    鼻唇嚢胞は口唇外鼻周辺に発症する非歯原性発育性の軟組織嚢胞で、1992年のWHO分類では鼻歯槽嚢胞と同義語とされていた疾患である。その病因は貯留嚢胞説、顔裂性嚢胞説、鼻涙管原基嚢胞説など諸説あるが定説はない。今回我々は、片側性完全唇顎裂に対して口唇形成術、口唇外鼻修正術、顎裂部自家骨移植術といった一連の手術を施行した患者の鼻前庭部に鼻唇嚢胞が発症した症例を経験したので報告する。(著者抄録)

    舌癌の原因として、鞍状歯列弓による舌への機械的刺激が疑われた症例に対して、歯列弓の側方拡大による矯正歯科治療を行い、良好な結果を得たので報告する。患者は上皮内癌に対する舌部分切除術を行った27歳0ヵ月の女性で、舌癌の要因の一つと考えられる下顎歯列の叢生の改善を希望して受診した。患者の下顎歯列は鞍状歯列弓を呈しており、舌癌発症部位に近接する下顎臼歯部は著しく舌側傾斜していた。歯列弓の側方拡大および非抜歯による矯正歯科治療を行い、下顎歯列の叢生および鞍状歯列弓を改善した。動的治療2年後、クリアリテーナーによる保定を行った。保定4年を経過しているが、舌癌の再発は認めず、良好な咬合を維持している。(著者抄録)

    Nasolabial cyst is a non-odontogenic developmental soft tissue cyst occurring in the periphery of the lip and nose, and is considered synonymous with nasal alveolar cyst according to the second edition of Histological Typing of Odontogenic Tumours by the WHO （1992）. Various theories have been proposed as to the etiology of this disease, including the retention cyst theory, the fissural cyst theory, and the embryonic nasolacrimal duct epithelial remnant theory, but none have been formulated. In the present study, we report a case of nasolabial cyst in the nasal vestibule of a patient who underwent a series of surgical procedures for unilateral complete cleft lip and alveolar, including labioplasty, secondary lip and nasal correction, and secondary bone grafting in the alveolar cleft.

    13トリソミーは、Patauによって1960年に報告された、全身奇形を合併する染色体異常疾患である。生命予後は極めて不良で、1歳以内に90%が死亡するとされている。13トリソミーをはじめ、染色体異常症例に対する治療方針は施設によって様々であり、一定の見解が得られていないのが現状である。今回我々は、両側性唇顎口蓋裂を伴った13トリソミー患児に対し、口唇形成術を施行し、術後肺炎の加療を要した一例を報告する。患者は2歳、女児。全身麻酔下にManchester変法による口唇形成術を施行した。1歳2ヵ月時に既に気管切開を受けており、術中の人工呼吸管理は気管切開チューブを利用した。術中の全身状態に大きな問題もなく、術後はICUにて管理を行った。しかし、術後3日目に重篤な肺炎に罹患し、小児科医の協力の下、人工呼吸器管理、カルバペネム系抗菌剤ならびに免疫グロブリン製剤の投与を行い、症状は改善し、術後14日目に退院した。術後1年6ヵ月経過後も全身状態は比較的安定し、創部の状態も経過良好である。(著者抄録)

    オトガイの突出と前歯部反対咬合および叢生を主訴に来院した初診時年齢22歳10ヵ月の女性に外科的矯正治療を行い、良好な治療結果を得たので報告する。患者の大臼歯関係はAngle Class IIIで骨格性下顎前突を呈しており、上下前歯部の叢生および下顎前歯の著しい舌側傾斜を認めた。術前矯正治療では、上顎歯列は第一小臼歯を抜去して叢生を改善し、下顎歯列は非抜歯で治療し、バイヘリックスで側方拡大した後、前歯の唇側傾斜を行った。その後、下顎枝矢状骨切り術で下顎骨を後方移動し、術後矯正治療を行った。治療後は良好な顔貌および咬合となり、顎口腔機能は開閉口路と咀嚼経路が安定した。保定2年5ヵ月を経過しているが、良好な咬合を維持している。(著者抄録)

    【目的】近年,インプラント治療患者数は増加傾向にあり,鹿児島大学病院では高い安全性と専門性の高い診療を担保するために複数の専門分野で連携した口腔インプラント専門外来を設置している.今回我々は今後の当専門外来の体制や開業医との住み分けの検証のため,過去5年間の治療について後ろ向き調査を行ったので報告する.【方法】2013年4月1日から2018年3月31日までの当専門外来受診患者のうち,インプラント体を埋入し,最終上部構造まで装着した症例を対象とした.評価項目は患者状態,埋入数・部位,骨造成の方法,最終上部構造の種類,脱落・撤去本数,他院からの紹介数とした.【結果】当専門外来を受診した総患者数は188名,最終上部構造まで装着した患者数は59名であった.初診時の平均年齢は56.4歳で,60歳代が20名(33.9%)と最も多かった.インプラント体の総埋入本数は167本,1名あたりの平均本数は2.83本で,下顎臼歯部が最も多かった.骨造成は39症例(66.1%)で行われた.最終上部構造は85装置で,材料はフルジルコニアが36装置で最も多かった.脱落したインプラント体は3名,4本で,他院で埋入したインプラント体は13名,14本を撤去した.他院からの紹介患者は51名であった.【結論】当専門外来は骨造成を行った症例の割合が高く,様々な分野を集約した大学病院の高度な医療が求められ,各分野の連携をさらに深めることが重要だと思われた.(著者抄録)

    Trisomy 13 is a chromosomal disorder with systemic malformations reported by Patau in 1960. The prognosis is markedly poor, and 90％ of affected infants do not survive beyond the first year of life. Treatment strategies for patients with chromosomal abnormalities, such as trisomy 13, vary among facilities, and no consensus has been reached. We describe the case of a patient with trisomy 13 and bilateral cleft lip and palate who underwent cheiloplasty and required postoperative treatment for pneumonia. A 2-year-old girl underwent cheiloplasty using a modified Manchester's technique under general anesthesia. She had already received a tracheostomy at the age of 1 year and 2 months, and so the intraoperative respiratory management was performed through the tracheostomy tube. The operation was completed without large fluctuation in her general condition and postoperative management was performed in the ICU. However, on the third postoperative day, she developed severe pneumonia. In cooperation with pediatricians, ventilator management and carbapenem antibiotics and immunoglobulin preparations were administered, her general condition improved, and she was discharged on the 14th postoperative day. At 1 year and 6 months since the cheiloplasty, her general condition is relatively stable and the surgical scar is not conspicuous.

    症例は79歳男性で、関節リウマチ(RA)を発症し、メトトレキサート(MTX)の投与を開始した。両側下顎大臼歯部の歯肉の痛みを自覚するもしばらく放置した。抗菌薬を処方されたが症状が改善しないため、精査加療目的で紹介受診した。右下6番ならびに左下5番頬側歯肉に、接触痛、発赤、歯の動揺を伴う壊死性潰瘍性病変を認め、著明な打診痛がみられた。パノラマX線写真では全顎的な中等度の水平的骨吸収像を認めた。初診から7日後、症状に改善傾向を認めなかったため、右下6番頬側歯肉より生検を施行した。病理組織学的には慢性潰瘍の病態で、大型の異型リンパ球ならびにCD30陽性細胞を認めた。さらに、PET-CT検査を行い、頭頸部領域に多数の集積像を認め、MTX関連リンパ増殖性疾患(MTX-LPD)を含めた腫瘍性病変を疑った。2年前に腰部から採取した生検材料についても再評価し、口腔と皮膚に異時性に発生したMTX-LPDと診断した。MTXを中止し、1ヵ月には潰瘍性病変は消失した。RAの治療は、MTXの代替えとして、サルファ剤とタクロリムスの内服へ変更し、病変の消失から2年経過した現在、再燃なく経過良好である。

    22歳男。半年前から左側下顎骨の膨隆を自覚していたが、疼痛がないため放置してした。口腔内所見として左下歯3〜7相当部歯肉に40×40×30mm大で境界明瞭、骨様硬、表面平滑な膨隆を認めた。パノラマX線では左下歯3〜7にかけて、内部に石灰化物を伴う不定形で境界明瞭なX線透過像を認めた。生検で「骨芽細胞腫の疑い」と臨床診断し、全身麻酔下に下顎区域切除術を行い、切除標本の病理組織所見から骨芽細胞腫と確定診断した。7年前に近医歯科医院で撮影されたパノラマX線写真が入手できたためその観察を行ったところ、左下歯6近心根周囲に境界不明瞭な透過像が認められ、骨芽細胞腫の初期像である可能性が考えられた。

    <p>Osteoblastoma is a rare tumor that accounts for less than 1% of all bone tumors and is characterized by the development of osteoblasts and osteoid tissue. It is classified as an intermediate type tumor with local aggressiveness. The tumor mass is usually larger than 2 cm, and there have been no reports of large osteoblastomas in Japan in recent years. We report the case of a large osteoblastoma, including a description of the previously obtained X-ray films.</p><p>　The patient was a 22-year-old man who visited our hospital with a chief complaint of swelling of the left side of the mandible. The tumor measured 40×40×30 mm, and X-ray films showed a radiolucent image including a calcified body with a clear margin. Osteoblastoma was diagnosed on the basis of two biopsies. Segmental mandibulectomy was performed with the patient under general anesthesia. There has been no recurrence after the operation, and the patient is making good progress. A panoramic X-ray film taken 7 years ago (at 15 years of age) revealed slight bone resorption resembling periodontitis in the region where the tumor had developed. Consequently, there was a high probability that this X-ray film indicated an early stage of this tumor.</p>

    <p>Methotrexate (MTX) has been recognized as a first-line treatment for rheumatoid arthritis (RA) because of its ability to reduce the proliferation of innate immune cells. Recently, the occurrence of methotrexate-associated lymphoproliferative disorders (MTX-LPD) in the oral cavity has been reported. We describe a case of MTX-LPD associated with asynchronous ulcers in the oral and skin regions that was diagnosed by oral lesions. A 79-year-old man had been receiving MTX for over 7 years as a treatment for RA. Two years earlier, a skin ulcer had developed in the lumbar region, which histopathologically diagnosed as a sarcoma, and he was treated with heavy-ion radiotherapy. Before he presented to our institution, he had severe bilateral gingival pain in the mandibular molar region for 2 months. The oral lesions appeared to be necrotic ulcers, and a pathological examination revealed lymphocytic infiltration involving atypical cells. Based on the patient's medical history and the results of a histopathological reexamination of the skin lesions, both the oral and skin lesions were diagnosed as MTX-LPD. Although the gingival lesions expanded over the next few weeks, the ulcers spontaneously disappeared after MTX treatment was withdrawn. There have been no signs of recurrence for 2 years.</p>

    上皮細胞は病原微生物の侵入に対して、最前線に位置し、物理的な生体防御機能を有している。病原微生物による上皮の崩壊、宿主の自己防衛免疫機構の障害ならびに病原微生物の感化によって、感染症は惹起される。歯根嚢胞は、感染根管由来の炎症性物質を介して、根尖部歯周組織に形成される感染症の一つである。根管治療を施しても、治療困難な程増大した歯根嚢胞に関しては、歯根端切除術あるいは抜歯が余儀なくされ、近代に至っても歯牙の予後に多大な影響を与えている。歯性感染症の中では比較的発症頻度の高い疾患であり、その発生は歯原性残存上皮であるMalassez上皮遺残の増殖に由来すると考えられているが、その発症機序、増殖機構に関しては未だ、不明な点が多い。本稿では、歯性感染病巣における上皮形成の意義について検討し、歯原性疾患についてのさらなる理解の可能性を考察する。(著者抄録)
