    <p><b><i>Background:</i></b> Although metabolic dysfunction-associated fatty liver disease is typically diagnosed using ultrasonography, the fatty liver index (FLI) offers a simple alternative. Brachial-ankle pulse wave velocity (baPWV) is an established marker of arterial stiffness and a predictor of cardiovascular events. This study investigated the association between FLI and baPWV in a general Japanese population.</p><p><b><i>Methods and Results:</i></b> This cross-sectional study included participants aged ≥18 years, excluding those with atrial fibrillation, lower extremity artery disease, severe aortic stenosis, or missing data. The primary outcome was an elevated baPWV (≥1,800 cm/s). Multivariable logistic regression analysis was performed to assess the association between FLI and elevated baPWV, considering FLI both as a categorical variable (low risk: FLI <30; moderate risk: FLI ≥30 and <60; high risk: FLI ≥60) and as a continuous variable (per 10-unit increase). The analysis included 10,122 individuals (mean age 54.3 years; 55% male). In multivariable-adjusted models, the odds of elevated baPWV were significantly higher in the moderate-risk (odds ratio [OR] 1.47; 95% confidence interval [CI] 1.20–1.79) and high-risk (OR 1.78; 95% CI 1.33–2.38) groups, using the low-risk group as the reference category. Each 10-unit increase in FLI was associated with significantly higher odds of the outcome (OR 1.16; 95% CI 1.10–1.22).</p><p><b><i>Conclusions:</i></b> FLI showed a significant association with arterial stiffness in the general Japanese population.</p>

    Aim Sarcopenia, a major cause of frailty in postmenopausal women, is linked to mitochondrial dysfunction, but the underlying mechanisms remain unclear. This study aimed to clarify whether mitophagy, a mitochondrial quality control mechanism, contributes to postmenopausal sarcopenia, to elucidate its underlying mechanism, and to assess whether it can be rescued. METHODS: C57BL/6 mice (12-week-old females) underwent ovariectomy to establish a menopause mouse model, or sham surgery, and the therapeutic effects of nicotinamide mononucleotide (NMN) were assessed. Human skeletal muscle myoblasts (HSMMs) differentiated under postmenopausal conditions with or without 17β-estradiol (E2), and Rab9 expression was modulated using CRISPR activation. RESULTS: Ovariectomized mice exhibited decreased muscle mass and strength. E2 deficiency in HSMMs inhibited skeletal muscle cell differentiation, promoted senescence, impaired mitochondrial function, and reduced mitophagy. However, E2 deficiency did not modulate light chain 3 and autophagy-related 7 but reduced Rab9 expression and the colocalization of Rab9 with lysosomal-associated membrane protein 2, suggesting that E2 mediates mitophagy through Rab9-dependent alternative autophagy. Furthermore, overexpression of Rab9 in E2-deficient HSMMs enhanced mitophagy, improved mitochondrial function, suppressed cellular senescence, and promoted skeletal muscle cell differentiation. The administration of NMN to ovariectomized mice increased Rab9 expression and improved sarcopenia through increased mitophagy. CONCLUSION: This study demonstrates that estrogen deficiency impairs mitophagy originated from Rab9-dependent alternative autophagy, leading to mitochondrial dysfunction and sarcopenia, while enhancement of Rab9 restores mitochondrial quality control and muscle function. These results identify Rab9-dependent mitophagy as a potential therapeutic target for postmenopausal sarcopenia.

    Background: Psychological frailty (PsF), encompassing cognitive and emotional vulnerabilities, remains understudied despite its potential impact on health outcomes in older adults. This cross-sectional study aimed to explore the characteristics of diverse aspects of PsF, including cognitive frailty (CF), lower cognitive function, and affective frailty (AF), and to examine their associations with health-related outcomes in community-dwelling older adults. Methods: Data were analysed from 420 community-dwelling older adults participating in the Tarumizu Study in Japan, excluding those exhibiting physical or social frailty. Participants were categorised into Robust, Lower cognitive function, CF, AF, and Multi-domain PsF groups. AF was assessed using five items from the Geriatric Depression Scale, focusing on emotional vulnerability. Health outcomes included self-rated health, subjective memory complaints (SMC), and functional ability of daily living. Associations were analysed using adjusted multiple regression models controlling for covariates. Results: PsF was present in 39.5% of the cohort (lower cognitive function: 18.1%, CF: 7.6%, AF: 11.0%, Multi-domain PsF: 2.9%). In the adjusted models, only AF showed a significant positive association with SMC (adjusted risk ratio = 1.62, 95% CI: 1.06–2.42, p = 0.023). No significant associations were found between any PsF domain and subjective health or daily functional ability in the adjusted models. Conclusions: AF, characterised by emotional vulnerability, was uniquely associated with SMC, suggesting emotional factors may play a more central role than cognitive decline in shaping perceptions of memory problems. These findings highlight the heterogeneous nature of PsF and underscore the need for nuanced assessments to inform targeted prevention and intervention strategies.

    Background: Gaze anxiety (scopophobia) is a frequently observed and reported symptom that is closely associated with social anxiety disorder (SAD). Considering the efficacy of cognitive behavioral therapy (CBT) for social anxiety, it may also be beneficial for individuals with gaze anxiety. Self-help CBT can offer a means of early intervention; however, its effectiveness remains unclear. Methods: This protocol describes the design of a clinical trial aimed at evaluating the efficacy of two self-help CBT interventions for gaze anxiety. This multicenter randomized controlled trial is planned across 12 institutions in Japan. The participants will be young adults aged 18–30 years who report anxiety and avoidance related to gazes. The enrolled participants will be randomly assigned to either a self-help book CBT group, a web-based CBT group, or a control group. The severity of gaze anxiety, as the primary outcome, will be assessed using online questionnaires at baseline, 3 months post-intervention, and the 6-month follow-up, with effectiveness evaluated through statistical analysis using mixed-effects models. Results: The clinical trial will be performed from April 2025 to March 2026, and the results of this study are expected to be available by mid-2026. The results of this trial will provide insights into the effectiveness of self-help CBT for gaze anxiety (scopophobia). Conclusions: This trial will examine whether self-help CBT improves access and whether it is effective for adolescents and young adults with social anxiety, especially gaze anxiety.

    Background: The percentage of people owning smartphones is rapidly increasing due to the digitalization of modern society. Among the smartphone’s features, route guidance using navigation applications establishes spontaneous and independent outdoor mobility, and can potentially support people’s health. Despite some known concerns regarding smartphone use while walking, navigation performance and eye-tracking behavior while walking with navigation applications in real-world environments are unclear. This study aimed to investigate the association between gaze behavior, particularly glancing at smartphones, and navigation performance among adult users of navigation applications while walking. Methods: This cross-sectional experimental study involved 33 healthy adults aged 20 years and older (20–81 years, 66.7% female) not requiring assistance with walking or activities of daily living. A route navigation task (RNT) of approximately 1,300 m was performed in a real outdoor environment. The participants were required to complete the route as quickly as possible, following the instructions of the navigation application. Simultaneously, the fixation time, number of fixations, number of saccades, total saccade angle, and number of glances at the smartphone were recorded using a wearable eye-tracking device. After checking the distribution of the obtained outcomes and performing correlation analysis, a generalized linear model (GLM) was created to investigate the relationship between the number of glances at the smartphone and the participants’ navigation performance and gaze behavior. To confirm the validity of the estimates for each GLM, a bootstrap sample of 2,000 iterations was used to evaluate the estimates. Results: Correlation analysis showed that the number of stops and route errors were moderately and positively correlated with the number of times participants glanced at their smartphones (P<0.05). In the original GLM, the number of stops and route errors showed a significant positive association with the number of glances at the smartphone (P<0.05). This significant positive relationship persisted even after examining the bootstrap sample. Conclusions: In healthy adults, frequent screen glances while walking with a navigation application may interfere with navigation performance. Establishing an alternative navigation system that does not rely on screen-checking may help pedestrians move more efficiently and safely.

    Hypertension is a significant risk factor for cardiovascular diseases, with its global prevalence doubling over the past three decades. Despite advancements in antihypertensive therapies, approximately 50% of patients with hypertension fail to achieve their target blood pressure (BP) levels, underscoring the need for innovative care strategies. Patient care teams comprising multidisciplinary healthcare providers have shown promise in improving BP management. This systematic review and meta-analysis were aimed at evaluating the effectiveness of patient care teams involving physicians in hypertension management. To this end, PubMed, Cochrane CENTRAL, and IchuShi-Web were comprehensively searched and 61 randomized controlled trials including 64,857 participants were identified. Compared with usual care, interventions by patient care teams significantly reduced office systolic BP (mean difference: −6.31 mmHg; 95% confidence interval: −7.71 to −4.90) and decreased the risk of uncontrolled BP by 27% (risk ratio: 0.73; 95% confidence interval: 0.68–0.79). Subgroup analyses demonstrated consistent BP reductions across various team leadership roles, such as physicians, nurses, and pharmacists, and across different intervention durations. These findings highlight the effectiveness of team-based BP management in achieving improved BP control, regardless of team composition or the follow-up period. Multidisciplinary care offers a viable approach to addressing the unmet needs of patients with hypertension, potentially improving cardiovascular outcomes. This evidence supports integrating patient care teams into hypertension management, particularly in settings requiring physician oversight. Future research should focus on refining team structures and tailoring interventions to diverse healthcare environments to enhance their impact. (Figure presented.)

    レンバチニブ投与に伴う血圧上昇と転帰の関係について検討した。対象は入院にて本剤の投与を開始した肝細胞癌患者67名(男性82.1%、年齢中央値71歳)である。血圧は入院当日と比較して投与開始翌日には高値となった。入院期間中、37.3%の患者が副作用グレード3(≧160/100mmHg)の高血圧を呈した。収縮期血圧の入院当日値から入院中最大値までの上昇量(ΔSBP)の中央値で患者を高/低群に分けて比較したところ、高群(ΔSBP≧26mmHg)は低群より死亡率が低かった。交絡因子を調整したCoxモデルにおいて、ΔSBP≧26mmHgは死亡のリスク低下と有意な関連を示した。レンバチニブ治療を受ける癌患者において、投与後の血圧上昇は予後を反映する有用な指標となる可能性がある。

    成人の血圧(BP)管理における遠隔医療/テレヘルスのためのデジタルヘルス介入(DHI)と通常のケアを比較した無作為化対照試験または観察研究117件(参加者68677名)のメタ解析を行った。群間の加重平均差(MD)のランダム効果メタ解析を用いてデータをプールした。ベースラインから各追跡調査時までの外来BPのプールされたMDをアウトカムとした。3ヵ月間の介入により収縮期BP(SBP)は通常ケアと比べて38研究で低下し、異質性のエビデンスが示された。SBP低下は介入期間が異なっても同様にみられた(3、6ヵ月(54件)、12ヵ月(43件)、>12ヵ月(9件))。SBPと同様に拡張期BPにも効果がみられた。介入提供法(スマートフォン・アプリケーション、テキストメッセージ、ウェブサイト)や施設タイプ(医療施設、非医療施設)によらず介入はSBPを有意に低下させた。介入は非高血圧コホートよりも高血圧コホートに有効であった。

    成人の血圧(BP)管理における遠隔医療/テレヘルスのためのデジタルヘルス介入(DHI)と通常のケアを比較した無作為化対照試験または観察研究117件(参加者68677名)のメタ解析を行った。群間の加重平均差(MD)のランダム効果メタ解析を用いてデータをプールした。ベースラインから各追跡調査時までの外来BPのプールされたMDをアウトカムとした。3ヵ月間の介入により収縮期BP(SBP)は通常ケアと比べて38研究で低下し、異質性のエビデンスが示された。SBP低下は介入期間が異なっても同様にみられた(3、6ヵ月(54件)、12ヵ月(43件)、>12ヵ月(9件))。SBPと同様に拡張期BPにも効果がみられた。介入提供法(スマートフォン・アプリケーション、テキストメッセージ、ウェブサイト)や施設タイプ(医療施設、非医療施設)によらず介入はSBPを有意に低下させた。介入は非高血圧コホートよりも高血圧コホートに有効であった。

    With the rapid development of information and communication technology, smartphone ownership has increased among older adults. While previous research has examined the impact of smartphone use on the health of older adults, this study focuses on the less-explored relationship between smartphone use, proficiency, and health-related outcomes including higher-level competence. The study was a cross-sectional survey of 208 community-dwelling older adults who used smartphones daily. Smartphone use, including years of use and applications, was assessed and categorized by age group. Health-related outcomes were measured, including cognitive and psychological function, higher-level competence, basic daily abilities, and physical function. The results indicated that smartphone proficiency decreased with age, with older adults primarily using basic smartphone applications. Notably, higher smartphone proficiency was positively associated with higher-level competence and physical functions. The study underscores the importance of considering smartphone proficiency when assessing older adults’ life skills and physical performance in our digital society.

    Objectives: This two-wave longitudinal study examined whether changes in satisfaction with meaningful activities (MA) were associated with subsequent subjective memory complaints (SMC) among community-dwelling older adults between 2019 and 2021. Method: Health surveys were conducted among local people at two time points: baseline (2019, before the COVID-19 pandemic) and follow-up (2021, during the pandemic). The participants were 275 community-dwelling older adults aged ≥65 years without cognitive decline at baseline. Satisfaction with MA was assessed using the Aid for Decision-Making in Occupation Choice and categorized as maintained/improved or decreased at follow-up. SMC was measured using four items developed by Tsutsumimoto and quantified according to the total number of complaints. Results: Older adults who maintained or improved satisfaction with MA showed no significant increase in SMC (p = 0.30), whereas those who demonstrated decreased satisfaction experienced a significant increase in SMC (p = 0.02). In the generalized linear mixed-effects model adjusted for covariates, a significant association was observed between decreased satisfaction with MA and increased SMC scores (β = 0.32, 95% CI = 0.09–0.55, p < 0.01). Conclusion: Maintaining and improving satisfaction with MA may be associated with a reduced risk of SMC increase in older adults.

    Objectives: To examine whether satisfaction with meaningful activities (MA) is associated with self-rated sleep quality and sleep duration among community-dwelling older adults. Methods: Data were drawn from 833 individuals aged ≥65 years who participated in a health survey between 2018 and 2019 in Japan. Sleep quality (good, moderate, or poor) and duration (< 6, 6–8, or ≥8 h) were assessed through a questionnaire. Participants selected the MA they considered most meaningful from a list of 95 activities in the Aid for Decision-Making in Occupation Choice, and their satisfaction with these MA were measured. Results: Ordinal logistic regression analysis, with sleep quality or duration as the dependent variable and satisfaction with MA as the independent variable, showed that higher satisfaction with MA was significantly associated with lower odds of poor sleep quality (OR: 0.84, 95% CI: 0.72–0.97, p =.03), but not with sleep duration. However, the association was not significant in sensitivity analyses that reversed the model. Conclusions: Satisfaction with MA may be associated with better sleep quality in older adults. Clinical Implications: Evaluating and enhancing satisfaction with MA may help identify older adults at risk of poor sleep quality, indicating that MA may be useful in supporting sleep.

    This cross-sectional observational study aimed to clarify the relationship between satisfaction with grave visitation and depressive symptoms among Japanese older adults. We analysed older adults who participated in a community health survey (n = 1025). Participants selected meaningful activities from the 95-item Aid for Decision-Making in Occupation Choice and rated their satisfaction on a five-point scale. Depression symptoms were assessed using the 15-item Geriatric Depression Scale. Grave visitation ranked fourth among all activities and was the most frequently chosen social activity. Higher satisfaction with grave visitation was associated with a significantly lower prevalence of depression, particularly in the hopelessness dimension. After adjusting for potential covariates in a Poisson regression analysis, satisfaction with grave visitation was significantly related to depression symptoms (prevalence ratio,.58; 95% confidence interval,.35–.96). Discussing beliefs about the deceased and ancestors and helping to increase satisfaction with grave visitation may help alleviate depressive symptoms.

    Objectives: Participating in religious activities benefits older adults’ mental health; however, the effects of grave visitation, a significant religious practice in Japan, on older adults’ mental health remain unclear. Therefore, this study examined the relationship between grave visitation frequency and depressive symptoms. Methods: A prospective cohort study of 273 community-dwelling older adults in Japan was conducted with baseline (2018) and follow-up (2022) assessments. Participants were categorized by visitation frequency: more or less than once a week. Changes in visitation frequency were classified as continued high, decreased, continued low, or increased. Depressive symptoms were assessed using the Geriatric Depression Scale (GDS-15). Multivariable linear regressions were used to examine the association between visitation and depressive symptoms. Results: Lower visitation at baseline was associated with worse GDS-15 scores in the follow-up. Those with decreased or continued low visitation had significantly worse depressive symptoms. After adjusting for baseline GDS-15 scores and covariates, the decreased group had significantly worse depressive symptoms than the continued high group. Conclusions: The findings indicate that changes in grave visitation frequency are significantly associated with depressive symptoms. Clinical Implications: Supporting older adults’ access to grave visitation through transportation or community programs may help maintain their well-being and emotional health.

    Purpose The purpose of the study was to investigate the effects of satisfaction with meaningful activities and their performance on the memory function among community-dwelling older adults 3 years later. Methods A longitudinal analysis was conducted on 288 community-dwelling older adults aged 65 years and more who had completed the survey in 2019 and 2022 as participants in the Tarumizu Study. Meaningful activities were assessed in 2019 using the Aid for Decision-Making in Occupation Choice for activities. Satisfaction with and performance of meaningful activities were assessed, and the bottom 25% were classified into two groups: low satisfaction and low performance. Memory function was assessed using the National Center for Geriatrics and Gerontology-Functional Assessment Tool in 2019 and 2022. Changes in memory function over 3 years were analyzed by satisfaction and performance of meaningful activities. Results Two-way analysis of variance with group (satisfaction with and performance of meaningful activities) and time (2019 and 2022) as factors showed no statistically significant group-time interaction for memory in the satisfaction with meaningful activities group (F = 1.4, p = 0.24). Meanwhile, a statistically significant group-time interaction was observed for memory in the meaningful activity performance group, indicating that high performance of meaningful activities may play a protective role against memory decline (F = 7.4, p = 0.007). Conclusion Higher performance of meaningful activities may protect against age-related changes in memory. A careful assessment of meaningful activities, and engagement that enhances performance may be useful in designing health support against memory decline.

    Although previous reports have shown that sodium-glucose cotransporter-2 (SGLT2) inhibitors have a blood pressure (BP) lowering effect, relevant long-term data is limited. This study aimed to evaluate the effect of the SGLT2 inhibitor ipragliflozin on BP, and associations between BP reduction and changes in cardiometabolic variables in diabetic patients. This was a sub-analysis of the PROTECT trial, a multicenter, randomized, open-label study to assess if ipragliflozin delays carotid atherosclerosis in patients with type 2 diabetes. Participants were randomized to ipragliflozin and control groups. The primary endpoint of the present sub-analysis was the trajectory of systolic BP over 24 months. Correlations between systolic BP changes and cardiometabolic variables were also evaluated. A total of 232 eligible participants with well-balanced baseline characteristics were included in each study group. Throughout the 24-month study period, mean systolic BP was lower in the ipragliflozin group. At 24 months, a between-group difference (ipragliflozin minus control) in mean systolic BP change from baseline was −3.6 mmHg (95% confidence interval, −6.2 to −1.0 mmHg), and the reduction in systolic BP in the ipragliflozin group was consistent across subgroups examined. Changes in systolic BP significantly correlated with those in body mass index in the ipragliflozin group, while no significant correlations with other cardiometabolic variables tested were observed. In conclusion, ipragliflozin treatment was associated with BP reduction throughout the 24-month follow-up period as compared to control treatment. BP reduction correlated with weight loss, which might be one of the mechanisms for the BP lowering effect of SGLT2 inhibitors. (Figure presented.)

    Introduction: The Mishima Islands in Kagoshima Prefecture, Japan, are remote islands designated as medically underserved. Supporting the people living in such remote islands requires a multifaceted examination of the livelihoods of the residents, including their health, work, and living environment. Methods: Students from the faculties of nursing, veterinary science, agriculture, and engineering participated in a 3-day virtual fieldwork session held synchronously from the campus of Kagoshima University and on a remote island in the Mishima Mura island group in Kagoshima Prefecture, Japan. Coursework and post-course surveys were analyzed to determine student engagement and learning. In particular, the analysis considered learning in relation to interprofessional learning and community-based learning. Results and discussion: Students expressed being able to conduct fieldwork online, collaborate with students from multiple faculties and graduate schools, and apply their expertise. These outcomes were reflected in the five-point Likert scale evaluation responses, highlighting a sense of accomplishment and satisfaction. Virtual fieldwork can contribute to student learning by introducing new concepts and allowing them to consider development issues in remote areas from multiple perspectives.

    Background: Considering concerns about conventional toe grip strength, we devised a method to measure toe pressure strength in the standing position, which is close to the actual motion. This study examined the association between toe pressure strength in the standing position and walking speed among older adults. Methods: This cross-sectional study included 150 communi-ty-dwelling older adults (81±8 years, 73% female) who participated in the physical fitness test. We analyzed the correlation between the participants’ maximum walking speed and physical function. Furthermore, we performed regression analysis with the maximum walking speed as the dependent variable to examine the association with toe pressure strength in the standing position. We also examined the association between maximum walking speed and toe pressure strength in the standing position by introducing a covariate. Results: Correlation analysis revealed a significant positive correlation between maximum walking speed and toe pressure strength in the standing position, with a moderate effect size (r=0.48, p<0.001). Moreover, multiple regression analysis with covariates showed an association between maximum walking speed and toe pressure strength in the standing position (standardization factor=0.13, p<0.026). Conclusion: Toe pressure strength in the standing position was associated with maximum walking speed. This finding clari-fies the significance of assessing toe pressure strength in the standing position and suggests that enhanced toe pressure strength in the standing position may increase maximum walking speed.

    Hyperuricemia is reportedly associated with the progression of carotid intima-media thickness (IMT), a surrogate of cardiovascular risks and events. However, factors associated with carotid IMT progression in patients with asymptomatic hyperuricemia are largely unknown. In this post-hoc analysis of the multicenter, randomized PRIZE study, we analyzed data from a total of 326 patients who underwent carotid ultrasonography in a blind manner at baseline and 24 months to evaluate carotid IMT. Mean and maximum IMT at the common carotid artery (CCA) were measured at a central core laboratory. Factors related to the absolute change in mean and maximum IMT from baseline to 24 months were explored. Overall, the adjusted mean [0.0032 (− 0.0214 to 0.0278) mm] and maximum [0.0011 (− 0.0327 to 0.0351) mm] CCA-IMT increased numerically from baseline to 24 months. Multivariable analysis identified higher body mass index, history of atherosclerotic cardiovascular disease (ASCVD), and lower mean CCA-IMT at baseline as significant factors associated with the increase in mean CCA-IMT. In addition, older age and lower mean CCA-IMT at baseline were significant factors for an increased absolute change in the maximum CCA-IMT at 24 months. The present sub-analysis of the PRIZE study showed higher body mass index, history of ASCVD, and older age as significant factors associated with CCA-IMT progression in patients with asymptomatic hyperuricemia. These factors may be considered when identifying the possible risk of atherosclerotic progression in this specific patient population of hyperuricemia.     Background: A method has been developed to assess toe pressure strength in the standing position, taking into account concerns about toe grip strength. Research question: Which is more associated to postural control capability, the conventional toe grip strength or the newly devised toe pressure strength in the standing position, which is close to the actual movement? Methods: This study is a cross-sectional study. This study included 67 healthy adults (mean age, 19 ± 1 years; 64% male). The postural control capability was measured using the center-of-pressure shift distance in the anterior–posterior axis. Toe pressure strength in the standing position was assessed using a toe pressure measuring device to evaluate the force of pressure on the floor surface by all toes. During measurement, care is taken to ensure that the toes do not flex. However, toe grip strength in the sitting position was measured using muscle strength for toe flexion in a conventional manner. Statistical analysis was conducted by performing a correlation analysis between each of the measured items. Additionally, multiple regression analysis was used to examine the functions associated with postural control capability. Result: Pearson's correlation analysis revealed that the postural control capability was correlated with toe pressure strength in the standing position (r = 0.36, p = 0.003). Multiple regression analysis demonstrated that only toe pressure strength in the standing position was significantly associated with the postural control capability, even after adjusting for covariates (standard regression coefficient: 0.42, p = 0.005). Significance: The results of this study indicated that toe pressure strength in the standing position was more strongly associated with the postural control capability in healthy adults than toe grip strength in the sitting position. It has been suggested that a rehabilitation program for toe pressure strength in the standing position would help improve postural control capability.

    Objectives: This study aimed to clarify the association between changes in satisfaction with meaningful activities and the occurrence of depressive symptoms before and during the coronavirus disease 2019 (COVID-19) pandemic. Design: A population-based prospective cohort study (Tarumizu Study). Setting and Participants: A total of 231 community-dwelling older adults aged ≥65 years without depressive symptoms at baseline. Methods: The participants completed baseline (June to December 2018, before the COVID-19 pandemic) and follow-up (September to December 2021, during the COVID-19 pandemic) assessments. Depressive symptoms were assessed using the 15-item Geriatric Depression Scale (GDS-15) and defined by a GDS-15 score of ≥5. Participants selected meaningful activities from the 95 activities included in the Aid for Decision-Making in Occupation Choice, and their satisfaction levels were evaluated. Satisfaction with the most meaningful activities was assessed as to whether it was maintained/improved or decreased at follow-up. Results: In the follow-up survey, 31 (14.3%) participants reported the occurrence of depressive symptoms. Compared with before the COVID-19 pandemic, the rates of meaningful activities during the COVID-19 pandemic decreased for interpersonal interaction and social life and increased for leisure, sports, self-care, and domestic life. Logistic regression analysis revealed that changes in satisfaction with meaningful activity were associated with the occurrence of depressive symptoms after adjusting for demographic variables, baseline GDS scores, cognitive and physical status, and instrumental activities of daily living (odds ratio, 2.78; 95% confidence interval, 1.17–6.59, P = .020). Conclusions and Implications: To avoid decreasing satisfaction with meaningful activities, supporting older adults may be protective against the occurrence of depressive symptoms. Our findings highlight the need for health care professionals to not only encourage older adults to participate in activities for their mental health but also consider whether the activities are meaningful or satisfying for them.

    Background: It has been shown that involvement in religious activities has a positive impact on psychological aspects. In this study, the relationship between grave visitation, a standard religious activity in Japan, and depression and apathy symptoms was investigated among older adults in Japan. Methods: A total of 638 older adults who participated in a community-based health check survey (Tarumizu Study 2019) were interviewed regarding the presence or absence of grave visitation, frequency, travel time, means of transportation, and flower offerings. Apathy and depressive symptoms were measured using the 15-item Geriatric Depression Scale (GDS-15). The participants were categorised into three grave visitation groups, namely, frequent (more than once per week), occasional (less than once per week), and non-visiting. Outcomes were compared between the frequency groups, and Poisson regression analysis was used to investigate the relationship between grave visitation frequency and apathy and depression. Results: Of the participants, 91.8% reported regular grave visitation. The non-visiting group had a significantly higher prevalence of apathy symptoms (44.2%) than the visiting groups. Furthermore, using the frequent group as the reference, Poisson regression analysis adjusted for potential covariates demonstrated that no grave visitation was significantly related to apathy (prevalence ratio, 1.43; 95% confidence interval, 1.00–2.05, P = 0.049). Conclusions: Not practising grave visitation was significantly related to apathy among older adults. Helping older adults to visit graves may prevent apathy by facilitating motivation and increasing activity.

    Evidence suggests that optimism has a positive impact on health status. Attentional bias modification (ABM) may be beneficial for enhancing optimism, but its effective application requires a detailed investigation of the association between attentional bias and optimism. This study aimed to determine the association between attentional bias and optimism based on different task types. Eighty-four participants completed the attentional bias measures using the dot-probe task (DPT), emotional visual search task (EVST) paradigms, and psychological assessments. Optimism was assessed using the Life Orientation Test-Revised with subscales for optimism and pessimism. Pearson’s correlation coefficient and multivariate linear regression analysis were applied to investigate the association between optimism and attentional bias. Neither the attentional bias derived from DPT nor EVST was significantly correlated with optimism total score or subscales. Regression analysis also showed no association between attentional bias and optimism (DPT, β = 0.12; EVST, β = 0.09), optimism subscales (DPT, β = 0.09; EVST, β = 0.17), or pessimism subscales (DPT, β = −0.10; EVST, β = 0.02). Our findings showed no evidence that attentional biases derived from either the DPT or EVST measures are associated with optimism or pessimism. Further studies are needed to effectively adapt the ABM to enhance optimism.

    Background and Objectives: Advancing age might impair real-world navigation ability. The use of mobile devices by older adults has grown rapidly in recent years. Navigation applications (apps) in mobile devices may facilitate the freedom of outings for older adults. Our aim is to investigate age-related differences in real-world app-based navigation walking in terms of accuracy, efficiency, and gaze behavior; and to explore clinical factors associated with navigation walking in older adults. Research Design and Methods: A total of 20 community-dwelling older adults and 16 young adults completed a route navigation task using a navigation app while recording their gaze behavior. Outcomes were compared in both groups and a general linear regression was used to explore clinical factors associated with app-based navigation walk in older adults. Results: Compared with young participants, older participants had more stops and root errors and less fixation time, smaller amplitude of saccades. Additionally, older adults were more likely to glance at their smartphones while app-based navigation walking. Furthermore, gait speed and the following assessment scores were significantly associated with navigation walking in older adults: Mini-Mental State Examination, Life-Space Assessment, and the short version of the Mobile Device Proficiency Questionnaire. Discussion and Implications: For app-based navigation walks, differences in accuracy and gaze behavior were found to exist with age. Additionally, efficient real-world navigation walks in older adults require the extent of life space and proficiency with mobile devices, along with walking speed and cognitive function. It is possible that age-related functional decline, such as the visual field and shifting attention between mobile devices and the real world, may have influenced the results. The study also suggests the need to understand the level of proficiency with mobile devices so that older adults can continue to go out freely. These findings give the basis for providing older adults with appropriate navigation assistance. Translational Significance: Real-world navigation ability is known to decline with age. Many older adults are now using mobile devices, but little is known about their accuracy and gaze behavior while using navigation applications. Our study revealed that older participants exhibited more stops and errors, smaller saccade angles and fixation durations, and glanced at smartphones more frequently than younger participants during navigation tasks. Efficient navigation was associated not only with cognitive and physical function but also with life space and mobile device proficiency. Our findings provide valuable insights for enhancing the use of technology by older adults and improving their navigation in the real world.

    Hypertension is a risk factor for atherosclerosis. Achieving the therapeutic target value of blood pressure (BP) prevents the onset of cardiovascular events; however, it is not clear how antihypertensive drug use and BP control status relate to arterial stiffness. The purpose of this study is to investigate the relationship between BP control status with or without antihypertensive drugs and arterial stiffness. Nine hundred eighty individuals (mean age: 68 ± 11 years) who participated in a community-based cohort study were enrolled. Arterial stiffness was evaluated using the cardio-ankle vascular index (CAVI). Higher BP was defined as a systolic BP ≥ 140 mmHg or diastolic BP ≥ 90 mmHg. Participants were divided into four groups: normal, non higher BP without antihypertensive drugs (n = 421); untreated, higher BP without antihypertensive drugs (n = 174); good control, non higher BP with antihypertensive drugs (n = 209); and poor control, higher BP with antihypertensive drugs (n = 176). In multivariable logistic analysis adjusted for age, sex, dyslipidemia and diabetes mellitus medication use, obesity, smoking, alcohol drinking, and heart rate at the CAVI measurement for a high CAVI—using a borderline cutoff value of 8.0—the other three groups were significantly associated with a high CAVI when compared with the normal group. By contrast, multivariable logistic analysis of a high CAVI using an abnormal cutoff value of 9.0 demonstrated that the poor control and untreated groups were significantly associated with a high CAVI, whereas the good control group was not. In conclusion, even with antihypertensive drugs, poor BP control is independently associated with a high CAVI.

    Patients with heart disease are more likely to experience social frailty due to physical inactivity, which may affect meaningful activities such as hobbies. This study aimed to investigate (1) the association between heart disease and social frailty in community-dwelling older adults and (2) the characteristics of meaningful activities in community-dwelling older adults with heart disease. Data from 630 older adults who participated in a community-based health survey were obtained, including clinical history, meaningful activities, social frailty and psychosomatic functions. Participants were divided into two groups: those with heart disease (n = 79) and those without (n = 551), and comparisons were made. Social frailty was observed in 23.7% of participants with heart disease, and logistic regression revealed significant associations with heart disease and social frailty after adjusting for potential covariates (OR, 1.97; 95% CI, 1.06 3.67; p = 0.032). Participants with heart disease did not differ significantly in terms of satisfaction or performance; their frequency of engagement in meaningful activities was significantly lower than without heart disease (p = 0.041). These results suggest that heart disease and social frailty are associated in community-dwelling older adults, and that this demographic is inclined to engage in meaningful activities less frequently.

    Background: Frailty is a multidimensional condition characterised by reduced physical and psychological resilience. Older adults also frequently demonstrate apathy, suggesting that it shares similar neuro-physiological pathways with frailty. This cross-sectional study aimed to investigate the association between apathy and frailty as defined by a comprehensive assessment. Methods: We analyzed 882 older adults (mean age: 74.4 ± 6.4 years; 62.1% female) who participated in a community-based health check survey (Tarumizu Study). Apathy was measured by the Geriatric Depression Scale-3A, a three-item subset of the Geriatric Depression Scale-15, and frailty by the Kihon Checklist of physical, psychological, functional, and social status. Associations were examined by multinomial logistic regression with frailty status (robust, pre-frailty, frailty) as the dependent variable, apathy as the independent variable, and sociodemographic factors, medications, cognitive function, functional capacity, and mood symptoms as potential confounders. Results: Apathy was observed in 23.7% of individuals, and logistic regression revealed significant associations with both pre-frailty and frailty after confounder adjustment (pre-frailty: odds ratio (OR) 1.80, 95% CI 1.22–2.64; frailty: OR 3.24, 95% CI 1.63–6.42). Participants with apathy also exhibited greater deficits in the Kihon Checklist subdomains instrumental activities of daily living (P = 0.022), physical function (P < 0.001), oral function (P < 0.001), and cognitive function (P = 0.001). Conclusions: Our findings underscore the importance of comprehensive frailty assessment and demonstrate that apathy can have pervasive deleterious effects on geriatric health.

    Objectives: This cross-sectional study investigates the associations among satisfaction with meaningful activities, and social frailty in community-dwelling Japanese older adults. Methods: We analyzed data from 596 older adults (mean age 74.2 ± 6.4 years, female 63.6%) who participated in the Tarumizu Study 2019, a community-based health survey. Participants selected meaningful activities from 95 activities using the Aid for Decision-Making in Occupation Choice (ADOC) tool. Satisfaction was evaluated from 1 to 5, and those who were assigned a rating of 4 or 5 were operationally classified as the high satisfaction group (n = 487), with others occupying the low satisfaction group (n = 109). Both groups were evaluated based on the ADOC, social frailty (Makizako's five items), physical function, depressive symptoms, cognitive function, and higher-level competence. Results: Of the 596 participants, 18.7% showed prevalence of social frailty. The low satisfaction group had a significantly higher prevalence of social frailty (low satisfaction 28.4% vs. high satisfaction 16.4%, P = 0.004) and depressive symptoms (low satisfaction 30.3% vs. high satisfaction 17.9%, P<0.01), and poor higher-level competence (P = 0.026) than the high satisfaction group. Logistic regression analysis showed that social frailty (Odds Ratio 1.78, 95% Confidence Interval 1.068–2.990, P = 0.027) was significantly associated with satisfaction with meaningful activity after adjusting for covariates. We found no significant differences in categories of meaningful activities between the low and high satisfaction groups (P = 0.549). Conclusions: This study suggested that social frailty was associated with satisfaction with meaningful activities, regardless of the degree or category of satisfaction.

    Objective: Vinegar has been reported to have a hypotensive effect. We aimed to investigate the relationship between the consumption of vinegar-based side dishes and blood pressure. Research methods & procedures: This cross-sectional study included 746 individuals (257 men and 489 women) aged ≥40 years from Tarumizu, Kagoshima, Japan. Nutrient intake was estimated based on the brief-type self-administered diet history questionnaire. The intake frequency of vinegar-based side dishes (Sunomono and pickles) was determined using a self-administered diet history questionnaire. Participants who did not consume vinegar-based side dishes for a month were defined as having no Sunomono or pickle eating habit. Blood pressure was categorized into four groups according to the Japanese Society of Hypertension Guidelines for the Management of Hypertension. The association between the intake of vinegar-based side dishes and blood pressure categories was analyzed using ordinal logistic regression analysis adjusted for age, body mass index, smoking history, excessive alcohol intake, living situation, energy intake, protein intake, sodium intake, potassium intake, and seaweed intake. Results: Approximately 13.6% men and 6.1% women had no Sunomono eating habits. In men, eating Sunomono, but not pickles, was significantly related to blood pressure categories (estimate, -0.702; 95% CI, -1.122 to -0.310), whereas more frequent consumption of Sunomono did not show an improvement in the blood pressure category. The relationship between eating Sunomono and blood pressure categories was not recognized in women. Conclusion: This was the first study assessing the association between consumption of vinegar-based side dishes and blood pressure categories. We highlighted the effect of Sunomono consumption on blood pressure categories in men. Consumption of Sunomono may improve blood pressure in men.

    当院外来を受診した65歳以上の高血圧症患者のうち、過去に認知障害と診断されていない患者312例を対象に、認知機能障害の発生率とその決定要因について検討した。評価項目は患者背景、ミニメンタルステート検査、手段的日常生活動作(IADL)などとした。認知機能障害の高血圧患者109例(男性54例、平均79.0±6.5歳)、非認知機能障害の高血圧患者203例(男性95例、平均75.0±5.4歳)を対象とした。その結果、認知機能障害の高血圧患者は、非認知機能障害の高血圧患者と比較して、高齢で、教育水準が低く、IADLスコアが低かった。重回帰分析の結果、年齢およびIADLが高血圧患者の認知障害と関連していることが示された。両群間で診察室血圧および家庭血圧、降圧剤処方数、各降圧剤の使用割合は同程度であった。また、本試験に参加した認知機能障害の高血圧患者とORANGEレジストリに登録されたMCIを有する高血圧患者を比較した。その結果、本試験に参加した認知機能障害の高血圧患者はORANGEレジストリ患者と比較して降圧剤処方数が多く、重度の高血圧であることが示された。ORANGEレジストリ患者では本試験の患者と比較してIADL障害を有していることが示された。以上から、高血圧高齢患者は、たとえ機能的に自立していたとしても、隠れた認知機能低下リスクが高いことが示唆された。

    Background: The psychological condition and frequency of going out (FGO) of community-dwelling older adults after the spread of COVID-19 will provide insights for supporting the daily lives of community-dwelling older adults. Going out is defined as moving from one’s own home to a place or region beyond one’s own home and is considered to reflect the daily and social life of community-dwelling older adults. This study investigates the relationship between the FGO and current psychological condition after the second wave of COVID-19 in community-dwelling older adults in Japan. Methods: This study adopted a self-administered questionnaire by mail. A total of 493 members of CO-OP Kagoshima were included in the analysis and divided into two groups according to the change in FGO. Multiple logistic regression analysis was conducted after a bivariate analysis to investigate the relationship between the FGO and psychological condition. Results: Significant differences were noted between the groups with decreased FGO and those with increased/unchanged FGO in general and pandemic-related psychological condition. Multiple logistic regression analysis showed a significant relationship between FGO and psychological condition, such as mental fatigue, not smiling as much as before, and anxiousness to go outside. Conclusion: Community-dwelling older adults may have experienced a decrease in the frequency of going out and a detrimental effect on their psychological condition after the COVID-19 pandemic. This finding may inform strategies to identify priorities for psychological approaches altered by COVID-19 to prevent confinement and stress in older adults.

    Blood pressure variability (BPV) is associated with the prognosis of cardiovascular diseases. However, it is unclear how BPV is related to various organs. The aim of this study is to investigate the association between BPV and multiple organ functions. A total of three hundred fifteen participants (114 males; mean age: 70 ± 9 years) participated in a community health checkup held in Tarumizu City. Home blood pressure (BP) was measured using a HEM-9700T (OMRON Healthcare, Kyoto, Japan). Day-to-day BPV was evaluated by the coefficient of variation (CV) of home BP measured in the morning for one month. N-terminal pro B-type natriuretic peptide (NT-pro BNP) and high-sensitivity (hs-)troponin T were measured as cardiac biomarkers. Liver stiffness and renal function were evaluated using the Fibrous-4 (Fib4) index and estimated glomerular filtration rate (eGFR), respectively. NT-pro BNP and hs-troponin T were divided by the median value. Fib4 index greater than 2.67 and eGFR less than 60 mL/min/1.73 m<sup>2</sup> were defined as high Fib4 index and low eGFR, respectively. In a multivariable logistic regression analysis, the CV of systolic BP was significantly associated with high NT-pro BNP, high Fib 4 index, and low eGFR, but not with high hs-troponin T. In contrast, the CV of diastolic BP was not associated with low eGFR, and the other three biomarkers had the same results as systolic BP. In conclusion, day-to-day BPV of systolic BP is independently associated with NT-pro BNP, eGFR, and Fib4 index, but not with hs-troponin T. In contrast, diastolic BPV was not found to be associated with eGFR.

    Hyperuricemia is related to an increased risk of cardiovascular events from a meta-analysis and antihyperuricemia agents may influence to cardiac function. We evaluated the effect of febuxostat on echocardiographic parameters of diastolic function in patients with asymptomatic hyperuricemia as a prespecified endpoint in the subanalysis of the PRIZE study. Patients in the PRIZE study were assigned randomly to either add-on febuxostat treatment group or control group with only appropriate lifestyle modification. Of the 514 patients in the overall study, 65 patients (31 in the febuxostat group and 34 in the control group) who had complete follow-up echocardiographic data of the ratio of peak early diastolic transmitral flow velocity (E) to peak early diastolic mitral annular velocity (e′) at baseline and after 12 and 24 months were included. The primary endpoint was a comparison of the changes in the E/e′ between the two groups from baseline to 24 months. Interestingly, e′ was slightly decreased in the control group compared with in the febuxostat group (treatment p = 0.068, time, p = 0.337, treatment × Time, p = 0.217). As a result, there were significant increases in E/e′ (treatment p = 0.045, time, p = 0.177, treatment × time, p = 0.137) after 24 months in the control group compared with the febuxostat group. There was no significant difference in the serum levels of N-terminal-pro brain natriuretic peptide and high-sensitive troponin I between the two groups during the study period. In conclusions, additional febuxostat treatment in patients with asymptomatic hyperuricemia for 24 months might have a potential of preventable effects on the impaired diastolic dysfunction.

    To treat older patients with hypertension, it is important to detect cognitive impairment at an early stage because of its potential influence on treatment efficacy and functional prognosis. In this study, we aimed to identify the incidence and determinants of cognitive impairment in hypertensive patients aged 65 years and above who visited our outpatient clinic and were not previously diagnosed with cognitive impairment. Among 312 patients with hypertension, we found that 35% (n = 109) and 7.7% (n = 24) had cognitive impairment and dementia, respectively, as defined by the Mini-Mental State Examination (≤27 or ≤23, respectively). Patients with cognitive impairment were older, had lower levels of education, and had lower instrumental activities of daily living (IADL) scores than those without cognitive impairment. Multiple regression analysis revealed that age and IADL were associated with cognitive impairment in patients with hypertension. Regarding the treatment of hypertension, the office and home blood pressure levels, number of antihypertensive medications prescribed, and proportion of the use of each antihypertensive drug was equivalent between patients with and without cognitive impairment. Finally, patients with unrecognized cognitive impairment showed distinct clinical characteristics, including high antihypertensive medication burden and preserved IADL, when compared to hypertensive patients in the different cohorts of definitive mild cognitive impairment of a similar age. These findings suggest that older hypertensive patients are at a high risk of masked cognitive decline, even if they are functionally independent.

    Lectin-like oxidized low-density lipoprotein (ox-LDL) causes vascular senescence and atherosclerosis. It has been reported that ox-LDL scavenger receptor-1 (LOX-1) is associated with the angiotensin II type 1 receptor (AT1R). While mitochondria play a crucial role in the development of vascular senescence and atherosclerosis, they also undergo quality control through mitochondrial dynamics and autophagy. The aim of this study was to investigate (1) whether LOX-1 associates with AT1R, (2) if this regulates mitochondrial quality control, and (3) whether AT1R inhibition using Candesartan might ameliorate ox-LDL-induced vascular senescence. We performed in vitro and in vivo experiments using vascular smooth muscle cells (VSMCs), and C57BL/6 and apolipoprotein E-deficient (ApoE KO) mice. Administration of oxidized low-density lipoprotein (ox-LDL) to VSMCs induced mitochondrial dysfunction and cellular senescence accompanied by excessive mitochondrial fission, due to the activation of fission factor Drp1, which was derived from the activation of the Raf/MEK/ERK pathway. Administration of either Drp1 inhibitor, mdivi-1, or AT1R blocker candesartan attenuated these alterations. Electron microscopy and immunohistochemistry of the co-localization of LAMP2 with TOMM20 signal showed that AT1R inhibition also increased mitochondrial autophagy, but this was not affected by Atg7 deficiency. Conversely, AT1R inhibition increased the co-localization of LAMP2 with Rab9 signal. Moreover, AT1R inhibition-induced mitochondrial autophagy was abolished by Rab9 deficiency, suggesting that AT1R signaling modulated mitochondrial autophagy derived from Rab9-dependent alternative autophagy. Inhibition of the Raf/MEK/ERK pathway also decreased the excessive mitochondrial fission, and Rab9-dependent mitochondrial autophagy, suggesting that AT1R signaling followed the Raf/MEK/ERK axis modulated both mitochondrial dynamics and autophagy. The degree of mitochondrial dysfunction, reactive oxygen species production, vascular senescence, atherosclerosis, and the number of fragmented mitochondria accompanied by Drp1 activation were all higher in ApoE KO mice than in C57BL/6 mice. These detrimental alterations were successfully restored, and mitochondrial autophagy was upregulated with the administration of candesartan to ApoE KO mice. The association of LOX-1 with AT1R was found to play a crucial role in regulating mitochondrial quality control, as cellular/vascular senescence is induced by ox-LDL, and AT1R inhibition improves the adverse effects of ox-LDL.

    <p>The risk of cardiovascular disease increases with age, causing chronic disability, morbidity, and mortality in the elderly. Cardiovascular aging and disease are characterized by heart failure, cardiac ischemia-reperfusion injury, cardiomyopathy, hypertension, arterial stiffness, and atherosclerosis. As a cell ages, damaged organelles and abnormal proteins accumulate. A system for removing these cytoplasmic substrates is essential for maintaining homeostasis. Autophagy assists tissue homeostasis by forming a pathway by which these substances are degraded. Growing evidence suggests that autophagy plays a role in age-related and disease states of the cardiovascular system, and it may even be effective in preventing or treating cardiovascular disease. On the other hand, overexpression of autophagy in the heart and arteries can produce detrimental effects. We summarize the current understanding of the close relationship between autophagy and cardiovascular senescence.</p>

    オルタナティブオートファジーの低下がドキソルビシンによる心筋障害および骨格筋障害の分子機序に関与することを明らかにし、治療方法確立を目指すことを目的として研究計画を立てた。A.基礎実験:(I) in vitro:H9c2ラット心筋芽細胞およびマウス骨格筋由来筋芽細胞株 C2C12 細胞を用いて以下の実験を行った。
    (I)H9c2細胞およびC2C12細胞に、ドキソルビシンを投与し、H9c2細胞およびC2C12細胞の分化・増殖を評価した。C2C12細胞の分化・増殖については、抑制されていることが観察された。H9c2細胞についても分化・増殖については、抑制されていることが観察された。ミトコンドリアをMito-tracker Red Stainingにおいて観察した。C2C12細胞においては、分裂したミトコンドリアの増加を認めており、fissionに傾いていることが確認された。H9c2細胞においても同様に、分裂したミトコンドリアの増加を認めており、fissionに傾いていることが確認された。(II) in vivo:正常マウスC57BL6(WT)を用いて、ドキソルビシン心筋障害モデルを作成した。(II)-1.生理学的変化の検討;WTマウス(12週齢)に対して、ドキソルビシン投与を行う。投与4週間後において非投与群と心機能を比較検討した。ドキソルビシン投与群においては、左室拡大を認め、左室駆出力の低下を認めた。電子顕微鏡では、心筋において、分裂したミトコンドリア像の増加を認めており、fissionが増加していると考えられた。

    脂質異常症モデルマウスのApoEノックアウト(KO)マウスと野生型(C57BL6)マウスとで以下を検討した。1)筋力の計測:野生型と比較しApoE KOでは筋力が低値であった。その傾向は加齢とともにより顕著であった。2)筋肉量の計測:下肢筋の重量も上記筋肉量と同様に、野生型と比較しApoE KOで低値であった。その傾向は加齢とともにより顕著であった。3)免疫組織化学染色を行ったところ、ApoE KOマウスではFRβ陽性マクロファージが認められた。4)ApoE KOマウスでは筋肉にAGEが蓄積していた。5)ミトコンドリア機能を比較したところ、野生型と比較しApoE KOでミトコンドリア機能(ATP産生能)は低値であり、酸化ストレス(H2O2産生)は高値であった。以上より、脂質異常ではより筋肉の障害を来し、その要因として、ミトコンドリア機能障害やFRβ陽性マクロファージの関与が示唆された。今後はこれらミトコンドリア機能障害とFRβ陽性マクロファージがどのように関連しているかについて検討を進めていく。
    培養筋肉細胞を用いた検討では、酸化LDLを添加した筋肉細胞ではコントロール刺激のみの筋肉細胞と比較し、ミトコンドリア機能障害(膜電位をJC-1やTMREにて測定)や酸化ストレス(mito-Sox Redにて評価)がより強かった。今後はマクロファージと筋肉細胞を共培養してさらなる研究を進める計画である。
    ヒトを対象にした研究では、元々当該講座が実施している地域在住高齢者を対象としたコホート研究において、筋肉量や筋力測定を行い、また採血にて検体採取を行い可溶性FRβの測定を行う研究を計画しているが、このヒト対象研究に関しては後述の通りの事情により現在研究が計画通りに進展していない。今後状況が改善次第、再開していく計画である。

    The role of macrophages in the pathogenesis of aortic aneurysms was investigated. In a mouse model of aortic aneurysm, we found that deletion of all macrophages suppressed the onset and progression of aortic aneurysm and promoted aortic aneurysm regression. Furthermore, in a mouse model of aneurysm, deletion of only activated macrophages suppressed the onset of aortic aneurysm and promoted regression of aortic aneurysm. In addition, immunohistochemistry using folate receptor β (FRβ) monoclonal antibody confirmed the presence of activated macrophages expressing FRβ in the removed aorta of patients undergoing aortic aneurysm surgery. A Kagoshima aortic aneurysm patient registry was established and blood soluble FRβ levels were measured in these patients.
