Background: Mobile devices have been used by many older adults and have the potential to assist individuals with subjective cognitive complaints (SCCs) in daily living tasks. Financial management is one of the most complex daily activity for older adults, as it is easily impaired in the prodromal stage of dementia and cognitive impairment. Aim: To investigate financial management ability among older adults from SCCs and mobile device proficiency. Methods: A self-administered questionnaire was sent to 529 participants who were ≥ 65 years and regularly use mobile devices. Participants were divided into four groups based on SCC prevalence and scores of the Mobile Device Proficiency Questionnaire (MDPQ-16). Financial management abilities were compared between groups using the Process Analysis of Daily Activities for Dementia subscale. Regression model and crosstabulation table were used to investigate associations in detail. Results: A significant difference in financial management ability was observed among the four groups (p < 0.001), with the dual impairment group showed significantly lower than the robust and SCC groups (p < 0.001). Educational history, sex, and MDPQ-16 score were significantly associated with participants’ financial management ability (p ≤ 0.01). The proportion of participants who could use ATMs and electronic money independently was significantly lower among those with low proficiency in mobile devices (LPM), regardless of SCC (p < 0.05). Conclusion: The LPM group showed an impaired ability to manage their finances, particularly in situations where they would use information devices. Therefore, healthcare professionals should assess not only the SCC but also their proficiency with mobile devices to predict their impairment in activities of daily living.

Aim: In response to the growing popularity of mobile devices among older adults in Japan, this study aimed to establish a reliable and valid measure of mobile device proficiency by developing a Japanese version of the Mobile Device Proficiency Questionnaire (MDPQ-J) for the Japanese population. Methods: To evaluate the reliability and validity of the MDPQ-J, we administered the questionnaire to 100 young or middle-aged participants (37.78 ± 13.90 years, 57.00% women), and 62 older participants (75.90 ± 6.01 years; 59.68% women). Results: The MDPQ-J showed a high degree of internal consistency (Cronbach's α = 0.98). Moreover, MDPQ-J scores in older participants tended to be lower than those in young or middle-aged participants. In terms of validity, the MDPQ-J score of older participants was significantly associated with age (ρ = −0.31), the amount of time using a mobile device per day (ρ = 0.64), and the system usability scale (ρ = 0.39). Additionally, the MDPQ-J scores were significantly positively associated with the importance (ρ = 0.37), performance (ρ = 0.57), and satisfaction (ρ = 0.29) associated with daily mobile device use in older participants. Conclusions: The high reliability and validity of the MDPQ-J in the Japanese population highlight its utility in adapting mobile devices for older adults, which is crucial in an increasingly digital society. This study explored the longitudinal connection between dialysis therapy in older adults and the incidence of cognitive impairment, and trends of general abilities. We conducted a retrospective propensity score matching (PSM) analysis of data from the care-needs certification survey in City A, Japan, spanning 2017 to 2021. Our sample comprised 11 443 initially cognitively intact older adults from the baseline year 2017. Among them, individuals had received dialysis therapy at baseline, and 216 individuals were included after PSM. The dialysis group had a significantly lower incidence of cognitive impairment during follow-up than the control group (P = 0.02). The competing risk of death was significantly higher in the dialysis group than in the control group (P <.001). Both groups showed a decline in their comprehensive ability over the time. There have been several reports of a negative association between dialysis and cognitive impairment, but more detailed investigation of the longitudinal relationship may be needed. Comprehensive support remains critical for both dialysis and non-dialysis older adults.

Purpose: The purpose of the study was to investigate the effects of satisfaction with meaningful activities and their performance on the memory function among community-dwelling older adults 3 years later. Methods: A longitudinal analysis was conducted on 288 community-dwelling older adults aged 65 years and more who had completed the survey in 2019 and 2022 as participants in the Tarumizu Study. Meaningful activities were assessed in 2019 using the Aid for Decision-Making in Occupation Choice for activities. Satisfaction with and performance of meaningful activities were assessed, and the bottom 25% were classified into two groups: low satisfaction and low performance. Memory function was assessed using the National Center for Geriatrics and Gerontology-Functional Assessment Tool in 2019 and 2022. Changes in memory function over 3 years were analyzed by satisfaction and performance of meaningful activities. Results: Two-way analysis of variance with group (satisfaction with and performance of meaningful activities) and time (2019 and 2022) as factors showed no statistically significant group-time interaction for memory in the satisfaction with meaningful activities group (F = 1.4, p = 0.24). Meanwhile, a statistically significant group-time interaction was observed for memory in the meaningful activity performance group, indicating that high performance of meaningful activities may play a protective role against memory decline (F = 7.4, p = 0.007). Conclusion: Higher performance of meaningful activities may protect against age-related changes in memory. A careful assessment of meaningful activities, and engagement that enhances performance may be useful in designing health support against memory decline.

With the rapid development of information and communication technology, smartphone ownership has increased among older adults. While previous research has examined the impact of smartphone use on the health of older adults, this study focuses on the less-explored relationship between smartphone use, proficiency, and health-related outcomes including higher-level competence. The study was a cross-sectional survey of 208 community-dwelling older adults who used smartphones daily. Smartphone use, including years of use and applications, was assessed and categorized by age group. Health-related outcomes were measured, including cognitive and psychological function, higher-level competence, basic daily abilities, and physical function. The results indicated that smartphone proficiency decreased with age, with older adults primarily using basic smartphone applications. Notably, higher smartphone proficiency was positively associated with higher-level competence and physical functions. The study underscores the importance of considering smartphone proficiency when assessing older adults’ life skills and physical performance in our digital society.

Purpose: It has recently been recommended that Rate of Force Development (RFD) be evaluated in addition to maximal muscle strength. There are no studies on RFD of toe pressure strength, and its importance in older adults and the extent to which it is associated with aging needs to be clarified. This study purpose was to examine the association between the RFD of toe pressure strength and timed up and go test (TUG) in an age-specific study. Methods: This study is a cross-sectional study. Participants in the study included 159 younger adults (26.3 ± 13.1 years, 52% male) and 88 older adults (75.0 ± 6.2 years, 26% male). The RFD of toe pressure strength was determined from the force–time curve obtained during the toe pressure strength assessment, and the ability to exert maximum muscle force in the shortest possible time was assessed. Regression analysis was performed for each group to test the association between RFD of toe pressure strength and TUG by age. Results: Younger adults showed no association between TUG and RFD of toe pressure strength, and significant association between TUG and RFD of toe pressure strength was found only in the older adults (standard regression coefficient = − 0.19, p = 0.048). Conclusion: This study showed a significant association between TUG and RFD of toe pressure strength in older adults. These findings show that RFD is one of the functions that should be assessed, particularly in older adults. Furthermore, it was suggested that approaching RFD could improve gait, standing, and sitting movements.

[目的]高齢の慢性期脳卒中片麻痺患者を対象に,杖使用の有無を判別する身体機能について検討することを目的とした。[対象と方法]対象は,高齢の慢性期脳卒中片麻痺患者とした。居住地域を含む基本情報に加えて,握力,膝伸展筋力,30-second chair stand test(30CST),開眼片脚立ち時間,最大歩行速度,timed up and go testを測定した。分析には,杖使用の有無を従属変数,居住地域を変量効果とした一般化線形混合モデルを用いた。[結果]分析対象者は,高齢の慢性期脳卒中片麻痺患者206名(平均年齢77±7歳,女性49%)であった。分析の結果,杖使用の有無には麻痺側の膝伸展筋力(推定値-0.13,p=0.049)と30CST(-0.18,p=0.013)が関係することが明らかになった。[結語]高齢の慢性期脳卒中片麻痺患者の杖使用の有無には,下肢筋力が関係することが明らかになった。杖の処方を検討する際には,下肢筋力に着目する必要性が示唆された。(著者抄録)

Background: Considering concerns about conventional toe grip strength, we devised a method to measure toe pressure strength in the standing position, which is close to the actual motion. This study examined the association between toe pressure strength in the standing position and walking speed among older adults. Methods: This cross-sectional study included 150 communi-ty-dwelling older adults (81±8 years, 73% female) who participated in the physical fitness test. We analyzed the correlation between the participants’ maximum walking speed and physical function. Furthermore, we performed regression analysis with the maximum walking speed as the dependent variable to examine the association with toe pressure strength in the standing position. We also examined the association between maximum walking speed and toe pressure strength in the standing position by introducing a covariate. Results: Correlation analysis revealed a significant positive correlation between maximum walking speed and toe pressure strength in the standing position, with a moderate effect size (r=0.48, p<0.001). Moreover, multiple regression analysis with covariates showed an association between maximum walking speed and toe pressure strength in the standing position (standardization factor=0.13, p<0.026). Conclusion: Toe pressure strength in the standing position was associated with maximum walking speed. This finding clari-fies the significance of assessing toe pressure strength in the standing position and suggests that enhanced toe pressure strength in the standing position may increase maximum walking speed.

Background and Objectives: Advancing age might impair real-world navigation ability. The use of mobile devices by older adults has grown rapidly in recent years. Navigation applications (apps) in mobile devices may facilitate the freedom of outings for older adults. Our aim is to investigate age-related differences in real-world app-based navigation walking in terms of accuracy, efficiency, and gaze behavior; and to explore clinical factors associated with navigation walking in older adults. Research Design and Methods: A total of 20 community-dwelling older adults and 16 young adults completed a route navigation task using a navigation app while recording their gaze behavior. Outcomes were compared in both groups and a general linear regression was used to explore clinical factors associated with app-based navigation walk in older adults. Results: Compared with young participants, older participants had more stops and root errors and less fixation time, smaller amplitude of saccades. Additionally, older adults were more likely to glance at their smartphones while app-based navigation walking. Furthermore, gait speed and the following assessment scores were significantly associated with navigation walking in older adults: Mini-Mental State Examination, Life-Space Assessment, and the short version of the Mobile Device Proficiency Questionnaire. Discussion and Implications: For app-based navigation walks, differences in accuracy and gaze behavior were found to exist with age. Additionally, efficient real-world navigation walks in older adults require the extent of life space and proficiency with mobile devices, along with walking speed and cognitive function. It is possible that age-related functional decline, such as the visual field and shifting attention between mobile devices and the real world, may have influenced the results. The study also suggests the need to understand the level of proficiency with mobile devices so that older adults can continue to go out freely. These findings give the basis for providing older adults with appropriate navigation assistance. Translational Significance: Real-world navigation ability is known to decline with age. Many older adults are now using mobile devices, but little is known about their accuracy and gaze behavior while using navigation applications. Our study revealed that older participants exhibited more stops and errors, smaller saccade angles and fixation durations, and glanced at smartphones more frequently than younger participants during navigation tasks. Efficient navigation was associated not only with cognitive and physical function but also with life space and mobile device proficiency. Our findings provide valuable insights for enhancing the use of technology by older adults and improving their navigation in the real world.

Background: Various functions are involved in prefrailty. However, no studies have examined more relevant functions. Therefore, this study examined the domains of the Kihon Checklist (KCL) associated with prefrailty by comparing them to robustness measures, using the KCL to comprehensively assess life-related functions in community-dwelling older adults. Methods: The 194 (mean age, 75±6 years) participants were community-dwelling older adults. Their robustness and preferences were assessed using the Japanese Cardiovascular Health Study criteria. Comprehensive life-related functions were assessed using the KCL, and each physical function was measured. Results: The main KCL characteristics associated with robustness and prefrailty were physical function (odds ratio [OR]=1.83; 95% confidence interval [CI], 1.17–2.88), nutritional status (OR=8.16; 95% CI, 2.96–22.48), and depressed mood (OR=3.46; 95% CI, 1.76–6.79). In particular, older adults had difficulty moving, including climbing stairs and getting up from a chair, which suggested a strong fear of falling. The participants also reported psychological characteristics such as low life fulfillment, a low sense of self-usefulness, and a strong sense of boredom. Conclusions: Prefrail individuals were characterized by poor physical function and nutritional status, as well as depressive mood. Prefrailty may be prevented or improved by approaches to improve physical function and fear of falling in addition to psychological interventions that encourage activity and a sense of self-usefulness.

垂水研究に参加した高齢者を対象として、墓参りと抑うつ症状およびアパシーとの関連を調べた。評価項目は高齢者うつ尺度、墓参りに関する項目(墓参りの有無、頻度、移動時間、移動手段、供花)などとした。高齢者を墓参状況により、頻繁群240名(週1回以上、平均74.9±6.4歳、女性170名)、不定期群346名(週1回未満、平均73.8±6.4歳、女性195名)、非墓参群52名(墓参せず、平均74.6±7.1歳、女性32名)に分けて検討した。その結果、非墓参群は墓参群と比較して、アパシー有病率が有意に高かった。潜在的共変量調整後のポアソン回帰分析の結果、非墓参群はアパシーと有意に関連していた。以上のように、高齢者において墓参りをしないこととアパシーには有意な関連があることが示された。

Purpose: We believe that toe pressure strength in the standing position, which is closer to the actual movement, is more associated with standing up in the older adults than the conventional toe grip strength. Therefore, the purpose of this study is to examine the association between toe pressure strength in the standing position and the presence of assistance in standing up in the older adults. Methods: Ninety-five community-dwelling older adults (82 ± 8 years old, 72% female) were included in this study. The patients were evaluated based on their need for assistance in standing up. Physical functions, including toe pressure strength in the standing position, toe grip strength, hand grip strength, knee extension strength, one-leg standing time with eyes open, and maximal walking speed, were measured. Results: When compared with and without assistance to stand up, the group requiring assistance had weaker toe pressure strength in the standing position than the group without assistance (p = 0.015, ES = 0.53). After adjusting for confounding factors, the final model revealed that toe pressure strength in the standing position was associated with the use of assistance in standing up (odds ratio 0.94 [0.88–0.99, p = 0.025]). Conclusion: Toe pressure strength in the standing position was associated with the use of assistance in standing up in older adults. Improving toe pressure strength in the standing position may facilitate the ability of older adults to stand up.
