目的：フレイルの予防・改善を目的とした運動介入において個別運動または集団運動といった運動の実施形態の違いは異なった効果をもたらすことが考えられる．本研究は地域在住のフレイルおよびプレフレイル高齢者を対象に運動介入形態の違い（個別または集団）が心身機能に及ぼす影響を明らかにすることを目的とした．方法：フレイルまたはプレフレイルを有する高齢者21名（平均77.1±3.3歳，女性90.5％）を個別群11名と集団群10名にランダムに割りつけ，週1回90分の運動を12週間実施した．個別群はトレーニングマシンを使用して行う筋力トレーニングを含むマルチコンポーネント運動プログラム，集団群は椅子座位中心，自重での筋力トレーニングを含むマルチコンポーネント運動プログラムを実施した．評価項目は身体組成，身体機能，認知機能とした．介入前後の比較および，群間における変化率の比較を行った．結果：介入前後の比較において個別群でBMI（p＝0.03），体脂肪量（p＜0.01）で有意な減少が認められ，5回立ち座り（p＜0.01），歩行速度（p＝0.02）が有意に改善した．集団群では，5回立ち座り（p＜0.01），情報処理機能（p＝0.03）で有意な改善が認められた．群間における変化率の比較では5回立ち座りにおいて個別群は集団群と比較して有意な改善が認められた（p＝0.01）．結論：運動の実施形態に関わらず，身体機能の改善が示され，個別群のみでBMI，体脂肪量の減少，集団群のみで認知機能の改善がみられた．フレイルおよびプレフレイル高齢者における運動介入において運動の実施形態による効果の特性を考慮することは，個人の背景に則した運動プログラム作成の一助になりうる．

Aim: To investigate the association between ACTN3 R577X polymorphism and appendicular skeletal muscle mass, walking speed, and muscle strength in older adults. Findings: RR homozygosity in the ACTN3 gene was associated with a lower likelihood of having low muscle mass. However, no significant association existed between ACTN3 polymorphism and walking speed or muscle strength. Message: ACTN3 R577X polymorphism may be associated with muscle mass but not physical function in terms of walking speed and muscle strength in older adults.

Objectives: The intent of the present study was to elucidate the association of eating alone and depressive symptoms with the development of disability among community-dwelling older adults, using a longitudinal study. Method: Participants included 4648 Japanese older adults (mean age 73.8 ± 5.4 years; 44.3% men) aged ≥ 65 years at the time of the examination. Eating status was divided into two categories: “Eating with others at least once a day” and “Other.” The 15-item Geriatric Depression Scale was used to measure depressive symptoms. Incident disability was certified by long-term care insurance (median duration: 36 months). Results: During a median follow-up at 36 months, 8.0% of the participants developed an incident disability. Adjusted for covariates, the participants who ate alone were associated with a higher hazard ratio of incident disability compared to those who ate with others (hazard ratio: 1.36, 95% confidence interval: 1.05–1.75). However, adjusted for the covariate depressive symptoms, eating alone was not significantly associated with incident disability. Structural equation models revealed that the indirect model confirmed eating alone habits were associated with disability via depressive symptoms. Conclusions: This study confirmed that eating alone was associated with an incident disability after adjusting for the covariates. Furthermore, the present study suggests an indirect relationship between eating alone and incident disability via depressive symptoms, the result of the structural equation model.

【目的】本研究の目的は、胃癌または大腸癌を有する患者の術前のPossible Sarcopenia(PS)と握力の低下、5回椅子起立時間の延長と術後の合併症の発生および在院日数との関連を検討することとした。【方法】対象は、胃癌または大腸癌の術後に自宅退院した患者75例とした。Asian Working Group for Sarco-penia 2019に準じて術前のPS、握力の低下、5回椅子起立時間の延長を判定し、術後の合併症の発生と在院日数との関連を調査した。【結果】術前のPSは、術後の合併症の発生と在院日数の長期化との関連が認められた。握力の低下、5回椅子起立時間の延長は、術後の合併症の発生と在院日数の長期化との関連が認められなかった。【結論】術前のPSは、胃癌または大腸癌を有する患者の術後の合併症の発生と在院日数の長期化との関連が示唆された。(著者抄録)

This study aimed to determine the effects of walking-only intervention (walking was the only exercise in which people participated) on physical function, fall-related outcomes, and health-related quality of life in community-dwelling older adults. We conducted a systematic search across five electronic databases, assessing risk of bias using Minds Manual for Guideline Development. Meta-analyses were performed, and pooled standardized mean differences were calculated. Nine studies (a total of 1,309 participants) were included, showing that walking-only interventions improved walking endurance (standardized mean difference: 1.11, 95% confidence interval: [0.08, 2.15]) and health-related quality of life (standardized mean difference: 0.71, 95% confidence interval: [0.18, 1.25]). However, there were no significant improvements in other outcomes. The certainty of the evidence based on the Grading of Recommendations, Assessment, Development, and Evaluation approach for all outcomes was graded as very low, primarily due to significant inconsistency and imprecision. Our results suggest that walking-only intervention can be effective for enhancing walking endurance and health-related quality of life for community-dwelling older adults. Further studies are required to investigate the effects of walking-only intervention. This need stems from the limited number of randomized controlled trials, heterogeneous intervention settings and results, and the very low certainty of the evidence.

日本は急速な超高齢社会を迎え,平均寿命の著しい伸びとともに,高齢者の生活機能も大きく変容し,健康水準の高い活力ある高齢者が増加している.本研究では,我が国の地域在宅高齢者を対象とした老化に関する多数のコホート研究の統合研究(「長寿コホートの統合的研究;Integrated Longitudinal Study on Aging in Japan:ILSA-J」)で収集された高次生活機能評価指標であるJST版活動能力指標(JST-IC)を用いて,現代の地域在宅高齢者における生活活動能力の現状を報告した.JST-ICでは総合スコアおよび4項目の下位尺度(新機器利用,情報収集,生活マネジメント,社会参加)について性別および年齢階級別の平均値と標準偏差を算出した.その結果,男女間では情報収集以外の全ての項目で性差が認められ,年齢階級間では男女ともに全ての項目で有意差が認められ,加齢に伴いスコアは低下していた.本研究では,高齢者の高次生活機能の一端を明らかにし,今後の利活用に資する資料とした.(著者抄録)

Background: This study aimed to investigate the potential association between the combination of low physical activity and low dietary diversity with mild cognitive impairment (MCI) in older Japanese adults. Methods: Data from 600 older adults (mean age, 74.1±6.4 years; women, 62.0%) were analyzed. We evaluated dietary variety based on the Food Frequency Score (FFS; maximum 30 points) by assessing the 1-week consumption frequencies of ten foods. An FFS of ≤16 indicated low dietary diversity. We assessed MCI using the National Center for Geriatrics and Gerontology Functional Assessment Tool. Physical activity levels was determined based on participant responses to two questions: “Do you engage in moderate levels of physical exercise or sports aimed at health?” and “Do you engage in low levels of physical exercise aimed at health?” Participants who responded “No” to both questions were classified as having low physical activity levels. We classified the participants into robust, low-dietary diversity, low-physical activity, and coexistence groups. Results: The overall prevalence of MCI was 20.7%, with rates in the robust, low dietary diversity, low physical activity, and coexistence groups of 17.7%, 24.7%, 25.0%, and 41.9%, respectively. Multiple logistic regression analysis revealed that low dietary diversity and physical activity were associated with MCI in older adults (odds ratio=2.80, 95% confidence interval 1.22–6.28). Conclusion: The results of the present study demonstrated the association of the co-occurrence of low dietary diversity and low physical activity with MCI. Older adults with both risk factors may require early detection, as well as physical activity and dietary interventions.

Background: The use of transport other than cars is a modifiable factor in the association between driving cessation and social frailty. Clarifying this relationship may serve as a new preventive measure against social frailty among current non-drivers. This study examined the potential association of driving status and transport use with social frailty, as well as between the frequency of transport use and social frailty, among current non-drivers. Methods: This study included 977 middle-aged and older adults (average age, 65.3±4.8 years). The participants were classified as transport users (more than a few times a week) and transport non-users (less than a few times a month). Based on driving status and transport use, the groups were further classified into current driver, current non-driver/transport user, and current non-driver/transport non-user groups. The relationships between driving status, transport use, and social frailty were examined using multiple logistic regression. Results: The current non-driver group and the transport non-user group were significant association with a higher social frailty. The current non-driver/transport user group showed no association with social frailty compared with the current driver group. The odds ratio for the social frailty rate for The current non-driver/transport non-user group was 2.14 (95% confidence interval, 1.25–3.73). Conclusions: Participants who neither drive nor take transport showed significant associations with increased social frailty. Compared with current driver/ transport use, current non-driver/transport non-use was associated with social frailty.

Background: Physical inactivity is a risk factor for locomotive syndromes and functional limitations in middle-aged and older adults. Therefore, strategies to promote physical activity should be considered. This study investigated whether users of physical activity monitors were more physically active and exhibited a lower risk of locomotive syndrome, compared with non-users. Methods: We analyzed data from 742 Japanese adults aged 60–79 years. The participants were surveyed for their use of physical activity monitors in their daily lives. We also assessed the prevalence of locomotive syndrome. Results: We observed significantly higher physical activity levels in users compared with non-users. Moreover, the use of a physical activity monitor was significantly associated with decreased odds of locomotive syndrome (adjusted odds ratio [aOR]=0.48). Significantly lower risk of locomotive syndrome were observed in individuals who had used a monitor for >2 years (aOR=0.42) or had set a personal step goal (aOR=0.32). Conclusion: Physical activity monitoring may increase engagement in physical activity-associated behavior. There-fore, monitoring could serve as a useful tool to promote physical health in middle-aged and older adults.

Objectives: Frailty is a significant cause of adverse health events including long-term care and hospitalization. Although information and communication technology (ICT) has become an integral part of modern life, it remains unclear whether ICT use is associated with frailty. Design: A cross-sectional study (Integrated Longitudinal Studies on Aging in Japan, ILSA-J). Setting and participants: Aged 75 and older data from the ILSA-J in 2017 (n = 2893). Methods: ICT use was measured using the technology usage sub-items of the Japan Science and Technology Agency Index of Competence. Specifically, the use of mobile phones, ATMs, DVD players, and sending e-mails were rated as “yes” (able to do) or “no” (unable to do), with the first quintile (≤1 point) defined as ICT non-users. Frailty was assessed using the Japanese version of the Cardiovascular Health Study criteria based on the phenotype model (e.g., weight loss, slowness, weakness, exhaustion, and low activity). Further, multivariate logistic regression analysis analyzed its association with ICT use. Subgroup analyses were stratified according to gender, years of education, and living arrangements. Results: Higher ICT use was not associated with frailty after adjusting for covariates (odds ratio [OR]: 0.53; 95%CI 0.39–0.73). Similar associations were found in the sub-groups of women (OR 0.45, 95%CI 0.30–0.66), <13 years of education (OR 0.48, 95%CI 0.34–0.67), living alone (OR 0.46, 95%CI 0.27–0.79), and living together (OR 0.57, 95%CI 0.38–0.85). No association existed between using ICT and frailty in the sub-groups of men and ≥13 years of education. Conclusions and implications: Higher ICT use is associated with the absence of frailty in individuals 75 years and older. Such benefits may be particularly pronounced in women, those with lower levels of education, and older adults living alone or with others.

Background: Dementia is a critical late-life health issue that occurs among members of aging societies. This study examined the relationships between eating out, dietary diversity, and mild cognitive impairment (MCI) among community-dwelling older adults. Methods: We analyzed data from 597 older adults (median age 73.0 years, interquartile range 69.0–78.0 years; 62.6% females). We applied the food frequency score to evaluate diet variety and the weekly consumption frequencies of ten food items were determined. The National Center for Geriatrics and Gerontology Functional Assessment Tool (NCGG-FAT) was used to evaluate MCI. Finally, we asked the participants how often they ate out each month; those who replied “none” were categorized into the “non-eating out” group. Results: The overall prevalence of MCI was 122 (20.4%), with a higher prevalence in the low dietary diversity group than in the high dietary diversity group (28.6% vs. 18.6%). After adjusting for covariates, the participants who self-described as not eating out were independently associated with low dietary diversity (odds ratio [OR]=1.97, 95% confidence interval [CI] 1.20–3.20), while low dietary diversity was associated with MCI (OR=1.72; 95% CI 1.02–2.87). Structural equation models revealed that not eating out had no direct effect on MCI but was associated with MCI via low dietary diversity (root mean square error of approximation=0.030, goodness-of-fit index=0.999, and adjusted goodness-of-fit index=0.984). Conclusions: Although non-eating out may not have a direct effect on MCI, an indirect relationship may exist between eating-out habits and MCI via dietary diversity status.

Aim: This study aimed to examine the relationships between levels of competence and impaired physical and cognitive functions in older adults. Methods: We used a data set of the Integrated Longitudinal Studies on Aging in Japan for 2017 including 5475 community-dwelling older adults. Levels of competence were assessed using the Japan Science and Technology Agency Index of Competence (JST-IC). Grip strength (low grip strength: <28 kg for men and <18 kg for women) and gait speed (slow gait speed: <1.0 m/s for both sexes) were evaluated as physical function measurements, and the Mini-Mental State Examination (cognitive decline: <24 on the Mini-Mental State Examination) was used to assess cognitive function. Results: The JST-IC had areas under the curve estimated from receiver operating characteristic analysis ranging from 0.65 to 0.73 for detecting low function as assessed by these tests. Restricted cubic spline curves showed that the shape of the association between the JST-IC and impaired function depended on sex and the test used. The comparison between perfect and imperfect JST-IC scores showed significant differences in the prevalence of low grip strength in both sexes, slow gait speed in women, and cognitive decline in men. Conclusions: It may be insufficient to identify those with impaired physical or cognitive function using the JST-IC. The shape of the association with the JST-IC varies across their measurements. Our findings can help interpret JST-IC scores in the context of low physical and cognitive functions. Geriatr Gerontol Int 2024; 24: 352–358.
