increase the risk of mortality and cardiovascular events in the general population. However, in patients with maintenance hemodialysis, PPI effects are under investigated. Methods: We analyzed the risk of PPIs for cardiovascular events using the Kagoshima Dialysis (KIDS) registry, a prospective, multicenter, observational study in patients with maintenance hemodialysis in Japan. Results: In all, 531 patients were enrolled from June 2015 to December 2018. One-year follow-up data were available for 376 patients (Use of PPIs at baseline (PPI group): 217 patients and without PPIs (No PPI group): 159 patients). The incidence of a composite outcome (all-cause mortality, non-fatal myocardial infarction, or non-fatal stroke) was higher in patients in the PPI group than the No PPI group (15.2% vs. 4.4%; hazard ratio (HR): 3.65, 95% confidence interval (CI): 1.61–8.23, P = 0.002). In the multivariate analysis, even after adjustment for covariates, the use of PPIs was an independent risk factor for a composite outcome (HR: 2.38, 95% CI: 1.02–5.54, P = 0.045). We performed propensity score matching analysis as a sensitivity analysis, showing a consistent result. The incidence of bleeding showed no difference between the two groups (15.7% vs. 11.3%; HR: 1.46, 95% CI: 0.83–2.59, P = 0.19). Conclusions: These results indicate that the use of PPIs in patients with maintenance hemodialysis might increase mortality and cardiovascular events without decreasing the risk of bleeding. Therefore, it should always be analyzed if a patient truly needs PPIs.

冠動脈疾患(CAD)を併発する重症虚血肢(CLI)患者の脳血管死亡と心血管死亡の危険因子を調査するため、後方視的コホート研究を施行した。2014年1月～2016年8月に入院したCLI患者連続137名を組み入れた。全患者が以前にCADへの血行再建術を施行されていた。CLIに対しては血管内治療を行い61名ではバイパス手術も併用した。CLIに対する血行再建術から2年以内の脳血管死亡と心血管死亡の複合アウトカムを主要評価項目とした。そのアウトカムに合致した死亡群23名(17%)(平均76±9歳、男性14名)と生存群114名(平均69±11歳、男性66名)を比較したところ、死亡群でのGeriatric Nutritional Risk Index(GNRI)は生存群よりも有意に低かった。主要評価項目に有意に関連する因子として血液透析と低栄養状態(GNRI 92未満)の2つが多変量Cox比例ハザード分析によって同定され、その2因子のいずれかを有する患者の生存率はいずれも認めない患者に比べ有意に低いことがKaplan-Meier法にて示された。さらにクロピドグレル使用に関しても死亡群の方がその使用率が有意に低く、低栄養状態の患者ではクロピドグレル非使用が主要評価項目との相関を強めることが判明した。低栄養状態への介入およびクロピドグレル使用はCLIへの重要な治療戦略になると考えられた。

Aim: Menopause causes arterial senescence and atherosclerotic development through decrease of estrogen. Recently, histone deacetylase SIRT1 has been reported to have protective effects against arterial senescence and atherosclerosis. However, the relationship between estrogen and SIRT1 in the context of menopause-induced arterial senescence is not well understood. The present study aims to investigate whether SIRT1 is involved in the etiology of menopause-induced arterial senescence and atherosclerotic development. Methods: Twelve-week old female apolipoprotein E-knockout (ApoE-KO) mice underwent ovariectomy (OVX) or sham surgery. Results: SIRT1 expression and endothelial nitric oxide synthase (eNOS) activation in the aorta were significantly lower in OVX mice than they were in sham mice (OVX vs. sham, n =5 per group). Senescence-associated β-galactosidase activity, protein expression of Ac-p53 and PAI-1, and aortic atherosclerosis lesions were significantly greater in OVX mice than they were in sham mice. Administration of 17β-estradiol (E2) for eight weeks to OVX mice restored aortic SIRT1 expression, activated eNOS, and retarded OVX-induced arterial senescence and atherosclerotic development (E2 vs. control, n=5 per group). The effects of E2 on SIRT1 upregulation, antisenescence and anti-atherosclerosis were attenuated by administration of a SIRT1 inhibitor, sirtinol. In vitro experiment using human endothelial cells demonstrated that E2 also increased SIRT1 expression and retarded oxidized low density lipoprotein-induced premature senescence, which were also abolished by sirtinol. These results suggested that estrogen modulated arterial senescence and atherosclerosis through SIRT1. Additionally a selective estrogen receptor modulator (SERM), bazedoxifene, also augmented SIRT1 and inhibited arterial senescence and atherosclerotic development (SERM vs. control, n =3 per group). Conclusions: Downregulation of SIRT1 causes OVX-induced arterial senescence and atherosclerosis in ApoEKO mice. Administration of estrogen or SERM enables OVX mice to restore these alterations by SIRT1 induction.

SIRT1がエストロゲンによって調節され、閉経に伴う老化および動脈硬化に関与しているか検討した。アポリポ蛋白質Eノックアウト(KO)マウスまたは血管内皮一酸化窒素合成酵素(eNOS)KOマウスに両側卵巣摘出術(OVX)またはシャム手術を行った。OVX後に17βエストラジオール(E2)ペレットを移植して、E2の作用を調べた。さらにヒト臍帯静脈内皮細胞(HUVEC)を培養し、E2処理を行った。シャム手術群よりもOVX群で大動脈でのSIRT1発現とeNOS活性化が有意に低かった。老化関連βガラクトシダーゼ活性、Ac-p53、プラスミノーゲン活性化因子阻害物質1の蛋白質発現、大動脈動脈硬化病変はシャム手術群よりもOVX群で有意に大きかった。E2投与によってOVX群の大動脈SIRT1発現、eNOS活性化が回復し、OVX誘発型動脈老化と動脈硬化発症が遅延した。E2のSIRT1発現亢進、抗老化、抗動脈硬化への影響はsirtinol投与によって軽減した。HUVECにおいてもE2によってSIRT1が発現亢進し、酸化LDL誘発型早期老化が遅延し、sirtinolによって無効化した。エストロゲンはSIRT1を通して動脈老化および動脈硬化を調節することが示唆された。

Seven participants had a diagnosis of dementia; the number of excluded participants differed based on different exclusion processes, and there was missing data of one participant with a dementia diagnosis. That is, one participant was excluded in the missing data before being excluded by the dementia diagnosis. Certification for long‐term care in Japan occurs at the disability level, not at the frailty level [1]. Therefore, in many Japanese cohort studies, certification for long‐term care is an exclusion criterion [2]. Also, in this study, participants who were already certified for long‐term care were excluded from the discussion of the frailty state. There can be multiple causes of chronic pain that cannot be identified in a single community‐based cohort study [3]. Further, in the current study, we did not explore the use of painkillers and, thus far, no discussion has taken place in this regard [4]. This is an aspect we would like to consider in future studies.

【目的】地域在住高齢者の有する腰痛と膝痛が，急性もしくは慢性疼痛であるかの違いによって身体機能に及ぼす影響を明らかにする。【方法】地域コホート研究（垂水研究2018）に参加した65 歳以上の高齢者735 名を対象とした。質問紙により急性・慢性疼痛の有無を聴取し，歩行速度および握力との関連を分析した。【結果】ロジスティック回帰分析の結果，慢性疼痛の有無は歩行速度低下と有意に関連していたが（調整後オッズ比2.55，p = 0.001），急性疼痛との関連はなかった（調整後オッズ比1.19，p = 0.632）（年齢，性別，GDS-15，内服薬数，ASMI，握力を共変量）。急性・慢性疼痛の有無と握力低下との関連はなかった（急性疼痛有無：調整後オッズ比1.22，p = 0.477．慢性疼痛有無：調整後オッズ比0.85，p = 0.530）。【結論】地域在住高齢者において，慢性疼痛を有することと歩行速度低下との関連が示唆された。

［目　的］ 地域在住高齢者における社会参加とサルコペニアとの関連を明らかにすることを目的とした。 ［方　法］ 地域コホート研究（垂水研究2018）に参加した65歳以上の地域在住高齢者859 名のうち，脳卒中等の既往歴のない751名を分析対象とした。Asian Working Group for Sarcopenia 2019に基づいてサルコペニアの有無，またJST版活動能力指標の社会参加に関する4項目より社会参加の状況を評価した。 ［結　果］ サルコペニアを有する者では，社会参加得点が有意に低値であった（p＜0.001）。従属変数をサルコペニアの有無，独立変数を社会参加得点としたロジスティック回帰分析の結果，社会参加とサルコペニアに有意な関連を認めた（オッズ比0.83，95％信頼区間0.71-0.97）（共変量：年齢，性別，教育歴，服薬数，転倒歴，運動習慣，うつ傾向，認知機能）。 ［結　論］ 地域在住高齢者の社会参加状況はサルコペニアと関連することが示唆された。

我が国では，さらなる高齢化により，心不全パンデミック等高齢者救急の増加が懸念されている．フレイル・サルコペニア・認知機能障害といった高齢者特有の身体的・精神的変化と共に，独居・老々介護といった社会的な問題を包括した治療戦略を考える必要がある．また，今後の高齢化を考えると，救急医療体制の確立も重要であるが，徹底した高血圧治療や健診受診・受療勧奨を含めた予防の徹底が重要である．
