症例は48歳の女性。201X年2月に胸痛のため近医を受診し、急性前壁中隔心筋梗塞の診断で入院となった。第8病日に胸痛が再度出現し、心電図にてII、III、aVFでST上昇を認め、急性下壁心筋梗塞の疑いで当院へ緊急搬送となった。冠動脈造影(coronary angiography;CAG)では左前下行枝(left anterior descending artery;LAD)#8および右冠動脈(right coronary artery;RCA)#3の完全閉塞を認め、責任病変であるRCA#3に対して経皮的冠動脈インターベンション(percutaneous coronary intervention;PCI)を施行した。血栓吸引を施行し、明らかな血栓は回収できなかったが、再灌流を得ることができた。その際の血管内超音波(intravascular ultrasound;IVUS)の所見で冠動脈解離の所見を認めたが、造影上はTIMI flow grade 3が得られたため、手技を終了した。急性期PCIより2週間後に施行したCAGではLAD#8は再灌流しており、同部位に冠動脈解離を疑わせる陰影欠損を認めた。RCA#3は造影上、有意狭窄や解離所見を認めなかった。初回は特発性冠動脈解離による急性前壁中隔心筋梗塞であり、2回目も同様の機序による急性下壁心筋梗塞が考えられた。短期間に2回の特発性冠動脈解離による心筋梗塞を発症した若年女性の1例を経験したので、報告する。(著者抄録)

Objective An increased red cell distribution width (RDW) has been reported to be associated with adverse outcomes in patients with heart failure (HF). This study aimed to evaluate the prognostic power of the combined measurement of RDW and B-type natriuretic peptide (BNP) concentrations in patients with HF. Methods and Results We retrospectively studied 116 patients (mean age, 63.7±14.3 years) who were admitted for the treatment of HF. Data including demographic information, vital signs, and laboratory and echocardiographic measurements at admission were collected from medical records. The observational period was defined as the number of days from hospitalization, and the study endpoint was defined as cardiovascular death. The mean RDW and BNP concentration at admission were 14.5±2.0% and 626±593 pg/mL, respectively. During a median observation period of 1,046 days, 22 patients died of cardiovascular disease. A univariate Cox proportional hazard analysis revealed that both RDW [hazard ratio (HR) 1.252, p = 0.0391] and BNP (HR 1.001, p = 0.0445) were significant prognostic indices for cardiovascular death. A receiver operating characteristic curve analysis revealed that the optimal cut-off RDW and BNP values for cardiovascular death were 14.9% and 686 pg/mL, respectively. The Kaplan-Meier survival curve revealed that the survival rate of patients with both RDW ≥ 14.9% and BNP ≥ 686 pg/mL showed the poorest prognosis in comparison to the patients in the other groups. Conclusion The combined assessment of the RDW and BNP concentrations may be useful for predicting mortality in patients with HF.

心不全患者116例(男性80例、女性36例、平均63.7±14.3歳)を対象に、赤血球容積粒度分布幅(RDW)とB型ナトリウム利尿ペプチド(BNP)の複合評価の有用性について検討した。RDWとBNPを組み合わせることで心血管死亡率を予測できるか評価した。追跡期間中央値1046日における死亡は24例で、このうち22例が心血管死であった(心不全15例、突然死5例、大動脈瘤破裂1例、脳卒中1例)。入院時の平均RDWは14.5±2.0%、平均BNPは626±593pg/mLであった。心血管死の予測因子について検討したCox比例ハザード分析では、年齢、慢性心不全の病歴、NYHA心機能分類、RDW、BNP、早期僧帽弁血流速度波形/早期僧帽弁輪血流速度比が抽出された。年齢補正後の二変量解析ではRDWとBNPの両者が心血管死亡率の有意な予測因子であることが示され、RDW 14.9%におけるAUCは0.59、感度は50.0%、特異度は73.1%、BNP 686pg/mLにおけるAUCは0.58、感度は45.5%、特異度は75.3%であった。また、Kaplan-Meier生存曲線では、RDW 14.9%以上かつBNP 686pg/mL以上の場合、死亡率が最も高くなっていた。心不全患者において、RDWとBNPの測定値評価の組み合わせが死亡率の予測に有用であると思われた。

特別養護老人ホームに入所している高齢者119名(男性26名、女性93名、平均86.7±7.8歳)を対象に、口腔機能としての咬合の不安定性と高血圧症、糖尿病、認知症等の併存症との関連性について検討した。咬合支持の評価にはEichner indexを用いて、歯科医師が咬合の安定性、歯数、義歯の使用を評価した。包括的な高齢者評価には高齢者機能評価簡易版(CGA7)を使用した。平均の歯数は0本(範囲0～4本)であった。被験者を咬合機能に応じて咬合安定群78名(66%)と不安定群41名(34%)の2群に分類した。高血圧症の罹患率は咬合安定群(83%)の方が不安定群(63%)より有意に高かったが(P=0.0149)、糖尿病の罹患率は咬合安定群(10%)の方が不安定群(27%)より有意に低かった(P=0.0190)。CGA7における手段的日常生活動作は咬合安定群(54%)の方が不安定群(24%)より有意に高値であった(P=0.0021)。多重ロジスティック回帰分析で咬合の不安定性は高血圧症と糖尿病の罹患率および手段的日常生活動作の低下と有意に相関した。

Endothelial dysfunction is observed in several cardiovascular diseases, where endothelium-dependent vasodilation is impaired by oxidative stress. However, the time course of endothelial function during the perioperative period of a minor-to-moderate surgery, and the effects of atherosclerotic risk factors and employed general anesthetics on recovery of endothelial function, are unknown. Endothelial function of 30 patients was evaluated as the reactive hyperemia index (RHI) of reactive hyperemia peripheral arterial tonometry. RHI was measured on day before surgery (control), immediately after surgery (Day 0), day after surgery (Day 1), and day 4 after surgery (Day 4) in patients with no functional limitations who were scheduled for oral and maxillofacial surgery of around 3 hours. Sevoflurane- or propofol-based anesthesia supplemented with an opioid analgesic remifentanil was employed. The control RHI was 2.26 ± 0.64. The RHI significantly decreased to the lowest level on Day 0 (1.52 ± 0.28), recovered on Day 1 (2.07 ± 0.58), and improved further on Day 4 (2.55 ± 0.83). Multiple linear regression analysis revealed that recovery of the RHI from Day 0 to Day 4 was impaired by diabetes mellitus (P = 0.0313), obesity (BMI ≥ 25; P = 0.0166), hyperuricemia (uric acid ≥ 6.0 mg/dL; P = 0.0416) and sevoflurane-based anesthesia (P = 0.0308). These findings suggest that endothelial function as evaluated by the RHI is severely suppressed on the day of a minor-to-moderate surgery, and that it improves until the 4th postoperative day on average. Recovery of endothelial function is impaired by diabetes mellitus, obesity, hyperuricemia, and sevoflurane-based anesthesia.

小～中等度手術の周術期における血管内皮機能の時間経過と、アテローム動脈硬化症の危険因子および全身麻酔が血管内皮機能の回復に及ぼす影響を調べた。3時間程度の口腔および顎顔面手術を行い、レミフェンタニルを追加したセボフルランまたはプロポフォールベースの麻酔を行った18歳以上の患者を対象とした。血管内皮機能は反応性充血末梢動脈圧力測定の反応性充血指標(RHI)によって手術前日、手術直後、1、4日後に評価した。30例(男性13例、女性17例、平均48.3±18.4歳)のデータ解析を行った。平均RHIは手術前日が2.26±0.64で、手術直後に1.52±0.28と最も低く、術後1日では2.07±0.58と回復し、4日目には更に改善して2.55±0.83となった。重回帰分析により、RHIの術後の改善は糖尿病、肥満、高尿酸血症、セボフルランベースの麻酔によって障害されることが示された。
