    Perioperative stress in pediatric patients is often difficult to assess via interviews; thus, an objective measure to assess perioperative stress is needed. To visualize perioperative stress, we observed autonomic nervous system (ANS) activity, circulatory dynamics, and psychological status in pediatric patients undergoing alveolar bone grafting under general anesthesia. This prospective observational study included 40 patients aged 8–12 years who were scheduled for alveolar bone grafting in our hospital. ANS activity was analyzed using heart rate variability the day before surgery, during general anesthesia, 2 h postoperatively, 24 h postoperatively, and the day before discharge. ANS assessment included LF/HF (sympathetic nervous system activity) and HF (parasympathetic nervous system activity). Additionally, heart rate (HR), systolic blood pressure (SBP), face scale (FS) score were recorded. Data from 31 patients, excluding dropouts, were analyzed. The ratio of change to the preoperative value was compared. After surgery, the LF/HF, HR, SBP, and FS score significantly increased (P < 0.01) and HF significantly decreased (2 h postoperatively: P < 0.05, 24 h postoperatively, before discharge: P < 0.01). SBP recovered to preoperative values 24 h postoperatively, and HR and FS scores recovered to preoperative values before discharge. However, even before discharge, LF/HF remained significantly higher than preoperative values, and HF remained significantly lower than preoperative values (P < 0.01). Conclusion We observed perioperative stress from multiple perspectives. Circulatory dynamics and psychological status recovered by the day before discharge; however, ANS activity did not. Therefore, evaluating ANS activity may be useful in visualizing potential perioperative stress in pediatric patients.

    Autologous bone grafting is the primary method for treating alveolar clefts. However, bone grafting materials are desired as alternatives to autogenous bone to reduce surgical invasiveness. Here, we present an animal study evaluating the effect of carbonate apatite (CA) on the spontaneous eruption of permanent teeth. The bone grafting materials included CA, natural bovine bone (BB), and hydroxyapatite (HA). In 15 8-week-old male beagle dogs, the left mandibular deciduous premolars (DP) two and three were extracted and subsequently filled with CA, BB, and HA. The animals were euthanized after a predetermined number of days, and samples were collected for microcomputed tomography and histological evaluation. Spontaneous eruption of the succeeding permanent teeth (P3 and P4) was observed in the CA group at 14 weeks. Delayed eruption of the succeeding permanent teeth was observed in the BB and HA groups. CA could serve as a viable alternative to autogenous bone for treating alveolar clefts.

    Aim: The effect of listening to music before tooth extraction on the autonomic nervous system and psychological state remains unclear. This prospective study explored the hypothesis that playing music before the extraction of an impacted mandibular third molar would stabilize patients' autonomic nervous system activity and circulatory dynamics. Methods: Thirty-six patients were randomized into two groups: control and music. Patients in the music group listened to music before tooth extraction. Heart rate variability and circulatory dynamics were recorded during the procedure. The Modified Dental Anxiety Scale and State Trait Anxiety Inventory scale scores were recorded before the procedure, and the latter was used to assess postoperative anxiety scores. Results: Sympathetic nervous activity significantly increased in the control group during the administration of local anesthesia, bone removal, and extraction (p < 0.01), although this increase was lower in the music group during the administration of local anesthesia and extraction (p < 0.05). In the music group, the postoperative anxiety score was significantly lower than the preoperative score (p < 0.05). Conclusion: Playing music preoperatively may help reduce sympathetic nerve activity during the administration of local anesthesia and tooth extraction.

    <p>  Multiple chemical sensitivity is a disease in which autonomic and neuropsychiatric symptoms, such as fatigue, headache, joint pain, insomnia, and dermatitis, occur in response to exposure to a trace amount of a chemical substance. Here, we report the management of intravenous sedation for tooth extraction and caries treatment in a patient with multiple chemical sensitivities.</p><p>  A 57-year-old woman visited a nearby dental clinic complaining of dentin hypersensitivity and requiring tooth extraction and treatment for a root fracture in the mandibular right second molar and caries in the mandibular right first premolar. However, she had a history of chemical hypersensitivity and dentophobia. She was referred to our hospital and was scheduled to undergo intravenous sedation in an environment with minimal exposure to inducers.</p><p>  We monitored her blood pressure, percutaneous arterial oxygen saturation (SpO<sub>2</sub>), and pulse, and a single dose of midazolam (2 mg) and propofol (10 mg) was administered after securing an intravenous route. The continuous administration of propofol was started at 3 mg/kg/h under 3 L/min of oxygen after the patient fell asleep. No symptoms or risks associated with multiple chemical sensitivity were observed during the tooth extraction or caries treatment.</p><p>  The patient was successfully treated in an environment that eliminated the triggers for the onset of symptoms of multiple chemical sensitivity after meticulous interviews with specialists in collaboration with each clinical department. Intravenous sedation using midazolam and propofol under 3 L/min oxygen was a successful approach for minimizing the psychosomatic mechanisms of multiple chemical sensitivity in a patient with dentophobia.</p>

    <b>Patients</b>: After a 38-year-old female underwent extraction of tooth 38, which was in close proximity to the mandibular canal, she presented with mental nerve dysesthesia. She was then prescribed vitamin B12. However, her symptoms remained unchanged one month after starting the medication; thus, she requested treatment at a pain clinic. Under the diagnosis of trigeminal neuropathy, she was treated with a stellate ganglion block and acupuncture. After 20 sessions of stellate ganglion block treatment, the pain shifted from dysesthesia to paresthesia; however, there was no significant improvement in subjective symptoms, and the patient was unsure about continuing the treatment. Improvement was observed in the precise tactile function and the electric current perception threshold tests, and these positive results were conveyed to the patient. The patient wanted to continue treatment. After 10 sessions of acupuncture, she was again re-examined. Further improvement was observed in the precise tactile function and the electric current perception threshold tests, and these positive results were conveyed to the patient. As the patient was satisfied with the treatment results and the improvement in her symptoms, she requested treatment cessation.
    <br><b>Discussion</b>: As in the present case, there are cases in which laboratory values improve despite no change in patient complaints about symptoms.
    <br><b>Conclusions</b>: Therefore, for patients, particularly those with orofacial pain, who are hesitant to continue treatment due to a lack of observable change in symptoms, a qualitative examination and provision of feedback on test results showing improvement can be useful in enhancing their decision-making regarding the treatment, thus contributing to the overall improvement in patient satisfaction.

    <p>This study aimed to determine independent factors for developing postoperative hypertension using 4 biomarkers in patients receiving oral and maxillofacial surgery under general anesthesia. Brain natriuretic peptide (BNP), N-terminal pro-B-type natriuretic peptide (NT-proBNP), high-sensitivity myocardial troponin T (hs-TnT), and high-sensitivity myocardial troponin I (hs-TnI) were measured and preoperative echocardiograms were examined. Episodes of systolic blood pressure (SBP) ≥ 170 mmHg or diastolic blood pressure ≥ 100 mmHg within 1 week after surgery were considered postoperative hypertension. We analyzed 213 (130 men; 83 women) patients, who were divided into a postoperative hypertension group (HT group, <i>n</i> = 32) and a normal group (N group, <i>n</i> = 181). The HT group showed a higher LVMI (113.5 versus 100.1), higher E/e' of the lateral wall (9.1 versus 7.7), and higher BNP (39.2 versus 22.9 pg/mL), NT-proBNP (400.1 versus 143.9 pg/mL), and hs-TnT (15.6 versus 10.3 ng/L) concentrations compared to the N group. NT-proBNP and hs-TnT concentrations positively associated with E/e', but BNP and hs-TnI did not. NT-proBNP (AUC = 0.64, cutoff value: 117.0 pg/mL) and hs-TnT (AUC = 0.61, cutoff value: 11.0 ng/L) concentrations were effective for discriminating E/e' ≥ 12. Multivariate logistic regression analyses showed that risk factors responsible for developing postoperative hypertension were NT-proBNP and hs-TnT using biomarkers and E/e' as independent variables, and NT-proBNP and SBP at admission using biomarkers and SBP at admission as independent variables. These findings suggest that NT-proBNP and hs-TnT concentrations, and SBP at admission, are useful to predict postoperative hypertension after minor to moderate surgery, and that left ventricular filling pressure is a primary factor associated with postoperative hypertension.</p>

    <p>  General anesthesia in patients with Parkinson disease should be performed with caution. Caution is particularly needed when patients with Parkinson disease have been prescribed opioids, since the combination of opioids and MAO inhibitors can cause serotonin syndrome and the use of fentanyl can increase muscle stiffness. The withdrawal of anti-Parkinsonian agents can cause the worsening of symptoms and the development of neuroleptic malignant syndrome, while their continued use may cause intraoperative circulatory changes.</p><p>  Here, the case of a 70-year-old male patient with previously diagnosed Parkinson disease who underwent surgical fixation of a left zygomatic and maxillary fracture is reported. The severity of the Parkinson disease was Ⅲ-Ⅳ on the Yahr Scale, and the patient was taking multiple anti-Parkinsonian drugs including dopamine/carbidopa, pramipexole hydrochloride, selegiline hydrochloride, entacapone, and droxidopa. A neurologist instructed the surgical team to limit the use of opioids and to administer levodopa intraoperatively. The patient’s routine medications were continued until the morning of surgery. General anesthesia was induced with intravenous thiamylal sodium, rocuronium bromide, and remifentanil, and the patient was orally intubated. Remifentanil was stopped immediately after intubation. The patient’s blood pressure was controlled with phenylephrine and ephedrine, as appropriate. General anesthesia was maintained with air, oxygen, and sevoflurane ; levodopa was administered intraoperatively. After an uneventful surgery, sugammadex sodium was administered, and the patient was extubated after awakening. Intravenous acetaminophen and flurbiprofen axetil were administered for postoperative analgesia. No worsening of the Parkinson disease symptoms was observed after the operation.</p><p>  In this case, we report a good surgical outcome with minimal opioid use and the continued perioperative administration of antiparkinsonian agents.</p>

    Purpose: Intravenous sedation is considered to decrease patient anxiety and tension during dental treatment, thereby reducing fluctuations in autonomic nerve activity and the risk of medical emergencies. We hypothesized that intravenous sedation would suppress the sympathetic nervous system during tooth extraction and relieve patient anxiety. Accordingly, the purpose of this study was to investigate the effects of intravenous sedation on autonomic nerve activity and psychological state during impacted mandibular third molar (IMTM) extraction. Methods: This prospective study included 40 healthy women scheduled for IMTM extraction under local anesthesia alone (control group; n = 20) or local anesthesia and intravenous sedation (sedation group; n = 20); of these, 34 women were evaluated. Heart rate variability (HRV) was analyzed to evaluate autonomic nerve activity. HRV and circulatory dynamics were measured during the treatment. The Modified Dental Anxiety scale and State-Trait Anxiety Inventory (STAI-S and STAI-T) scores were obtained before treatment. STAI-S scores were also obtained after treatment. Descriptive and bivariate statistical analyses were conducted, and a P-value <.05 was considered statistically significant. Results: The low-frequency/high-frequency ratio was lower in the sedation group than in the control group (P <.01). Compared with the control group, the sedation group showed a significantly greater decrease in post-treatment STAI-S scores relative to pre-treatment scores (P <.01). Conclusions: The findings suggest that sedation suppresses the sympathetic nervous system during IMTM extraction and decreases anxiety after the extraction. Further research should elucidate the mechanisms by which sedation suppresses the sympathetic nervous system.
