    Objectives: To evaluate periodontal wound healing following scaling and root planing (SRP) in conjunction with the application of sodium hypochlorite/amino acids and cross-linked hyaluronic acid (xHyA) gels in dogs. Materials and Methods: In four beagle dogs, 2-wall intrabony defects were created and metal strips were placed around the teeth. Clinical parameters were measured 4 weeks after plaque accumulation. The experimental root surfaces were subjected to SRP with either the subgingival application of a sodium hypochlorite/amino acid gel and a xHyA gel (test group) or SRP alone (control group) using a split-mouth design. Clinical parameters were re-evaluated at 6 weeks. The animals were sacrificed at 8 weeks for histological analysis. Results: The test group showed significant improvements in all clinical parameters compared to the control group. Histologically, the test group exhibited statistically significantly greater new bone formation [i.e., length of newly formed bone, new bone area] compared with the control group (p < 0.05). Furthermore, statistically significantly greater formation of new attachment [i.e., linear length of new cementum adjacently to newly formed bone with inserting collagen fibers] and new cementum was detected in the test group compared with the control group at 8 weeks (p < 0.05 and p < 0.01, respectively). Conclusion: The adjunctive subgingival application of sodium hypochlorite/amino acid and xHyA gels to SRP offers an innovative novel approach to enhance periodontal wound healing/regeneration. Clinical relevance: The present findings have for the first-time shown histologic evidence for periodontal regeneration in support of this novel treatment modality.

    The relationship between oral health and the development of Alzheimer’s disease (AD) in the elderly is not yet well understood. In this regard, the association between aging or neurodegeneration of the trigeminal nervous system and the accumulation of amyloid-β(1–42) (Aβ42) oligomers in the pathogenesis of AD is unknown. We focused on selective autophagy in the trigeminal mesencephalic nucleus (Vmes) and the diffusion of Aβ42 oligomers with respect to aging of the trigeminal nervous system and whether the degeneration of Vmes neurons affects the diffusion of Aβ42 oligomers. We used female 2- to 8-mo-old transgenic 3xTg-AD mice and App NL-G-F knock-in mice and immunohistochemically examined aging-related changes in selective autophagy and Aβ42 oligomer processing in the Vmes, which exhibits high amyloid-β (Aβ) expression. We induced degeneration of Vmes neurons by extracting the maxillary molars and examined the changes in Aβ42 oligomer kinetics. Autophagosome-like membranes, which stained positive for Aβ, HO-1, and LC3B, were observed in Vmes neurons of 3xTg-AD mice, while there was weak immunoreactivity of the membranes for intraneuronal Aβ in App NL-G-F mice. By contrast, there was strong immunopositivity for extracellular Aβ42 oligomers with the formation of Aβ42 oligomer clusters in App NL-G-F mice. The expression of Rubicon, which indicates age-related deterioration of autophagy, increased the diffusion of Aβ42 oligomer with the age of Vmes neurons. Tooth extraction increased the extracellular immunopositivity for Aβ42 oligomers in App NL-G-F mice. These results suggest that autophagy maintains homeostasis in Vmes neurons and that deterioration of autophagy due to aging or neurodegeneration leads to the diffusion of Aβ42 oligomers into the extracellular space and possibly the development of AD.

    Background: The establishment of symbiotic microbiota in pregnant women is important for both the mother and her offspring. Little is known about the salivary symbiotic bacteria in pregnancy, and analysis of composition of microbiome (ANCOM) is useful to detect small differences in the number of bacteria. The aim of this study was to investigate the differences in the salivary bacteria between healthy pregnant and non-pregnant women using ANCOM. Methods: Unstimulated saliva samples were collected from 35 healthy pregnant women at 35 weeks gestation and 30 healthy non-pregnant women during menstruation. All participants underwent a periodontal examination. Estradiol and progesterone levels were examined by enzyme-linked immunosorbent assay. DNA extracted from the saliva was assessed by 16S ribosomal RNA amplicon sequencing and real-time PCR. Results: Salivary estradiol and progesterone levels were significantly increased in pregnant women. The alpha and beta diversities were higher in pregnant women than in non-pregnant women. The largest effect size difference noted when the microbiota of the pregnant and non-pregnant women were analyzed was that for Bifidobacteriales. Levels of Bifidobacterium dentium, but not of Bifidobacterium adolescentis, were significantly increased in pregnant women, and the levels were significantly correlated with progesterone concentration. Conclusion: The results suggest that Bifidobacterium and progesterone levels are elevated in the saliva of healthy pregnant women compared with non-pregnant women.

    Aim: To histologically evaluate the effects of cross-linked hyaluronic acid (xHyA) with or without a collagen matrix (CM) on periodontal wound healing/regeneration in class III furcation defects in dogs. Materials and Methods: Class III furcation defects were surgically created in the mandibular premolars in six beagle dogs. The defects were randomly treated as follows: open flap debridement (OFD) + CM (CM), OFD + xHyA (xHyA), OFD + xHyA + CM (xHyA/CM) and OFD alone (OFD). At 10 weeks, the animals were euthanized for histological evaluation. Results: The newly formed bone areas in the xHyA (4.04 ± 1.51 mm2) and xHyA/CM (4.32 ± 1.14 mm2) groups were larger than those in the OFD (3.25 ± 0.81 mm2) and CM (3.31 ± 2.26 mm2) groups. The xHyA (6.25 ± 1.45 mm) and xHyA/CM (6.40 ± 1.35 mm) groups yielded statistically significantly (p <.05) greater formation of new connective tissue attachment (i.e., new cementum, with inserting connective tissue fibres) compared with the OFD (1.47 ± 0.85 mm) group. No significant differences were observed in any of the histomorphometric parameters between the xHyA and xHyA/CM groups. Complete furcation closure was not observed in any of the four treatment modalities. Conclusions: Within their limits, the present results suggest that the use of xHyA with or without CM positively influences periodontal wound healing in surgically created, acute-type class III furcation defects.

    Objectives: The oral cavity is one of the main entry sites for SARS-CoV-2. Gingival keratinocytes express transmembrane serine protease 2 (TMPRSS2), responsible for priming the SARS-CoV-2 spike protein. We investigated whether periodontitis increased the expression of TMPRSS2. Methods: To investigate gene expression in periodontitis, we analyzed the expression of specific genes from (1) the Gene Expression Omnibus (GEO) dataset of 247 human gingival tissues and (2) an experimentally-induced periodontitis mouse model. Human gingival tissues with or without periodontitis were immunohistochemically stained using an anti-TMPRSS2 antibody. Analysis of the TMPRSS2 promoter was performed using a ChIP-Atlas dataset. TMPRSS2 expression was detected in cultured human keratinocytes using quantitative reverse transcription (qRT)-PCR and Western blot analysis. Results: GEO dataset analysis and an experimentally-induced periodontitis model revealed increased expression of TMPRSS2 in periodontitis gingiva. The keratinocyte cell membrane in periodontitis gingiva was strongly immunohistochemically stained for TMPRSS2. Using ChIP-Atlas and GEO datasets, we screened for transcription factors that bind to the TMPRSS2 promoter region. We found one candidate, estrogen receptor 1 (ESR1), highly expressed in periodontitis gingiva. Analysis of the GEO dataset revealed a correlation between ESR1 and TMPRSS2 expression in gingival tissues. An ESR1 ligand induced TMPRSS2 expression in cultured keratinocytes. Conclusions: Periodontitis increases TMPRSS2 expression in the cell membrane of gingival keratinocytes.

    Aim: To evaluate periodontal wound healing/regeneration of one-wall intra-bony defects treated with recombinant human fibroblast growth factor-2 (rhFGF-2) and beta-tricalcium phosphate (β-TCP), carbonate apatite (CO3Ap), or deproteinized bovine bone mineral (DBBM) in dogs. Materials and Methods: The stability of rhFGF-2 adsorbed onto the bone substitutes was evaluated by Enzyme-Linked Immunosorbent Assay (ELISA). One-wall intra-bony defects (5 × 5 × 5 mm) created in five adult male beagle dogs were treated with rhFGF-2 alone (rhFGF-2), rhFGF-2 with β-TCP (rhFGF-2/β-TCP), rhFGF-2 with CO3Ap (rhFGF-2/CO3Ap), or rhFGF-2 with DBBM (rhFGF-2/DBBM). Histological outcomes (e.g., linear length of new cementum adjacent to the newly formed bone with inserting collagen fibres [NA] as the primary outcome) were evaluated at 10 weeks post surgery. Results: Significantly higher amount of rhFGF-2 was adsorbed onto CO3Ap compared with β-TCP. Among the treatment groups, the rhFGF-2/DBBM group showed the highest amount of periodontal tissue regeneration. The rhFGF-2/DBBM group showed significantly greater formation of NA (3.22 ± 0.40 mm) compared with rhFGF-2 (1.17 ± 1.00 mm, p <.01) group. Additionally, new bone area in the rhFGF-2/DBBM group (9.78 ± 2.30 mm2) was significantly higher than that in the rhFGF-2 (5.08 ± 1.26 mm2, p <.01), rhFGF-2/β-TCP (5.91 ± 1.27 mm2, p <.05), and rhFGF-2/CO3Ap (6.51 ± 1.49 mm2, p <.05) groups. Slight ankylosis was found in the rhFGF-2/β-TCP (1/9 sites), rhFGF-2/CO3Ap (3/10 sites), and rhFGF-2/DBBM (1/9 sites) groups. Conclusions: Within their limitations, the present data indicate that DBBM seems to be a suitable carrier for rhFGF-2 and that rhFGF-2/DBBM treatment promotes favourable periodontal regeneration compared with rhFGF-2, rhFGF-2/β-TCP, and rhFGF-2/CO3Ap treatments in one-wall intra-bony defects.

    Objective: The aim of this in vitro study was to investigate the expression of SARS-CoV-2 entry and processing genes in human gingival fibroblasts (HGnF) following treatment with Porphyromonas gingivalis-derived lipopolysaccharide (PgLPS) or inflammatory cytokines/mediators. Design: We assessed the expression of SARS-CoV-2 entry and processing genes; angiotensin-converting enzyme 2 (ACE2), cellular serine proteases transmembrane serine protease 2 (TMPRSS2), Furin, and basigin (BSG) in HGnF by real-time PCR. To further asses the contribution of PgLPS and inflammatory cytokines/mediators to proliferation and SARS-CoV-2 entry and processing gene expression, HGnF were treated with PgLPS, IL1β, TNFα, and PGE2. Results: The expression for ACE2 in HGnF was significantly elevated after PgLPS or IL1β, TNFα, PGE2 treatment. The expression of TMPRSS2 was increased by PgLPS, IL1β, or PGE2 while BSG was elevated by PgLPS and IL1β. The expression of BSG and FURIN decreased after TNFα treatment. Conclusion: SARS-CoV-2 entry and processing genes are expressed in human gingival fibroblasts and their expressions are altered by PgLPS, IL1β, TNFα and PGE2 treatment.

    高血圧患者への降圧薬として用いられるカルシウム拮抗薬は,歯肉増殖症を発症させることが知られている。歯肉増殖症は著しい歯肉の増殖に加え,歯列不正など,審美的な問題を生じることがある。その発症機序については不明な点も多いが,プラークは歯肉増殖症発症の病因に関係している。治療としては,可能であれば内科医へ薬剤の変更依頼,プラークコントロールとスケーリング・ルートプレーニングを中心とした歯周基本治療を行い,必要に応じて歯周外科手術が適応となる。本症例報告患者は高血圧症を有する41歳男性で,カルシウム拮抗薬による薬物性歯肉増殖症を伴う慢性歯周炎(ステージIII,グレードC)と診断され著しい歯肉腫脹,出血,上顎前歯部の動揺と歯列不正をきたしていた。内科医による服用薬の変更後,口腔衛生指導およびスケーリング・ルートプレーニング(SRP)やPMTCなどの歯周インフェクションコントロールを徹底し,再評価後,歯肉増殖は改善されたが,4mm以上のプロービングデプスが残存した。患者が多忙であり歯周外科の同意が得られないため,再SRPと局所薬物配送システムを行った。また,患者には繰り返しプラークコントロール,プロフェッショナルケア,血圧コントロールの重要性を説明し,モチベーションを高めた。その結果,非外科的歯周治療のみで良好な歯周組織状態及び歯列の改善が得られた。(著者抄録)

    Aggregatibacter actinomycetemcomitans is a facultative anaerobic Gram-negative bacterium associated with periodontal diseases, especially aggressive periodontitis. The virulence factors of this pathogen, including adhesins, exotoxins, and endotoxin, have been extensively studied. However, little is known about their gene expression mode in the host. Herein, we investigated whether culture conditions reflecting in vivo environments, including serum and saliva, alter expression levels of virulence genes in the strain HK1651, a JP2 clone. Under aerobic conditions, addition of calf serum (CS) into a general medium induced high expression of two outer membrane proteins (omp100 and omp64). The high expression of omp100 and omp64 was also induced by an iron-limited medium. RNA-seq analysis showed that the gene expressions of several factors involved in iron acquisition were increased in the CS-containing medium. When HK1651 was grown on agar plates, genes encoding many virulence factors, including the Omps, cytolethal distending toxin, and leukotoxin, were differentially expressed. Then, we investigated their expression in five other A. actinomycetemcomitans strains grown in general and CS-containing media. The expression pattern of virulence factors varied among strains. Compared with the other five strains, HK1561 showed high expression of omp29 regardless of the CS addition, while the gene expression of leukotoxin in HK1651 was higher only in the medium without CS. HK1651 showed reduced biofilm in both CS- and saliva-containing media. Coaggregation with Fusobacterium nucleatum was remarkably enhanced using HK1651 grown in the CS-containing medium. Our results indicate that the expression of virulence factors is altered by adaptation to different conditions during infection.

    The periodontal inflamed surface area (PISA) is a useful index for clinical and epidemiological assessments, since it can represent the inflammation status of patients in one contentious variable. However, calculation of the PISA is difficult, requiring six point probing depth measurements with or without bleeding on probing on 28 teeth, followed by data input in a calculation program. More simple methods are essential for screening periodontal disease or in epidemiological studies. In this study, we tried to establish a convenient partial examination method to estimate PISA. Cross-sectional data of 254 subjects who completed active periodontal therapy were analyzed. Teeth that represent the PISA value were selected by an item response theory approach. The maxillary second molar, first premolar, and lateral incisor and the mandibular second molar and lateral incisor were selected. The sum of the PISAs of these teeth was significantly correlated with the patient’s PISA (R2 = 0.938). More simply, the sum of the maximum values of probing pocket depth with bleeding for these teeth were also significantly correlated with the patient’s PISA (R2 = 0.6457). The simple model presented in this study may be useful to estimate PISA.

    Japan Diabetes Complication and Prevention prospective (JDCP) study was conducted to examine the association between glycemic control and oral conditions in a large database of Japanese patients with diabetes. It included a total of 6099 patients with diabetes (range, 40–75 years) who had been treated as outpatients between 2007 and 2009. The mean number of present teeth at baseline was 19.8 and women with type 2 diabetes had fewer teeth than men with type 2 diabetes. Within the previous year, 17% of all patients had lost teeth. At baseline, 32% had experienced gingival swelling, 69% had brushed more than twice a day, 37% had used interdental cleaning aids, and 43% had undergone regular dental checkups. Multiple logistic regression analysis indicated that type 1 patients with HbA1c ≥ 7.0% were at higher risk of having fewer than 20 teeth (odds ratio [OR] 2.38; 95% confidence interval [CI] 1.25–4.78), and type 2 patients with HbA1c ≥ 8.0% also were at high risk of having fewer than 20 teeth (OR 1.16; 95% CI 1.00–1.34), after adjustment for nine possible confounding factors. In conclusion, patients with diabetes were found to be at high risk of tooth loss, and the poorer the glycemic control, the higher the risk of tooth loss in these patients.

    大規模観察研究(JDCP study)に登録された通院治療中の糖尿病患者6099名(うち2型93.8%、男性59.0%、年齢40〜75歳)を対象に、登録時の血糖コントロール状態と口腔所見の関連を解析した。登録時の現在歯数は19.8±9.4本で、2型糖尿病者では男性に比べて女性で歯数が少なかった。対象者の17%が過去1年間に歯を喪失し、32%に歯肉腫脹の既往があり、69%が一日の歯磨き回数2回以上で、37%が歯間清掃具を使用し、43%が定期歯科健診受診者であった。交絡因子を調整した多重ロジスティック回帰分析の結果、20歯未満になるオッズ比(95%CI)は、HbA1c 7.0%以上の1型糖尿病者で2.38(1.25〜4.78)であり、HbA1c 8.0%以上の2型糖尿病者で1.16(1.00〜1.34)であった。以上から、糖尿病患者は、血糖コントロール状態の悪化に伴って歯の喪失リスクが高まることが明らかになった。

    Objectives: The aim of this study was to evaluate the combined effects of recombinant human bone morphogenetic protein - 9 (rhBMP-9) loaded onto absorbable collagen sponges (ACS) and low-intensity pulsed ultrasound (LIPUS) on bone formation in rat calvarial defects. Materials and methods: Circular calvarial defects were surgically created in 18 Wistar rats, which were divided into LIPUS-applied (+) and LIPUS-non-applied (−) groups. The 36 defects in each group received ACS implantation (ACS group), ACS with rhBMP-9 (rhBMP-9/ACS group), or surgical control (control group), yielding the following six groups: ACS (+/−), rhBMP-9/ACS (+/−), and control (+/−). The LIPUS-applied groups received daily LIPUS exposure starting immediately after surgery. At 4 weeks, animals were sacrificed and their defects were investigated histologically and by microcomputed tomography. Results: Postoperative clinical healing was uneventful at all sites. More new bone was observed in the LIPUS-applied groups compared with the LIPUS-non-applied groups. Newly formed bone area (NBA)/total defect area (TA) in the ACS (+) group (46.49 ± 7.56%) was significantly greater than that observed in the ACS (−) (34.31 ± 5.68%) and control (−) (31.13 ± 6.74%) groups (p < 0.05). The rhBMP-9/ACS (+) group exhibited significantly greater bone volume, NBA, and NBA/TA than the rhBMP-9/ACS (−) group (2.46 ± 0.65 mm3 vs. 1.76 ± 0.44 mm3, 1.25 ± 0.31 mm2 vs. 0.88 ± 0.22 mm2, and 62.80 ± 11.87% vs. 42.66 ± 7.03%, respectively) (p < 0.05). Furthermore, the rhBMP-9/ ACS (+) group showed the highest level of bone formation among all groups. Conclusion: Within their limits, it can be concluded that LIPUS had osteopromotive potential and enhanced rhBMP-9-induced bone formation in calvarial defects of rats. Clinical relevance: The use of rhBMP-9 with LIPUS stimulation can be a potential bone regenerative therapy for craniofacial/peri-implant bone defects.

    Aim: To clinically and histologically evaluate in dogs the healing of gingival recessions treated with coronally advanced flap (CAF) with or without cross-linked hyaluronic acid (HA). Materials and methods: Gingival recession defects were surgically created on the vestibular side of both maxillary canines in 8 dogs. After 8 weeks of plaque accumulation, the 16 chronic defects were randomly treated with either CAF alone or CAF and HA-gel (CAF/HA). Clinical and histological outcomes were evaluated at 10 weeks post-surgically. Results: Compared to baseline, the clinical measurements at 10 weeks revealed a statistically significant decrease in gingival recession for both CAF (p < 0.01) and CAF/HA (p < 0.001) groups. Statistically significant differences were found in clinical attachment level (p < 0.05) and width of gingival recession (p < 0.01) favouring the CAF/HA group. Bone formation was statistically significantly greater in the CAF/HA group than in the CAF group (1.84 ± 1.16 mm vs., 0.72 ± 0.62 mm, respectively, p < 0.05). Formation of cementum and connective tissue attachment were statistically significantly higher in the CAF/HA group compared with the CAF group (i.e. 4.31 ± 1.78 mm versus 2.40 ± 1.35 mm and 1.69 ± 0.98 mm versus 0.74 ± 0.68 mm, respectively (p < 0.05)). Conclusions: The present data have for the first time provided histologic evidence for periodontal regeneration of gingival recession defects following treatment with CAF and HA. Clinical relevance: The use of HA in conjunction with CAF may represent a novel modality for treating gingival recession defects.

    Bone morphogenetic protein-9 (BMP-9) has been shown to potently induce osteoblastic differentiation of periodontal ligament fibroblasts (PDLFs) and may be a candidate therapeutic agent for periodontal tissue healing/regeneration, but the effect of the inflammatory environment of periodontitis on such approaches is unclear. We investigated whether interleukin-1β (IL-1β) affected BMP-9-mediated osteoblastic differentiation of human (h) PDLFs. IL-1β suppressed BMP-9-induced osteogenic differentiation of hPDLFs, as evidenced by reduced alkaline phosphatase (ALP) activity and mineralization, and the downregulated expression of BMP-9-mediated bone-related genes, RUNX2, SP7, IBSP, and SPP1. In hPDLFs, with or without BMP-9, IL-1β increased the protein expression of activin A, a BMP-9 antagonist, and decreased follistatin protein, an antagonist of activin A. Similarly, IL-1β upregulated the expression of the activin A gene and downregulated that of the follistatin gene. Notably, follistatin re-established BMP-9-induced ALP activity suppressed by IL-1β. Activin A inhibited the expression of BMP-9-responsive genes and BMP-9-induced ALP activity, while follistatin re-established them. Finally, extracellular signal-regulated kinase 1/2 (ERK1/2), p38, and nuclear factor-kappa B (NF-κB) inhibition significantly blocked IL-1β-induced activin A gene expression. Our data indicate that IL-1β inhibits BMP-9-induced osteoblastic differentiation of hPDLFs, possibly by promoting activin A production via the ERK1/2, p38, and NF-κB pathways.

    <p>Gingival hyperplasia is one of the well-known side effects of calcium channel blockers, which are often used for the treatment of hypertension. Gingival hyperplasia may induce malalignment of the teeth and aesthetic problems. Although the mechanisms of gingival hyperplasia are not yet clearly understood, inflammatory conditions of the periodontal tissue caused by dental plaques are known to be involved in the pathogenesis of gingival hyperplasia. In general, a change of the antihypertensive drug to one without the side effect of gingival hyperplasia by the patient's attending physician, and thorough initial treatment, including plaque control and scaling and root planing are effective for the treatment of calcium channel blockers-induced gingival hyperplasia. Periodontal surgery may be performed, if necessary.</p><p>Herein, we report the case of a 41-year-old male patient, a hypertensive under treatment with the calcium channel blocker amlodipine besilate<sup>®</sup>, who was diagnosed as having gingival hyperplasia and chronic periodontitis (Stage III, Grade C). He presented with marked gingival enlargement and bleeding, mobility of the anterior maxillary teeth, and disordered teeth. We attempted periodontal infection control, including instructions on oral hygiene, and professional care, including scaling and root planing and professional mechanical tooth cleaning. After re-evaluation, persistence of deep pockets with a pocket depth of more than 4 mm at multiple sites was observed, although the gingival hyperplasia had almost completely resolved. We planned a flap operation, but we could not do it, because the patient was very busy with his career and did not provide informed consent. So, we repeated scaling and root planing, and used a local drug delivery system in order to eliminate periodontopathic microorganisms. Furthermore, we motivated the patient by repeatedly explaining the importance of plaque control, professional care, as well as blood pressure control. As a result, the periodontal tissue and dentition improved with non-surgical therapy.</p>

    歯周治療の成功と治癒した歯周組織を良好に維持するには,歯周基本治療での歯周病の原因因子の排除が重要である.重度慢性歯周炎患者および侵襲性歯周炎患者では,歯周基本治療におけるスケーリング・ルートプレーニング(SRP)による歯周病原細菌の機械的除去効果の向上を目的に抗菌薬の経口投与を併用することも少なくない.また,抗菌薬を併用し短期間で全顎SRPを行うことで,処置部位への歯周病細菌の再感染を抑制し,治癒効果を高めるとされるフルマウスディスインフェクションも検証されている.本症例は,12,11の歯肉腫脹と疼痛を主訴に来院した45歳女性で,歯肉縁下プラークからPorphyromonas gingivalis,Aggregatibacter actinomycetemcomitans,Treponema denticola,Tannerella forsythiaが検出された,2次性咬合性外傷を伴う重度慢性歯周炎と診断された患者に対し,歯周基本治療時に,経口抗菌薬を併用した2週間でのSRPの実施と,暫間補綴物による外傷性咬合の除去を行うことで,徹底的に原因因子を除去した.その後,歯周外科治療と口腔機能回復治療を行い,サポーティブペリオドンタルセラピー(SPT)に移行した.SPT5年経過した現在も,歯周病原細菌は検出されず,良好な口腔内状態を維持できている.本症例は,歯周基本治療時の抗菌薬を併用したSRPを中心とした徹底的な原因除去後に,適切な歯周外科治療と口腔機能回復治療を行ったことが,良好な治療成績と病状安定につながったと思われる.(著者抄録)

    【目的】本研究は、歯周病治療を行った患者のSPTを継続できる要因を検討することを目的とした。【対象および方法】鹿児島大学病院で歯周病治療を行い、SPTを1年以上継続している患者106名(継続群)と、SPTを中断後再来院した24名(中断群)を対象に、質問紙調査と歯周病の臨床パラメーターの後ろ向き評価を行った。【結果および考察】継続群では「忙しくても健診を受ける」、「規則正しい食習慣」の項目でスコアが高い傾向があった。また受診の理由は「受けると安心する」、満足の理由は「健診の間隔」が有意に多かった。臨床パラメーターでは、中断群は中断後のPPDや歯の動揺度が有意な悪化傾向を示した。また、中断群では喫煙者が有意に多かった。健康志向で自己管理の意識が高い患者はSPTへのコンプライアンスも良好である可能性が考えられた。また、患者にとってSPTの間隔が適度で、受診によって安心感が得られると、受療行動へ繋がると示唆された。【結論】SPTの継続のためには、口腔内の状態に対して患者の安心感に繋がるような肯定的なフィードバックをすることや、患者が治療に主体的に関わるアドヒアランス向上を促進することが重要であると考えられた。(著者抄録)

    <p>The Japan Diabetes Complications and its Prevention (JDCP) prospective study was conducted to analyze the association between glycemic control and oral conditions in a large database of Japanese patients with diabetes. It included a total of 6,099 patients with diabetes (range, 40-75 years) who were treated as outpatients between 2007 and 2009. The mean number of teeth at baseline was 19.8 and women had fewer teeth than men in type 2 diabetes. Within the previous year, 17 % had lost teeth. At baseline, 32 % had experienced gingival swelling, 69 % brushed more than twice a day, 37 % used interdental devices, and 43 % received dental regular checkups.     新規模型は、評価者が評価しやすい上顎切歯を対象として作製した。人工歯はエジェクターを用いてプッシュ式で簡便に取り外すことができ、客観的臨床能力試験(OSCE)実施時に迅速な交換が可能であった。新規模型を使用して、平成27年度歯学部4年生(52名)の基礎実習終了時および平成28年度臨床研修歯科医(28名)の研修開始時に、歯周組織検査及びスケーリング・ルートプレーニング(SRP)の試験をOSCE形式で行った。プロービング値の正確さは、学生、臨床研修歯科医、医局員と臨床経験が長くなるにつれ正確になる傾向がみられた。歯の動揺度に関しては、測定結果の正確性は得られず、想定した動揺度よりも小さく評価する傾向を認めた。新規模型の使用感のアンケート調査では、学生、臨床研修歯科医、医局員のいずれにおいても、歯周組織検査、歯肉縁下SRPに関して概ね良好な結果であった。模型に関する使用感において、歯肉・人工歯の交換はすべて2分以内にでき、交換しやすさにおいても概ね良好な評価が得られた。

    尋常性天疱瘡の臨床症状の一つに歯肉上皮の剥離があり、そのため口腔清掃時や摂食時の疼痛が起こるため、生活の質の低下に繋がる。また、その治療後も再燃が考えられる疾患のため、定期検診では口腔内だけでなく、全身面や精神面にも注意すべきである。今回、歯肉の発赤、腫脹および疼痛を主訴とする41歳の女性患者を報告する。かかりつけ歯科のもとで壊死性潰瘍性歯周疾患の疑いで治療を受けたが改善しないため、当科を紹介された。医科皮膚科にて、血液検査、歯肉・皮膚組織の病理組織検査から尋常性天疱瘡と診断され、医科入院下にてプレドニゾロン(副腎皮質ステロイド)による治療が開始された。歯科では医科入院中、週に一回歯科衛生士による徹底した口腔衛生指導、専門的歯面清掃および精神面の支援、病態を悪化させる食事などに関して食生活指導を行った。口腔内症状は徐々に改善し、歯肉状態に合わせた口腔清掃用具とセルフケア方法の変更を行った。退院後のsupportive periodontal therapy中に一部歯肉上皮剥離の再発が認められ、セルフケアの方法を見直した。尋常性天疱瘡は再燃することがあるため、患者が安心して日常生活を送れるよう早期に異変に気付き歯科医師、医科と連携を図り、患者支援をすることが大事だと考えられた。(著者抄録)

    36歳女性。右上2番の動揺と歯石を主訴に来院した。歯周組織検査の結果、中等度の広汎型慢性歯周炎と診断した。歯周基本治療のみで著しい改善がみられ、SPTにて管理を行っていた。左下4番の垂直性骨欠損がみられ、PPDは5mmであったが、歯周基本治療により3mmに減少した。SPT 5年目に第III相臨床試験において、左下4番の垂直性骨欠損部位にFGF-2製剤を用いた歯周組織再生療法を行った。治療後は骨増加率の回復、PPDのさらなる減少(2mm)を認めた。

    本研究は切迫早産(TPL)妊婦の歯周組織状態と糖代謝異常の関連を調べることを目的とした。被験者は15名の正常妊娠妊婦、14名の切迫早産で糖代謝異常の妊婦(TPL/糖代謝異常)、16名の切迫早産で糖代謝正常の妊婦(TPL/糖代謝正常)とした。全ての妊婦にプロービングポケット深さなどの歯周組織検査を実施した。被験者から採取した唾液、歯肉縁下プラーク、絨毛膜組織のPorphyromonas gingivalis(P.gingivalis)、Fusobacterium nucleatum(F.nucleatum)、Prevotella intermedia(P.intermedia)、Aggregatibacter actinomycetemcomitans、Treponema denticola、Tannerella forsythia(T.forsythia)の検出はPCR法で行った。歯周組織検査結果と口腔内の歯周病原細菌の検出率は、TPL/糖代謝異常とTPL/糖代謝正常の間に有意差はなかった。絨毛膜組織絨ではTPL/糖代謝正常はF.nucleatumとP.gingivalisが検出されたのに対し、TPL/糖代謝異常はF.nucleatum、P.intermedia、T.forsythiaが検出された。正常妊娠妊婦の絨毛膜組織はいずれの歯周病原細菌も検出されなかった。本研究は歯周病が切迫早産妊婦の糖代謝異常のリスク因子と示せなかった。しかし絨毛膜に存在するF.nucleatum、P.intermedia、T.forsythiaが糖代謝異常と関連する可能性を示唆した。(著者抄録)

    歯科受診成人患者を対象にインプラント周囲疾患の有病率と危険因子を調査する多施設横断研究を施行した。2012年10月からの1年間に、歯科インプラントの経過観察のために歯科病院または歯科診療所を定期的に受診していた日本人成人患者267例(女性157例、年齢62.5±10.7歳)を組み入れた。全例においてインプラント治療前後の歯周管理は歯周病専門医によって行われ、インプラント荷重開始後3年以上経過していた。経過観察時に専門医による歯周組織の評価を行い、健康/インプラント周囲粘膜炎/インプラント周囲炎に分類した。インプラント周囲粘膜炎の患者単位での有病率は33.3%、インプラント周囲炎では9.7%であった。インプラント周囲疾患の強い危険因子として同定されたのは、口腔衛生状態が悪いことと歯周炎の既往との2点であった。インプラント導入前後での歯周治療の質と、患者のコンプライアンスおよび意識の高さとが、インプラント周囲組織の健康を維持する上で重要だと考えられる結果であった。

    <p>We investigated the prevalences and risk factors for peri-implant diseases in Japanese adult dental patients attending a follow-up visit at dental hospitals or clinics as part of their maintenance program. This cross-sectional multicenter study enrolled patients with dental implants who attended regular check-ups as part of a periodontal maintenance program during the period from October 2012 through September 2013. Patients with implants with at least 3 years of loading time were included in the study. The condition of peri-implant tissue was examined and classified into the following categories: healthy, peri-implant mucositis, and peri-implantitis. Patients were also evaluated for implant risk factors. A total of 267 patients (110 men, 157 women; mean age: 62.5 ± 10.7 years) were analyzed. The prevalence of patient-based peri-implant mucositis was 33.3% (<i>n</i> = 89), and the prevalence of peri-implantitis was 9.7% (<i>n</i> = 26). Poor oral hygiene and a history of periodontitis were strong risk factors for peri-implant disease. The present prevalences were lower than those previously reported. The quality of periodontal therapy before and after implant installation and patient compliance and motivation, as indicated by plaque control level, appear to be important in maintaining peri-implant tissue health.</p>

    <p>As pemphigus vulgaris can cause exfoliation of the gingival epithelium as one of its symptoms, tooth brushing and eating can be painful, leading to a decreased quality of life. Pemphigus vulgaris can relapse even after the lesions have healed, and careful attention should be paid not only to the intraoral conditions, but also to the systemic and mental conditions of the patient at the time of periodic dental examinations. We report the case of a 41-year-old woman who complained of redness, swelling, and gingival pain. She was treated for a suspected necrotizing ulcerative periodontal disease by a family dental office, but her symptoms did not improve. Thus, she was referred to our department. In our hospital's Department of Dermatology, she was diagnosed as having pemphigus vulgaris based on the results of blood tests and a histopathological examination of the gingiva and skin tissues. She was hospitalized in the Department of Dermatology and treated with prednisolone, a corticosteroid. During hospitalization, thorough oral hygiene instructions and professional tooth cleaning focusing on the sites where the patient had difficulty cleaning her teeth were provided, and mental support and guidance regarding eating habits and meals that might exacerbate her condition were conducted by a dental hygienist once a week. The patient's oral symptoms were gradually ameliorated, and we modified her oral cleaning tools and self-care method based on her gingival condition. After discharge, a recurrence of epithelial exfoliation in part of the gingiva was observed during the supportive periodontal therapy period, and her self-care method was once again reconsidered. Since pemphigus vulgaris can exhibit a relapse of symptoms, it is important for dental staff, including dental hygienists and dentists, to notice abnormalities in the gingival conditions of patients as early as possible and to cooperate with medical staff to support patients so that the patients can live their daily lives with ease.</p>

    <p>本研究は切迫早産 (TPL) 妊婦の歯周組織状態と糖代謝異常の関連を調べることを目的とした。被験者は15名の正常妊娠妊婦, 14名の切迫早産で糖代謝異常の妊婦 (TPL/糖代謝異常), 16名の切迫早産で糖代謝正常の妊婦 (TPL/糖代謝正常) とした。全ての妊婦にプロービングポケット深さなどの歯周組織検査を実施した。被験者から採取した唾液, 歯肉縁下プラーク, 絨毛膜組織の<i>Porphyromonas gingivalis</i> (<i>P. gingivalis</i>), <i>Fusobacterium nucleatum</i> (<i>F. nucleatum</i>), <i>Prevotella intermedia</i> (<i>P. intermedia</i>), <i>Aggregatibacter actinomycetemcomitans</i>, <i>Treponema denticola</i>, <i>Tannerella forsythia</i> (<i>T. forsythia</i>) の検出はPCR法で行った。歯周組織検査結果と口腔内の歯周病原細菌の検出率は, TPL/糖代謝異常とTPL/糖代謝正常の間に有意差はなかった。絨毛膜組織絨ではTPL/糖代謝正常は<i>F. nucleatum</i>と<i>P. gingivalis</i>が検出されたのに対し, TPL/糖代謝異常は<i>F. nucleatum, P. intermedia</i>, <i>T. forsythia</i>が検出された。正常妊娠妊婦の絨毛膜組織はいずれの歯周病原細菌も検出されなかった。本研究は歯周病が切迫早産妊婦の糖代謝異常のリスク因子と示せなかった。しかし絨毛膜に存在する<i>F. nucleatum</i>, <i>P. intermedia</i>, <i>T. forsythia</i>が糖代謝異常と関連する可能性を示唆した。</p>

    新型コロナウイルスによって引き起こされたパンデミックは世界中の医療体制や経済活動に大きな影響を与え,我々の社会生活に多くの変化をもたらした。新型コロナウイルスの感染者の多くは軽度から中等度の症状を示すが,一部は重症となり死に至る。しかし,重症化のリスク因子の解明を含めた新型コロナウイルス感染症の病態の全容やメカニズムなどについて未だ不明な点がある。新型コロナウイルス感染症および歯周病の有病率は高齢者で高く,両疾患とも他の多くの基礎疾患との関連が指摘されている。本ミニレビューではCOVID-19/SARS-CoV-2感染と歯周病の関連性について現在までの知見の一部を紹介する。(著者抄録)

    骨形成蛋白質(BMP)は発生や骨格形成、造血・神経発生、組織の恒常性維持など様々な機能を担っている。BMP-2および-7は古くから骨再生において中心的な役割を果たすと考えられ、詳細に検討されてきた。BMP-9は結合する受容体やシグナル伝達経路、抑制機構などにおいてBMP-2と異なる特徴を多く持ちながら、強力な骨芽細胞への分化能、骨誘導能を有している。BMP-9の骨再生療法、歯周組織再生療法への応用の可能性について述べた。
