Non-small cell lung cancer (NSCLC) is the most frequent cause of cancer-related death worldwide. Although many molecular-targeted drugs for NSCLC have been developed in recent years, the 5-year survival rate of patients with NSCLC remains low. Therefore, an improved understanding of the molecular mechanisms underlying the biology of NSCLC is essential for developing novel therapeutic strategies for the treatment of NSCLC. In this study, we examined the role of miR-130b in NSCLC. Our results showed that high expression of miR-130b in clinical specimens was significantly associated with poor overall survival in patients with NSCLC. Moreover, miR-130b expression was significantly increased in NSCLC clinical specimens from patients with vascular and lymphatic invasion. Consistent with this, overexpression of miR-130b promoted invasion and matrix metalloproteinase-2 (MMP-2) activity in A549 cells. Argonaute2 immunoprecipitation and gene array analysis identified tissue inhibitor of metalloproteinase-2 (TIMP-2) as a target of miR-130b. Invasion activity promoted by miR-130b was attenuated by TIMP-2 overexpression in A549 cells. Furthermore, TIMP-2 concentrations in serum were inversely correlated with relative miR-130b expression in tumor tissues from the same patients with NSCLC. Overall, miR-130b was found to act as an oncomiR, promoting metastasis by downregulating TIMP-2 and invasion activities in NSCLC cells.

A 46-year-old woman was found to have an aneurysm of the superior segmental pulmonary artery in the right lower lung lobe on computed tomography images. Moreover, angiography revealed dilated bronchial arteries flowing into the aneurysm with neovascularization, and the contrast medium was partially pooled in the basal segment of the same lobe. The patient’s hemoptysis could not be controlled by an interventional radiology procedure. Therefore, lobectomy was carried out instead of aneurysmectomy. There has been no recurrence for 4 years after surgery. We considered that that angiographic information allowed for the most appropriate operation in this case.

A tracheocutaneous fistula may develop when a tracheostomy orifice epithelializes during a prolonged course of healing or undernutrition. Various techniques for closing such fistulae have been reported. However, a standard procedure has not yet been established. We, herein, present a case involving a 35-year-old woman who developed a tracheocutaneous fistula after tracheostomy. We closed the fistula using two skin flaps to cover the tracheal lumen and skin defect, respectively. The advantage of this technique is that it allows the tracheal lumen to be covered by inversed skin epithelium and ensures that the suture line of the skin does not match up with that of the subcutaneous tissue.

An 82-year-old man underwent a left upper lobectomy for a solitary tumor on suspicion of lung cancer. Histopathologic findings of the resected specimen showed clear cell renal cell carcinoma, which was diagnosed as a metastasis from kidney cancer concealed for 31 years after nephrectomy. The Ki-67 labeling index of the metastatic tumor was high (36.1%). A few cases of recurrent renal cell carcinoma after a long interval from initial diagnosis have been seen. However, pulmonary metastasectomy more than 30 years after radical nephrectomy for renal cell carcinoma has not been reported. This remarkable case provides new and valuable clinical insights into metastatic renal cell carcinoma.

A 44-year-old man with locally advanced central lung cancer was treated by right lower lobe lung autotransplantation after pneumonectomy using an extracellular phosphate-buffered solution for cold lung preservation. The advantage of the ex vivo operation made it possible to perform safe and definitive cancer resection without massive bleeding. Cold lung preservation brought some advantages, such as reducing the risk of ischemia-reperfusion injury compared with warm ischemia and allowing enough time to achieve microscopically negative margins. The patient returned to normal life with minimum loss of pulmonary reserve and no recurrence of lung cancer for 6 months.

重症筋無力症(MG)に対し両側胸腔アプローチによる胸腔鏡下拡大胸腺摘出術を行った9例(男性4例、女性5例、平均年齢64.1歳)について検討した。胸腺腫の合併は7例でみられ、術前画像で非浸潤性胸腺腫と診断したものを当術式の適応とした。更に術前処置としてはステロイド使用歴のある7例にはprednisoloneの連日投与が行われた。その結果、手術時間は平均370分で、術中出血量は平均89ml、術後入院期間は平均8.9日であった。術後経過は合併症として左横隔神経損傷による軽度呼吸苦を1例で認められたが、その後、症状は軽快した。また、MG症状の再燃を1例で認められたが、その他8例はMG症状の再燃や胸腺腫の再発はみられなかった。

Background. Pulmonary hamartoma is the most common benign lung tumor type. Most such cases are parenchymal hamartoma, while some are endobronchial hamartoma. Case. A 40-year-old man presenting with obstructive pneumonia was admitted to our hospital. An endobronchial tumor in the left lower lobar bronchus was observed on chest computed tomography. A bronchoscopic examination revealed a smooth-surfaced polypoid tumor occluding the left lower lobar bronchus. A transbronchial biopsy revealed no diagnostic findings, so bronchoscopic resection using electrosurgical snaring was performed. Accordingly, a histopathological analysis led to the diagnosis of endobronchial cartilaginous hamartoma. Since a tumor obstructing the orifice of the left B6c had persisted, video-assisted left S6 segmentectomy was performed for radical surgery. The postoperative pathological findings revealed that some parts of the endobronchial tumor had expanded to the pulmonary parenchyma. Conclusion. We herein report a case of pulmonary hamartoma that was confirmed to have expanded to the pulmonary parenchyma after bronchoscopic resection for an endobronchial tumor. In some cases of endobronchial hamartoma, expansion to the pulmonary parenchyma occurs, and caution must be exercised in its treatment.

74歳女性。臍ヘルニアの精査目的に施行された胸部CTにて右肺下葉の無気肺、中間気管支幹の内腔を占める病変を指摘され、精査目的に当科へ紹介となった。血液検査では異常所見はみられなかったものの、胸部CTでは右気管支中間幹から下幹内腔に脂肪濃度の腫瘤性病変が確認され、脂肪腫が疑われた。硬性気管支鏡下で右中間幹から下葉気管支に存在する腫瘍を切除したところ、病理組織学的にEndbronchial lipomatous hamartomaと診断され、術後4日目に患者は自宅退院となった。目下、術後4年経過で再発は認められていない。

Background. Mitomycin-C (MMC) reportedly has an effect on granulation suppression, and many cases of tracheal stenosis caused by benign disease have received topical MMC treatment worldwide. However, in Japan, this treatment modality is still rarely used. Case Report. A 31-year-old man received treatment for a fracture of his left femur under general anesthesia when he was 18 years old. He became aware of discomfort in his pharyngeal portion one year previously, and he had wheezing 8 months ago. He went to hospital and was prescribed medicine. However, as no improvement was made, he was finally referred to our Respiratory Medicine department. Bronchoscopy showed scarring stenosis of the entire tracheal circumference approximately 3 cm away from the vocal cord to the peripheral side, so he was referred to us for bronchial intervention. In the operation, we resected scar tissue, and applied MMC to the resected area. Result. The postoperative course was non-eventful and the patient was discharged from hospital one week later. At bronchoscopic examination 3 months after surgery, there was no restenosis by granulation and/or scar tissue hyperplasia. There has been no recurrence 3 years after surgery. Conclusion. Although long-term follow-up is necessary to confirm these results, we believe that the suppressive effect of MMC on granulation hyperplasia could be effective in preventing relapse of tracheal stenosis.

A 63-year-old male with multiple ground-glass nodules (GGNs) in both lung fields detected on CT was followed up over five years. CT indicated that a section of the GGNs was growing. Therefore, left upper lobectomy was performed under a video-assisted thoracoscope to remove three GGNs. The final histological diagnosis for each GGN was inflammatory changes. Limited resection is often adopted for such cases because slow-growing GGNs usually have a low-grade malignant potential even if they are cancerous. However, it is sometimes difficult to obtain a diagnosis and decide on treatment strategies in cases whose multiple GGNs exist inside the lung parenchyma. Furthermore, in some case reports of growing GGNs, pathological findings were not malignant even though radiological images suggested lung cancer. Considering these observations, it is necessary to treat each case individually. In addition, the frequency of growing GGN cases should be clarified throughout Japan.

We report a case of a solitary fibrous tumor (SFT) associated with hypoglycemia. A 54-year-old woman experienced loss of consciousness and dysarthria when awakening. On an investigation, hypoglycemia and a huge intrathoracic mass were detected, which was diagnosed as SFT by needle biopsy. Other endocrinological investigations were normal except for low values of immunoreactive insulin and C-peptide. Since the symptoms were suspected to be related to the tumor, she underwent complete resection of the tumor. After the operation, all symptoms disappeared, and insulin-like growth factor II (IGF-II) was stained positive by immunohistochemistry. High molecular weight IGF-II was also detected from a preoperative blood sample. We propose that high molecular weight IGF-II produced by SFT caused the hypoglycemia. There have been no signs of recurrence for one year since the operation.
