片側性口唇口蓋裂に対し上顎歯列弓を形成する治療法として、術前鼻歯槽形成法(PNAM)とHotz床を用いた治療とで短期効果を比較する後方視的研究を行った。片側性口唇口蓋裂に罹患した日本人患児の自験例で、口唇修復術を施行前の術前顎矯正治療として2003〜2009年にPNAMを施行した27例(男児15例、初診時日齢7.4±7.1、修復術施行時の日齢146.9±48.4)と、2009〜2016年にHotz床を用いて治療した29例(女児15例、初診時日齢9.8±5.4、修復術施行時の日齢143.4±49.7)の診療録を再調査した。初診時と口唇修復術施行時との両時点における上顎歯列模型をスキャンデータ化し、両群の上顎歯列形成を比較した。PNAM群ではHotz床群に対し両時点間で生じた切歯点の後方移動量がより大きく、口蓋裂のmajor segmentにおける口蓋破裂縁点の後方移動量も大きいことがいずれも有意に示された。またPNAM群の切歯点は両時点間での比較において口蓋裂側方向へ側方移動していることも有意に示された。さらに修復術施行時点においてPNAM群ではHotz床群に比べ顎裂幅や歯槽弓長が有意に短かった。PNAM施行によってmajor segmentの口蓋裂側への回転が生じていたことから、PNAMはより対称的な上顎歯列弓を形成するのに貢献すると考えられたが、同時に歯槽弓長を短くしてしまう可能性も示された。

小児期の口唇裂口蓋裂患児に対する顎裂部への自家腸骨海綿骨移植術は、一般的な治療法として広く行われている。腸骨採取部位の軟組織の厚みや腸骨稜の突出は術後の創部緊張に影響し、創治癒に影響すると思われる。そこで本研究では、小児の肥満度と腸骨採取部の術後創部状態の相関を明らかにするため、創部状態の評価およびRohrer Index(RI:体重(kg)/身長(cm)3×10^7)との相関を調べた。2009年から2016年までに当科で顎裂部腸骨海綿骨移植術を行ったCLP患者62名(72側、手術時平均年齢10.6歳)を対象とした。術後3ヵ月以降の創部写真を用いて評価者3名による創部状態の評価を行い(Score 1-5)、Score 1、2を創部良好群、Score 4、5を創部不良群とした。また、各患者の年齢、体重、身長、RIと創部Scoreとの相関をピアソンの相関係数を用いて評価し、創部良好群と創部不良群との間の各項目の数値をt検定にて比較した。その結果、次の知見を得た。1.創部状態の評価を行った結果、創部良好群は72側中35〜41側(48.6〜56.9%)、創部不良群は19〜24側(26.4〜33.3%)であった。2.創部良好群の平均RIは125であり、不良群の114よりも有意に高く(p<0.05)、RIの高い症例は良好な創痕となることが示された。その他の項目には有意差を認めなかった。創部ScoreとRIとの間には負の中等度相関を認めた。3.RIが低い症例では腸骨採取部の皮下組織が薄く、縫合閉鎖が困難になるとともに、腸骨稜の突出による皮膚緊張が強くなり、創の治癒に影響を及ぼすと考えられる。従って、RIの低い痩せ型の患児に対しては創部緊張を緩和するために皮下で十分に創縁を引き寄せる必要があると思われる。(著者抄録)

Autologous iliac cancellous bone grafting in the alveolar cleft is used as a general treatment for patients with cleft lip and palate （CLP）. The thickness of the soft tissue and protrusion in the donor site of the ilium may affect the wound tension, which affects healing. In this study, to clarify the factors affecting wound condition, we evaluated the wound condition of the donor site, and investigated the correlation between the wound condition and the Rohrer Index （RI ＝ weight （kg） / height （cm）³ × 10⁷）.<br>The subjects were 62 patients with CLP who underwent iliac bone grafts in the alveolar cleft （72 sides, mean age 10.6 years） in our department from 2009 to 2016. The oral surgeons evaluated the wound healing using photographs of the donor site three months after the graft. The wounds were classified with a wound healing score from 1 to 5. Scores of 1 or 2 indicated good healing （good wound group）; a score of 4 or 5 indicated poor healing （poor wound group）. We also analyzed the correlation between the wound score and the age, weight, height, and RI of each patient using Pearson's correlation coefficient. In addition, we compared the values of the good wound and poor wound groups using Student's t-test. We obtained the following findings:<br>1．The good wound group included 35-41 （48.6-56.9％） of the 72 sides, and the poor wound group included 19-24 （26.4-33.3％） sides.<br>2．The mean RI of the good wound group was significantly higher than that of the poor wound group （good RI＝ 125; poor RI＝114; p ＜ 0.05）. Moreover, there was a moderate negative correlation between the wound healing score and the RI. There were no significant differences between other variables.<br>3．Cases with a low RI may have been due to insufficient subcutaneous tissue at the donor site; suture closure was more difficult and increased skin tension due to increased protrusion of the iliac crest. In conclusion, we suggest that a suture technique that relieves wound tension is necessary for infants with a low RI.

1981年4月から2011年3月迄に受診した、口唇裂・口蓋裂患者一次症例を対象とした。1981年4月から1991年3月迄を前期、1991年4月から2001年3月迄を中期、2001年4月から2011年3月迄を後期とした。口唇裂・口蓋裂患者一次症例は1100名(男性564名、女性536名)で、口唇裂が368名(33.5%)、唇顎口蓋裂が408名(37.0%)、口蓋裂が324名(29.5%)であった。初診時年齢は、生後3ヵ月未満が全体の85.6%と殆どを占めた。先天異常を合併した患者は122名(11.1%)に認め、ロバンシークエンス30例、心奇形44例、発達遅滞19例、染色体異常17例、消化器奇形12例であった。調査期間別の総患者数の推移は、前期336例、中期380例、後期384例であった。裂型別の症例数は、いずれの期間も唇顎口蓋裂、口唇裂、口蓋裂の順であった。いずれの期間も生後3ヵ月未満の受診割合が80%以上であった。紹介元分布では、いずれの期間も産科・婦人科の紹介が最も多く見られた。

<p>This study analyzed 1,100 patients with primary cleft lip, cleft palate, or both who visited our specialized outpatient department during the 30-year period between 1981 and 2011.</p><p>The results were as follows:</p><p>1 . This study identified 1,100 primary cases of cleft lip, cleft palate, or both among patients attending the outpatient department.</p><p>2 . Of the 1,100 patients, 368 had cleft lip (33.5%), 408 had cleft lip and cleft palate (37.0%), and 324 had cleft palate (29.5%).</p><p>3 . There were 564 male patients and 536 female patients, corresponding to a male-to-female ratio of 1.05:1.</p><p>4 . The age of patients at the first visit was recorded, and we found that 684 patients (62.2%) presented to the department within 1 month of birth.</p><p>5 . The average birth weight of the patients was 2,934.4 g. The birth weight of 400 patients, representing the largest proportion (36.4%), ranged from 2,500 g to 2,999 g.</p><p>6 . A total of 870 patients (79.0%) were living in Kagoshima Prefecture. The proportion of remote island residents in Kagoshima Prefecture was 5.0%.</p><p>7 . The largest proportion of patients was introduced to our department by obstetricians. The second largest proportion of patients was introduced to us by dentists or oral surgeons.</p><p>8 . A total of 122 patients (11.1%) had a congenital anomaly.</p><p>9 . The number of visits, which started in 2006, has increased since 2011. The number of prenatal consultations ranged from 1 to 6 per year. The proportion of patients who sought prenatal consultations was low.</p><p>10. A comparison of the study variables according to the survey period showed that the total number of patients had increased slightly in the intermediate and late periods. The most common abnormality throughout the study was cleft lip and cleft palate. In the early and late periods, there were higher proportions of men than women. In the intermediate period, there was a higher proportion of women. During the early period, less than 50% of patients presenting at the clinic were younger than 1 month. During the intermediate and late periods, 70% of the patients were younger than 1 month at their first outpatient visit. The proportion of low-birth-weight infants (birth weight less than 2,500 g) has gradually increased. High proportions of patients had cleft palate as a congenital malformation in all periods.</p>
