Aim: We aimed to visually evaluate the place of articulation of palatalized articulation (PA) using electropalatography (EPG) and to analyze the involvement of fistula and velopharyngeal function (VPF) as factors in the occurrence of PA. Methods: Ten patients with postoperative cleft palate and auditory PA were included. The test sounds were three alveolar sounds, [t], [d], and [s], with [a] inserted before each consonant and [a], [e], and [o] inserted afterward. The test sounds were consecutively recorded five times using the EPG, and a cumulative frequency pattern was created by accumulating the maximum contact frames at the time of each consonant production to visually determine the PA. The place of articulation in the cumulative frequency pattern was subsequently evaluated in detail. The relationship between the place of articulation and fistula and VPF was statistically analyzed. Results: All 10 patients had PA for any of the tested sounds. The place of articulation was divided into three patterns: anterior, middle, and posterior. The relationship between the place of articulation and the fistula for PA showed a significant difference between the groups with and without a fistula (U = 0, p = 0.0167, p < 0.05). The relationship with VPF showed no statistically significant differences. Conclusion: Using EPG to visually evaluate the place of articulation of the PA, we could divide it into three positions. Furthermore, there was an association between the place of articulation and fistula, suggesting that the place of articulation is more posterior in the presence of a fistula.

Clinical and statistical observations were made on 91 patients with cleft lip and palate （CLP patients） who underwent orthognathic surgery between 1981 and 2020 at the Department of Oral and Maxillofacial Surgery, Kagoshima University Hospital, and compared to 410 patients with jaw deformities without CLP （non-CLP patients）. The results were as follows.<br>　1. The male-to-female ratio of CLP patients was 1.1：1, with no gender difference, and the average age was 17.5 years. In contrast, the male-to-female ratio of non-CLP patients was 1：2.4, and the average age was 22.2 years.<br>　2. Regarding the number of CLP patients by clinical diagnosis, mandibular prognathism was observed in 37 cases （40.7 %）, the combination of mandibular prognathism and maxillary retrognathism in 31 cases（34.1 %）, and maxillary retrognathism in 18 cases（19.8 %）. On the other hand, in non-CLP patients, mandibular prognathism was observed in 352 cases（85.9 %）, the combination of mandibular prognathism and maxillary retrognathism in 24 cases（5.9 %）, and maxillary retrognathism in 9 cases（2.2 %）. Patients with CLP tended to have a higher rate of maxillary retrognathism.<br>　3. Regarding the number of CLP patients by procedure, sagittal split ramus osteotomy （SSRO） was performed in 37 cases（40.7 %）, the combination of Le Fort I osteotomy and SSRO in 31 cases（34.1 %）, and Le Fort I osteotomy in 16 cases（17.6 %）. On the other hand, in non-CLP patients, SSRO was performed in 344cases（83.9 %）, the combination of Le Fort I osteotomy and SSRO in 34 cases（8.4 %）, and Le Fort I osteotomy in 7 cases（1.7 %）. Patients with CLP tended to have a higher rate of Le Fort I osteotomy.<br>　4. In CLP patients, the average duration of surgery was 5 hours and 28 minutes and blood loss was 383 g. In non-CLP patients, the average duration of surgery was 4 hours and 20 minutes and blood loss was 255 g.<br>　CLP patients had significantly longer operative times and more blood loss than non-CLP patients. In particular, blood loss was 3.1 times higher for maxillary single operations. These results suggest that orthognathic jaw surgery in patients with CLP may take longer and be more invasive than in non-CLP patients. Thus, it is considered more important to study and pass on surgical techniques in orthognathic surgery for CLP patients.

The aim of this retrospective cohort study was to compare the recurrence rate and speech outcomes between two techniques for palatal fistula closure of cleft palate (CP). Patients with CP who underwent secondary palatal fistula closure using the single hinge-flap method with double-breasted mattress suture (hinge-flap group) and those who were treated with the conventional sliding palatal flap method (sliding-flap group) were retrospectively evaluated for demographic and perioperative variables. Recurrence rate of palatal fistula, perceptual speech outcomes, and nasalance scores were further reviewed in patients who met the inclusion criteria. A total of 31 patients, 21 in the hinge-flap group and 10 in the sliding-flap group, were included in this study. The fistula recurrence rate in the hinge-flap group (0%) was significantly lower than that in the sliding-flap group (30.0%) (P = 0.027). In the speech assessment, hypernasality and nasalance scores decreased post-operatively in both groups and significance was observed in the hinge-flap group (P = 0.013, P < 0.001, respectively). Articulation disorders were significantly improved in the hinge-flap group (P = 0.001). Within the limitations of the study it seems that the single hinge-flap method with double-breasted mattress suture should be preferred whenever appropriate.

Nasolabial cyst is a non-odontogenic developmental soft tissue cyst occurring in the periphery of the lip and nose, and is considered synonymous with nasal alveolar cyst according to the second edition of Histological Typing of Odontogenic Tumours by the WHO （1992）. Various theories have been proposed as to the etiology of this disease, including the retention cyst theory, the fissural cyst theory, and the embryonic nasolacrimal duct epithelial remnant theory, but none have been formulated. In the present study, we report a case of nasolabial cyst in the nasal vestibule of a patient who underwent a series of surgical procedures for unilateral complete cleft lip and alveolar, including labioplasty, secondary lip and nasal correction, and secondary bone grafting in the alveolar cleft.
