Background: Intra-oesophageal acid stimulation increases masseter muscle activity. However, the infusion speeds used in previous studies are significantly slow, with minimal acid volume (30 mL over 30 min). Additionally, it is unclear how masseter muscle activity, symptoms of discomfort, stress levels and saliva quantity change over time. Objectives: The time course of masseter muscle activity, heartburn and discomfort symptoms, salivary cortisol concentration to assess stress, autonomic nervous system (ANS) activity and saliva quantity under the influence of intra-oesophageal acid infusion were evaluated at a faster injection rate and larger volume than in previous studies. Methods: Ten healthy adults underwent polygraphic monitoring, consisting of electromyography of the masseter muscle and electrocardiography during intra-oesophageal acid infusion (10 mL/min, 10 min). Symptoms of heartburn and discomfort were assessed using the visual analogue scale (VAS), and saliva quantity was measured. Friedman's test was used for multiple comparisons. Results: Masseter muscle activity, VAS scores for heartburn and discomfort, as well as saliva quantity during the 10-min acid infusion, increased significantly compared with that before acid infusion (p < 0.001, 0.001, 0.019 and 0.047, respectively) and decreased 10 and 20 min after acid infusion (p = 0.004, 0.004 and 0.007, respectively). No significant changes were observed in the salivary cortisol concentration or ANS activity. Conclusion: Intra-oesophageal acid infusion stimulated symptoms of heartburn and discomfort and increased masseter muscle activity and saliva production, which may not be related to psychological stress.

Objectives: Late-stage secondary alveolar bone grafting (SBG) has a poor prognosis; however, the case selection and timing of orthodontic treatment have not been thoroughly investigated. This study aimed to clarify the prognosis of early-SBG and late-SBG in patients with unilateral or bilateral cleft lip and palate. Materials and Methods: Fifty-six patients underwent early-SBG performed before the eruption of the cleft-adjacent lateral incisors or canines (early-SBG group). Ten patients underwent late-SBG requiring significant tooth movement for gap closure; SBG was performed before phase II treatment for treatment efficiency, and tooth movement began approximately 3 months post-SBG (late-SBG group). Pre-SBG cleft width was measured; bone-bridge formation using the Chelsea scale and cleft gap closure post-SBG were evaluated. Types A and C bone bridges were considered adequate. These items were statistically compared between groups. Results: Cleft width in the early-SBG group was significantly smaller than that in the late-SBG group (p < 0.001). There were no significant differences in the percentage of adequate bone-bridge formation (p = 0.055) and cleft-gap closure (p = 1.000) between groups. However, the ratio of type A bone bridges was significantly higher in the early-SBG group than in the late-SBG group (p = 0.035). Conclusion: These results suggest that, even in late-SBG, early orthodontic tooth movement to the graft bone leads to the formation of an adequate bone bridge, similar to early-SBG. However, the maintenance of cervical grafted bone after late-SBG may be less than that after early-SBG.

交通事故による前歯部審美障害を主訴に来院した初診時年齢24歳5ヵ月の女性にマルチブラケット装置による矯正歯科治療および補綴治療を行い、良好な治療結果を得たので報告する。患者は交通事故で上顎前歯を脱臼し、上顎左側中切歯は喪失しており、開咬を伴う骨格性III級を呈していた。矯正歯科治療により、上顎は左側側切歯を中切歯の位置に排列し、右側は第二小臼歯を抜去して臼歯を近心移動した。下顎は左側第一小臼歯と右側第二小臼歯を抜去し、前歯の舌側移動を行った。動的治療後に上顎前歯の補綴治療を行い、保定5年3ヵ月を経過しているが、良好な咬合を維持している。(著者抄録)
