インターロイキン(IL)-1αはエナメル上皮腫細胞の挙動に影響を与えるか評価した。ヒト濾胞型エナメル上皮腫から確立されたAM-3細胞を実験材料とした。リアルタイム逆転写PCR法と酵素免疫測定法による評価の結果、AM-3細胞をIL-1αで刺激することでマトリックスメタロプロテアーゼ9のmRNA発現と分泌量は有意に増加することが示された。AM-3細胞とマクロファージ由来RAW264.7細胞の運動性はIL-1αによって有意に亢進し、IL-1受容体(IL-1R)拮抗薬にて抑制された。またAM-3細胞の侵襲性も同様にIL-1αで有意に亢進し、IL-1R拮抗薬で抑制された。ヒト包皮線維芽細胞のHFF-2細胞に対しAM-3細胞が示す侵襲性を改変2層コラーゲンゲル半球培養モデルで評価したところ、その侵襲性は、IL-1R拮抗薬または抗IL-1a中和抗体で処理することで抑制されることが判明した。以上の所見から、IL-1α自身、またはIL-1αに依存して間質細胞が産生する未知の化学誘引物質が、エナメル上皮腫細胞の局所侵襲性に重要であると考えられた。

智歯抜歯術の前に音楽を流すことで、患者の自律神経系の活動と循環動態が安定するかどうかを前向き試験で検証した。埋伏下顎智歯の抜歯術を受ける20～40歳の女性患者36名を、対照群と介入群に無作為に割り付けた。介入群では、抜歯術の直前に歯科用椅子の上で仰臥位を取ってもらい、音楽(ラフマニノフ)をヘッドホンで20分間聴きつつ安静にするよう指示した。処置前にはModified Dental Anxiety Scaleと状態-特性不安尺度(STAI)のスコアを評価し、処置中には心拍数変動と循環動態を記録した。処置後にはSTAIスコアを再度評価した。心拍数変動の低周波成分/高周波成分の比を交感神経活動度の指標として評価した結果、対照群では、局所麻酔投与、骨削除、抜歯の各処置中の交感神経活動度が、術前安静時に比べ有意に上昇していた。両群を比較した場合、局所麻酔投与および抜歯の処置中の交感神経活動度は介入群の方が有意に低かった。介入群では、不安の評価としたSTAIスコアが術前に比べ術後で有意に低下していた。以上から、抜歯術前に音楽をかけておくことで、局所麻酔投与中および抜歯処置中の交感神経活動を鎮める助けになると考えられた。

当科開設以来,1981年から2021年までの40年間に鹿児島大学口腔顎顔面外科を受診した口唇裂・口蓋裂一次症例1,446名について臨床統計的観察を行った。結果は以下の通りである。1.40年間に当科を受診した一次症例は1,446名であった。2.裂型別患者数では,唇裂が483名(33.4%),唇顎口蓋裂が551名(38.1%),口蓋裂が412名(28.5%)であった。3.男女比は,男性748名,女性698名で1.07:1であった。4.初診時年齢は,生後から20歳の間において,生後1ヵ月未満が939名(65.0%)で最も多かった。5.出生時体重は,平均2,936.2gで,2,500～2,999gが527名(37.0%)で最も多かった。6.患者の居住地は,鹿児島県内が1,130名(78.2%)であり,鹿児島県外が316名(21.8%)であった。7.初診時の来院経路(紹介元医療施設)は,院外の産科・婦人科からの紹介が最も多く,次いで小児科・周産期・母子センターからの紹介が多かった。8.先天異常を合併した患者は,134名(9.3%)に認められた。9.2006年から開始した往診数,出生前カウンセリング数は計276件で,2011年以降,増加してきた。出生前カウンセリングを行うことで,家族への精神的負担を軽減できた。10.裂型別の症例数は,いずれの期間も唇顎口蓋裂,唇裂,口蓋裂の順であった。男女比は,I期,III期,IV期で男性に多く,II期は女性に多くみられた。初診時年齢は,I期では生後1ヵ月未満の受診が半数以下であったが,II期,III期,IV期は約70%程度が生後1ヵ月以内に受診していた。2,500g未満の低出生体重児の割合は,I期10.4%,II期16.8%,III期21.4%,IV期12.4%であった。近年では,小児科・周産期・母子センターからの紹介が多くなっていた。先天異常の合併の割合は,口蓋裂が5.1%で,次いで,唇顎口蓋裂が3.1%,唇裂が1.0%であった。(著者抄録)

Aim: It is widely known that orthognathic surgery affects changes in facial soft tissue morphology. However, the effect of orthognathic surgery on facial expression remains to be verified. The purpose of this study was to examine the pros and cons of the effects of orthognathic surgery on facial expression in skeletal Class III patients.Methods: Fifteen patients (5 males, 10 females) were captured pre-surgery and at 6 months' post-surgery using Kinect camera while performing facial soft tissue movements (opening mouth, smiling, and blowing). The three-dimensional coordinates, amount of movement, and velocity of the eight facial soft tissue landmarks during motion were obtained. As controls, 15 Japanese healthy volunteers with Class I occlusion were included.Results: Differences in the three-dimensional coordinate values during movement were confirmed, and there were some landmarks in the lower face region of the preoperative patient that showed significant differences compared with the control. Total amount of movement and velocity of Pogonion during smiling were significantly smaller than those of the control group, but there was no significant difference between the control and postoperative groups at any landmarks after surgery. It was suggested that patients with skeletal Class III may have limited movement in some movements, and it was confirmed that with orthognathic surgery, the aspect of movement would be similar to that of the control.Conclusions: Orthognathic surgery may be beneficial to the facial muscles without limiting facial movements.

Purpose: To observe the pre- and postoperative three-dimensional (3D) nasal morphologies following mandibular ramus cortical bone augmentation (CBA) for a concaved nasal dorsum in definitive correction of an underdeveloped unilateral cleft lip (UCL) nose. Subjects and Methods: Three Japanese women with complete unilateral cleft lip and palate (UCLP) underwent nose correction using CBA on the flat or concaved nasal dorsum in combination with an extended spreader cartilage graft. The measurement items included the nasal dorsal angle, the nasal tip angle, and the x- and z- axis values of the nasal dorsum ridge and nasal tip points. The results were compared with those of thirteen sex- and age-matched women with complete UCLP who underwent the same nasal correction excluding CBA. Postoperative changes in cortical bone were also observed radiographically. Results: The nasal dorsal angle increased and nasal tip angle decreased postoperatively in the patients with CBA, becoming more similar to those of subjects without CBA. Postoperatively, improvement of the lateral nose profile with natural hardness was achieved in three all patients with CBA. 3D analyses of patients with CBA demonstrated an increased height of the nasal dorsum ridge and nasal tip points in the center of the face. Radiological observation revealed remaining transplanted cortical bone, although it became smaller. Conclusions: Mandibular ramus cortical bone augmentation may be one of the options for molding up the severely concaved nasal dorsum on definitive correction of an underdeveloped nose in Asian patients with complete UCLP, without damaging other parts of the body.

Objective: Orthognathic surgery has become useful procedures for patients with cleft lip and palate (CLP) to improve the inharmonious relationship between the maxilla and mandible. The purpose of this study is to assess the postoperative stability and skeletal morphology following orthognathic surgeries for patients with skeletal class III deformity with CLP. Methods: This study retrospectively evaluated the long-term stability of orthognathic surgery and the final profile of skeletal morphology in 30 patients using lateral cephalometric imaging. Sagittal split ramus osteotomy was performed for 10 patients (CLP-S), Le Fort I osteotomy was performed for 5 patients (CLP-L) and two-jaw surgery was performed for 15 patients (CLP-T). As a control, we included 5 patients treated by two-jaw surgery, 7 patients treated by SSRO without CLP and 20 CLP patients with orthodontic treatment alone. Results: In the CLP group at 2 years postoperatively, the mean relapse in SNA was 19.8% (CLP-T) and 18.1% (CLP-L), and the mean relapse in SNB was 17.6% (CLP-S) and 26.9% (CLP-T). No significant differences were observed in the relapse ratio of the same operation between with and without CLP. In comparison of surgical and non-operative patients with CLP, there were significant differences in preoperative skeletal factors, but the profiles were similar at the end of treatment. Conclusion: Our orthognathic surgeries provided the same level of stability for patients with skeletal class III deformity with or without CLP, creating harmonious intermaxillary relationship.
