To practice psychosomatic medicine in general practice, we have found that the recognition of psychosomatic medicine is still not sufficient. While our task is to promote enlightenment of psychosomatic medicine, it is required to enhance the psychosomatic medicine on post- and undergraduate education. However, even though we increase the conventional lecture hours, it would end in simple cramming of knowledge and it has been difficult to reach the learning goal of psychosomatic medicine, which includes ethic education, manner education and reliable doctor-patient relationship. Further, as a result of the categorical lectures, it has been difficult to deepen the understanding of psychosomatic medicine containing various factors. In the future, it will be important to develop new the curriculum of psychosomatic medicine based on the outcome by cooperating several courses.

食欲・食行動は個体が生命を維持するために最も重要な感覚・行動の一つであり、外部・内部環境に対する個体の状態・反応の評価に重要である。グレリンは成長ホルモン分泌促進因子受容体の内因性のアゴニストとして発見されたペプチドである。グレリンの発現・分泌は絶食により増加、摂食により減少し、外因性に投与されたグレリンは、食欲を増加させる。その他、成長、腸管運動、呼吸機能、心機能、糖代謝、免疫能、情動、認知機能、生殖など、多彩な作用に関与している。そのため、神経性食欲不振症、機能性胃腸症、がん性悪液質などの疾患に対するグレリンの臨床応用が期待されている。また、近年、漢方薬である六君子湯が、グレリンシグナルを増強させる作用を有していることが注目されている。心身ホルモンとして多彩な作用を有するグレリンは心身医療において、その重要性がさらに増加していくものと予想される。(著者抄録)

内臓知覚過敏には、消化管炎症後の粘膜感作、十二指腸での酸・脂質への知覚過敏の関与が示唆されている。機能性ディスペプシアで、Helicobacter pylori(H.pylori)感染が知覚過敏に関与しているかはいまだ明らかではないが、H.pyloriは慢性炎症、胃酸分泌異常を引き起こし、またH.pyloriの除菌により症状が改善する報告がみられることから、病態への関与が注目されている。H.pylori感染では、宿主側の因子として炎症性サイトカインを介した抗体反応、リンパ球の応答、菌側の因子としてH.pyloriが産生するvacuolating toxin A(VacA)とcytotoxin-associated gene A(CagA)が炎症免疫応答反応に関係している。H.pylori感染と知覚過敏の関連について、臨床から分子レベルまでのトランスレーショナルなメカニズムの解明が望まれる。(著者抄録)

悪液質(cachexia)やサルコペニアの病態では、たんぱく質の同化障害と異化亢進を特徴とする代謝障害が生じている。代謝障害はテストステロン、インスリン様成長因子1(IGF-1)、成長ホルモンなどの同化ホルモンの低下により生じるが、悪液質においてはサイトカインがレプチン様作用を起こして食欲不振や代謝障害を惹起させる特徴がある。グレリンやレプチンなどの食欲調節ペプチドは、食欲やエネルギー代謝のみならず、筋代謝にも作用していることも明らかになり、悪液質およびサルコペニアの病態解明には、脳-消化管-脂肪-筋の連関に目を向けることが重要である。(著者抄録)

Appetite and feeding behavior are the most important sense and behavior for the maintenance of one's life. Ghrelin is a peptide that has been discovered as an endogenous agonist of the promotion factor receptor for growth hormone secretion. Manifestation and secretion of ghrelin increase by fasting and decrease by ingestion, and the exogenously administered ghrelin increases appetite. In addition, it is involved in various actions such as growth, bowel motility, respiratory function, cardiac activity, carbohydrate metabolism, immunocompetence, affection, cognitive function and generation. Moreover, clinical applications of ghrelin to diseases such as anorexia nervosa, functional gastrointestinal symptoms and cancer cachexia are anticipated. In recent years, the fact that Rikkunshito has an effect to enhance ghrelin signal attracts researchers' attention. Ghrelin, which has various effects as physical and mental hormone, will rise its significance in psychosomatic medicine.

目的:摂食障害患者は、未治療のまま長期経過する例が多く、やせが遷延するとさまざまな合併症が生じうる。合併症はときに重篤であり注意が必要である。方法:当科において2001〜2010年に入院加療を受けた摂食障害患者を分析し、合併症のリスクファクターについて検討する。結果:当科において、30代の患者数が増加しており、未治療の長期経過例や遷延例が増加している可能性が示唆される。病型は、神経性食欲不振症(制限型)が最も多く、神経性大食症(非排出型)は有意に増加傾向を示した。合併症は、白血球減少症や軽度肝機能障害の合併率が高く、イレウス、播種性血管内凝固症候群など重篤な合併症をきたした例も認めた。BMIが低値になると、合併症の発症頻度が増加した。また、重篤な合併症は、治療初期に発症し、BMI<13の高度なやせを呈し病前体重より高度な体重減少を認めた症例で生じていた。結論:早期発見および早期治療の導入が合併症リスクを減らすために、最も重要である。(著者抄録)
