症例は68歳女性で、左頬部に大きな腫瘤が出現したため受診した。腫瘍表面は粒状で45×30mmの大きさがあり、皮膚表面へも15×15mmの大きさで進展し、同側の口腔交連および上下口唇の3分の1以上を巻き込んでいた。患者は無歯顎者であった。同腫瘍の生検の結果、扁平上皮癌(T4aN2bM0)であることが明らかになった。術前に放射線療法とS-1での化学療法を行い、腫瘍を縮小させた後に最初の手術として病変部の頬部を全層性に広範切除し、広背筋皮弁で欠損部を再建する手術を施行した。その結果、欠損部の充填には成功したものの、顔面下半部の変形が残存した。上記手術から1年後、移植皮弁を減量し、口腔交連と口唇紅を再建する手術を施行した。同手術では顔面の対称性を達成するため術前に患者の顔面の三次元的画像を取得し、ソフトウェア上で再建計画を立てた。上口唇は移植皮弁の皮膚の一部を流用して形成し、下口唇は有茎の舌皮弁から新規に作成した。さらに上下顎の歯肉の粘膜骨膜組織をそれぞれ頬部へ持ってきて合わせることで口腔前庭の膨らみを形づくった。初回手術から5年後の評価では、日常生活活動度は術前と同じであり、口唇の対称性が保たれ経口摂食の妨げになっていなかった。顔面下半部の状態も含め整容性、機能性の面で良好な帰結が得られており、腫瘍の再発・転移の徴候も認められなかった。

口腔扁平上皮癌における術前の頸部リンパ節転移診断の精度を高めるため、CT、MRI、超音波、18F-FDG PET画像検査の結果を統合利用するチャート式診断基準システムを開発し、その臨床有用性について検討した。当院で手術を施行した口腔扁平上皮癌患者106名を組み入れ、治療時期に応じて、I群(2005年10月〜2008年8月、45名)、II群(2008年9月〜2010年3月、29名)、III群(2010年4月〜2012年3月、32名)、に分類した。頸部リンパ節転移の診断においては、I群では上述の各画像診断法を統合した判定はせず、個別に利用していた。II群、III群では各画像診断法の所見を統合した診断基準が適用されており、II群ではCT/MRIと超音波検査の両者ともに陰性の場合に転移陰性、III群ではII群の基準に加えPET/CTのmaximum standardized uptake value(SUVmax)が3.2未満の場合に転移陰性とそれぞれ診断していた。cN0と診断されていたのは67例で、50例では経過観察が適用されそのうち12例(24%)では原発巣の再発を見ずに頸部リンパ節転移を発症した。術後転移率はI、II、III群でそれぞれ33、24、13%と、診断基準システムが新しいほど改善したが、陽性的中率は50、40、33%と悪化していた。
