Background: Smartphones have become an integral part of our daily lives. Although many scales can assess smartphone usage, they rely on respondents' subjective self-reports and suffer from considerable cognitive bias. Therefore, quantitative measurement of smartphone's recorded screen time is an effective way to assess smartphone usage. Objective: This study assesses the effects of age, gender, and subjective psychological factors (depression, social anxiety, sleep quality, stress perseverance, and loneliness) on smartphone usage. Based on previous research, we hypothesized that there would be (1) gender-, (2) age-, and (3) psychological state-specific differences in smartphone usage. Methods: We conducted psychological tests, obtained participants’ weekly screen times, and performed ordinary least squares (OLS) regression and k-means clustering. Results: Twenty-four female participants and 25 male participants were analyzed. Only the subjective loneliness indicated statistical significance between genders. OLS regression analysis indicated that among females, age showed a negative coefficient (age, −76.5), and both the social anxiety (LSAS-J, 20.0) and loneliness (UCLA-LS, −44.5) had significant coefficients. In contrast, among males, age had no significant coefficients, and only the depression (BDI-III, 77.7) showed a significant positive relationship. Conclusions: Our findings suggest that smartphone users' motivations and practicalities differ by gender and age group. To support these findings, further studies with larger sample sizes and more variables should be conducted.

今回,罹病期間が長期に及び精神神経症状を伴った顎関節症患者に対し,柴胡桂枝湯を投与したところ著効した症例を経験した。患者は,79歳女性で,10年前より両顎関節周囲の疼痛があり,顎関節症と診断された。消炎鎮痛剤投与やオーラルアプライアンス治療を受けていたが症状は改善しなかった。増悪因子はストレスであり心理社会的因子の関与が示唆されたが,精神療法を受けることは希望されず,漢方内科を受診した。胸脇苦満と両側上腹部の腹直筋の緊張,心下支結(中かんの圧痛)といった腹候に着眼して柴胡桂枝湯を処方した。柴胡桂枝湯を5.0g/日,2週間の投与であったが顎関節症の症状は速やかに改善し,精神症状の改善も得られた。本症例のように心理社会的因子の関与が強い顎関節の治療には漢方治療が有用な選択肢となりうるし,随証治療を行うことで,速やかに症状を改善できる可能性がある。(著者抄録)

Adropin is a 43-amino acid peptide that is highly conserved among mammals. First identified in the mouse liver in 2008, adropin is broadly expressed throughout the body and has been implicated in various pathological conditions, including obesity, altered food intake, insulin resistance, glucose intolerance, and other disorders related to energy metabolism. However, its precise physiological role remains unclear. In this study, we developed a specific competitive enzyme-linked immunosorbent assay (ELISA) for adropin using an in-house generated anti-adropin polyclonal antibody. We then examined plasma adropin levels in mice under different energy metabolic conditions: fed a normal diet, subjected to short-term fasting, and fed a long-term high-fat diet. In addition, we assessed hepatic and hypothalamic expression of Enho mRNA, which encodes adropin. We observed a wide range of plasma adropin levels, spanning from those in normal healthy mice on a high-fat diet to preclinical and obese diabetic mice. Both plasma adropin concentrations and hepatic Enho mRNA expression increased in response to feeding and high-fat diet intake. Multiple regression analysis revealed a significant negative correlation between plasma adropin and plasma glucagon concentrations. These findings suggest that adropin secretion is modulated by the peripheral hormone glucagon and may contribute to the maintenance of energy metabolic homeostasis. In conclusion, the ELISA developed in this study provides a useful and reliable tool for investigating the mechanisms underlying energy metabolism.

Omega-3 (ω-3) polyunsaturated fatty acids in fish oil have been shown to prevent diet-induced obesity in lean mice and to promote heat production in adipose tissue. However, the effects of fish oil on obese animals remain unclear. This study investigated the effects of fish oil in obese mice. C57BL/6J mice were fed a lard-based high-fat diet (LD) for 8 weeks and then assigned to either a fish oil-based high-fat diet (FOD) or continued the LD for additional 8 weeks. A control group was fed a standard diet for 16 weeks. Mice fed the FOD showed weight loss, reduced adipose tissue mass, and lower plasma insulin and leptin levels compared to those fed the LD. Rectal temperatures were higher in the FOD and LD groups compared to the control group. Energy intake was lower in the FOD group than the LD group but similar to the control group. The FOD and LD groups exhibited increased expression of heat-producing genes such as Ppargc1a, Ucp1, Adrb3, and Ppara in brown adipose tissue but not in white adipose tissue. The FOD reduced food consumption and increased rectal temperature and heat-producing genes in brown adipose tissue. Fish oil may therefore be a potential therapeutic approach to obesity.

Cancer cachexia, which encompasses a range of sickness behaviors such as anorexia, reduced locomotor activity, and depression, diminishes quality of life and increases mortality in cancer patients. Rubiscolin-6 is a hexapeptide derived from ribulose-1,5-bisphosphate carboxylase/oxygenase (Rubisco), a major plant leaf protein. Previous reports have demonstrated that rubiscolin-6 functions as an agonist of the δ opioid receptor, inducing diverse effects such as orexigenic, antinociceptive, and antidepressant-like effects. In this study, we investigated the effects of rubiscolin-6 on sickness behavior in colon26-bearing cachexia model mice. Intraperitoneal administration of rubiscolin-6 enhanced food intake in cachexic mice with chronic anorexia. It also increased locomotor activity, which decreased in cachexic mice, as assessed by the open field test. Moreover, the tail suspension test revealed that rubiscolin-6 alleviated depression-like behavior in mice with cachexia. These findings imply that rubiscolin-6 could be used to ameliorate the sickness behavior associated with cancer cachexia.

Major advances have been made in cancer treatment, but the prognosis for elderly cancer patients with sarcopenia and frailty remains poor. Myokines, which are thought to exert preventive effects against sarcopenia, have been reported to be associated with the prognosis of various cancers, but their effect on head and neck squamous cell carcinoma (HNSCC) is unknown. The aim of this study was to clarify the influence of exercise on the control of HNSCC and to examine the underlying mechanism involved. Mice were injected with HSC-3-M3 cells, a human cell line of highly metastatic and poorly differentiated tongue cancer, at the beginning of the study. Just prior to transplantation, blood was collected from the mice, and the levels of myokines were measured by ELISA. Oncostatin M (OSM), a selected myokine, was added to HSC-3-M3 cells, after which the cell proliferation ability, cell cycle, and protein expression were analyzed in vitro. Tumor cell viability was lower (control: 100%, exercise: 75%), tumors were smaller (control: 26.2 mm3, exercise: 6.4 mm3), and survival was longer in the exercise group than in the control group in vivo. OSM inhibited HSC-3-M3 cell proliferation in a concentration-dependent manner in vitro. The addition of OSM increased the proportion of cells in the G0/G1 phase, decreased the proportion of cells in the G2/M phase, and increased the expression of the CDK inhibitors p21 and p27. These results indicate that exercise may directly inhibit the proliferation of HNSCC cell lines via OSM.

Introduction: School refusal is one of the serious problems with children’s mental health, and various studies have examined its prevalence and factors among students. Although many studies suggested that anxiety and depression are deeply associated with school refusal, there is little agreement as to effective interventions. The purpose of this paper is to evaluate the efficacy and safety of mindfulness yoga intervention in children with school refusal. Method: This study is a multicenter, exploratory, open cluster-randomized controlled trial. 43 participants aged 10–15 years with school refusal were randomly assigned to a non-yoga group with treatment as usual (TAU) which includes cognitive behavioral therapy based on self-monitoring, or a yoga group (4-week mindfulness yoga program provided by video sessions + TAU). The primary outcome was symptoms of anxiety evaluated by Spence Children’s Anxiety Scale-Children (SCAS-C). Participants were assessed in four time periods: a 2-week baseline (Day −14), a baseline (Day 1), a post-test after 4 weeks of treatment (Day 29), and an 8-week follow-up (Day 85). Statistical analysis was conducted by a linear mixed effect model using SAS version 9.4. Results: 43 participants were included in the Full-analysis set (FAS) (21 in the mindfulness yoga group and 22 in the non-yoga group). The estimates of SCAS-C at post-test adjusted for baseline values in each treatment group were 39.9 in the mindfulness yoga group and 39.4 in the non-yoga group. The between-group difference for the estimates was 0.4 (80%CI −4.8 to −5.6, p = 0.54), which indicated mindfulness yoga program has no significant effect on anxiety compared with TAU. However, on an exploratory analysis of the subscale of SCAS-C, significant improvement was observed on the Physical Injury Fears subscale. The pulse rate was significantly lower in the yoga group compared to the non-yoga group. Conclusion: This study indicated the safety of a mindfulness yoga intervention for children with school refusal, but the effectiveness of the intervention for anxiety was limited. Further research is needed to determine the long-term effects of yoga and how it can best be integrated with other therapies.

Rationale: Anorexia nervosa is characterized by an extreme fear of weight gain. Clinicians often prescribe meal replacement shakes if patients are unable or unwilling to consume typical foods. However, these shakes sometimes lack essential micronutrients, such as selenium, which may lead to health risks. Moreover, selenium deficiency induces macrocytic anemia. Herein, we present a case of a patient with anorexia nervosa with macrocytic anemia due to selenium deficiency, which was alleviated by selenium supplementation. Patient concerns: An 18-year-old female was admitted to our hospital. The patient was diagnosed with anorexia nervosa. Ultimately, she was unable to walk independently because of fatigue and electrolyte disturbances. Clinical findings: On admission, the height, weight, and body mass index of the patient were 158.5 cm, 27.1 kg, and 10.8, respectively. Our treatment for anorexia nervosa showed relative effectiveness, and the patient's body weight recovered to 29.2 kg by day 60. However, the mean corpuscular volume increased from day 20, suggesting macrocytic anemia. Diagnoses, interventions, and outcomes: Despite our vitamin B12 and folic acid supplementation interventions, the mean corpuscular volume continued to rise. On day 60, the patient was diagnosed with selenium deficiency, and selenium administration of 100 μg/day was initiated. Outcomes: The macrocytic anemia in the patient was alleviated, and treatment for anorexia nervosa was continued in our hospital. Lessons: To the best of our knowledge, this is the first case of macrocytic anemia induced by selenium deficiency with anorexia nervosa comorbidity, underscoring the importance of selenium supplementation in patients with anorexia nervosa, especially in those with macrocytic anemia.

Background: Anxiety disorder is the most prevalent psychiatric disorder. Benzodiazepines, which are often used for anxiety in patients with anxiety disorder, have various side effects. Lavender, one of the most commonly used essential oils in aromatherapy, has the potential to reduce benzodiazepine use for anxiety disorders. Methods: This study is a multicenter, double-masked, randomized, placebo-controlled clinical trial. The study will recruit patients aged 20–59 years old with generalized anxiety disorder and panic disorder among anxiety disorders. The bottle containing the test solution (lavender aroma essential oil or distilled water) will be given to the patients. Patients will carry the bottles with them in their daily life and use the drops on tissue paper when anxious. The primary endpoint is the number of times anxiolytics used in 28 days. Discussion: If the use of benzodiazepines could be reduced by sniffing lavender aroma, which is inexpensive and safe, it would contribute not only to the risks associated with benzodiazepine use but also to the health care economy and could even be added as a standard treatment. Stress is inevitable in humans and stress changes our physical and mental states. Stress has been studied epidemiologically, biologically, and psychologically. First defined in 1990, emotional intelligence (EI) affects psychological stress management. In contrast, the prefrontal cortex (PFC) is suggested to play a vital role in stress management. Human PFC activity can be inferred from the balance of oxygenated and deoxygenated hemoglobin in cerebral blood flow, which can be measured and calculated using functional near-infrared spectroscopy (fNIRS). An important cognitive activation task to activate the PFC is the verbal fluency task (VFT). Therefore, if the PFC is activated by the VFT and monitored by fNIRS, and the activity correlates with EI, fNIRS can be used to measure EI. In this study, Psychological tests using the self-rating depression scale, state-trait anxiety inventory (STAI), and trait emotional intelligence questionnaire-short form (TEIQue-SF) were conducted to evaluate the correlation with VFT performance. Relative oxygenated and deoxygenated hemoglobin concentrations were measured using an fNIRS device, and their correlation with VFT performance was tested. Spearman correlation coefficient was used to determine correlations. Results were as follows. Although VFT performance did not correlate with the oxygenated hemoglobin concentration ([Oxy-Hb]) changes, [Oxy-Hb] was elevated in all channels. VFT performance was significantly negatively correlated with the Zung self-rating depression scale (ρ = 0.063, P =.759), trait anxiety or anxiety level as a personal characteristic of STAI (ρ = 0.243, P =.232), and state anxiety or anxiety about an event of STAI (ρ = -0.138, P =.500), whereas no correlation was found with the TEIQue-SF (ρ = 0.303, P =.132). Healthy individuals PFC activity is not severely affected by their mental state and cognitive activation successfully activates the PFC, supporting the hypothesis that EI is correlated with frontal cortical activation during the VFT in a nonclinical population. EI may play a vital role in reducing stress associated with depression and anxiety in our social lives. Although we failed to show a statistical correlation between TEIQue-SF and [Oxy-Hb] due to a sample size shortage, our preliminary study was the first to attempt to show the PFC activity of EI through a hemodynamic response. Future research may elucidate the role of EI in reducing psychological stress in social life.

Rationale: Refeeding syndrome (RS) is a fatal condition caused by rapid calorie intake during starvation. Self-neglected fasting in psychiatric disorders is associated with RS. However, overeating resulting from circumventing the clinician's instructions does not have a reportedly high risk of RS. Patient concerns: A 47-year-old undernourished woman with borderline personality disorder was hospitalized for nausea, vomiting, and diarrhea. Clinical findings: She had not eaten much for 10 days and had lost weight (56.5-51.1 kg) over 3 weeks. No abnormalities were indicated on physical examination and imaging examinations. Diagnoses, interventions, and outcomes: Infectious diseases and malignancies were excluded from the differential diagnosis. On the third day of admission, the patient's serum phosphorus level significantly decreased to 0.7 mg/dL, and additional sodium phosphate was administered intravenously. On the fourth day, despite our instructions, the patient was found to be eating nonhospital food from the first day of admission. In conjunction with her history, a final diagnosis of RS was made. After appropriate treatments, the patient was discharged on the 15th day of hospitalization. The patient's nausea, vomiting, and diarrhea were improved. Lessons: When undernourished patients have psychiatric disorders, including borderline personality disorder or schizophrenia, the occurrence of RS should be considered based on the patients' poor adherence to physicians' instructions.

【目的】月経随伴症状は月経中の頭痛や下腹部痛ばかりではない。月経前はイライラや抑うつといった精神症状が、月経中や月経後は腹痛や腰痛といった身体的症状が中心となる。本研究では、月経随伴症状の出現時期で漢方の使い分けが必要かにつき検討した。【方法】対象は、鹿児島大学漢方診療センターで経験した月経中の痛みが主訴の子宮内膜症患者35例と子宮筋腫患者7例である。月経前・月経中・月経後の3群に分け、それぞれの時期に随伴症状の特徴と有効な漢方薬の違いを検討した。【成績】月経痛の程度は40%以上重度であった。両疾患を合わせて月経随伴症状の改善率は、月経前は61.5%(8/13)、月経中は28.6%(12/42)、月経後66.7%(2/3)であった。特に子宮内膜症患者の証は多彩で方剤決定に工夫が必要だった。月経前では頭痛や肩こりイライラなどの症状が多く利水剤・清熱剤・駆お血剤の効果が高く、月経後は補剤をベースとした方剤の効果が高かった。【結論】月経中の症状は多彩な漢方薬の使い分けが必要だが、非月経時期の痛みや症状に関してはある程度パターン化した使い分けが可能かもしれない。今後、症例を重ね検討したい。(著者抄録)

Background: Kampo medicine was introduced into the core curriculum model of medical education in Japan in 2007, and has been taught at all 80 medical universities across the country. However, as there are differences in terms of content and between universities, Kampo medicine education needs to be standardized. Therefore, this study aimed to examine the effects of combining multiple strategies, including lectures, problem-based learning (PBL), and simulator training for the development of a standardized educational curriculum for Kampo medicine. Methods: We offered a lectures-only class (Kampo strategy-lectures [Ks-L]), a class with lectures and decoction (Ks-LD), a class with lectures, decoction, and PBL (Ks-LDP), and a class with lectures, decoction, PBL, and simulator training (Ks-LDPS). We analyzed the data from a 12-item questionnaire conducted by the Faculty Development Committee of Kagoshima University Faculty of Medicine to evaluate the curriculum. Results: Ks-LDPS was rated significantly higher than the other modules in active learning, and overall satisfaction. Conclusions: The results of this study suggest that combining multiple strategies rather than offering classes with lectures promoted a positive attitude to Kampo medicine and deep self-learning among students, thereby improving the educational effects of Kampo medicine education.

老化脳に観察されるミクログリア炎症反応による活性化と、グレリンおよびサーチュイン1(SIRT1)遺伝子との関連について調査した。5週齢の雄性klotho遺伝子欠損マウスを老化モデルマウスとして用い、年齢をマッチさせた野生型マウスとともに、血漿中グレリン濃度を比較した。また、全マウスの海馬から採取したミクログリアは初代培養で調整し、klotho遺伝子欠損マウスおよび野生型マウスの各1群に対しては、グレリン産生を誘発させる六君子湯の経口投与を行った。グレリンが投与されたマウス群は投与後1週間後に、グレリン非投与の野生型マウス群およびklotho遺伝子欠損マウス群と共に、血液と脳組織を全身麻酔下で採取し、定量的に評価した。その結果、klotho遺伝子欠損マウスの血漿中アシルグレリン濃度は野生型マウス群に比べ、有意に低濃度であったが、六君子湯を投与すると血漿中アシルグレリン濃度の上昇が認められた。さらに、klotho遺伝子欠損マウスの海馬ではアメボイド型の活性化ミクログリアが認められ、六君子湯が投与されたklotho遺伝子欠損マウスの海馬では、ラミファイド型の休止ミクログリアも観察され、神経新生の回復が認められた。他にも、六君子湯が投与されたklotho遺伝子欠損マウスでは、海馬と視床下部でSIRT1活性の有意な上昇も観察された。今回の実験結果から、ミクログリア活性化を標的としたグレリン-SIRT1シグナル伝達カスケード活性化に基づく治療的介入が、老化脳の神経炎症や関連する神経疾患の治療に有益となることが示唆された。

マウスのマイクログリア細胞株であるMG6細胞を用いて、白朮の抗炎症作用について検討し、その主要成分であるアトラクチレノリドIIIの効果について評価した。リポポリサッカライド(LPS)の適用によってinterleukin-1β(IL-1β)およびTNF-α mRNA発現の著明な増加が認められた。そのLPSによるIL-1β mRNA発現の増加は、白朮50ng/mLおよび500ng/mLの適用によって、それぞれLPS反応の48%および28%にまで有意に抑制された。また、蒼朮ではIL-1β mRNA発現の増加に対して、50ng/mLの適用では効果がなく、500ng/mLの適用でLPS反応の26%にまで有意な抑制が認められた。さらに、アトラクチレノリドIIIの作用について検討したところ、アトラクチレノリドIII 10μMでは効果は認められなかったが、30μMおよび100μMの適用によって、LPSの反応がそれぞれ82%および40%にまで有意に抑制された。

【目的】チョコレート嚢胞は血の異常をベースとして、月経痛の重症度と冷えの頻度は逆相関になることを報告してきた。子宮腺筋症(A群)、チョコレート嚢胞(C群)と子宮筋腫(M群)の月経痛の詳細を漢方医学的に検討した。【方法】当科で経験したA群28例、C群84例、M群52例である。月経痛の程度を、なし、軽度(鎮痛剤が不要)、中等度(鎮痛剤が必要だが日常生活に支障なし)、重度(鎮痛剤が必要で日常生活に支障が有り)と定義し、三群間で比較した。原則として、治療前と治療後1ヵ月、3ヵ月と6ヵ月後に、問診・切診・腹診の所見を記録した。【成績】各疾患群間で患者背景に差は無かった。月経痛が重度の割合はA:57.1%、C:51.8%、M:36.5%とMが低く、月経痛の程度が月経周期間で異なる割合は、A:0%、C:81.0%、M:5.7%とCで高かった。Aをびまん型(5例)局在型(19例)粘膜下型(4例)に分けると、重度月経痛の割合が20.0%、68.4%、25.0%で、局在型で高かった。Mを漿膜下(5例)筋層内(18例)粘膜下(29例)に分けると、重度月経痛の割合が20.0%、16:7%、51.7%と粘膜下で高く、1〜2ヵ月で急激に月経痛が悪化した例は粘膜下の23例(79.3%)だけにみられた。子宮腺筋症は熱が、チョコレート嚢胞は冷えが、月経痛の重症度と密接に関係していた。筋層内筋腫と粘膜下筋腫では、月経痛が重度な症例に実証が多く、筋層内筋腫で実証の率が高く、粘膜下筋腫では冷えがやや高い傾向がみられた。治療後は、実証が多くなり、気(特に粘膜下筋腫)と血の異常が改善され、冷えが少なくなる特徴があった。【結論】今回の研究で、漢方医学的には良性疾患によりそれぞれの特徴を持ち、西洋学的治療も漢方医学的所見の改善をすることが明らかになった。(著者抄録)

＜文献概要＞摂食行動は,生体のエネルギーの恒常性を維持するために重要な機能であり,末梢からの情報が視床下部(ホメオスタティック調節系)と中脳辺縁系ドーパミン神経系(報酬系)に作用して調節されている.さらに,食文化の発展に伴い,高カロリー食およびスクロースはnatural reward として作用し,それらの食物に対する嗜好性や報酬強化による摂食調節のメカニズムも明らかになりつつある.また,ストレスが視床下部だけでなく,報酬系や情動をつかさどる大脳辺縁系に影響して摂食障害を引き起こす可能性が示唆されている.

悪液質がもたらす栄養不良には、食欲不振が大きく影響している。さらに、炎症性サイトカインに誘発される全身の炎症反応による代謝異常のため、骨格筋分解の亢進、インスリン抵抗性、脂質分解の亢進などの同化障害と異化亢進された状態にある。この代謝障害が高度になり、悪液質のステージが、"前悪液質(precachexia)"、"悪液質(cachexia)"、"不応性悪液質(refractory cachexia)"と進行すると、栄養補給を行っても有効に同化することができず、栄養不良は不可逆的な状態になる。そのため、悪液質のステージを評価し、適切な栄養管理を行うことが重要となる。(著者抄録)

おもに胃で産生されるグレリンは28アミノ酸残基のペプチドであり,空腹信号を胃から脳へ伝達する(脳腸相関).グレリンはグレリン脂肪酸転移酵素(ghrelin O-acyltransferase;GOAT)によりセリンにオクタノイル基(C8:0)が付加されてアシルグレリンとなり食欲亢進活性を示す.アシルグレリンは空腹状態で胃から分泌され,脳内のグレリン受容体(GHS-R1a)に結合し摂食を亢進する.アシルグレリンは恒常性維持のための摂食と快楽的な摂食の両方に関与している.食事誘発性の肥満においては,アシルグレリンレベルは正常なBMI値の人に比べて低く,脳内でグレリン受容体の減少によりアシルグレリンシグナルが低下する.神経性食思不振症の患者では健常人に比べてアシルグレリンレベルは高い.この章では,アシルグレリンの食欲に対する機能や作用機構と肥満や神経性食思不振症でのグレリンシステムの破綻について概説する.(著者抄録)

Cachexia is characterized by anorexia, weakness, weight loss, and muscle wasting. The understandings of the pathological mechanism of anorexia and muscle metabolism in view of the crosstalk between brain, gut, adipose tissue and muscle will open the new way for the management of cachexia. Cytokines-induced systemic inflammatory in cachexia causes anabolic/catabolic imbalance. Cachexia represents a continuum with three stages of clinical relevance : pre-cachexia, cachexia and refractory cachexia. In the refractory cachexia stage, the burden and risks of artificial nutritional support likely outweigh any potential benefit, and therapeutic interventions focus typically on alleviating the suffering associated with cachexia. It is important to appreciate the stage of cachexia and perform appropriate nutritional management.

身体疾患のなかで、その発症や経過に心理社会的背景が密接に関与し、器質的あるいは機能的障害が認められる病態が心身症である。機能性ディスペプシア(FD)も心身症の側面をもち、その病態をとらえるには心理社会的側面に目を向ける必要がある。ストレスは、視床下部-下垂体-副腎皮質系や自律神経系、免疫系を介した脳腸相関により消化器症状を引き起こすことが知られている。また、虐待やトラウマが胃の知覚の異常や運動異常をきたすなど、遺伝や養育環境を含む生育歴やエピジェネティクスも重要な因子であることが報告されてきた。したがって、FDの診療は消化器症状のみに焦点を当てた対症的治療では不十分であり、心理社会的因子など患者の背景を十分に把握し、より上流にある原因に対してアプローチする、全人的かつ個別的医療を行うことが必要になる。(著者抄録)

To practice psychosomatic medicine in general practice, we have found that the recognition of psychosomatic medicine is still not sufficient. While our task is to promote enlightenment of psychosomatic medicine, it is required to enhance the psychosomatic medicine on post- and undergraduate education. However, even though we increase the conventional lecture hours, it would end in simple cramming of knowledge and it has been difficult to reach the learning goal of psychosomatic medicine, which includes ethic education, manner education and reliable doctor-patient relationship. Further, as a result of the categorical lectures, it has been difficult to deepen the understanding of psychosomatic medicine containing various factors. In the future, it will be important to develop new the curriculum of psychosomatic medicine based on the outcome by cooperating several courses.

食欲・食行動は個体が生命を維持するために最も重要な感覚・行動の一つであり、外部・内部環境に対する個体の状態・反応の評価に重要である。グレリンは成長ホルモン分泌促進因子受容体の内因性のアゴニストとして発見されたペプチドである。グレリンの発現・分泌は絶食により増加、摂食により減少し、外因性に投与されたグレリンは、食欲を増加させる。その他、成長、腸管運動、呼吸機能、心機能、糖代謝、免疫能、情動、認知機能、生殖など、多彩な作用に関与している。そのため、神経性食欲不振症、機能性胃腸症、がん性悪液質などの疾患に対するグレリンの臨床応用が期待されている。また、近年、漢方薬である六君子湯が、グレリンシグナルを増強させる作用を有していることが注目されている。心身ホルモンとして多彩な作用を有するグレリンは心身医療において、その重要性がさらに増加していくものと予想される。(著者抄録)

Appetite and feeding behavior are the most important sense and behavior for the maintenance of one's life. Ghrelin is a peptide that has been discovered as an endogenous agonist of the promotion factor receptor for growth hormone secretion. Manifestation and secretion of ghrelin increase by fasting and decrease by ingestion, and the exogenously administered ghrelin increases appetite. In addition, it is involved in various actions such as growth, bowel motility, respiratory function, cardiac activity, carbohydrate metabolism, immunocompetence, affection, cognitive function and generation. Moreover, clinical applications of ghrelin to diseases such as anorexia nervosa, functional gastrointestinal symptoms and cancer cachexia are anticipated. In recent years, the fact that Rikkunshito has an effect to enhance ghrelin signal attracts researchers' attention. Ghrelin, which has various effects as physical and mental hormone, will rise its significance in psychosomatic medicine.

目的:摂食障害患者は、未治療のまま長期経過する例が多く、やせが遷延するとさまざまな合併症が生じうる。合併症はときに重篤であり注意が必要である。方法:当科において2001〜2010年に入院加療を受けた摂食障害患者を分析し、合併症のリスクファクターについて検討する。結果:当科において、30代の患者数が増加しており、未治療の長期経過例や遷延例が増加している可能性が示唆される。病型は、神経性食欲不振症(制限型)が最も多く、神経性大食症(非排出型)は有意に増加傾向を示した。合併症は、白血球減少症や軽度肝機能障害の合併率が高く、イレウス、播種性血管内凝固症候群など重篤な合併症をきたした例も認めた。BMIが低値になると、合併症の発症頻度が増加した。また、重篤な合併症は、治療初期に発症し、BMI<13の高度なやせを呈し病前体重より高度な体重減少を認めた症例で生じていた。結論:早期発見および早期治療の導入が合併症リスクを減らすために、最も重要である。(著者抄録)

背景と目的:摂食障害(神経性食欲不振症および神経性過食症)は欧米のみならず、本邦においても思春期女性を中心に増加傾向が認められている。摂食障害は多元的な発症因子をもち、その病態生理においては明確な知見が得られていない。脂肪細胞から分泌されるアディポネクチンは、肥満者においてはその血中濃度が低く、アディポネクチンの低下は2型糖尿病や心血管疾患の発症と関連が深いといわれている。最近、アディポネクチンは生体内において多量体構造(高分子量:HMW、中分子量:MMW、低分子量:LMW)をとって存在し、これらが体重増減に関して相反する機能をもっていることが明らかとなった。今回、われわれは神経性食欲不振症におけるアディポネクチン各分画を測定し、その役割についての検討を行った。症例と方法:女性の神経性食欲不振症(AN)患者9名および年齢と身長を合わせた健常者9名において血中のアディポネクチン3分画を測定した。結果:アディポネクチン総量に対するHMWの割合(%HMW)およびLMWの割合(%LMW)はAN群において有意に高値を示した。2群をあわせた18名において、BMIに対して%HMWは正の相関を、%LMWは負の相関を認めた。また、摂食障害調査票(EDI-2)において、HMW、%HMWおよびLMWは「社会的不安感」と強い正の相関を示し、LMWは「衝動の制御」と強い正の相関を示した。さらにモーズレイ強迫神経症質問紙(MOCI)において、%HMWは「洗浄」と強い正の相関を示した。考察:アディポネクチンのHMWとLMWの血中濃度はANにおいて、低栄養に応じた反応をしているものと考えられた。また、心理テストとの関連を加味すると、アディポネクチン各分画は、摂食障害患者の摂食行動と精神病理において異なった役割を果たしていると推測され、ANをはじめとする体重異常を呈する疾患においては、アディポネクチンはその総量に加え、各分画を経時的に評価する必要があると考えられた。(著者抄録)

機能性ディスペプシア(FD)は心身症の代表疾患であり、心理社会的ストレスがFDの発症や症状の持続増悪に関与していることは広く認識されている。FDの腹部症状、不安感および行動は相互に悪循環を呈し増悪し、社会機能にも影響を及ぼすと不登校や退職などの原因にもなり得る。FDの治療は、腹部症状に対する薬物療法のみではコントロール困難で治療に難渋することが多い。また、FDはうつ病などの精神疾患を合併することが少なからずあり、お互いがお互いを悪化させ得るため、精神症状の評価も重要である。FDでは患者の腹部症状に加え、準備因子、発症因子、持続増悪因子について聴取し、身体-心理-社会的要因の全体像を把握した上で薬物療法、食事運動指導、心理療法を併用した適切な治療計画をたてることがポイントとなる。FDの患者が不安感を感じることは病態の1つであり、プライマリ・ケアにおいても特別視することなく、心理的社会的評価を含めた全人的医療を心がけることが大切である。(著者抄録)

Eメールを使った低強度認知行動療法を20代女性患者に行った症例について報告した。主訴は「大人になったら自然と痩せるかと思っていたが変わらない。もう何からどうしていいかわからない。痩せたいけど、すぐ諦めてしまうので、励ましてほしい」というものであった。カンファレンスで治療方針と役割分担を計画し、入院中はカロリー制限食、運動療法、生活リズムの改善等の介入を行った。外来では医師が生活指導、肥満症に対する行動療法、心理士が家族、友人関係における不安、自己効力の低さに焦点を当て認知療法を行った。しかし外来治療当初にリバウンドし、もうだめとの発言があった。医師らと相談の上、1日1回のメールサポート(MS)を行うことした。MS開始後、半年間で30kg以上の減量に成功した。減量行動は定着しつつあった。MSの頻度も週1回まで減少した。減量と共に生活全般が活動的になり、現在は就職している状態となった。

グレリンは,1999年に胃から同定された強力な食欲促進ペプチドであり,血中あるいは迷走神経を経由して中枢にシグナルを伝え,視床下部に存在する神経ペプチドY(neuropeptide Y;NPY)やアグーチ関連ペプチド(agouti-relatedpeptide;AgRP)を介して,食欲や消化管運動に促進性の影響を及ぼす.このように末梢から放出されるホルモンが中枢の機能をコントロールすることは,臨床応用の観点からも重要な意義をもち,今後,脳腸相関の解明や,その異常病態の治療に有用なアプローチとなるものと期待される.(著者抄録)

先進国だけでなく,一部の発展途上国でも肥満やそれに伴う耐糖能異常,高血圧,脂質異常症などが重複するメタボリックシンドロームの患者数の増加が社会問題化しつつある.エネルギーバランスの異常が病態に大きく関与すると考えられる.グレリンやpancreatic polypeptide(PP)ペプチドファミリーなどの食欲・体重調節ペプチドを中心に病態解析が進められている.PPおよびpeptide tyrosine-tyrosine(PYY)はおもにY4あるいはY2受容体を介して摂食抑制作用を示し,体重を減少させる.最近PYY3-36がヒトにおいては恒常性維持領域だけでなく,大脳辺縁系および高次大脳皮質領域の神経活動調節にも関与することが示されている.消化管を含めた末X臓器と摂食中枢との間で行われる食欲・体重調節機構の解明が,メタボリックシンドロームの病態の解明や治療につながることが期待される.(著者抄録)

Objective : Anorexia nervosa (AN) continues to be a refractory disease because of its unknown pathogenesis. The role of adiponectin in AN has not been clarified. Moreover, few reports have described the relations between adiponectin isoforms and AN in the physical and psychological states. Therefore, we measured plasma adiponectin and its isoforms levels in patients with AN to examine their roles in AN. Methods : Eighteen women participated in this study : nine patients with AN and nine age-matched healthy controls. We examined plasma adiponectin and its isoforms levels in all subjects and administered three types of psychological test to patients with AN : the Eating Disorders Inventory-2, the Maudsley Obsessional-Compulsive Inventory, and the Beck Depression Inventory-2. Results: We found that the percentage of high-molecular-weight (HMW) to total adiponectin (%HMW) was significantly low and the percentage of low-molecular-weight (LMW) to total adiponectin (%LMW) was significantly high in the AN group compared with the control group. The %HMW positively and the %LMW negatively correlated with body mass index in the entire study population. The %HMW was also positively correlated with psychological symptoms such as social insecurity or cleaning evaluated with the Eating Disorders Inventory-2 or the Maudsley Obsessional-Compulsive Inventory. Conclusions: Our study indicates that all adiponectin isoforms should be evaluated in patients with AN in addition to total adiponectin. The decreased %HMW and the increased %LMW that were correlated with the body mass index and some components of psychopathology in our patients may indicate a complex role of adiponectin isoforms in maintaining energy homeostasis and emotion during extreme malnourishment.

When a psychologist with little experience gives psychological support to the geriatric patients/clients in palliative care, he/she faces various difficulties, such as the age difference between the psychologist and a patient, psychosocial problems of the patients, and so on. In the field of palliative care, listening attentively and showing empathy to the patients are considered to be important methods ; however, treating patients' anxiety and depression, especially when related to death, are subtle problems. Inexperienced psychologists need an equivocal methodology, which, however, is not being practiced sufficiently, yet it seems not enough. The first half of the current article reviews the psychological problems of the geriatric patients in palliative care from the perspective of cognitive-behavioral therapy. The second half precents a summary of a case which has been a struggle and a learning experience to the author, and discusses how inexperienced psychologists can contribute to this field.

22歳女性が入院した。身長157cm、体重154.8kg、BMI63.6kg/m2であった。食事のカロリーを制限し、運動を処方したが、患者の体重はほとんど減少しなかった。抗肥満薬として防風通聖散を投与した。BFの投与後、運動による効果が向上し、四肢の寒冷感の改善が認められた。3ヵ月の間に、体重は145.3kgに、BMIは59.7kg/m2に減少した。安静代謝率が2260から2440kcal/日に上昇した。

機能性消化管障害にうつや不安障害が合併していることは度々経験される。今回われわれは、職場での社交不安を伴った機能性消化管障害の患者に対し、従来の認知行動療法における不安管理訓練をベースにしながらも、催眠療法を意識し、筋弛緩のみならずイメージ深化の技法を用い、リラクセーションの側面を強化した治療を導入した。さらに並行して、認知再構成法や、エクスポージャーなど意識下、かつ現実場面における認知行動療法も施行し、奏功した。本症例は、認知行動療法と催眠療法のエッセンスを併用しており、認知行動催眠療法といえるものであると考えている。このアプローチの経過と効果について報告する。(著者抄録)

Corticotropin-releasing factor(CRF)は、1981年にヒツジの視床下部から分離されたペプチドであり、ストレス応答において重要な役割を果たす。近年、構造がCRFと類似し、CRF受容体を介して作用を発現するCRFファミリーペプチドとしてUrocortin1(Ucn1)、Urocortin2(Ucn2)、Urocortin3(Ucn3)が分離、同定された。CRF受容体には2つの受容体が存在し、CRF receptor type 1(CRF-R1)は主として不安行動に関与し、CRF receptor type 2(CRF-R2)は摂食行動に関わっている。CRF、Ucn1、Ucn2、Ucn3は中枢投与によって摂食が抑制され、CRF-R2は投与後早期ではなく、後期の摂食抑制に関わることが示唆されている。消化管運動は摂食行動に影響し、CRFペプチドファミリー投与による消化管運動は、胃ではCRF-R2を介する。マノメトリー法を用いてマウスにUcn3を静脈内投与した実験においても胃、十二指腸の消化管運動が抑制された。Ucn2はCRF-R2を介して抗不安作用を示すとの報告もあり、CRF-R2に親和性の高いUcn3は機能性ディスペプシアや摂食障害、肥満などの治療への臨床応用の可能性が考えられる。(著者抄録)

摂食障害の病態はいまだ完全に解明されておらず、難治性疾患の一つになっている。他の、特に器質的疾患に比べ、適当な疾患動物モデルが存在せず、動物実験の結果がそのままヒトに外挿できないことが、摂食障害の病態の解明、治療法の開発における研究にとって大きな障害となっている。摂食障害の病態は、現在のところ、環境、遺伝的要因を背景にし、個体レベルにおいては、(1)上位中枢における認知、(2)視床下部におけるペプチドを中心とした摂食調節、(3)摂食調節ペプチド受容体の感受性、(4)末梢からの摂食調節シグナルの伝達経路、(5)末梢からのペプチドを中心とした摂食調節シグナルのレベル、などにおける異常が複合しているものと考えられる。これまでに、さまざまな摂食調節ペプチドの血中レベルの異常が、摂食障害において報告されている。本稿では、摂食障害の病態の解明、治療法の開発における末梢の摂食調節ペプチドに焦点を当てた研究について、特に神経性食欲不振症に関して、その現状と今後の可能性を提示する。(著者抄録)

Eating disorder is one of the intractable diseases, and the pathology is incompletely understood. Comparing to other organic illness, an appropriate animal disease model is non-existent. For that reason, there is an obstacle to elucidate the pathogenesis of eating disorder and the development of its treatment due to the difficulty in extrapolating the results of animal experiment to human beings. At present, the pathogenesis of eating disorder is considered to be a complex of abnormalities at the whole-body level with environmental and genetic factors, such as 1) cognition in the upper central nervous system, 2) feeding regulation in the hypothalamus that peptides play a pivotal role, 3) sensibility of feeding-regulatory peptide receptors, 4) feeding-regulatory signal transduction pathway from periphery, and 5) the level of feeding-regulatory signal from periphery that peptides play a pivotal role. Abnormalities in blood concentration of feeding-regulatory peptides have been reported in eating disorder. We present the current states and the future possibilities of researches focused on peripheral feeding-regulatory peptides regarding elucidation of the pathology of eating disorder, especially anorexia nervosa, and the development of its treatment.

心理社会的因子によって身体症状が生じるのが心身症である。日常診療において、身体的症状をとらえるためには、その背景にある心理社会的な側面にも目を向ける必要がある。ストレスと症状との関係においては、視床下部-下垂体-副腎軸(HPA-axis)と脳腸軸(Brain-gut axis)が重要な役割を担っている。とくに副腎皮質刺激ホルモン放出因子(CRF)が最重要因子の1つとして関与していることが知られている。CRFの受容体は主として脳と腸管に存在し、CRF1型受容体を介して下部消化管運動が亢進、CRF2型受容体を介して胃運動が抑制される。また、絶食時に発現、分泌が上昇するグレリンは、ストレス時にも上昇することが明らかになっており、腹部症状に及ぼす影響についての研究結果が待たれるところである。(著者抄録)

体脂肪組織からその量に応じて放出されるレプチンは、体脂肪の蓄積状況を脳内に伝えるシグナルであり、視床下部に存在する食欲調節物質を介して、体重(体脂肪量)を一定に保持するというフィードバックループの存在が証明された。食欲不振や体重減少を主徴とする癌性悪液質は、担癌生体から放出されるサイトカインの過剰病態と考えられ、この過剰なレプチン様シグナルが飢えに対する生体の応答を阻害し、持続的な食欲不振、基礎代謝量の亢進、体重減少を引き起こす。癌性悪液質の治療は、食欲を増加させ、体脂肪や筋肉の崩壊を阻止し、QOLの維持、向上をはかるとともに、化学療法や放射線療法の耐性を高め、予後を改善することにある。サイトカインによる過剰なレプチン様シグナルを是正し、食欲・体脂肪量調節ループを適切に作動させることが治療の目標となる。疼痛、疲労、抑うつといった癌の周辺症状とあわせ、心身両面から包括的な治療を行う必要がある。(著者抄録)

がん患者は、しばしば食欲不振、体重減少、全身倦怠感などの症状を呈し、悪液質(cachexia)という状態に至る。悪液質における体重減少は、飢餓時とは異なり、脂肪組織のみならず骨格筋の多大な喪失を呈する。悪液質の要因として、食欲低下とエネルギー消費の増大だけでなくサイトカインや腫瘍由来物質の産生が関与している。近年、がん患者の食欲不振の原因として注目されているのが、脳内視床下部に存在するneuropeptide Y(NPY)などの食欲促進ペプチドとproopiomelanocortin(POMC)などの食欲抑制物質との不均衡である。グレリンは、胃から分泌され、迷走神経や視床下部に存在する受容体GHSR 1aを活性化し、NPYやAgRPの発現と遊離を増加させ、摂食促進および消費エネルギー抑制に作用する。また、消化管にも液性、神経性に作用し、胃酸分泌促進作用や蠕動運動亢進による胃排出促進作用を有する。(著者抄録)

脳・消化管ペプチドは、脳と消化管の双方に存在するペプチドであり、消化管機能の調節のみならず、食欲・エネルギー代謝や情動、学習・記憶に深くかかわると考えられている。食欲・体重調節に関しては、1994年のレプチンの発見以来、その概要が明らかになり、脳・消化管ペプチドはレプチンの下流に存在し、体脂肪量の過不足に応答する食欲・体重調節機構の根幹を担うことが証明された。脳・消化管ペプチドのヒト行動に及ぼす役割に関しては、動物での成績に加え、血中・脳脊髄液中ペプチド分泌動態やペプチド投与効果の観察など、ごく限られた知見しかない現状にあった。しかし、近年の遺伝子解析技術の進歩や、ペプチドからの創薬、脳画像解析技術の進歩などから、脳・消化管ペプチドのヒト行動に及ぼす役割が明らかになりつつある。脳・消化管ペプチドがどこまで、われわれの行動を規定しているかは大問題であるが、そのアプローチ法と問題点について述べた。(著者抄録)

Energy homeostasis is controlled by a complex neuroendocrine system consisting of peripheral and central signals, particularly leptin and brain-gut peptides. Animal experiments have demonstrated that brain-gut peptides are important not only in the regulation of feeding and metabolism, but also in other neuroendocrine and behavioural functions. Knowledge about gut-brain peptides is increasingly used in the study of the pathophysiology of behavioural disorders. Genetic studies including polymorphisms in genes encoding peptide or peptide receptors, as well as administration studies with peptide agonist/antagonist together with brain imaging have opened a new avenue for the study of peptide signaling in human behavior and behavioural disorders. This review briefly describes brain-gut peptide family and its role in feeding and other human behaviors, with a focus on the factors influencing peptide signaling such as peptide auto-antibodies, synaptic rearrangement, food-derived peptides, and environmental factors involved.

日本において近年、肥満症、メタボリックシンドロームの増加や、若年女性の摂食障害が問題となっており、食欲・体重調節機構の解明は急務となっている。1994年のレプチンの発見以来、末梢臓器である消化管、脂肪組織と摂食中枢との関連を中心にその調節機構の解明が徐々に進んでいる。古くより消化管から分泌されるホルモンは食欲に影響することが知られており、末梢である消化管と摂食中枢は、互いの機能を調節している。オレキシンなどの例外を除き、摂食を促進する消化管ペプチドはエネルギー消費を抑制し、体重を増加させる作用を持ち、摂食を抑制するものは、エネルギー消費を促進し、体重を減少させる作用を持つ。食事の栄養素は、摂取することにより摂食を抑制する方向に働くが、例えば必須アミノ酸の一つであるリジンは欠乏食を与えると、リジン嗜好性が高まり、栄養素の欠乏症による恒常性の破綻を回避するために重要であると考えられる。肥満に対して長期的に体重減少効果を認める治療は、胃バイパス術をはじめとする肥満外科手術であり、高血圧、脂質異常症、糖尿病など、メタボリックシンドロームの改善にも極めて有効であることが示されている。例えば、糖尿病の著明な改善が認められるが、その機序としてインクレチンと反対の作用をもつ未知のペプチドが存在し、胃バイパス術によりその作用が減弱し、糖尿病が改善するという「Anti-incretin theory」などが提唱されている。今後も消化管を含めた末梢臓器と摂食中枢との間で行われる食欲・体重調節機構の解明が一層進んでいき、肥満だけなく、神経性食思不振症や悪液質といった食欲を低下させる病態の解明や治療につながることが期待される。(著者抄録)

Peptide tyrosine-tyrosine(PYY)は摂食により血中濃度が上昇する消化管ペプチドであり、消化管運動、消化液分泌などの制御のほかに摂食抑制作用があり、体重調節、摂食行動調節にかかわっている。PYY3-36の摂食抑制作用は、Y2受容体欠損マウス、Y2受容体アンタゴニストの中枢投与、迷走神経切断ラットや電気生理学的実験などから、Y2受容体、迷走神経を介した経路が考えられ、摂食行動調節については恒常性維持領域だけでなく、大脳辺縁系および高次大脳皮質領域の神経活動を介していることがわかった。生活習慣によるエネルギーバランスへの影響に関する検討が、消化管ペプチドを中心として、そのメカニズムの解析とともに進行し、一部の消化管ペプチドは抗肥満薬として臨床応用されつつある。今後さらに、生活習慣病の発症・進行予防に向けた新たな知見が急速に集積していくものと期待される。(著者抄録)

神経性食欲不振症(AN)は高度の体重減少を呈する疾患であり、特異的な精神症状のほかに、内分泌機能障害、循環器障害、脳萎縮などのさまざまな合併症をきたし、ときには致死例もみられる。その合併症の多くは低栄養に伴うものであるが、ときに再栄養による重篤な合併症が起こるため、特に痩せが高度の場合の初期治療には、きめ細やかな栄養管理が必要とされる。(著者抄録)

ニューロペプチドY(NPY)は中枢神経系を中心に非常に多く分布しており、その生理作用は摂食調節やエネルギー調節のほか、行動、情動系、アルコール摂取、消化管運動、痙攣調節など多岐にわたっている。NPY受容体には6種類のサブタイプが存在し、各サブタイプのアゴニスト、アンタゴニスト化合物投与による薬理作用の検討やトランスジェニック動物を用いた各サブタイプ遺伝子のノックアウトあるいは過剰発現による表現型の観察により、その生理作用が検討されている。摂食調節におけるNPY受容体の役割についても広く研究されており、とくにY1およびY5受容体が摂食亢進作用として、Y2受容体が摂食抑制作用として報告されている。しかし単一受容体サブタイプだけでは摂食調節機構を説明することは困難であり、NPY受容体サブタイプ間における相互作用や他の神経ペプチドによる代償作用あるいは重複作用が想定されている。(著者抄録)

著者らは、森田療法における神経症障害の重要概念である「とらわれ」の精神病理の程度を測る「神経症性障害のとらわれ自己評価スケール(SSTN)」を開発しており、今回、神経症性障害の「とらわれ」の精神病理を明らかにすることを目的に、神経症性障害の93名(中国人)を対象に2週間の森田療法と薬物療法による介入を行い、介入前後にSSTN、SAS(Zungの自己評価式不安尺度)を用いて評価を行い、正常対照者91名(中国人)と比較、検討した。その結果、患者群において、介入前後の2回のSSTNとSASの総得点に正の相関が認められ、2週間の治療により患者群の「とらわれ」と不安の程度が軽減され、「とらわれ」は不安とともに消長することが示唆された。

There are many patients with depression and panic disorder who are suffering from somatic symptoms such as abdominal complaints. We showed a report of stress related disorders which are consistent with other reports. We find nothing special in these patients at somatic service, but their symptoms do not improve only by medication or counseling. Especially it is not good for the patients to be addicted to benzodiazepine, and co-dependent in the doctor-patients relationship. It is necessary for them to be treated by psychosomatic approach as those with functional dyspepsia. In this paper, we introduce proper medication, CBT, breathing, Health-art theory (propounded by Yujiro Ikemi), and complementary-alternative therapy for them.

摂食障害と摂食関連ペプチドとの関連を検討するため、拒食症(神経性食欲不振症;AN)患者11名(A群)、肥満10名(B群)、健常者11名(C群)を対象に、三菱ELISAおよびPhoenix RIAを用いて測定を行った。BMI、体脂肪はA群はC群に比較して有意に低値であった。摂食関連ペプチドのうち、obestatin、desacyl-ghrelinはC群に比較してA群は有意に高値、B群は有意に低値を示した。Acyl-ghrelinはC群に比較してA群は高値であったが有意差はなく、B群は有意に低値を示した。ObestatinはBMIと有意な負の相関を、desacyl-ghrelinおよびacyl-ghrelinは有意な正の相関を示した。次に、制限型AN患者10名および健常者10名に対して糖負荷テストを行ったところ、obestatinはPYYのような負荷後の上昇は認めず、ghrelinと同様にゆるやかな抑制パターンを示した。Obestatinも中枢性摂食異常患者の栄養状態と関係する可能性が示唆された。

Anorexia nervosa(AN)の治療においてはrefeedingに伴う「治療抵抗性」をはじめとしてintrinsic/extrinsic risk factorsを無視できない局面がある。Food regulatory systemは、ANにおいてはmixed signalingを呈しており、食欲調節のみならず情動調節においてもそのambivalentな病態と密接な関連があると考えられているが、いまだANの中核病態にかかわるkey mediatorを見出せてはいない。Food regulatory systemを軸としてさらなるintrinsic/extrinsic risk factorsの探索を進めていく必要があるが、中でもalarmin systemは栄養・炎症・免疫と密接に関連したhomeostatic regulatorであり、特にANにおいては「治療抵抗性」との密接な関連が示唆される。今後は有機的な治療構造の確立のためにもintrinsic/extrinsic risk factorsの観点に立ち、特にfood regulatory systemとalarmin systemのcrosstalkを中心にANの病態解析を進めていく必要があるものと思われる。(著者抄録)

Treatment of patients with anorexia nervosa (AN) involves serious difficulties such as "treatment-resistance" during refeeding that need careful consideration of psychopathology of AN from intrinsic/extrinsic risk factors. A lot of literature on AN has revealed that, mixed signaling in the food regulatory system which appears to mirror not only energy intake but also emotions, may underlie characteristic ambivalent eating behaviors in this disorder; however, the key mediator relevant to core psychopathology of AN has not yet to be determined. Further investigations exploring new intrinsic/extrinsic risk factors of pathogenesis of AN are required, and the alarmin system, a homeostatic regulator relevant to nutrition, inflammation, and immune system, is especially noteworthy because of its close association with "treatment-resistance" in AN. We believe that elucidation of pathogenesis of AN from the view of intrinsic/extrinsic risk factors, specifically crosstalk between food regulatory and alarmin systems, is needed to take advantage of possible therapeutic applications in this serious disorder.

脳腸ペプチドは食欲・体重調節に重要な役割を有することが知られているが、摂食行動と密接に関わる消化管機能の調節にも深く関わっている。グレリンは胃から見出された強力な食欲促進ペプチドであり、視床下部に存在する神経ペプチドY(neuropeptide Y;NPY)やアグーチ関連ペプチド(agouti-related peptide;AgRP)を介して、食欲や消化管運動に促進性の影響を及ぼす。グレリンにはアシル化された分子型と、アシル基のない分子型(デスアシルグレリン)が存在し、また近年、同じグレリン遺伝子から産生される消化管ペプチドであるオベスタチンが見出された。アシルグレリンは摂食や消化管運動に促進作用を有するのに対し、デスアシルグレリンは促進、抑制両方向に作用、またオベスタチンは抑制もしくは作用なしと報告されている。本稿では、グレリンファミリーペプチドによる食欲・消化管運動調節を概括し、議論の多いこの点を含めて述べる。(著者抄録)

Perfluorinated compounds (PFCs) have been employed as surface treatment agents in a variety of products. Perfluorooctanoic acid (PFOA), a PFC that is found globally in the environment and in human tissues, has been increasing significantly in serum levels over the past 50 years. Here, we demonstrated that PFOA inhibits feeding behavior as potently as the endogenous peroxisome proliferator-activated receptor (PPAR)-α ligand, oleoylethanolamide (OEA), via the activation of PPAR-α, the vagal nerve and hypothalamic neuropeptides. Peripherally administered PFOA decreased food intake as potently as OEA. PFOA decreased gastric emptying and increased the expression level of the gene encoding urocortin 1 in the hypothalamus and the immunoreaction for urocortin 1 in the paraventricular nucleus. Vagotomy attenuated the inhibitory effects of PFOA on feeding. The inhibition of food intake and body-weight gain by PFOA was completely mitigated in PPAR-α -/- mice. Our studies demonstrated that the ubiquitous environmental pollutant PFOA works as an imitator of OEA mimicking its action in the feeding regulatory system, providing a new mode of action as represented by environmental 'anorexigens'.

Gastrointestinal (GI) hormones are well known to be important regulators not only in GI functions but also in feeding behavior. Ghrelin, a motilin-related family of regulatory peptide from the stomach, is the recent addition of this list, which has a charactelistic n-octanoylated serine at position 3. In addition to its ability to stimulate growth hormone (GH) secretion, ghrelin increases appetite and gastric motility/emptying and induces a positive energy balance leading to body weight gain. Ghrelin is a complementary, yet antagonistic, signal to leptin reflecting acute and chronic changes in energy balance, the effects of which are mediated by hypothalamic neuropeptide Y (NPY) and agouti-related peptide (AgRP). Ghrelin induces the fasted pattern of gastroduodenal motility (phase III or hunger contractions) , and the ghrelin-NPY axis is involved in the physiological occurrence of the interdigestive phase III contractions in rodents. On the other hand, desacyl form of ghrelin that accounts for approximately 90% of total circulating ghrelin, decreases food intake and gastric emptying, and disrupts the fasted pattern of gastric motility. Obestatin, a recently identified ghrelin gene product, may also act inhibitory in appetite and GI motility regulation although much more controversial than ghrelin. The findings suggest that ghrelin gene products may differently regulate appetite and gastrointestinal motility through different endocrine/neural pathways along the gut-brain axis.

Elevated anxiety symptoms have been reported to be present in many patients with diabetes mellitus. The underlying mechanisms by which diabetes mellitus influences behavior remain to be determined. We assessed feeding and anxiety behaviors in spontaneously diabetic Ins2Akita mice. We measured blood glucose, body weight, and food and water intakes in C57BL/6 heterozygote Ins2Akita mice. The behavioral properties of Ins2Akita mice were assessed in an open-field test and an elevated plus-maze. The gene expression of hypothalamic neuropeptides was examined in non-food-deprived Ins2Akita mice. Body weights of the Ins2Akita mice were less than those of the age-matched C57BL/6 mice, as controls. Food and water intakes were increased in the Ins2Akita mice. In the open-field test, the Ins2Akita mice had decreased locomotor activity and increased immobilization time. The Ins2Akita mice exhibited anxiety behavior in the elevated plus-maze. RT-PCR analysis showed decreased proopiomelanocortin (POMC) mRNA expression and increased agouti-related protein (AGRP) mRNA expression in Ins2Akita mice. There were no significant differences in hypothalamic ghrelin mRNA expression. These observations indicate that Ins2Akita mice, which are characterized by hypoinsulinemia and hyperglycemia, exhibited hyperphagia and anxiety behavior; the mechanism of action involved the activation of hypothalamic AGRP and the inactivation of hypothalamic POMC. In addition, Ins2Akita mice are a useful model for understanding the mechanisms involved in the psychological complications of diabetes mellitus. Further, melanocortin systems may be therapeutic targets not only for diabetes but also for its associated complications.

モチリンは十二指腸を中心とした消化管に存在し,空腹期において消化管の平滑筋に対して直接,あるいは迷走神経を介して作用し,胃からはじまり順次回腸に向けて伝播していく,一連の強力な収縮運動に関与している.1999年に成長ホルモン放出促進因子レセプター(GHS-R)の内因性リガンドであるグレリンが胃から同定された.グレリンは28個のアミノ酸よりなるペプチドで,脂肪酸による修飾を特徴としている.グレリンとそのレセプターは,モチリンおよびそのレセプターとの間での構造上の相同性が高く,グレリンが新たなモチリンペプチドファミリーの一員であることが示唆される.空腹期のグレリンの投与は,胃前庭部,十二指腸における空腹期強収縮運動の頻度を増加させるとともに,食後期の投与は,収縮波を空腹期のパターンに変化させる.一方,脂肪酸による修飾のないデスアシルグレリンの投与は,脂肪酸によって修飾されたグレリンとは逆に上部消化管の空腹期強収縮運動の頻度を抑制する.グレリンの消化管運動調節作用は,グレリンおよびそのレセプターが消化管運動異常に関係する,さまざまな疾患に対しての創薬のターゲットとなる可能性を示唆している(著者抄録)

成長ホルモン放出促進因子レセプター(GHS-R)の内因性リガンドで,アシル化を特徴とするアシルグレリンが胃から同定され,その多彩な作用が報告されてきた.一方,生体内には,これまで生理活性がないとされる,アシル化されていないデスアシルグレリンが,アシルグレリンよりも多く存在している.最近,デスアシルグレリンが未知の新たなレセプターを介して,摂食,消化管運動,心臓,糖代謝,脂肪生成などに影響を及ぼすことが報告された.また,中鎖脂肪酸の摂取によるアシルグレリンの増加や,新たなグレリン測定法の開発も報告された.デスアシルグレリンの新たな作用の発見および測定法によるグレリン値の相違などは,グレリンが関与するさまざまな病態における,グレリンの役割を再評価する必要性を示唆している.今後,脂肪酸の修飾のメカニズムやデスアシルグレリンのレセプターの解明などに向けた新たな知見が急速に集積していくものと考えられる(著者抄録)

グレリンは胃から分泌され,脳に作用して,摂食亢進作用など多彩な生理作用を有することが知られている.グレリンにはAcylated ghrelinと,Des-acyl ghrelinの2種類が知られており,Des-acyl ghrelinは血液中のtotal ghrelinの90%以上を占めているにもかかわらず,その生理作用は良く知られていなかった.最近の我々の研究で,Des-acyl ghrelinは摂食行動や消化管運動に対してacylated ghrelinと拮抗する作用を有することが明らかになった.また,acylated ghrelinとdes-acyl ghrelinは,脳内で全く異なる神経核や神経伝達物質を介してこの反応を起こすことがわかった.摂食行動や消化管運動の制御をめぐる脳と腸の機能相関に関して,最近の研究成果をもとに解説する(著者抄録)

Ghrelin, an endogenous ligand for growth-hormone secretagogue receptor (GHS-R), was recently identified in the stomach with structural resemblance to motilin. Ghrelin induces a state of positive energy balance and body weight gain by promoting food intake, decreasing energy expenditure and stimulating GH secretion. The expression and secretion are increased by fasting and are decreased by feeding. In addition, diet-induced weight loss increases plasma ghrelin level in humans. Moreover, ghrelin influences anxiety, memory retention, cardiac performance, gut motility, glucose metabolism and adipose tissue. These observations suggest that gastric peptide ghrelin and the receptors may be potential therapeutic targets for various diseases.

Anorexia and body weight loss are characteristic of many diseases, including cancer and AIDS. Neuropeptides play a pivotal role in the physiological mechanisms regulating food intake and body weight. Neuropeptide Y (NPY) is a key molecule of the orexigenic network for energy intake and for normal adaptive feeding response to energy deficits. Therefore, the NPY receptors (notably, Y1 and Y5) may be one of the most significant target classes for treatment of anorexia and body weight loss. In addition, several anorexigenic peptides including corticotropin-releasing factor (CRF), cholecystokinin (CCK), leptin and melanocortin (MC) are emerging as potential targets for anorexia. Antagonists for CRF 2 receptor, CCK A receptor, MC 4 receptor and leptin receptor may be useful in stimulating food intake. The development of highly specific and selective non-peptide antagonists for these receptors is awaited. Anorexia is a crucial and critical disease. Increasing knowledge of its pathophysiology could lead to innovative new medicines for anorexia-cachexia syndrome. © 2000 Ashley Publications Ltd. ISSN 1460-0412.

Aim: Previous studies have demonstrated that corticotropin-releasing factor (CRF) produces behavioural, physiological and immunological responses similar to those induced by stress. However, these findings have been validated largely in laboratory rats. Methods: We examined the effects of intracerebroventricular (i.c.v.) administration of CRF on anxiety and food intake in mice. Using the elevated-plus maze, we measured anxiety levels after i.c.v. CRF in mice. We also measured food intake for 2 h after i.c.v. CRF. Results: CRF increased the normal preference for the closed arms of the maze at a very low dose of 3 pmol, indicating an anxiogenic effect. CRF powerfully suppressed food intake at the doses of 3-300 pmol for over 2 h. Conclusion: Our results demonstrate that i.c.v. CRF evokes anxiogenic behaviour and suppresses feeding with the same dose-response relationships in mice. CRF may thus play a role in integrating the overall responses to stress through co-ordinated actions in the brain of this species.

A 35 year old male, who had taken medication for atrial flutter since 29 years old, was admitted to the hospital, for sudden bloody diarrhea. An endoscopic examination showed edematous mucosa with petechial hemorrhages and narrowing air filled segment in the ascending colon. Repeated cultures of feces were negative for pathogenic organisms. Mucosal biopsy revealed mucosal erosion with infiltration of acute inflammatory cells. A diagnosis of ischemic colitis was made based on definite clinical and pathologic criteria. Intravenous fluids and bowel rest improved his clinical symptoms and 2nd colonoscopy performed 2 weeks later showed obvious improvement of the mucosal changes. Ischemic colitis affects seniles with clinical evidences such as atherosclerosis and hypertension. A review of Japanese literature from 1986 through 1994 revealed only 19 cases of ischemic colitis in young adults under 40 years of age.
