Background: A method has been developed to assess toe pressure strength in the standing position, taking into account concerns about toe grip strength. Research question: Which is more associated to postural control capability, the conventional toe grip strength or the newly devised toe pressure strength in the standing position, which is close to the actual movement? Methods: This study is a cross-sectional study. This study included 67 healthy adults (mean age, 19 ± 1 years; 64% male). The postural control capability was measured using the center-of-pressure shift distance in the anterior–posterior axis. Toe pressure strength in the standing position was assessed using a toe pressure measuring device to evaluate the force of pressure on the floor surface by all toes. During measurement, care is taken to ensure that the toes do not flex. However, toe grip strength in the sitting position was measured using muscle strength for toe flexion in a conventional manner. Statistical analysis was conducted by performing a correlation analysis between each of the measured items. Additionally, multiple regression analysis was used to examine the functions associated with postural control capability. Result: Pearson's correlation analysis revealed that the postural control capability was correlated with toe pressure strength in the standing position (r = 0.36, p = 0.003). Multiple regression analysis demonstrated that only toe pressure strength in the standing position was significantly associated with the postural control capability, even after adjusting for covariates (standard regression coefficient: 0.42, p = 0.005). Significance: The results of this study indicated that toe pressure strength in the standing position was more strongly associated with the postural control capability in healthy adults than toe grip strength in the sitting position. It has been suggested that a rehabilitation program for toe pressure strength in the standing position would help improve postural control capability.

Objectives: This study aimed to clarify the association between changes in satisfaction with meaningful activities and the occurrence of depressive symptoms before and during the coronavirus disease 2019 (COVID-19) pandemic. Design: A population-based prospective cohort study (Tarumizu Study). Setting and Participants: A total of 231 community-dwelling older adults aged ≥65 years without depressive symptoms at baseline. Methods: The participants completed baseline (June to December 2018, before the COVID-19 pandemic) and follow-up (September to December 2021, during the COVID-19 pandemic) assessments. Depressive symptoms were assessed using the 15-item Geriatric Depression Scale (GDS-15) and defined by a GDS-15 score of ≥5. Participants selected meaningful activities from the 95 activities included in the Aid for Decision-Making in Occupation Choice, and their satisfaction levels were evaluated. Satisfaction with the most meaningful activities was assessed as to whether it was maintained/improved or decreased at follow-up. Results: In the follow-up survey, 31 (14.3%) participants reported the occurrence of depressive symptoms. Compared with before the COVID-19 pandemic, the rates of meaningful activities during the COVID-19 pandemic decreased for interpersonal interaction and social life and increased for leisure, sports, self-care, and domestic life. Logistic regression analysis revealed that changes in satisfaction with meaningful activity were associated with the occurrence of depressive symptoms after adjusting for demographic variables, baseline GDS scores, cognitive and physical status, and instrumental activities of daily living (odds ratio, 2.78; 95% confidence interval, 1.17–6.59, P = .020). Conclusions and Implications: To avoid decreasing satisfaction with meaningful activities, supporting older adults may be protective against the occurrence of depressive symptoms. Our findings highlight the need for health care professionals to not only encourage older adults to participate in activities for their mental health but also consider whether the activities are meaningful or satisfying for them.

Evidence suggests that optimism has a positive impact on health status. Attentional bias modification (ABM) may be beneficial for enhancing optimism, but its effective application requires a detailed investigation of the association between attentional bias and optimism. This study aimed to determine the association between attentional bias and optimism based on different task types. Eighty-four participants completed the attentional bias measures using the dot-probe task (DPT), emotional visual search task (EVST) paradigms, and psychological assessments. Optimism was assessed using the Life Orientation Test-Revised with subscales for optimism and pessimism. Pearson’s correlation coefficient and multivariate linear regression analysis were applied to investigate the association between optimism and attentional bias. Neither the attentional bias derived from DPT nor EVST was significantly correlated with optimism total score or subscales. Regression analysis also showed no association between attentional bias and optimism (DPT, β = 0.12; EVST, β = 0.09), optimism subscales (DPT, β = 0.09; EVST, β = 0.17), or pessimism subscales (DPT, β = −0.10; EVST, β = 0.02). Our findings showed no evidence that attentional biases derived from either the DPT or EVST measures are associated with optimism or pessimism. Further studies are needed to effectively adapt the ABM to enhance optimism.

[目的]本研究の目的は,介護予防を目的とした体力測定会の参加頻度に影響する基本チェックリスト(Kihon Checklist:KCL)の各項目を検討することである.[対象と方法]対象は,全6回の体力測定会のうち4回以上参加した継続参加群91名,1回のみ参加した単発参加群64名の計155名とした.体力測定会への参加頻度に影響するKCLの各項目を検討するために,継続参加群および単発参加群を従属変数とした2項ロジスティック回帰分析を実施した.[結果]体力測定会への参加頻度に影響するKCLの各項目として,「6ヵ月間で2kg以上の体重減少があるか」が選択された.[結語]体力測定会への参加を継続するには,体重減少が生じないよう栄養状態に注意する必要性が示唆された.(著者抄録)

地域在住高齢者を対象とした横断研究を実施し、主観的記憶障害を有する高齢者における冷蔵庫管理の意義について検討した。2020年9月～10月に鹿児島県在住の高齢者を対象に自記式質問紙調査を実施し、282名を解析対象とした。調査項目は主観的記憶障害の有無、初期認知症徴候観察リスト、冷蔵庫管理に関する質問などとした。高齢者を主観的記憶障害あり群74名、主観的記憶障害なし群192名に分けて検討した。その結果、主観的記憶障害あり群ではなし群と比較して、冷蔵庫管理ミスが有意に多かった(P=0.008)。主観的記憶障害あり群では主観的記憶障害なし群と比較して、「冷蔵庫に同じものがたくさんある/賞味期限が切れたものがある」「冷蔵庫に何が入っているかわからない」「冷蔵庫の中に必要なものが見つからない」の4項目で冷蔵庫管理に関するエラーが多くみられた。さらに、主観的記憶障害は「本人が冷蔵庫の中身を知らない」という項目と関連していた(OR 7.44、P=0.02)。以上のように、主観的記憶障害を有する高齢者では冷蔵庫管理に関していくつかの問題を抱えていることが示された。

This retrospective study aimed to clarify the risk of older adults' nursing home placement in terms of basic movements and activities of daily living (ADLs) by analyzing data from a long-term care insurance certification survey in 2016-2018 in City A. Of the 21,520 people certified as needing care, 16,865 could be followed up until 2018. Data on sex, age, household structure, and level of care required were obtained. Those who lived at home and at nursing homes were categorized as the “Unchanged group” and the “Changed group,” respectively. Multivariate binomial logistic regression analysis was performed, with group type as the dependent variable and basic movement and ADL scores as the independent variables. For factor analysis according to care level, participants were classified into support need levels 1 and 2, care need levels 1 and 2, and care need levels 3, 4, and 5. For those categorized into support need levels 1 and 2, standing on one leg and transferring (basic movements) and urination and face cleaning (ADLs) were associated with nursing home placement. For those in care need levels 1 and 2, getting up and transferring (basic movements) and bathing, urination, face cleaning, and hair styling (ADL) were significantly associated with nursing home placement. For those in care need levels 3, 4, and 5, sitting and transferring (basic movements) and self-feeding and defecation (ADL) were significant. Occupational therapists must focus on older adults' declining ADLs and basic movements and relay the necessary information to patients, families, and other healthcare professionals to ensure appropriate and prompt care delivery.

高齢者は認知機能の低下に伴い複雑な手段的日常生活活動(IADL)から障害され次第に基本的日常生活活動(BADL)に困難をきたす.主観的認知機能低下や軽度認知機能障害の段階においても服薬管理などのIADLが低下し,家電製品の使用に困難を有す.筆者らが開発した生活行為工程分析表は,生活行為を詳細に工程分析することにより,ADLの障害部分と残存部分を明らかにすることができる.たとえば,洗濯では「洗濯物をしまう」は障害されやすいが,「洗濯物を干す」「洗濯物を取りこむ・たたむ」といった手続き的記憶を活用できる工程では認知症の重度者でも残存しやすかった.生活行為工程分析に基づいた3ヵ月の目標指向的なリハビリテーションによって,IADL,とくに目標として多かった洗濯において介入効果が得られた.在宅での生活行為を詳細に分析することで支援ポイントが明確となり,環境への介入等によって認知機能は変化せずとも自立度を高める可能性が示された.また認知機能が低下しても残存する工程を生かすことで在宅生活の継続の一助となり得る.(著者抄録)

Objectives: Occupation-based intervention (OBI) involves daily and meaningful activities for evaluation and intervention. Recently, the “aid for decision-making in occupation choice for hand” (ADOC-H) was developed to facilitate OBI in patients with hand injuries. We aimed to examine the efficacy of OBI using the ADOC-H combined with physical function-based interventions (PBI) for patients with distal radius fractures (DRF). Material and methods: Patients with DRF were retrospectively allocated to two groups, ADOC-H group (n = 14) and PBI group (n = 14), and compared. Results: Improvements in the Pain Catastrophizing Scale magnification and Hospital Anxiety and Depression Scale and Disabilities of the Arm, Shoulder, and Hand questionnaire scores were significantly higher in the ADOC-H group than in the PBI group (p <.05). The groups showed no differences in measure of physical function, such as range of motion and grip strength. Conclusion: OBI using the ADOC-H combined with PBI is clinically useful for patients with DRF as it promotes use of the injured hand for daily activities in a step-by-step approach, improving psychological difficulties in using the hand.

認知的,精神心理的,社会的フレイルへの対策では,身体的側面を含めた包括的な介入が基本となり,多職種による集学的なアプローチが軸となる。そのなかでも認知的,精神心理的,社会的側面への支援では,個々人の生活歴や価値観,環境要因などを考慮する必要があるため,とくに,作業療法の専門性を活かしたアプローチは有効と考えられる。作業療法では,認知機能障害に伴う生活行為の問題や精神心理症状といった個別性の高い生活課題への支援や,個々人にあった社会参加の促進などが重要であろう。また,地域や集団に対しても生活行為に焦点をあて,個々人の生活状況や価値観,目標を取り入れた作業療法アプローチは有益と考えられる。しかし,本領域における作業療法のエビデンスは不足しているため,今後の積極的な作業療法の効果検証が求められる。(著者抄録)

【はじめに・目的】我々は、足指把持力の評価において懸念すべき点を考慮した「立位での足指圧迫力」の評価方法を考案した。これまでに、筋力評価としての妥当性（釜﨑，2020）や転倒リスクを判別する臨床的有用性を報告してきた（釜﨑，2020）。しかし、足指の筋力評価において、どちらがより有用な評価方法なのかは明らかにされていない。そこで本研究の目的は、足指把持力と立位での足指圧迫力のどちらが重心移動能力と関係するのかを検証し、足指筋力のより有用な評価方法を示すこととした。ひいては、重心移動能力の向上を目的とした理学療法に寄与すると考える。

【方法】本研究は横断研究である。対象は、某大学のリハビリテーション学部生とした。リクルートは呼びかけおよびチラシの掲示で行った。なお、対象者の中に国内外トップレベルの競技者はいなかった。重心移動能力は、重心動揺計を用いて、A-P軸の最大重心移動距離によって評価した（Fukuyama K, 2013）。立位での足指圧迫力は、足指押力測定器を用いて評価した。踵は浮かさず、足指で床を圧迫するように力を入れることを指示した。なお、測定時の重心移動は許可しているが、機械の特性上垂直方向に足指で力を加えないと測定値は上昇しない構造になっている。また、足指把持力、握力、膝伸展筋力、navicular dropも評価した。統計処理は、各測定項目の相関をPearsonの相関分析で検討した後、重心移動能力を従属変数とした重回帰分析を実施した。model 1は立位での足指圧迫力と足指把持力を独立変数に投入し、model 2では共変量と考えられる変数を投入し交絡を調整した。重回帰分析の必要サンプルサイズは、効果量“large”、αエラー0.05、検出力0.8、独立変数6に設定した結果46名であった。

【結果】分析対象者は、必要サンプルサイズを満たす成人67名（19±1歳、男性64%）であった。相関分析の結果、重心移動能力と有意な相関があったのは立位での足指圧迫力のみであった（r=0.36, p=0.003）。次に、握力、膝伸展筋力、性別、年齢で調整した重回帰分析の結果、重心移動能力には立位での足指圧迫力のみが関係した（標準回帰係数：0.42, p=0.005）。

【考察】足指把持力は、バランス能力に関係しないと報告されており（Uritani D, 2016, Yamauchi J, 2019）、同様の結果が示された。一方、重心移動能力と立位での足指圧迫力には有意な相関があった。したがって、重心移動能力には足指把持力よりも立位での足指圧迫力の方が寄与しているという我々の仮説が支持された。重回帰分析では、立位での足指圧迫力のみが重心移動能力と有意に関係することが明らかになった（model 2）。この結果は、動員される筋が異なるためであると推察する。足指把持力は外在筋の筋力を反映する（Uritani D, 2016）が、立位での足指圧迫力は内在筋を評価していると考える。つまり、立位での足指圧迫力は、よりバランス能力を捉える内在筋を評価していることから、重心移動能力に関係したと推察する。本研究によって、健康成人の重心移動能力には、足指把持力よりも立位での足指圧迫力が関係し、立位での足指圧迫力を評価する臨床的有用性が示された。ひいては、立位での足指圧迫力を増強することで、重心移動能力を向上させる可能性が示唆された。

【説明と同意、および倫理的配慮】対象者には、研究の内容と目的を説明し、理解を得たうえで同意を求めた。本研究への参加は自由意志であり、参加を拒否した場合でも不利益にならないことを説明した。また、対象者は大学生であったため、成績には影響しないことを説明した。本研究は西九州大学倫理審査委員会の承認（21HUR23）を得て実施した。

65歳以上の地域住民を対象に、心臓足首血管指数(CAVI)と自己報告による睡眠の時間・質の関連性を横断的に調査した。2018～2019年に健康診断を受けた鹿児島県垂水市の高齢者コホート(垂水研究)から、65歳以上の997名(女性622名、年齢中央値74歳)を解析に組み入れた。睡眠時間で3群に分類した。その結果、睡眠時間が8時間以上であった長睡眠群は110名(女性56名、年齢中央値77歳)、6～8時間の中睡眠群は438名(女性269名、年齢中央値74歳)、6時間未満の短睡眠群は449名(女性297名、年齢中央値73歳)であった。この3群で平均CAVIを比較すると、長睡眠群では9.6±1.3となり、中睡眠群(9.1±1.2)または短睡眠群(9.1±1.1)よりも有意に高かった(P<0.001)。各種の因子(年齢、性別、収縮期血圧、現喫煙状態、BMI、運動頻度、学歴、フレイル、睡眠薬、睡眠の質、昼寝時間)で調整を行った多変量回帰分析の結果、長睡眠群のCAVIは中睡眠群よりも有意に高い(p=0.016)ことが証明された。こうした結果とは対照的に、睡眠の質とCAVIの有意な関連性は示されなかった。本解析結果から、高齢者では睡眠時間が長いことが動脈硬化度と相関し、睡眠の質は相関しないことが示唆された。

Older adults are more likely to have age-related sleep problems, which may result in the reduction of cognitive functions. This study was designed to examine the relationship between sleep onset problem and subjective cognitive complaints (SCC) among community-dwelling older adults during the coronavirus disease 2019 pandemic. In this study, 186 older adults aged 65 and above were enrolled and were instructed to respond to an online survey. This survey comprised questions regarding sleep quality (four items such as sleep duration, use of sleep medication), SCC (six domains), and sociodemographic information (eight items such as age, gender, stress condition). We classified the participants into two groups according to the presence or absence of sleep onset problem and examined the relationship between each SCC domain. The sleep onset problem (+) (n = 70) group had significantly higher frequency of scheduled memory decline, misplacement, disorientation in time, word recall decline, and forgetfulness. Furthermore, the sleep onset problem affected the participants’ scheduled memory after adjusted for potential covariates (OR, 2.28; 95%CI, 1.13–4.73; p = 0.02). Older adults with sleep onset problem may need to be evaluated for SCC and supported in term of both sleep status and SCC.

日本の24施設の外来および通所認知症高齢者143例(男性39例、平均83.18±7.19歳)を対象とした横断研究を実施し、認知機能障害の重症度と基本的日常生活動作プロセスとの関連について検討した。認知機能障害の重症度により、軽度(ミニメンタルステート検査20点以上)53例、中等度(10点以上20未満)73例、重症(10点未満)17例に分けて検討した。評価項目は認知症に対する生活行為工程分析表による基本的日常生活動作とした。その結果、基本的日常生活動作能力は、認知機能障害の重症度が高くなるにつれて有意に低下していた。実施可能な基本的生活動作の割合は軽度で58～100%、中等度で38～97%、重度で0～88%であった。一部の基本的日常生活動作プロセスは認知機能障害の重症度と関連していなかった。以上から、認知症患者の詳細な基本的日常生活動作評価は、過剰な介護を回避するための介護者教育や基本的日常生活動作障害の予測に有用であることが示唆された。
