甲状腺内視鏡手術は整容性に優れ患者満足度も高い。一方で鎖骨下・前胸部・腋窩法など様々なアプローチに加え吊り上げ式や送気法など施設により様々である。本稿ではVANS変法における操作の定型化および場面ごとの手術のコツについて述べる。視野展開:甲状腺はエンドスワブなどで面を意識した圧排を行い,前頸筋は内視鏡用筋鈎を使用し確実な視野展開を行う。上極(上甲状腺動静脈)処理:血管周囲はく離鈎により上甲状腺動静脈を挙上し安全な血管処理が可能となる。反回神経周囲の操作および麻痺防止への対策:術中神経モニタリング装置(IONM),持続モニタリングの使用で確実な反回神経の同定ならびに牽引麻痺に対する予防を図る。エネルギーデバイスによる熱損傷や過剰牽引に対して注意し操作を行う。適切なデバイスの選択も重要である。安全な甲状腺内視鏡手術実施には,手術手技のポイントを理解し,十分な技術の習得が重要である。(著者抄録)

<p>We report a case of male breast cancer with a <i>BRCA2</i> mutation in a patient with a long-term history (7 years) of 5α-reductase inhibitor (5-ARI) use.</p><p>A 48-year-old man had been taking a 5-ARI for seven years to treat androgenetic alopecia. One year after discontinuing the medication, he noticed a mass in his left chest wall and presented to our hospital. Needle biopsy revealed invasive ductal carcinoma, and genetic testing confirmed the presence of a <i>BRCA2</i> mutation. The patient underwent total mastectomy with sentinel lymph node biopsy. Final pathological findings revealed invasive ductal carcinoma, scirrhous type, classified as pT1bN0M0, Stage I. Immunohistochemistry showed estrogen receptor (ER) -positive, progesterone receptor (PgR) -positive, HER2-negative status, and a Ki-67 index of 17%. The patient declined Oncotype DX testing, and tamoxifen monotherapy was selected as adjuvant therapy. However, he discontinued tamoxifen 10 months after initiation due to hair loss.</p><p>5-ARIs are widely used for the treatment of male-pattern hair loss ; however, a causal relationship between 5-ARI use and male breast cancer remains unestablished. Further case accumulation and investigation are warranted.</p>

Our recently created RNA-sequence-based microRNA (miRNA) expression signature in breast cancer clinical specimens revealed that some miR-30 family members were significantly downregulated in cancer tissues. Based on TCGA database analyses, we observed that among the miR-30 family members, miR-30a-3p (the passenger strand derived from pre-miR-30a) was significantly downregulated in breast cancer (BC) clinical specimens, and its low expression predicted worse prognoses. Ectopic expression assays showed that miR-30a-3p transfected cancer cells (MDA-MB-157 and MDA-MB-231) had their aggressive phenotypes significantly suppressed, e.g., their proliferation, migration, and invasion abilities. These data indicated that miR-30a-3p acted as a tumor-suppressive miRNA in BC cells. Our subsequent search for miR-30a-3p controlled molecular networks in BC cells yielded a total of 189 genes. Notably, among those 189 genes, cell-cycle-related genes (ANLN, MKI67, CCNB1, NCAPG, ZWINT, E2F7, PDS5A, RIF1, BIRC5, MAD2L1, CACUL1, KIF23, UBE2S, EML4, SEPT10, CLTC, and PCNP) were enriched according to a GeneCodis 4 database analysis. Moreover, the overexpression of four genes (ANLN, CCNB1, BIRC5, and KIF23) significantly predicted worse prognoses for patients with BC according to TCGA analyses. Finally, our assays demonstrated that the overexpression of ANLN had cancer-promoting functions in BC cells. The involvement of miR-30a-3p (the passenger strand) in BC molecular pathogenesis is a new concept in cancer research, and the outcomes of our study strongly indicate the importance of analyzing passenger strands of miRNAs in BC cells.

症例は68歳男性で、両側鼠径ヘルニアに対する腹腔鏡下鼠径ヘルニア修復術(TAPP)後3日目に嘔気、嘔吐、服膨満感が出現した。血液検査、Xp、腹部単純・造影CT各所見より内ヘルニアによる腸閉塞を疑った。術後10日目に再手術を行った結果、腹膜閉鎖部の中央が離開し間隙に小腸の陥入を認めた。小腸と腹膜の癒着を剥離し、小腸を引抜いて腸閉塞の解除を行った。腹膜の離開部は3-0 barbed suture糸で縫合閉鎖し癒着防止フィルムを貼付した。TAPP後の腹膜縫合閉鎖では、腸管陥入を防ぐため腹膜に過度の緊張を掛けず間隙の無い縫合を行うことが必要である。医学中央雑誌における1995年-2023年までのTAPP後における腸閉塞の報告は17例で、そのうち本例を含めて15例が腹膜離開を伴っていた。

A female in her 60's presented with a left-sided breast mass. A core needle biopsy specimen showed diffuse proliferation of a round cell tumor, which was positive for vimentin, NKX2.2, BCOR, and focal CD99 on immunohistochemistry (IHC). No fusion genes of the Ewing family sarcomas were detected. With a tentative diagnosis of primary breast sarcoma (PBS), total mastectomy was performed after chemotherapy. The resected tissues showed proliferation of round or spindle-shaped tumor cells with a high nuclear-to-cytoplasmic ratio, exhibiting solid and fascicular arrangements but no epithelial component or organoid pattern. While IHC indicated no particular histological diagnosis, genomic examination revealed gene alterations in MED12 p.G44D, MLL2 (KMT2D) p.T1496fs*27, and EGFR variant III (vIII). Moreover, a retrospective IHC study showed overexpression of EGFRvIII. A malignant phyllodes tumor (PT) with extensive sarcomatous overgrowth was indicated as an integrative diagnosis. This is a rare case of a malignant PT harboring EGFRvIII. The present case provides an importance of accurate diagnosis and genomic analysis of rare breast tumors, as malignant PT and PBS are different in its treatment strategy and prognosis.

センチネルリンパ節生検はcN0乳癌に対する標準術式として定着し,RI・色素併用法が推奨される.207例について,センチネルリンパ節へのRI-リンパ流を手術前日にRI注入時のSPECT-CTを用いて計測し,RI-リンパ流を規定する臨床病理学的因子,特に腋窩郭清の必要なセンチネルリンパ節マクロ転移の関与を検討した.RI-リンパ流はリンパ節転移の有無とは必ずしも相関せず,微小転移>転移陰性>マクロ転移の順に低下し,マクロ転移を伴う群で微小転移のみの群に比べ有意に低値であった.一方,RI-リンパ流は年齢と負の相関を示し,特に閉経後で低値であった.また,内下部(B)区域に腫瘍が占拠する症例で低値の傾向を示し,腫瘍径やBMIとは有意な相関はみられなかった.これらの結果から,若年,特に閉経前のB区域以外に占拠する症例でRI-リンパ流が低値な場合には腋窩郭清への術式変更があり得,高齢あるいはB区域の症例でもRI-リンパ流が良好であればその可能性は低いと予測できる.(著者抄録)

Purpose: To develop and identify machine learning (ML) models using pretreatment clinical and 2-deoxy-2-[¹⁸F]fluoro-d-glucose positron emission tomography ([¹⁸F]-FDG-PET)-based radiomic characteristics to predict disease recurrences in patients with breast cancers who underwent surgery. Procedures: This retrospective study included 112 patients with 118 breast cancer lesions who underwent [¹⁸F]-FDG-PET/ X-ray computed tomography (CT) preoperatively, and these lesions were assigned to training (n=95) and testing (n=23) cohorts. A total of 12 clinical and 40 [¹⁸F]-FDG-PET-based radiomic characteristics were used to predict recurrences using 7 different ML algorithms, namely, decision tree, random forest (RF), neural network, k-nearest neighbors, naive Bayes, logistic regression, and support vector machine (SVM) with a 10-fold cross-validation and synthetic minority over-sampling technique. Three different ML models were created using clinical characteristics (clinical ML models), radiomic characteristics (radiomic ML models), and both clinical and radiomic characteristics (combined ML models). Each ML model was constructed using the top ten characteristics ranked by the decrease in Gini impurity. The areas under ROC curves (AUCs) and accuracies were used to compare predictive performances. Results: In training cohorts, all 7 ML algorithms except for logistic regression algorithm in the radiomics ML model (AUC = 0.760) achieved AUC values of >0.80 for predicting recurrences with clinical (range, 0.892–0.999), radiomic (range, 0.809–0.984), and combined (range, 0.897–0.999) ML models. In testing cohorts, the RF algorithm of combined ML model achieved the highest AUC and accuracy (95.7% (22/23)) with similar classification performance between training and testing cohorts (AUC: training cohort, 0.999; testing cohort, 0.992). The important characteristics for modeling process of this RF algorithm were radiomic GLZLM_ZLNU and AJCC stage. Conclusions: ML analyses using both clinical and [¹⁸F]-FDG-PET-based radiomic characteristics may be useful for predicting recurrence in patients with breast cancers who underwent surgery.

Background: Delays in breast cancer diagnosis can allow the disease to progress to an incurable stage. However, factors that cause patients to delay seeking treatment are unclear. In this study, we aimed to identify behavioral economic factors and personality characteristics of patients with breast cancer who had a delayed diagnosis. Methods: We analyzed questionnaires completed by 41 patients with breast cancer. A delayed diagnosis was defined if the time between the first symptom and the medical visit was more than 6 months. Results: We found 11 patients who had a delayed diagnosis. The significant characteristics associated with patients with breast cancer who had delayed diagnosis were: (i) less experience with breast cancer screening; (ii) progressive disease stage; and (iii) low time and future time preference. We found no significant behavioral economic factors other than time preference, and personality that differed between patients with breast cancer who did and did not have a delayed diagnosis. Conclusion: Low time preference rate is a characteristic of patients with breast cancer who had a delayed diagnosis.

<p>Sentinel node biopsy for treatment of cN0 early breast cancer has been established and a double method with radioisotope (RI) and dye is recommended. We mainly use RI and Indocyanine Green (ICG) supplementally.</p><p>Clinicopathological factors influencing RI-lymph flow to sentinel nodes and prediction of axillary node dissection were evaluated among 207 patients with cN0 breast cancer. RI-lymph flow was calculated by RI-counts per volume in sentinel nodes and the injection site using SPECT-CT ; SIEMENS Symbia Intevo6^®. RI-lymph flow did not always correlate with sentinel node metastasis. It increased in patients with nodal micro-metastasis of diameter ≦2mm, although it decreased in patients with macro-metastasis >2mm, for whom axillary node dissection was considered.</p><p>RI-lymph flow decreased in the elderly, especially in menopausal females and in patients with cancer located at the inner-lower quadrant, did not relate to the invasive size of the cancer nor body mass index. In these results, RI-lymph flow was considered to be useful for predicting sentinel node macro-metastasis and surgical conversion to axillary node dissection. That is, they are possible in younger, especially premenopausal females with cancer located in sites other than the inner-lower quadrant and low RI-lymph flow. They are unlikely in the elderly or patients with cancer located in the inner-lower quadrant and high RI-lymph flow.</p>

Breast cancer is primarily classified into ductal and lobular types, as well as into noninvasive and invasive cancer. Invasive cancer involves lymphatic and hematogenous metastasis. In breast cancer patients with distant metastases, a neutrophil-derived serine protease; cathepsin G (Cat G), is highly expressed in breast cancer cells. Cat G induces cell migration and multicellular aggregation of MCF-7 human breast cancer cells; however, the mechanism is not clear. Recently, platelet-activating factor (PAF)-acetylhydrolase (PAF-AH), the enzyme responsible for PAF degradation, was reported to be overexpressed in some tumor types, including pancreatic and breast cancers. In this study, we investigated whether PAF-AH is involved in Cat G-induced aggregation and migration of MCF-7 cells. We first showed that Cat G increased PAF-AH activity and elevated PAFAH1B2 expression in MCF-7 cells. The elevated expression of PAFAH1B2 was also observed in human breast cancer tissue specimens by immunohistochemical analysis. Furthermore, knockdown of PAFAH1B2 in MCF-7 cells suppressed the cell migration and aggregation induced by low concentrations, but not high concentrations, of Cat G. Carbamoyl PAF (cPAF), a nonhydrolyzable PAF analog, completely suppressed Cat G-induced migration of MCF-7 cells. In addition, PAF receptor (PAFR) inhibition induced cell migration of MCF-7 cells even in the absence of Cat G, suggesting that Cat G suppresses the activation of PAFR through enhanced PAF degradation due to elevated expression of PAFAH1B2 and thereby induces malignant phenotypes in MCF-7 cells. Our findings may lead to a novel therapeutic modality for treating breast cancer by modulating the activity of Cat G/PAF signaling.

Background/Aim: The development and application of cancer immunotherapy to pancreatic cancer has not progressed because its efficacy has not been proven in clinical trials. In this study, we aimed to explore potential targets of immune checkpoint inhibitor therapy for pancreatic cancer treatment. Materials and Methods: We collected resected specimens from 40 patients with pancreatic cancer who underwent resection at our Institution without any preoperative treatment. We evaluated the expression of molecules in the programmed death receptor-1 (PD-1), T cell immunoglobulin mucin-3 (Tim-3)/Galectin-9, and CD155/T cell immunoreceptor with Ig and ITIM domains (TIGIT) pathways using immunohistochemical staining. The correlation between the expression pattern of these molecules and patient prognosis were assessed using Kaplan-Meier analysis. Results: An increased number of CD8⁺ T cells in pancreatic cancer tissue was significantly associated with a better patient prognosis. Additionally, patients with a higher ratio of PD-1 expression to CD8⁺ T cells had a worse prognosis. We observed no correlation between the Tim-3/Galectin-9 and CD155/TIGIT pathways and patient prognosis. Conclusion: Modifications in the immune environment to increase T cell infiltration into tumors could result in the PD-1 pathway becoming a potential target to treat pancreatic cancer using immune checkpoint inhibition.

<p>Periareolar mammoplasty is a step 2-1 procedure of OPBCS Volume displacement. Partial mastectomy is done via periareolar incision which is inconspicuous postoperatively. It is mainly used for cancer lesions located on the upper area of the breast. Partial mastectomy with a unique shape via circular incision is performed, then, volume displacement is performed to avoid deformity of the breast. Periareolar mammoplastyは,OPBCS Volume displacementレベル2-(1)の手技である。傍乳輪切開部より病変部へ到達し乳房部分切除を実施するため切除瘢痕が目立ちにくいのが特徴で,おもに乳房上部病変の部分切除時に用いられる。傍乳輪切開の形状を生かした乳房部分切除,または乳輪より大きいサイズの円弧の円周上の切開部からの乳房部分切除を行う。切除部の陥凹や乳頭乳輪複合体(以下乳頭乳輪,NAC:nipple areolar complex)のゆがみ・偏位が生じないようにvolume displacementを行う。NACの血流維持に留意して実施することが重要である。(著者抄録)

乳房部分切除後の乳房外側1/4領域の欠損を再建するためのimmediate volume replacementによる乳房オンコプラスティックサージャリーについて、原法とその後に改変された修正法の成績を後ろ向きに検討した。乳房温存手術と胸背部筋膜脂肪皮弁を用いた当該手術を施行された早期乳癌患者を対象とした。原法に追加された修正法を、胸背部筋膜脂肪皮弁(TDAFF)の遠位端に三日月形の真皮を追加する手技(修正法1)、TDAFF近位端に三日月形の真皮を追加する手技(修正法2)、修正法2に加え乳腺下形成を追加する手技(修正法3)、修正法3に加えと三日月型真皮をメルセデスベンツ状型に変更した手技(修正法4)に分割し、これら原法と修正手技における形成期間、術後合併症、腫瘍学的成績、整容的成績を比較した。原法および修正1～4法の患者数はそれぞれ26、9、15、23、10例、術後合併症はそれぞれ5例、0例、2例、1例、0例で、局所再発は原法のみ3例、遠位再発は原法1例、修正法1で1例、修正法2で3例見られた。良好または優良と評価された整容性成績は、それぞれ73%、56%、73%、83%、100%であった。本乳房オンコプラスティックサージャリーの成績は、修正法により改善したことが示された。

Background: The treatment of early breast cancer using breast conservation therapy (BCT) commonly ensures local control and acceptable cosmetic results. We report a useful technique including the use of a thoracodorsal adipofascial cutaneous flap for reconstructing defects in the outer quadrant area after partial mastectomy, which achieved excellent results. Methods: During the past 15 years, some modifications have been added to the original method at a rate of one modification every 2–5 years. We classified these modifications into the original method and four modified methods. Modification I: addition of a crescent-shaped dermis on the distant edge of the thoracodorsal adipofascial flap (TDAFF), Modification II: addition of a crescent-shaped dermis on the proximal edge of the TDAFF, Modification III: addition of inframammary formation plus Modification II, and Modification IV: change of a crescent-shaped dermis to a Benz-shaped (shaped like the Mercedes Benz logo) one plus Modification III. We compared the plastic period, postoperative complications, oncological results, and cosmetic results among the original and four modified groups. Results: The patient number was 26, 9, 15, 23, and 10 in the original and Modification I, II, III, and IV groups, respectively. The median observation period was 115, 92, 67, 51, and 32 months, respectively. Postoperative complications were seen in 5 (19%), 0, 2 (13%), 1 (5%), and 0 patients, respectively. Local recurrence was seen in 3 (12%), 0, 0, 0, and 0 patients, respectively. Distant recurrence was seen in 1 (4%), 1 (11%), 3 (20%), 0, and 0 patients, respectively. Cosmetic results evaluated as good–excellent were seen in 19 (73%), 5 (56%), 11 (73%), 19 (83%), and 10 (100%) patients, respectively. Conclusions: Oncoplastic surgery using an immediate volume replacement technique with a thoracodorsal adipofascial flap was improved by adding some modifications.

症例は46歳,女性.2013年,他院で右乳癌に対して右乳房部分切除術+腋窩リンパ節郭清術を施行.最終病理診断は浸潤性小葉癌,pT3N1(1/22)M0 pStageIIIAの診断であった.2017年6月に便秘が持続し,精査の結果,直腸癌(signet-ring cell carcinoma)cT4bN0M0 cStageIIの診断となった.CTで子宮や仙骨への浸潤が疑われ,人工肛門造設術を実施した後にFOLFOXIRI+bevacizumab(BV),S-1+BVを投与し,その後Hartmann手術を行った.直腸手術検体の免疫染色はER陽性,PgR陰性,HER2 1+,E-cadherin陰性で原発性乳癌と形態も類似しており,浸潤性小葉癌の直腸転移と診断した.直腸腫瘍術後はレトロゾール内服治療を行っており,術後3年間で再発病変なく経過している.浸潤性小葉癌が消化管転移をきたすことは稀ではないことが知られているが,実臨床では診断に難渋することも少なくない.乳癌直腸転移の診断と治療方針について考察した.(著者抄録)

症例は63歳女性で、早期乳癌と診断された。罹患部位が側方に限局しており乳頭-乳輪領域に近接していること、手術手技の説明後にインフォームドが得られたこと、手術瘢痕部の長さに懸念を抱いていることから、乳房部分切除術と二重正三角形皮弁切除を併用する乳房オンコプラスティックサージェリーの適応症例と判断した。円柱型の部分切除を計画し、切除マージンは2.0cmに設定した。また、2枚の正三角形皮弁を用いることとし、1枚は切除領域、もう1枚は外胸部領域に一直線上の配置となるよう計画を立てた。乳輪周囲の三日月形領域を12時方向から6時方向にかけて脱上皮化を施し、円柱状乳房組織、正三角形皮弁および脂肪組織を一括切除した。乳房下部線を縫合した後、皮膚-腺皮弁を水平方向にスライドさせ欠損部の充填を図った。術後の病理所見では切除断端は陰性であり、術後放射線療法とアロマターゼ阻害剤によるホルモン療法を施行した。出血、感染症、脂肪壊死、乳頭-乳輪領域の血流障害といった合併症は認められず、術後72ヵ月、再発所見はみられず乳房の良好な対称性が獲得されている。

The treatment of early breast cancer using oncoplastic breast surgery (OBS) has been gradually increasing in popularity and is recognized for its efficacy in local control and excellent cosmetic results. We herein report a useful technique for obtaining symmetry of the breast shape for an early breast lesion located in an outer area, close to the nipple-areola, in a Japanese patient with ptotic, fatty breasts. We designed two equilateral triangles: one just upon the resected area and the other on the axilla. They were located on a straight line, with one top pointed to the cranial side and one to the caudal side. A crescent area around the areola was de-epithelialized in the 12 o’clock and 6 o’clock directions. Columnar-shaped breast tissue and an equilateral triangular skin flap and fatty tissue were removed together. To fill the defect, a skin-glandular flap was slid horizontally after suturing the inframammary line. Although an incision scar was formed on the breast and lateral chest wall in a Z-shape, this new technique was able to achieve not only cancer control but also excellent cosmetic results.

乳腺の紡錘細胞癌は全乳癌の0.1～0.2%と稀で,化学療法抵抗性や早期遠隔再発などで予後不良とする報告が多い.取扱い規約は比較的予後の良い線維腫症様化生癌も含めているが,WHO分類は両者を区別している.乳腺過誤腫は「breast in breast」と称され,乳腺良性腫瘍の5%未満の稀な腫瘍である.過誤腫に発生した乳癌の報告は24例と少なく,組織型は乳管癌16(浸潤13,非浸潤3),小葉癌6(浸潤4,非浸潤2),両者の併存1で,特殊型は粘液癌の1例のみであった.今回著者らは,過誤腫内乳癌の25例目で組織型では初めての紡錘細胞癌の症例を経験した.症例は85歳の女性で,左乳房腫瘤を主訴に受診した.画像上,腫瘤は二重構造を示し,脂肪濃度で高エコーの8cm大の境界明瞭な腫瘤内に軟部濃度で低エコーの4.5cm大の不整な腫瘤が存在した.低エコー部分からの針生検は紡錘細胞癌の診断であった.乳房部分切除を行い,最終病理診断は「乳腺過誤腫内に発生した紡錘細胞癌,pT2NxM0:StageIIA,ER(-),PgR(-),HER2/neu(-),Ki-67 LI=17%」であった.(著者抄録)

症例は40歳代女性で、下腹部痛を主訴とした。13年前に乳癌(pT1aN0M0、病期IA)による左乳房切除術と補助療法を受けていた。乳癌はエストロゲン受容体(ER)とプロゲステロン受容体(PgR)陽性、ヒト上皮成長因子受容体2(HER2)陰性であった。今回の受診では、造影CTで左肺に多発腫瘤、右腎に1個の腫瘤が発見され、下大静脈内の腫瘍血栓も明瞭に認められた。腎病変は原発腎癌と診断された。外科的切除するには病巣が大き過ぎたため、腎細胞癌に対してニボルマブとイピリムマブによる分子標的治療を開始した。しかし、開始3ヵ月後でも腫瘍は縮小しなかった。そこで腎と肺の病巣に対しCTガイド下針生検を行ったところ、両病巣とも乳癌の転移巣(ERとPgRに陽性、HER2陰性)であることが判明した。アベマシクリブ、タモキシフェン、リュープロレリンによる併用治療を開始したが、その数ヵ月後には肝機能と腎機能は悪化し、治療開始から6ヵ月後に転移病変進行のため死亡した。

<p>The patient was a 46-year-old woman who had received right breast conserving surgery and axillary lymph node dissection (II) for right breast cancer elsewhere 5 years previously. The final pathological diagnosis was invasive lobular carcinoma, pT3N1 (1/22) M0 pStage IIIA. Four years later, she complained of severe constipation and, following a close examination, she was diagnosed with rectal cancer (signet-ring cell carcinoma) cT4bN0M0, cStage II. Infiltration into the uterus and sacral bone was suspected with a CT scan. After colostomy, we started administration of FOLFOXIRI plus bevacizumab (Bev), S-1 plus Bev, followed by Hartmann's procedure. Immunostaining of the rectal lesion showed ER (+), PgR (-), HER2 (1+), and E-cadherin (-). The rectal lesion was similar in morphology to the primary breast cancer. The diagnosis of rectal metastasis of invasive lobular carcinoma was made. Oral letrozole treatment was started, and she has been followed without metastasis for 3 years after the rectal surgery. It is known that invasive lobular carcinoma sometimes causes gastrointestinal metastasis, but its diagnosis is often difficult in clinical practice. The diagnosis and treatment policy for rectal metastasis of breast cancer are discussed.</p>

<p>Spindle cell carcinoma is an extremely rare histological breast tumor type, accounting for 0.1-0.2% of all breast cancers. Its prognosis has been said to be poor, except for a sub-type of fibromatosis-like metaplastic carcinoma.</p><p>Mammary hamartoma is also a rare breast tumor known as “breast in breast”, accounting for 5% or less of all benign breast tumors. Although coexistence of malignancy with a hamartoma is uncommon, 24 cases of breast cancer arising in mammary hamartomas have been reported. Most of their histological types were ductal or lobular carcinoma, 17 invasive, 5 non-invasive, and one mixed, except for one mucinous carcinoma. This report presents the 25th case of breast cancer arising in a mammary hamartoma, and the first case whose histological type was spindle cell carcinoma.</p><p>An 85-year-old woman was referred to our hospital with a left breast tumor. On imaging, the tumor consisted of a double structure : the outer structure was well demarcated and homogeneous, high-echoic, adipose tissue density, 8.0 cm in diameter ; and the inner was irregular, low-echoic, soft tissue density, and 4.5 cm in diameter. She underwent partial mastectomy, and the pathological diagnosis was spindle cell carcinoma arising in a left mammary hamartoma, pT2NxM0 ; Stage IIA.</p>

妊娠25週の30歳代女性。左乳房に発赤および疼痛が出現した。乳房炎と診断され抗菌薬を投与されたが、症状は改善しなかった。その後、両下腿前面に多発性紅斑病変、両下肢に関節炎が出現した。結節性紅斑を伴う肉芽腫性乳房炎(GM)を疑い、プレドニゾロン(PSL)を投与した。下肢症状は改善したが、左乳房症状は持続した。左乳房膿瘍の針生検によりGMが確認された。妊娠中であったため、脂溶性抗菌薬を用いず、PSLのみ投与した。乳房症状が徐々に改善したためPSL用量を漸減し、中止した。PSL投与期間は17ヵ月であった。乳房生検標本の病理所見およびグラム染色により最終的に嚢胞性好中球性肉芽腫性乳房炎と診断した。

症例は68歳女性で、出生以来、左乳房下に副乳を有しており、20年前より副乳から軽度の血性分泌物を認めていたが、細胞診では悪性所見はみられなかった。今回、副乳下部に分泌物を伴わない軽度圧痛を呈する腫瘤を自覚して近医を受診し、穿刺吸引細胞診で悪性所見がみられたため当院に紹介された。身体所見で左正常乳房下に小豆大の副乳を認め、マンモグラフィー、超音波検査、MRIにて左乳腺下部に境界不明瞭な不規則かつ不均一な腫瘤が検出され、腫瘤と真皮結節との交通がみられた。超音波ガイド下針生検を施行したところ、浸潤性乳管癌であり、左乳房下部に生じた副乳癌と診断を下した。初診1ヵ月後、乳腺拡大切除術とセンチネルリンパ節生検を施行、術後の組織病理所見と免疫染色結果から副乳組織に生じた浸潤性乳管癌と診断した。浸潤サイズは径1.4cmでリンパ脈管浸潤はみられず、手術マージンは1.5cm以上であった。また、腫瘍はエストロゲン受容体、プロゲステロン受容体およびHer2/neuに陰性を示しており、リンパ節に転移所見はみられなかった。術後8ヵ月、経過は良好であり、アジュバントホルモン療法としてレトロゾール2.5mg/日の投与を予定している。

Our recent research has revealed that passenger strands of certain microRNAs (miRNAs) function as tumor-suppressive miRNAs in cancer cells, e.g., miR-101-5p, miR-143-5p, miR-144-5p, miR-145-3p, and miR-150-3p. Thus, they are important in cancer pathogenesis. Analysis of the miRNA expression signature of breast cancer (BrCa) showed that the expression levels of two miRNAs derived from pre-miR-99a (miR-99a-5p and miR-99a-3p) were suppressed in cancerous tissues. The aim of this study was to identify oncogenic genes controlled by pre-miR-99a that are closely involved in the molecular pathogenesis of BrCa. A total of 113 genes were identified as targets of pre-miR-99a regulation (19 genes modulated by miR-99a-5p, and 95 genes regulated by miR-99a-3p) in BrCa cells. Notably, FAM64A was targeted by both of the miRNAs. Among these targets, high expression of 16 genes (C5orf22, YOD1, SLBP, F11R, C12orf49, SRPK1, ZNF250, ZNF695, CDK1, DNMT3B, TRIM25, MCM4, CDKN3, PRPS, FAM64A, and DESI2) significantly predicted reduced survival of BrCa patients based upon The Cancer Genome Atlas (TCGA) database. In this study, we focused on FAM64A and investigated the relationship between FAM64A expression and molecular pathogenesis of BrCa subtypes. The upregulation of FAM64A was confirmed in BrCa clinical specimens. Importantly, the expression of FAM64A significantly differed between patients with Luminal-A and Luminal-B subtypes. Our data strongly suggest that the aberrant expression of FAM64A is involved in the malignant transformation of BrCa. Our miRNA-based approaches (identification of tumor-suppressive miRNAs and their controlled targets) will provide novel information regarding the molecular pathogenesis of BrCa.

乳癌の分子病態に密接に関与している癌抑制型マイクロRNA(miRNAs)、miR-99a-5pならびにmiR-99a-3pにより制御される発癌遺伝子を調査した。その結果、3種類のデータセットから、乳癌細胞においてmiR-99a-5pおよびmiR-99a-3pの標的遺伝子として、113種類の遺伝子が同定された。また、これら113種類のうち19種類の遺伝子はmiR-99a-5pにより、95種類の遺伝子ではmiR-99a-3pにより制御され、FAM64A遺伝子のみがmiR-99a-5pとmiR-99a-3pに制御されていた。さらに、これら遺伝子のうち、C5orf22遺伝子、YOD1遺伝子およびFAM64A遺伝子を含む16種類の遺伝子高発現により、乳癌患者の生存率低下が有意に予測されることがThe Cancer Genome Atlas(TCGA)データベースにより確認され、FAM64A遺伝子発現と乳癌サブタイプの分子病理との関連を調べた結果からは、乳癌患者から採取した臨床検体からFAM64A遺伝子の上方制御が認められたことから、FAM64A遺伝子発現はLuminal-AおよびLuminal-Bサブタイプ患者間では、有意に異なることが明らかにされた。以上より、乳癌の悪性形質転換にはFAM64A遺伝子の異常発現が関与していることが示され、本研究で用いたmiRNAを用いたアプローチ法は、乳癌の分子病理解明に役立つことが示唆された。

Purpose: In endoscopic surgery, surgeons occasionally encounter difficulties due to visual field obstruction by muscles or blood vessels. In these situations, specialized instruments that can effectively retract these obstructions are required. Recently, we developed a new detachable wire-rimmed retractor (KN retractor) for narrow-space surgery. Methods: We evaluated the utility of this KN retractor in 15 patients with thyroid and parathyroid disease. Of those, five patients with papillary thyroid cancer had gasless endoscopic hemithyroidectomy with central node dissection, five underwent endoscopic total thyroidectomy for Graves’ disease, and the remaining five received endoscopic parathyroidectomy with gas insufflation. Results: Surgeons were able to perform meticulous operations in a satisfactory visual field supported by the KN retractor. In all patients, the strap muscles were preserved without cutting. The average operating time was 149, 154, and 81 min in patients who underwent hemithyroidectomy with central node dissection, total thyroidectomy, and parathyroidectomy, respectively. Gas insufflation was successfully completed in all cases while maintaining sufficient airtightness. Conclusions: The KN retractor is suitable for both the gasless lifting method and gas insufflation surgery in a narrow space. We believe that the KN retractor is a new device that will greatly improve the safety and shorten the operation time in endoscopic surgery.

新規開発した着脱式鋼線筋鉤(KNリトラクター)の有用性を、甲状腺・副甲状腺疾患患者15例において評価した。このうち甲状腺乳頭癌の5例には吊り上げ法による内視鏡下甲状腺葉切除術および中心リンパ節郭清、グレーブス病の5例には内視鏡下甲状腺全摘術、残りの5例には送気法による内視鏡下副甲状腺摘出術を施行した。KNリトラクターにより良好な視野が確保されるとともに、全例で舌骨下筋の切断が回避された。平均手術時間は3術式でそれぞれ149分、154分、81分であった。また送気法においては十分な気密性が維持された。KNリトラクターは吊り上げ法あるいは送気法による手術の両方に適しており、narrow spaceでの内視鏡手術における良好な視野の確保にきわめて有用であると考えられた。

A 39-year-old gravida 1 para 1 pregnant Japanese woman underwent skin-sparing mastectomy and axillary lymph node dissection with immediate breast reconstruction (IBR) using a tissue expander (TE) at 32 weeks of pregnancy under general anesthesia. Inserted TE (300 cc) was expanded during breast feeding while the volume was 240 cc of the resected breast tissue. One month after delivery, 2 months after surgery, the contralateral healthy breast increased in size and the inframammary line was deviated toward a caudal site which was larger than 300 cc-inflated TE. She stopped breast feeding to receive a systemic chemotherapy after one months-breast feeding. At 3 months after delivery, the healthy breast size was smaller than the 250 cc-expanded breast and both the inframammary lines were at the same level. She was diagnosed local recurrence 3 month-postoperatively, so we resected the recurrent lesion and exchanged TE to silicon breast implant immediately. Finally, a good symmetry was obtained after insertion of the 220 cc SBI. At an IBR using TE, we should know the dynamic change of breast volume and the level of inframammary line of the healthy breast during those phases of pregnancy, delivery, and nursing.

甲状腺癌患者の内視鏡下リンパ節切除に適用される改良型双方向アプローチビデオ補助頸部手術(BAVANS)を紹介した。BAVANSは180°異なる2方向からアプローチする手技であり、前胸部アプローチ(腋窩アプローチ)では甲状腺切除と中心リンパ節の上部を切除し、顎下腺アプローチでは中心リンパ節の中部と下部の切除を行う。改良型BAVANSは複数の視野角を有する硬性内視鏡を用いるものであり、前頸部におけるカメラポート数を減らしながらも簡便な操作性を保持し、BAVANS法と同等の質を有する。改良型BAVANSの平均手術時間は片側性中心リンパ節切除(CND)において193分、甲状腺全摘+両側性CNDで245分であり、平均出血量は甲状腺全摘+両側性CNDでは26±16.4mL、甲状腺亜全摘+片側性CNDで15.6±26.8mLであった。また、平均リンパ節採取数は片側性CNDにおいて10±4.5、両側性CNDで7.4±3.8となっていた。甲状腺癌患者のリンパ節切除に際して、2方向からのアプローチは有用性が高く、完全切除のために有望な手技であると考えられた。

症例は54歳,閉経後の女性.性同一性障害のため,女性から男性への適合目的に男性ホルモンを投与中であった.乳癌検診で右乳房石灰化を指摘され,近医を受診した.ステレオガイド下針生検で,粘液癌の診断であった.術前診断は,cT1N0M0 StageIAであり,乳房全切除とセンチネルリンパ節生検を施行した.最終病理診断は,mucinous carcinoma,pT1cN0M0,StageIA,ER:score5,PgR:score5,HER2:1+,Ki-67:1%であった.術後,アロマターゼ阻害薬を内服し,術後24ヵ月再発なく経過中である.アンドロゲンは,本人希望により術後に再開した.性同一性障害のために,アンドロゲンを投与中に発生した乳癌の報告は稀である.アンドロゲン投与による乳癌発症リスクと,発症後の予後に与える影響について,今後更なるデータの蓄積が望まれる.(著者抄録)

症例は71歳女性で、右乳房に有痛性の巨大な腫瘍を認めたため来院した。22年前に乳房の一部を切除されていたが、残りの腫瘍は残存していた。1年前に行った細針吸引細胞診で潜在的悪性腫瘍と診断されていた。その後、コア生検が施行されたが、病変が良性か悪性腫瘍かの確定診断には至らなかった。今回の来院では、画像検査所見と生検所見から悪性腫瘍が疑われたため、右乳房切除術とセンチネルリンパ節生検を施行した。出血性結節性病変は28mm大であり、乳房組織の中心に認めた。アポクリン乳頭状病変の微小病巣を複数認めたが、筋上皮細胞層はなく、あまりに微小なため悪性か良性かを確定できなかった。壊死領域全体に乳頭状および管状構造が残存しており、乳頭状病変が壊死したと考えられた。周囲の乳房組織に上皮内癌は認められなかった。外側および深部断端と切除センチネル節に腫瘍細胞の浸潤はなかった。アジュバント療法は行わず、術後1年間再発せずに生存している。

症例は45歳女性で、右乳房の外側下方に腫瘤を認めたため来院した。穿刺吸引生検と細胞診では悪性腫瘍陰性であった。3年後、腫瘤が三つに増加していることを認めたため再受診し、細胞診は悪性腫瘍陰性であった。局所麻酔下で腫瘍摘出術が施行され、病理学的診断は浸潤性腺管癌、病理学的断端陽性であり、当院にてT1cN2aM0ステージIIIA乳癌と診断した。痩せ型であり、乳房インプラント、筋肉皮弁、血管吻合による自家乳房再建を用いた乳房再建は拒否し、局所皮弁を用いたvolume replacementと遊離真皮脂肪移植のみ同意した。そこで、乳房部分切除と腋窩リンパ節郭清、複数の局所皮弁を用いたvolume replacementと外側腹部からの遊離真皮脂肪移植を実施した。病理学的にはpT1N1aM0ステージIIAと診断された。術後放射線療法と全身ホルモン療法を実施し、現在まで遠隔および局所再発は認めていない。

<p>A 54-year-old postmenopausal woman with gender identity disorder was administered androgen as part of the female-to-male transformation. Right breast calcification was detected during breast cancer screening, and she underwent physical examination at a local hospital. She was diagnosed with mucinous carcinoma via needle biopsy under stereotactic guidance. With a pre-operative diagnosis of T1N0M0 Stage IA, she underwent a total mastectomy with sentinel lymph node biopsy. Histological examination revealed mucinous carcinoma (pT1cN0M0 Stage IA), ER (score 5), PgR (score 5), HER2 (1+), and Ki-67 index (1%). At the patient's request, androgen supplementation was re-started after the surgery. The occurrence of breast cancer during androgen administration for gender identity disorder is extremely rare. The mechanism and clinical outcome will be evaluated after further data collection.</p>

トリプルネガティブ乳癌(TNBC)の新規治療標的を特定するため、臨床検体を用いてTNBCのmicroRNA発現シグネチャーを調べた。外科的切除術を施行した乳癌患者より採取した35検体を用いて、small RNAシーケンシング、qRT-PCR法、マイクロアレイ遺伝子発現解析、in silico解析などを行った。TNBC組織では、計104のmicroRNAで発現異常がみられ、56で発現亢進、48で発現低下がみられた。発現が低下したmicroRNAの中でmiR-204-5pが最も顕著な低下を示した。次に、miR-204-5pの抗腫瘍作用を調べた。乳癌細胞でのmiR-204-5pの異所発現によって癌細胞の遊走と浸潤が低下した。ゲノムワイド遺伝子発現解析とin silicoデータベース検索によって、32の遺伝子がmiR-204-5pの標的と推定された。AP1S3、RACGAP1、ELOVL6、LRRC59の高発現は乳癌患者の予後不良と有意に関連していた。ルシフェラーゼレポーターアッセイにより、miR-204-5pがAP1S3の発現を直接調節していることが分かった。TNBC臨床検体ではAP1S3の過発現がみられ、癌細胞の悪性度を高めていることが示された。

症例は64歳女性と67歳女性で、2例とも乳癌に対し、乳房温存手術および放射線治療を受けてから2、3年後に残存乳房に結節が出現した。この結節は、それぞれ皮膚生検と開胸下生検により血管肉腫と診断された。病変の拡大範囲を明らかにするため、術前にマッピング生検が行われた。2例とも、乳房切除術、拡大皮膚切除術および中間層皮膚移植が行われた。病理検査では、2例とも腫瘍の完全切除が確認され、術後に化学療法が行われた。2例とも4年間で局所または遠隔再発は認められなかった。

症例は24歳女性で、手拳大の右乳房腫瘤が出現し、次第に小児頭大に増大した。右乳房に皮膚露出と出血を伴う腫瘍を認め、針生検にて悪性葉状腫瘍、紡錘細胞癌、間質肉腫などが疑われたため、貧血管理と手術目的で当院に緊急入院となった。右乳房に31×26×16cm大の巨大腫瘤を認めた。長径25cmにわたって腫瘍が露出しており、壊死と出血を認めた。造影CTで、右乳房に26cm大の内部不均一な腫瘤と右腋窩リンパ節の腫大を認めた。針生検病理像から悪性葉状腫瘍、紡錘細胞癌、間質肉腫が疑われたため、右乳房悪性腫瘍の術前診断で手術を施行した。術中の肉眼所見でも胸壁浸潤は認めず、腫瘍の完全切除が可能であった。病理組織学的所見では、大部分で強い核異型と多数の核分裂像を認め、悪性葉状腫瘍の診断であった。切除断端は陰性、摘出リンパ節にも転移は認めなかった。

症例は82歳、女性。他院で右乳癌に対し、乳房部分切除術およびセンチネルリンパ節生検が施行された。乳腺欠損部に乳房外側と側胸部の脂肪弁が授動・補填された。最終病理はpTis(45×55×13mm)、g、ly(-)、v(-)、surgical margin(+)<3mm、N0(sn)であった。術後2ヵ月より放射線療法と内分泌療法が実施された。術後2年6ヵ月、硬結部に発赤が出現し一部が自壊した。針生検で脂肪壊死および膿瘍と診断された。保存的に観察されていたが、疼痛が出現し自壊部から膿性排液が続いた。術後2年9ヵ月、加療目的に当院を紹介受診した。経過および画像所見より右乳房温存術後の脂肪壊死と診断し、右乳房切除術を施行した。切除標本に悪性所見はなく、皮膚瘻孔を伴った脂肪壊死と膿瘍が認められた。術後は内分泌療法を継続し、乳房切除術後1年8ヵ月現在、再発所見なく経過している。(著者抄録)

甲状腺癌内視鏡手術の術式として、頭側-尾側の二方向からアプローチするBidirectional approach of Video-Assisted Neck Surgery(BAVANS)法を開発し、これを用いて中央区域リンパ節完全郭清術を施行した手術成績を検討した。当術式では、リンパ節郭清に先立ち、まずガスを用いない標準的な前胸部アプローチまたは腋窩アプローチにて内視鏡下甲状腺切除術を施行した。その後に、執刀医は患者の頭部近くに立ち位置を変え、下顎領域上頸部病変の正面にポートを3本挿入し、頭部越しに尾側へ向けて傍気管病変へアプローチした。BAVANS法により頭尾側方向への優れた視野が確保でき傍気管リンパ節へのアクセスも容易であった。2011年9月に当院でBAVANS法を導入してから、パイロット試験として甲状腺乳頭癌16例の手術を当術式にて施行し、いずれも術後の経過は良好であった。8例では甲状腺全摘術または亜全摘術が行われ、5例では両側中央区域リンパ節郭清術が施行されていた。片側中央区域リンパ節郭清における回収リンパ節の平均数は9個で、標準的な直視下手術における回収数よりも多かった。全例で術後5時間以内に経口摂取を開始できた。1例で術後Horner症候群を発症したが、その他の合併症は見られなかった。
