(1) Background: The present study aimed to examine physical, cognitive and emotional factors affecting falls in community-dwelling older adults with and without pain; (2) Methods: Data from 789 older adults who participated in a community-based health survey were analyzed. Participants completed questionnaires on the presence of pain and previous falls. Muscle weakness (handgrip strength < 26.0 kg for men and < 18.0 kg for women) and low skeletal muscle mass (appendicular skeletal muscle mass index < 7.0 kg/m2 for men and < 5.7 kg/m2 for women) were determined. Mild cognitive impairment (MCI) and depressive symptoms were assessed using the National Center for Geriatrics and Gerontology-Functional Assessment Tool and 15-item geriatric depression scale (GDS-15), respectively; (3) Results: In participants with pain, MCI and GDS-15 were associated with previous falls after adjusting for age, sex, education and medication use. In participants without pain, muscle weakness and low skeletal muscle mass were associated with previous falls when adjusting for the above covariates; (4) Conclusions: Falls in participants with pain were associated with cognitive and emotional factors, whereas falls in those without pain were associated with physical factors. Fall prevention interventions for older adults with pain may require tailored strategies to address cognitive and emotional factors.

診断時のGeriatric Nutritional Risk Index(GNRI)スコアから、肺動脈性肺高血圧症(PAH)または慢性血栓塞栓性肺高血圧症(CTEPH)の患者の予後を予測できるか調査した。対象患者は104例(平均60±14歳、女性84.6%)で、71例(68.3%)がWHO機能クラスIIIまたはIVであった。追跡調査期間中(中央値24ヵ月)、16例(15.4%)が死亡した。6例(37.5%)の死因は肺高血圧症(PH)と関連しており、残り10例(62.5%)の主な死因は感染症(3例)、呼吸器疾患(4例)、がん(2例)、その他(1例)であった。PH再入院または全死因の複合転帰は、低GNRI群の22例中10例(45.5%)および高GNRI群の82例中17例(20.7%)で発生した。PH再入院の発生率は、高GNRI群の方が低GNRI群よりも高かった。カプラン・マイヤー分析では、高GNRI群は低GNRI群よりも累積イベント無発生率が有意に高かった。多変量Cox比例ハザードモデルでは、低GNRIが高GNRIよりも高い全死因死亡率またはPH再入院率と有意に関連していた。以上の結果より、栄養失調はPH患者の予後予測因子であることが示された。

夜間頻尿・多尿は畜尿障害だけではなく、夜間尿産生が亢進することによって発症することもある。尿産生にはナトリウム(Na)利尿と水利尿があり、代表的な疾患として各々高血圧と心不全が考えられる。食塩感受性高血圧で塩分摂取過多となると、昼間だけではNaを排泄することができず、夜間に排泄をせざるを得なくなる。そのため、夜間高血圧によるNa排泄のため尿産生に導かれ夜間多尿を呈する。一方で、心不全では夜間臥位により静脈還流が上昇するため、水利尿をして心臓への負担軽減を促すような適応現象が起こり、夜間尿産生亢進による夜間多尿を呈する。前者はサイアザイド系利尿薬でNa排泄を促し、後者はループ利尿薬で水分排泄を促すことが、夜間頻尿・多尿の治療となりうると考えられる。(著者抄録)

Aim: To compare the understanding of hypertension and antihypertensive treatment in Japanese patients (aged <75 years vs. ≥75 years) with blood pressure (BP) targets as per the 2014 Japanese guidelines. Methods: A 10-question survey was administered before and after treatment. Results: Majority of patients aged ≥75 years did not achieve their BP targets (75%); >50% of these patients had little knowledge of hypertension and poor understanding of their physician’s explanation of it. Conclusions: Elderly patients with hypertension (aged ≥75 years) require daily BP monitoring and detailed and repeated explanation of hypertension and BP targets.

Intima-media thickness (IMT), plaque score (PS), and brachial-ankle pulse wave velocity (baPWV) are noninvasive parameters used for the evaluation of atherosclerosis. There have been no reports investigating the associations between IMT, PS, or baPWV and known risk factors for atherosclerosis in the same participants. This study aimed to compare the contributing risk factors among IMT, PS, and baPWV. We enrolled 3128 male participants who underwent health checkups. Carotid ultrasonography was used to measure IMT and PS, and baPWV was measured using an automatic device. We analyzed the association between these parameters and risk factors for atherosclerosis including obesity, smoking, alcohol, high blood pressure (BP), impaired fasting glucose (IFG), dyslipidemia (DL), and high-sensitivity C-reactive protein (hs-CRP). Multivariate logistic regression analysis revealed high IMT (>0.9 mm) to be independently associated with high BP. The presence of atherosclerosis (PS ≥ 1.1 mm) was independently associated with high BP, IFG, and DL. In addition, advanced atherosclerosis (PS ≥ 5.1 mm) showed a significant association with high BP, IFG, DL, log hs-CRP, and smoking. High baPWV (≥1600 cm/s) was significantly correlated with high BP, IFG, and log hs-CRP. IMT, PS, and baPWV were all associated with different risk factors among male Japanese participants. Only high BP was a common risk factor for all three parameters. Furthermore, hs-CRP was associated with advanced atherosclerosis but not with the presence of atherosclerosis.
