    Background: Myasthenia gravis (MG) without thymoma is often treated with extended thymectomy to improve symptom control and reduce immunotherapy requirements. Schwannoma of the recurrent laryngeal nerve (RLN) is rare, and surgical resection is usually curative. However, operative manipulation carries a risk of temporary or permanent vocal fold paralysis. When these conditions coexist, choosing staged versus single-stage surgery is non-trivial. A synchronous operation can consolidate perioperative care and recovery but demands careful planning to prevent respiratory complications, particularly if RLN palsy or myasthenic crisis occur. Case presentation: A 65-year-old woman with generalized, thymoma-negative MG (acetylcholine-receptor antibody positive) had no hoarseness or dysphagia. Flexible laryngoscopy confirmed mobile vocal folds bilaterally. Chest radiography showed mediastinal widening without diaphragmatic paralysis. Contrast-enhanced CT and MRI revealed a right anterior mediastinal cystic lesion and a solid mass in the left tracheoesophageal groove abutting the RLN. ^18F-fluorodeoxyglucose positron emission tomography demonstrated uptake in the solid lesion (SUV_max 4.89). Transesophageal endoscopic ultrasound–guided fine-needle aspiration yielded spindle cells compatible with schwannoma, and the mass was considered RLN-derived. A single-stage median sternotomy was undertaken. Extended thymectomy was performed with bilateral phrenic exposure and en bloc removal of thymic/perithymic fat, followed by nerve-sparing enucleation of the left RLN tumor through a longitudinal epineurial window and meticulous intracapsular dissection, preserving macroscopic neural continuity. The postoperative intensive care unit course was uneventful: no dyspnea or dysphagia, normal voice, and laryngoscopy confirmed intact vocal fold mobility. A chest radiograph on postoperative day 5 was unremarkable, and the patient was discharged on day 7. Histopathology showed alternating Antoni A/B areas with virtually no mitoses; tumor cells were strongly S-100–positive, confirming schwannoma. Conclusions: In carefully selected patients with stable MG and no preoperative vocal fold dysfunction, single-stage median sternotomy enables safe concomitant extended thymectomy and RLN schwannoma enucleation. Success hinges on nerve-sparing technique and proactive perioperative planning for airway protection and potential MG crisis.

    Purpose: Pleural lavage cytology (PLC) is a recognized prognostic marker in non-small cell lung cancer (NSCLC); however, the impact of serial intraoperative changes remains unclear. Methods: We retrospectively analyzed 439 patients who underwent curative NSCLC resection. PLC was performed at three intraoperative points: after thoracotomy (pre-PLC), after lung resection, and after lavage at chest closure (post-PLC). Associations between recurrence-free survival (RFS) and pleural dissemination were evaluated by a Kaplan–Meier analysis and Fine and Gray competing risks regression. Results: Forty-one patients had at least one positive PLC result. RFS was the lowest in pre-PLC(+)/post-PLC(+) (n = 10), intermediate in pre-PLC(–)/post-PLC(+) (n = 11), and best in post-PLC(−) (n = 20). Importantly, post-PLC(−) patients included 13 patients with pre-PLC positivity, yet their RFS matched that of consistently negative cases (n = 398). The cumulative incidence of pleural dissemination exhibited a similar pattern. In a multivariate analysis, post-PLC positivity, but not pre-PLC positivity, independently predicted poor RFS (hazard ratio, 3.06; p < 0.001). Conclusion: Post-PLC, but not pre-PLC, provides decisive prognostic information for recurrence and pleural dissemination, likely reflecting residual lavage-resistant tumor clusters. Importantly, combining pre- and post-PLC results refines risk stratification and identifies the poorest-outcome subgroup that may benefit from adjuvant therapy.

    Tracheal adenoid cystic carcinoma (ACC) often shows submucosal extension, leading to positive margins. A male patient presented with dyspnea due to an intraluminal tumour arising from the membranous mid-trachea. Rigid bronchoscopy debulking confirmed ACC. In a second-stage operation, circumferential resection of approximately 3.8 cm of trachea with end-to-end anastomosis achieved macroscopic clearance. Intraoperative frozen sections, however, revealed tumour cells within lymphatic channels of the tracheal sheath at the distal cartilaginous-side margin opposite the membranous lesion, precluding further resection because of the limit of safe tracheal length. The anastomosis was therefore completed with anticipated microscopic residual disease, and postoperative radiotherapy from the anastomosis to the carina was delivered. The patient remains recurrence-free at follow-up. This case highlights circumferential extramural lymphatic spread of tracheal ACC and underlines the importance of systematic intra- and postoperative pathological assessment and of considering tracheal sheath and peritracheal tissue in defining resection and adjuvant strategies.

    Objectives The recent increase in sublobar resections has been driven by favourable long-term outcomes and advances in stapling devices. However, maintaining an adequate resection margin remains a critical oncological requirement. This study aimed to investigate whether deep wedge resection induces subpleural alveolar injury that could lead to margin overestimation by creating undetectable internal cavities. Methods We retrospectively analysed 33 consecutive patients who underwent wedge resection and CT imaging of resected lung specimens between December 2018 and February 2025. CT was performed on inflated specimens to better visualize internal lung architecture. We assessed the presence of an “empty space” adjacent to the staple line and correlated it with clinical factors, including depth of wedge resection (WR). Additionally, ex vivo porcine lung models were used to simulate deep WR, analyse compression effects, and identify histological damage caused by stapler compression. Results CT imaging revealed empty spaces adjacent to the staple line in 10 of 33 specimens (30.3%), with a mean cavity length of 8.25±3.2 mm. This artifact was significantly associated with deeper WR (≥26.3 mm) and increased stapler cartridge usage (median: 4 vs 3, P=.0298). In porcine experiments, compression to 2 mm thickness resulted in internal parenchymal rupture without pleural tearing, replicating the clinical findings. Conclusions This study identified a potential mechanism by which deep wedge resection may lead to overestimation of the pathological margin due to stapler-induced parenchymal rupture. Further large-scale studies integrating oncological outcomes are warranted to clarify how wedge resection and segmentectomy should be appropriately selected for deep peripheral lung lesions.

    <p>Purpose: Pulmonary vein stump thrombus (PVST) is a relatively common complication after left upper lobectomy that can cause vital organ embolism. We previously found that patients with PVST on postoperative day 7 show risky hemodynamic features around the pulmonary vein stump on 4-dimensional (4D) flow magnetic resonance imaging (MRI), which may contribute to thrombus development. However, it remains unclear whether such hemodynamics persist later.</p><p>Methods: Eleven patients who underwent left upper lobectomy for lung cancer received 4D flow MRI on postoperative day 7 and again after over 3 months. Hemodynamic parameters were used to classify each case as risky or non-risky for PVST.</p><p>Results: According to a total of 24 examinations in 11 patients, 7 were classified as risky and 17 as non-risky. PVST developed in 6 patients during various postoperative phases, and all PVST cases developed under the risky conditions. Furthermore, PVST did not develop under non-risky conditions, suggesting that our risk assessment is valid as a predictive marker for PVST.</p><p>Conclusion: Our results suggest that late postoperative hemodynamic assessments, as well as early postoperative assessments, are useful for identifying patients at high risk of PVST. A late postoperative hemodynamic assessment may contribute to determining when to discontinue anticoagulants.</p>

    Background: In recent years, surgical procedures have shifted from open thoracotomy and laparotomy to minimally invasive endoscopic surgery, making endoscopic forceps training essential. In our department, we teach endoscopic forceps training to medical students in dry laboratories. We examined differences in the skill acquisition of medical students undergoing endoscopic forceps training. Methods: Ninety-eight fourth-grade students at Kagoshima University Graduate School of Medicine underwent endoscopic forceps training every day in a dry laboratory during a two-week period in our department. Before the start of the training, we administered a questionnaire. We focused on the differences in skill acquisition levels between the first and second halves of the training period. Results: Twenty-eight of the 63 male students and 9 of the 35 female students aspired to become surgeons (hereafter: “aspiring surgeon group”). All students showed a significant improvement in their skill acquisition from the first to the second half (p < 0.001). At the start of training, there was no difference in skill acquisition between the 37 in the aspiring surgeon group and the other students. However, during the second half of the training, the aspiring surgeons acquired skills significantly faster (p = 0.007). Among them, male aspiring surgeons, in particular, acquired skills significantly faster. Focusing on the speed of skill acquisition, there was no correlation between sex (p = 0.18) or sports activity (p = 0.64) and improvement in endoscopic forceps skills. Conclusions: In dry laboratory endoscopic forceps training among medical students, all participants showed skill improvement with repeated practice. However, aspiring surgeons demonstrated significantly faster improvement, likely due to clear goal setting. As demonstrated in this study, goal-setting in student education may be associated with skill acquisition.

    <p>Tracheal mucosa-associated lymphoid tissue (MALT) lymphoma is rare, and its diagnosis can be challenging. We herein report the case of a 70-year-old woman with a history of right upper lobectomy and lymph node dissection for lung adenocarcinoma (pT2aN0M0, stage IB) performed 6 years prior to presentation. Follow-up chest computed tomography incidentally revealed a tracheal nodule. Bronchoscopy revealed a smooth, elevated lesion. An initial biopsy using endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) was inconclusive. Therefore, we performed an endobronchial ultrasound-guided miniforceps biopsy (EBUS-MFB), which yielded sufficient tissue for diagnosis. Histopathological examination revealed diffuse infiltration of centrocyte-like cells, consistent with a low-grade B-cell lymphoma such as MALT lymphoma. No other organ involvement was found, and a diagnosis of tracheal MALT lymphoma was made. EBUS-MFB is a useful and safe technique for diagnosing rare tracheal lesions, such as MALT lymphoma, when EBUS-TBNA results are inconclusive.</p>

    Objectives Two major types of stapling devices exist: those with disposable built-in knives and those with nondisposable built-in knives. This study investigated whether the stapler type influences the incidence of local recurrence, including margin recurrence and pleural dissemination, after curative sublobar resection for lung cancer. Methods We retrospectively reviewed patients who underwent sublobar resection at our institution between 2010 and 2021. We compared disease-free survival, overall survival, and local recurrence between procedures using a stapler with a disposable knife and those using a stapler with a nondisposable knife. Results A total of 269 patients were included: 84 were treated with the disposable-knife stapler and 185 with the nondisposable-knife stapler. Local recurrence occurred in 22 of 269 patients (8.2%), including 9 of 84 (10.7%) in the disposable group and 13 of 185 (7.0%) in the nondisposable group (P =. 72). Patients who developed local recurrence tended to be older, male, have a smoking history, squamous cell carcinoma, absence of a ground-glass component, positive stapling cartridge cytology, partial resection, right lower lobe tumours, elevated carcinoembryonic antigen, and higher maximum standardized uptake values. In a propensity score-matched study (78 patients per group), no significant differences in disease-free survival, overall survival, or local recurrence were detected between the stapler types. Conclusions No statistically significant differences in oncological outcomes were observed between stapler types in this retrospective study; however, the absence of a significant difference does not rule out a real effect. Further large-scale research is warranted.

    This article translates the guidelines for preoperative pulmonary function assessment in patients with lung cancer who will undergo surgery, established by the Japanese Association of Chest Surgery on May 17, 2021, from Japanese to English. The last version of these guidelines was created on April 5, 2011. Over the past decade, changes in clinical practice have occurred that do not align with the current guidelines, prompting a revision in conjunction with the introduction of new evidence this time. This guideline was developed with reference to the internationally adopted GRADE (Grading of Recommendations Assessment, Development, and Evaluation) system. Extraction of evidence, systematic review, and quality assessment are entrusted to each guideline review committee and the Pulmonary Function Assessment Working Group. Committee members are also responsible for determining the selection of evidence and the extraction period, with a particular emphasis on adopting items considered to be of special importance. The recommended assessment and management is categorized into a general overview, pulmonary function assessment, cardiopulmonary exercise test, pulmonary function assessment for lung cancer with interstitial pneumonia, preoperative smoking cessation, and pulmonary rehabilitation. These are described by the strength of recommendation, the strength of evidence, and the consensus rate.

    OBJECTIVES: Predicted postoperative forced expiratory volume in 1 s (ppoFEV1) and predicted postoperative diffusing capacity of the lungs for carbon monoxide (ppoDLco) are the two most significant parameters for predicting the risk of cardiopulmonary complications after major lung resection for lung cancer. Although the severity of pulmonary emphysema on computed tomography may be an important risk factor for postoperative complications, the clinical significance of measuring the severity of pulmonary emphysema, in combination with ppoFEV1 and ppoDLco, has never been evaluated. METHODS: We measured the severity of pulmonary emphysema, representing the percentage of low-attenuation area (< −950 Hounsfield units), in the whole lung field, in addition to ppoFEV1 and ppoDLco, in 451 patients who underwent major lung resection for primary lung cancer. We also measured the volume of the upper and lower lobes of emphysematous and non-emphysematous lungs. RESULTS: Postoperative cardiopulmonary complications developed in 60 patients (13.3%). According to a receiver operating characteristic curve analysis for the diagnostic potential of postoperative complications, the area under the curve was highest for the severity of emphysema, followed by ppoDLco and ppoFEV1. According to a stepwise multivariable logistic regression analysis, the severity of emphysema and ppoDLco was identified as independent risk factors for postoperative complications. Neither the heterogeneous distribution of emphysema nor that of non-emphysema was associated with the occurrence of complications in patients with upper lobe disease or in patients with lower lobe disease. CONCLUSIONS: The severity of emphysema on computed tomography is a relevant risk factor for cardiopulmonary complications after major lung resection.     Background/Aim: Immune checkpoint inhibitors (ICIs) have emerged as a standard treatment for recurrent non-small cell lung cancer (NSCLC). However, the factors predicting their efficacy in postoperative recurrence remain unclear. Patients and Methods: We retrospectively analyzed 30 patients who underwent complete surgical resection of NSCLC and subsequently received ICI monotherapy for recurrence. Clinicopathological factors, including the programmed death ligand 1 (PD-L1) expression and number of dissected lymph nodes (DLNs), were evaluated for their association with the treatment response and prognosis. Results: The high expression of PD-L1 (≥50%) in surgical specimens was significantly associated with higher response rates, prolonged progression-free survival (PFS), and overall survival (OS) after the initiation of ICI therapy. In contrast, patients with ≥15 DLNs had a significantly shorter PFS than those with <15 DLNs, particularly in patients without extrathoracic recurrence. A multivariate analysis identified the expression of PD-L1, the number of DLNs, and the presence of extrathoracic metastasis as independent prognostic factors for ICI-PFS. Conclusion: Extensive lymphadenectomy may worsen the prognostic outcomes of ICI monotherapy, possibly by impairing tumor-draining lymph node-mediated immunity. These findings highlight the importance of lymphatic preservation and support the clinical rationale for neoadjuvant ICI therapy for resectable NSCLC.

    <p>Anticoagulant is used to treat pulmonary vein stump thrombus after left upper lobectomy. However, the optimal length of anticoagulant therapy remains controversial. We report a case of pulmonary vein stump thrombus that regenerated after discontinuation of anticoagulation therapy despite the fact that the thrombus had disappeared after such therapy. A 71-year-old man underwent left upper lobectomy for lung cancer. Three months after the operation, contrast-enhanced CT showed a thrombus in the left superior pulmonary vein stump, and anticoagulant therapy was administered for three months. Irrespective of the disappearance of the left superior pulmonary vein stump thrombus, it regenerated 3 months after the discontinuation of anticoagulant. The newly regenerated thrombus disappeared after readministration of the anticoagulant. There is no report on patients with a recurred pulmonary vein stump thrombus after primary anticoagulant therapy. This case alerts us to the possibility of late postoperative development of pulmonary vein thrombus after successful primary treatment.</p>

    <p><b><i>Background. </i></b>There have been reports of small-cell lung cancer found in cases of hilar and mediastinal lymphadenopathy with no primary lesion. We experienced a case of T0N1M0 small-cell lung cancer. <b><i>Case. </i></b>A 61-year-old man had a history of endoscopic submucosal dissection (ESD) for middle esophageal cancer 8 years ago and for gastric cancer 5 years ago. The gastric cancer had been completely resected endoscopically according to the pathology. However, with regard to the esophageal cancer, he was requested to be followed up, although additional excision was advised due to the depth to the wall. The left main bronchial lymph node was found to be swollen at follow-up. This node was located near the esophageal cancer that had been treated with ESD, which initially suggested esophageal cancer metastasis. As no other lesions were found, including in the esophagus after ESD, the node was resected for a diagnosis. The left main bronchial lymph node was removed thoracoscopically. The pathological findings resulted in a diagnosis of lymph node metastasis of small-cell lung cancer. There was no apparent primary lesion on image findings or endoscopically, so it was assumed to be a lung primary tumor and judged to be T0N1M0 small-cell lung cancer. <b><i>Conclusion. </i></b>We experienced a case of small-cell lung cancer of unknown primary origin with hilar lymphadenopathy. We believe that additional treatment including chemotherapy will be necessary in the future.</p>

    背景.食道気道瘻の治療は手術、内視鏡的治療(食道側・気管支側アプローチ)が挙げられるが、治療に難渋する。今回、食道胃接合部癌術後の食道気道瘻に対し食道側アプローチと、気管支側アプローチとしてEndobronchial Watanabe Spigot(EWS)を組み合わせ瘻孔閉鎖を得られた症例を経験したので、報告する。症例.食道癌術後の76歳男性。20XX年5月に、食道胃接合部癌で、腹部食道胃全摘、脾胆嚢切除、食道空腸吻合を施行された。20XX+2年から食事中に咳嗽・喀痰を認め、食道気道瘻と判断した。その後、食道側より内視鏡用クリップ、コイル+ヒストアクリル充填術を試みるも改善なく、カバー付きステントは違和感が強く抜去した。20XX+4年10月に食道気道瘻に対しEWSによる気管支充填を行った。経鼻胃管よりインジゴカルミン液を注入し、責任気管支を同定後、EWSで気管支充填を行った。充填術後は喀痰排出が著明に減少した。その後、内視鏡的に食道側からのインターベンションで瘻孔閉鎖が得られた。結論.末梢気道に生じた瘻孔に対して食道側・気管支両側からの内視鏡的治療を組み合わせることは非常に有用であり、その中でEWSは食道気道瘻閉鎖に有効であった。(著者抄録)

    本研究は、癌に対する胸腔鏡下肺葉切除術後の疼痛に対処する方法について検討した。胸腔鏡下肺葉切除およびリンパ節郭清術を待機していた患者を対象として、肋間神経ブロックおよび硬膜外鎮痛法を比較する無作為化比較試験を実施した。高用量celecoxibの経口投与を、肋間神経ブロック群では術後3時間で開始し、硬膜外鎮痛群では硬膜外鎮痛の中止後に開始した。肋間神経ブロックを受けた患者(男性6名、女性15名、平均年齢66.7歳)および硬膜外鎮痛法を受けた患者(男性16名、女性6名、平均年齢69.0歳)を対象とした。術後の有害事象および術後合併症の発現率は両群間で同等であったが、処置関連の問題の発現率は硬膜外鎮痛群で肋間神経ブロック群よりも有意に高かった。鎮痛処置時間は肋間神経ブロック群で硬膜外鎮痛群よりも有意に短かった。視覚的アナログ尺度で評価した術後0〜7日目の術後疼痛には2群間に有意差がなかった。肺活量および歩行距離で評価した術後の生理的機能にも群間に有意差がなかった。

    症例は65歳、男性。同時多発肺癌に対して、胸腔鏡補助下右肺上中葉切除術と上縦隔ならびに#7リンパ節郭清術を施行し、郭清部位にfibrin glueを散布して閉胸に至った。術後2日目に乳糜胸水を認め、絶食管理でも1日1.5l以上の排液が続くため、術後3日目に再手術とした。麻酔導入後、仰臥位で体表エコー下に鼠径リンパ節へインドシアニングリーン(ICG)を穿刺注入し、体位変換を行った。胸腔内を観察すると、近赤外光カメラで気管前に蛍光ICGの貯留を認め、責任部位を同定し得た。同部位をZ縫合で修復してfibrin glueを散布し、さらに気管支断端を被覆した遊離脂肪織の一部を上縦隔にも充填し、fibrin glueで固定した。再手術後4日目に食事を再開しても乳糜の再燃なく、再手術後12日目に自宅退院した。ICG蛍光法は、乳糜の漏出部位の同定に簡便でかつ有用であった。(著者抄録)

    <p>Purpose: The diagnostic potential of conventional multi-planar reconstruction (MPR) images, which consist of horizontal, frontal, and sagittal section, in approximating the anatomical distance between tumors and intersegmental planes remains unclear. The aim of the present study was to clarify the validity of decision-making for segmentectomy based on MPR imaging and identify a specific tumor location that is likely to result in the overestimation of the anatomical margin on MPR images.</p><p>Methods: The study population included 33 patients who were considered eligible for segmentectomy based on the observation of MPR images, and verified using a commercially available image-analysis software whether the decision-making based on MPR images was indeed correct or not.</p><p>Results: MPR image-based assessment resulted in the overestimation of the anatomical margin in as many as 8 (24%) of the 33 patients. Overestimation predominantly occurred in cases involving patients with tumors at certain segments (right S1, right S2, right S3, left S3, and left S4) that had a complex and oblique intersegmental plane.</p><p>Conclusion: Conventional MPR image-based assessment frequently resulted in the overestimation of the anatomical margin. We recommend using software-based assessment preoperatively in patients with tumors in the risky segments, particularly in cases involving indistinct tumors.</p>

    <p>A 65-year-old man underwent right upper and middle lobectomy with upper mediastinal and subcarinal lymph node dissection for synchronous right upper and middle lobe cancer. We applied fibrin glue to the upper mediastinum and subcarina to prevent postoperative bleeding and chylorrhea. Two days after surgery, we identified chylous pleural effusion. In spite of fasting, the amount of discharge persisted at over 1.5 L per day. Therefore, we performed reoperation three days after the initial operation.</p><p>After the induction of general anesthesia, indocyanine green (ICG) was injected into the inguinal lymph node under echography-guidance. After rethoracotomy, we could identify the responsible site in the paratracheal region, in which the pooling of infrared ICG was recognized using a near-infrared thoracoscope. We repaired the responsible site by Z suture. In addition, the sutured site was reinforced with fibrin glue followed by covering with a free fat pad.</p><p>The diet was started on the fourth day after reoperation, which did not lead to recurrence of chylothorax. The patient was discharged from the hospital on the twelfth day after reoperation.</p><p>Near-infrared fluorescence imaging of ICG was straightforward and useful to identify the leakage point of chyle.</p>

    <p><b><i>Background.</i></b> While surgical and endoscopic approaches are the treatments of choice for patients with esophago-pulmonary fistula, this complication is generally intractable. We herein report a case of delayed postoperative esophago-pulmonary fistula that was closed via bronchoscopy and enteroscopy using an Endobronchial Watanabe Spigot (EWS). <b><i>Case.</i></b> A 76-year-old man underwent surgery for gastroesophageal junction carcinoma. Subtotal esophagectomy combined with gastrectomy, splenectomy, cholecystectomy and esophagoenterostomy was performed. The patient complained of coughing and sputum production immediately after liquid swallowing two years after the operation and was diagnosed with esophago-pulmonary fistula. We attempted to obstruct the fistula with clipping, coiling, and histoacryl embolization several times, which did not resolve the condition. Stenting with a covered expandable metallic material was also unsuccessful because it gave the patient discomfort. As we were able to identify the responsible subsegmental bronchus by instilling indigo carmine via a nasogastric tube, embolization of the responsible subsegmental bronchus with EWS was performed four years after the initial operation. The patient's symptoms eventually resolved by subsequent treatment via an enteroscopic approach. <b><i>Conclusion.</i></b> Combined bronchoscopic and enteroscopic therapy with EWS led to the successful healing of an intractable esophago-pulmonary fistula.</p>

    C-arm cone-beam CT(CBCT)下でのビデオ支援胸腔鏡手術(VATS)によるデブリドマンを安全かつ確実に施行し得た複数の小房性器質化膿胸の67歳女性症例について検討した。右膿胸の治療を行っていた。胸部CT検査から右胸腔内の三つの独立した腔における液体貯留が示された。発症から約7週間が経過していたために、肺と胸壁間に強固な癒着が生じたと考えられた。CBCTを使用して三つの膿胸腔のそれぞれをVATSにより完全にデブリドマンすることを決定した。一つの腔は胸壁との狭い範囲内のみに存在し、肋軟骨の後方に位置していた。CBCT画像上ですべての目的とする解剖学的構造をクリックすることにより、レーザー投影によって腔の位置が体表面に容易に描写され、最善の皮膚切開の位置を決定する助けとなった。他の腔では初回デブリドマン後のCBCTから腔の除去が不十分であることが示された。追加のデブリドマンにより膿胸腔の十分な縮小が得られた。

    【背景】スリガラス影主体の肺癌病変の縮小手術で、術中に触知困難な場合は切除マージン評価が難しい。そこで、切除標本を体外で含気させCT撮影し腫瘍存在および切除マージン測定が可能か検討した。【症例】症例1)82歳、女性。右下葉S10に肺癌疑いの病変、症例2)70歳、女性。右下葉S9に肺癌疑いの病変、それぞれに対し部分切除をした。【方法】それぞれ部分切除後の切除標本にできるだけ細い穿刺針で、腫瘍からなるべく遠く離れた胸膜から単回穿刺し、シリンジでゆっくり十分に空気を注入した。そして十分に膨らんだ切除標本を密閉容器に入れ素早くCT室で撮影した。【結果】体内と同様に切除標本を含気させCT撮影し、触知困難病変の確認と切除マージンもCTで測定できた。【結論】触知困難な病変に対し、術中に切除肺を含気させCT評価を行うことで、標本を損傷せずに腫瘍の存在確認および切除マージンの評価は簡便で有用であった。(著者抄録)

    背景.Pembrolizumabをはじめとした免疫療法では、免疫関連有害事象を認めることが知られている。Pembrolizumab投与後に難治性大腸炎を発症した症例を経験したので報告する。症例.症例は49歳女性で、左上葉肺癌に対する5次治療としてPembrolizumab投与を行った。6コース目の投与後に腹痛・下痢・血便を認め、免疫関連有害事象である大腸炎を発症した。プレドニゾロン、抗TNFα抗体を用いて加療したが、抵抗を示しCyclosporineの投与を要した。Cyclosporineの血中濃度上昇とともに症状は改善した。免疫関連有害事象である大腸炎には、重篤な経過をたどるものもある。しかし、難治性大腸炎に関する報告は少なく、対応方法には一定した見解はない。結論.Cyclosporine投与は、Pembrolizumabによる難治性大腸炎に対し有効な治療方法である可能性がある。迅速な対応を可能にするために、十分な症例の蓄積が必要である。(著者抄録)

    <p><b><i>Background. </i></b>Immune checkpoint inhibitors (ICIs), such as Pembrolizumab, can cause immune-related adverse events (irAEs). We herein report a 49-year-old woman who developed refractory colitis after Pembrolizumab administration. <b><i>Case. </i></b>The patient had a left upper lobe lung adenocarcinoma that had been treated with Pembrolizumab as the fifth-line treatment. After the sixth administration of Pembrolizumab, she developed colitis as an irAE, complaining of abdominal pain, diarrhea and bloody stool. As the colitis did not respond to Prednisolone or anti-TNFα antibody, Cyclosporine was administered. The colitis was substantially improved after sufficient dosing of Cyclosporine. Some patients treated with ICIs die of colitis as an irAE. However, this is not well documented in the literature, and no consensus has been reached regarding appropriate treatment. <b><i>Conclusion. </i></b>The accumulation of a sufficient number of cases will reveal the efficacy of Cyclosporine in treating intractable colitis induced by Pembrolizumab.</p>

Therefore, the resected specimen was inflated outside the body, and CT was performed to assess whether it was possible to confirm the presence of the tumor and measure the resection margin.</p><p>Case. 1) A 82-year-old female underwent partial resection of the right lung for suspected lung cancer in the lower right lobe S10. 2) A 70-year-old female underwent partial resection of the right lung for suspected lung cancer in the lower right lobe S9.</p><p>Method. Each excisional specimen after partial excision was punctured as thinly as possible with a thin needle as far as possible from the pleura distant from the tumor, and a syringe was used to inject air slowly and sufficiently. Then, a well-expanded resected specimen was placed in a closed container and taken into the CT room.</p><p>Results. By inflation of the resected specimen similar to that inside the body and performing CT, it was possible to confirm the resection of the unpalpable lesion and measure the excision margin by CT.</p><p>Conclusion. It was convenient and useful to confirm the existence of the tumor and evaluate the surgical margin without damaging the specimen by inflating the resected lung during the operation and performing CT evaluation for a palpable lesion.</p>

    背景.顆粒細胞腫は良性疾患であるが、局所切除後に断端再発する症例が少なからず存在する。しかし、再発の原因は不明なものが多い。症例.65歳、女性。健康診断の胸部X線検査で右肺野に異常陰影を指摘された。胸部CT検査で右上葉S2に25×20mm大の結節影を認め肺癌が疑われた。気管支鏡検査で右B2に白色隆起性病変を認め、同部位から生検を行った。HE染色では小型円形核と好酸性顆粒を伴う豊富な細胞質を有する組織球様の腫瘍細胞が充実性に増殖しており、免疫染色ではS-100蛋白陽性、NSE陽性、CD68陽性であることから顆粒細胞腫と診断された。腫瘍が右上葉気管支中枢側に存在したため、胸腔鏡下右上葉切除術を施行した。術後の病理組織学的検査では良性の顆粒細胞腫と診断されたが、腫瘍の一部が隣接するリンパ節へ浸潤していた。結論.隣接するリンパ節への浸潤を伴った顆粒細胞腫を経験した。顆粒細胞腫は良性と診断されたものでも周囲への浸潤を認めるものがあり、これが切除後再発に関与している可能性がある。切除の際に十分なマージンを確保することが肝要であり、葉切除が必要な症例も存在する。(著者抄録)

    【はじめに】肺低形成は新生児期における呼吸障害の原因となり得るが、肺葉に限局した低形成の場合、成人期に指摘され、悪性腫瘍を合併することがある。【症例】我々はこれまで4例の肺癌合併中葉低形成を経験した。男性3例、女性1例で、年齢はすべて60歳代であった。平均中葉容積率は全肺の0.24%と著明に縮小していた。肺癌組織型は、4例中3例が定型的カルチノイド、1例が小細胞肺癌であった。4例とも右中葉切除術を施行した。【考察】肺低形成の多くは、先天性臓器異常に続発する二次性であるが、今回の4例は合併異常を有さず、病変も中葉に限局していた。このため呼吸障害を発症することなく成人期まで経過したと考えられた。全例、神経内分泌腫瘍であったが、肺低形成と神経内分泌腫瘍との因果関係は不明である。【結語】右中葉低形成に肺癌を発症した4例を経験した。(著者抄録)

    背景. 大細胞神経内分泌癌は比較的稀な組織型で、多臓器に同時に発生したとする報告はない。症例. 75歳、男性。持続する咳嗽に対する胸部CTで前縦隔に75mmの腫瘤と左肺下葉に11mmの結節を認めた。CTガイド下前縦隔腫瘤生検で神経内分泌癌が疑われた。縦隔腫瘤は左腕頭静脈、左内胸動静脈、左横隔神経、左肺上葉に浸潤していたが完全切除できた。左肺下葉の病変は部分切除した。縦隔と肺の腫瘍細胞は細胞質に乏しく、不整で大きな核を有し、核小体が明瞭で、胞巣状もしくは柵状に増生していた。ともに壊死を伴い、高倍率10視野あたり11個を超えて核分裂像を認めた。免疫組織化学染色でchromogranin A、synaptophysin、CD56及びCK(MNF116)が陽性で、ともに大細胞神経内分泌癌と診断した。腫瘍細胞の大きさや核形態、並びに細胞配列像の違いやTTF-1の染色性から、胸腺原発大細胞神経内分泌癌及び肺原発大細胞神経内分泌癌と診断した。結論. 肺と胸腺に同時期に発生した大細胞神経内分泌癌を一期的に切除できた、極めて稀な1例を経験した。(著者抄録)

    症例は80歳代、女性。膵頭部腫瘍に対して幽門輪温存膵頭十二指腸切除術が施行された。病理所見から浸潤性膵管内乳頭粘液性腺癌(intraductal papillary mucinous carcinoma、invasive:浸潤性IPMC)、pT3N0M0、Stage IIIと診断された。術後2年の胸部CTで肺野結節影を指摘され、増大するため診断と治療を兼ねた胸腔鏡下左肺下葉切除術(ND2a-1)及び舌区部分切除術が施行された。腫瘍のHE病理組織像は既往の膵腫瘍の組織像と酷似しており、さらに免疫染色所見が一致したため、浸潤性IPMCの肺転移と診断された。浸潤性のIPMCは予後不良で、その肺転移巣を切除することは極めて稀である。原発性肺癌との鑑別が困難な浸潤性のIPMCの孤立性肺転移に対して診断的意義を含んだ根治切除を施行した。(著者抄録)

    CT検診の普及によりスリガラス陰影を主体とした小型肺癌の発見率は増加し,一方で人口の高齢化により高齢者肺癌手術症例の比率は顕著に増加した.結果として,切除範囲やアプローチの縮小化を目指した低侵襲手術の需要が増している.原発性肺癌に対する標準術式は肺葉切除と肺門縦隔リンパ節郭清であるが,一定の条件を満たした症例に対して肺部分切除,区域切除,選択的リンパ節郭清といった縮小手術が日常臨床で行われている.そこで工夫しなければならない点が幾つかあり,ICG蛍光法が重要な役割を果たしている.まず,触知困難な小型病変の同定である.従来のVATSマーカーによる経皮的マーキングでは空気塞栓のリスクを伴うが,ICG蛍光法は肺を穿刺することなく病変を同定し得る方法である.比較的深部の病変も同定可能になり,サージカルマージン確保の観点からもその有用性が期待されている.次に,区域切除における切離ラインの決定にICG蛍光法が用いられている.従来の選択的ジェット換気による含気虚脱ラインによる同定は,胸腔鏡下手術において視野の妨げになることがあるが,ICG蛍光法では肺を含気させる必要がなく,手技も簡便である.そして,リンパ節郭清の個別化においてICG蛍光法が期待される.肺はセンチネルリンパ節の同定が解剖学的に困難であり,その概念が確立してない.ICG蛍光法を用いた最新の肺のリンパ節マッピングについて本文中で触れる.(著者抄録)
