＜ポイント＞「肥満」と「肥満症」は区別されており、「肥満症」は、「健康障害を合併するか、合併症が予測される場合で、医学的に減量の介入を必要とする病態」と定義される。ヒトでは、嗜好、ストレス、報酬や社会的因子に基づく摂食行動が特徴的であり、認知・情動性の摂食行動調節機構が存在する。「肥満症」に対しては、性格特性と社会面の問題まで包括的に評価したうえで、全人的治療を行う。「肥満症」の減量治療にはエンパワーメント・アプローチに基づいて、行動変容を促すことで、セルフエフィカシーを向上させていく。肥満手術や薬物治療の適応となることもある。(著者抄録)

おもに胃で産生されるグレリンは28アミノ酸残基のペプチドであり,空腹信号を胃から脳へ伝達する(脳腸相関).グレリンはグレリン脂肪酸転移酵素(ghrelin O-acyltransferase;GOAT)によりセリンにオクタノイル基(C8:0)が付加されてアシルグレリンとなり食欲亢進活性を示す.アシルグレリンは空腹状態で胃から分泌され,脳内のグレリン受容体(GHS-R1a)に結合し摂食を亢進する.アシルグレリンは恒常性維持のための摂食と快楽的な摂食の両方に関与している.食事誘発性の肥満においては,アシルグレリンレベルは正常なBMI値の人に比べて低く,脳内でグレリン受容体の減少によりアシルグレリンシグナルが低下する.神経性食思不振症の患者では健常人に比べてアシルグレリンレベルは高い.この章では,アシルグレリンの食欲に対する機能や作用機構と肥満や神経性食思不振症でのグレリンシステムの破綻について概説する.(著者抄録)

In Kagoshima University Hospital, Kampo Medical Center was established in 2012. There, we try to perform several achievement to improve clinical, research, and educational performance. Kampo Medical Center is consist of the member from several departments, including obstetrics and gynecology department, pain department, dentistry department as well as psychosomatic medicine department. Kampo Medical Center has the function to cooperate each department and cross-sectional work, then act in the field of clinical, research, and education. We induce the simulated training and problem based learning to learn Kampo medicine. In research field, we publish several paper and reported the mechanism and the effect of Rikkunshito in cancer. Herein, we discuss about the works Kampo Medical Center has done for the development of Kampo medicine.

Cachexia is characterized by anorexia, weakness, weight loss, and muscle wasting. The understandings of the pathological mechanism of anorexia and muscle metabolism in view of the crosstalk between brain, gut, adipose tissue and muscle will open the new way for the management of cachexia. Cytokines-induced systemic inflammatory in cachexia causes anabolic/catabolic imbalance. Cachexia represents a continuum with three stages of clinical relevance : pre-cachexia, cachexia and refractory cachexia. In the refractory cachexia stage, the burden and risks of artificial nutritional support likely outweigh any potential benefit, and therapeutic interventions focus typically on alleviating the suffering associated with cachexia. It is important to appreciate the stage of cachexia and perform appropriate nutritional management.

一般診療の中で心身医学を実践するにあたり、心身医学の認知はいまだ十分とは言い難い状況が続いている。心身医学の啓蒙を進めることが課題であり、卒前および卒後の心身医学教育の充実が求められている。しかし、従来型の講義形式では、講義時間を増やしても単純な知識の詰め込みに過ぎず、信頼される医師-患者関係など、倫理教育や態度教育も必要な心身医学の学習目標に到達することは難しかった。また、系統別講義を行ってきた結果、多要因を含む心身医学への理解を深めることができない現状にあったといえる。今後は複数の科目を連携させ、「最終的に何ができるようになったか」というアウトカムを基盤とした心身医学のカリキュラム開発を検討していくことが課題となると考える。(著者抄録)

身体疾患のなかで、その発症や経過に心理社会的背景が密接に関与し、器質的あるいは機能的障害が認められる病態が心身症である。機能性ディスペプシア(FD)も心身症の側面をもち、その病態をとらえるには心理社会的側面に目を向ける必要がある。ストレスは、視床下部-下垂体-副腎皮質系や自律神経系、免疫系を介した脳腸相関により消化器症状を引き起こすことが知られている。また、虐待やトラウマが胃の知覚の異常や運動異常をきたすなど、遺伝や養育環境を含む生育歴やエピジェネティクスも重要な因子であることが報告されてきた。したがって、FDの診療は消化器症状のみに焦点を当てた対症的治療では不十分であり、心理社会的因子など患者の背景を十分に把握し、より上流にある原因に対してアプローチする、全人的かつ個別的医療を行うことが必要になる。(著者抄録)
