＜文献概要＞目的:猫ひっかき病は,一般的に予後良好な人畜共通感染症である。眼病変が合併した場合も,典型的には視力予後が良好であることが多い。今回,非典型的な臨床像を呈する猫ひっかき病の1例を経験したため報告する。症例:患者は41歳,男性。飼い猫から咬傷を受け,その1ヵ月後に発熱と悪寒が出現したが,リンパ節腫脹はなかった。同時期に右眼の視力低下,飛蚊症を自覚した。さらに約1ヵ月後に両眼の著明な視力低下を自覚し,近医から鹿児島大学病院眼科(当科)へ紹介となった。矯正視力は右15cm指数弁,左0.06。両眼に視神経乳頭腫脹,漿液性網膜剥離,黄斑浮腫,硝子体出血を認めた。当科初診から1週間後に左眼に星芒状白斑が出現した。臨床所見から猫ひっかき病を疑い,アジスロマイシン内服,ステロイドパルス療法を行ったところ,両眼の視神経乳頭腫脹や漿液性網膜剥離は改善した。さらにプレドニゾロン,ドキシサイクリン内服を行ったが,視力の改善は限定的だった。その後,抗Bartonella henselae抗体陽性が確認され,猫ひっかき病による視神経網膜炎と診断した。結論:猫ひっかき病では視神経網膜炎を呈することがある。非典型的で重篤な眼所見を合併する場合には鑑別に挙がりにくく視力予後が不良である可能性があり,十分な問診と早期の治療介入の検討が望ましい。

Purpose: This study aimed to evaluate the shape of the extraocular muscles (EOMs) in normal subjects using the en-face images of anterior segment optical coherence tomography (ASOCT). The EOM insertion and the direction of the muscle fibers were investigated. Subjects and methods: A total of 97 healthy normal subjects (194 eyes) at Okayama University Hospital (age, 47.1 ±21.5 years; range, 8-79 years) participated in the study. A series of 256 tomographic images of the rectus EOMs were captured using the C-scan function of the AS-OCT (CASIA2, TOMEY Co., Japan), and the images were converted to en-face images in multiTIFF format. The anterior chamber angle to EOM insertion distance (AID) and the angle of the muscle fibers from the insertion site (angle of muscles) were measured from the images. The correlations of AID and angle of muscles with age and axial length were investigated and evaluated. Results: AID and angle of muscles were significantly correlated with age or axial length in some EOMs. The AIDs of medial rectus (MR) (P = 0.000) and superior rectus (SR) (P = 0.005) shortened with age. The AIDs of MR (P = 0.001) and inferior rectus (IR) (P = 0.035) elongated with axial length, whereas lateral rectus (LR) (P = 0.013) shortened. The angles of MR (P = 0.001) and LR (P = 0.000) were found to have a more downward direction toward the posterior in older subjects. Conclusion: En-face images can be created by AS-OCT, and the shape of the EOMs in normal subjects using these image measurements was available. With the ability to assess the EOMs, AID and angle of muscles are expected give useful information for treating and diagnosing strabismus-related diseases.

It is known that social factors affect the choice of treatments, and special attention has been paid to sex differences. The purpose of this study was to determine whether regional and sex differences exist in the treatment of rhegmatogenous retinal detachment (RD). We used Japan-RD Registry database of 2523 patients aged ≥ 40 years between February 2016 and March 2017 in 5 Japanese regions. Regional differences of patients’ perioperative factors were analyzed. The factors affecting the proportion of patients who underwent surgery within one week of the onset, defined as early-surgery, were examined by logistic regression. We observed regional differences in perioperative factors, especially in the use of phacovitrectomy, general anesthesia, and air-tamponade, which was higher in certain regions. (Fisher’s exact test, all P = 0.012) The proportion of early-surgery was significantly higher among men in Kyushu region (Odds ratio (OR) 1.83; 95% confidence interval (CI) 1.08–3.12; P = 0.02), and it was also significantly higher after adjusting for covariates (OR 1.89; 95% CI 1.06–3.42; P = 0.02). Regional and sex differences exist in the treatment of RD in Japan. Although there was no significant differences in the anatomical outcomes, women in certain regions of Japan are less likely to receive early surgical intervention for RD.

Optic neuritis is an ocular disorder whose pathogenesis has not been fully determined, although autoimmune mechanisms have been suggested to be involved in its development. In recent years, anti-aquaporin-4 antibody (AQP4-Ab) and anti-myelin oligodendrocyte glycoprotein antibody (MOG-Ab) have been shown to play major roles in the development of optic neuritis. Because these two antibodies target different tissues, optic neuritis can be classified by the type of antibody. AQP4-Ab-positive optic neuritis responds poorly to steroid therapy and has a poor prognosis in terms of visual acuity. On the other hand, MOG-Ab-positive optic neuritis responds favorably to steroid therapy but is likely to recur when the dosage of steroids is reduced or discontinued. We first present the high incidence of idiopathic optic neuritis and discuss these relatively newer disease concepts of AQP4-Ab-positive optic neuritis and MOG-Ab-positive optic neuritis.

データサイエンスはデータ解析を行う学問であるが,この状況が近年劇的に変化してきている.人工知能(AI),第5世代(5G)通信ネットワーク,モノのインターネット(IoT)などが社会実装され,社会を一気に変えつつある.この変化は医療においてはデジタル医療革命と呼ばれ,世界中に広がりつつある.この変化に対する我々の研究を紹介する.I.画像解析におけるAIの可能性 眼科は診療や研究に画像を用いることが多いが,客観性や定量性が不十分であったので,AIを導入して客観性や定量性のある画像解析を研究した.脈絡膜の光干渉断層計(OCT)en face画像について層別化を試みた.support vector machine(SVM)を用いたAIにより脈絡膜en face画像を層判別するアルゴリズムを開発した.学習用データの各特徴量からSVMモデルを作成し,検証用データを用いて,脈絡膜の各層に対する決定係数を算出した.その結果,決定係数は平均0.985であった.このことは,これまで定性的にしか区別できなかった画像をAIにより客観的かつ高い再現性を持って分類できることを示すものである.また,同法で得られたen face画像について,脈絡膜血管走行のパターン分類を行った.中心性漿液性脈絡網膜症(CSC)を対象としてen face画像を取得し,血管面積,血管長と平均血管径を定量し,正対称方向に走行する血管割合(symmetry index)を算出した.その結果,CSC眼ではHaller層血管が拡張しているだけでなく,上下で非対称性の走行を取っていた.この構造的差異は疾患眼だけではなく僚眼にもみられたので,CSCにおける先天性素因の存在を確認することができた.正常眼の黄斑直下の脈絡膜には太いHaller層血管が存在しないため血管が拡張しても黄斑部への影響は少ないが,CSC眼では非対称性にHaller層血管が走行するため,Haller層血管が拡張すると黄斑下の脈絡膜内層が圧排され,血液網膜関門が破綻して,滲出液が網膜下に貯留するのがCSCであると考える.II.AIによる新しい研究テーマの創出 眼底所見から性別判定が可能であることをAIは証明したが,その証明過程は不明である.そこで眼底写真に関連する定量化された蓄積データを集め,それらのみで性別を予測できるか否かについて調べた.その結果,約80%の確率で眼底から性別を予測できた.女性の眼底は男性よりも緑色が強く,上耳側動脈が黄斑の近くを走行するなどの特徴があった.それらが何を反映しているかを研究したところ,生下時に60%程度は男女差の特徴がすでに備わっており,第二次性徴を過ぎてそれが80%に増加することが分かった.人間が思いもつかなかった現象や事実をAIが発見して,それを人間が研究したという事実は,眼科研究史的にも大きな意味を持つ.今後は研究テーマの発掘もAIが中心となるかもしれない.III.ビッグデータの重要性 ビッグデータこそが今後の眼科におけるデータサイエンスの中心である.日本網膜硝子体学会(JRVS)は,2016年にJRVS理事施設で施行された全網膜剥離手術症例について登録し,3,446例が集積された(J-RDレジストリ).さまざまな解析が行われ,経毛様体扁平部硝子体切除術と強膜内陥術を選ぶ術者の傾向,視力変化,剥離部位による手術成功率の差など,2016年時点の日本の網膜剥離についての基礎データが完成した.さらに傾向スコアマッチングを用いてこのデータを解析したところ,シリコーンオイルタンポナーデがガスタンポナーデよりも網膜障害が強いこと,内的網膜下液ドレナージは術後に網膜前膜を生じやすいというエビデンスなどが得られた.ほかにも,治療における男女差など現在も多くの知見が得られ続けている.ビッグデータは今後の眼科医療を正しく進めるうえで重要なものである.しかし,日本はその点で立ち遅れており,その早期の確立が必要である.(著者抄録)
