    Objective: The revision of the “Regulations for Designating Training Facilities for Physical Therapists and
    Occupational Therapists” and the “Guidelines for Physical Therapists and Occupational Therapists Training
    Facilities” applied to students enrolled from 2020 stipulates that clinical training involving participation in
    medical treatment will be conducted. It also states that it is desirable to have a ratio of approximately 2:1 of
    the number of trainees at a training facility to the number of clinical educators at the facility. Method: In the
    first year when these were applied, we conducted a fact-finding survey of practical training at our school,
    particularly focusing on the number of students per clinical educator. We also conducted a questionnaire survey
    on the barriers to the smooth implementation of the transition to a collaborative learning model of one clinical
    educator to two students. Result: The response rate to the questionnaire was 80%. Approximately 20% of
    facilities used a collaborative learning model with one clinical educator and two students, while 54.8% had a
    1:1 clinical educator to student training model. Regarding why they are not able to conduct practical training for
    multiple students, 81% cited “problems with the hospital’s guidance system for students.’’ Conclusion: It was
    suggested that many training facilities are still searching for effective ways to conduct practical training.

    <p>【Objective】 This study aimed to clarify the current state of disaster preparedness and identify regional differences among child development support and after-school day care service facilities in four disaster-affected prefectures in Japan (Iwate, Miyagi, Fukushima, and Kumamoto). 【Methods】 A self-administered postal survey was conducted with 762 facilities, from which 246 valid responses were collected. Cross-tabulations and chi-square (χ<sup>2</sup>) tests were performed on 17 preparedness-related items. For items showing statistically significant differences, multiple comparisons were conducted. 【Results】 Significant regional differences were observed in 10 of the 17 items, including participation in disaster preparedness training, creation of disaster preparedness maps, methods of information sharing, and stockpiling of emergency supplies. Facilities in Fukushima Prefecture demonstrated higher levels of preparedness, while those in Kumamoto Prefecture faced challenges in developing comprehensive preparedness systems. 【Discussion】 These regional differences appear to be influenced by the specific characteristics of the disasters experienced and by variations in administrative support systems. Moving forward, policy efforts should aim to standardize preparedness across all facilities and establish ongoing support systems, particularly for smaller institutions. 【Conclusions】 Substantial regional disparities exist in disaster preparedness among child development support and after-school day care service facilities in the four prefectures surveyed.</p>

    Background: Mobile devices have been used by many older adults and have the potential to assist individuals with subjective cognitive complaints (SCCs) in daily living tasks. Financial management is one of the most complex daily activity for older adults, as it is easily impaired in the prodromal stage of dementia and cognitive impairment. Aim: To investigate financial management ability among older adults from SCCs and mobile device proficiency. Methods: A self-administered questionnaire was sent to 529 participants who were ≥ 65 years and regularly use mobile devices. Participants were divided into four groups based on SCC prevalence and scores of the Mobile Device Proficiency Questionnaire (MDPQ-16). Financial management abilities were compared between groups using the Process Analysis of Daily Activities for Dementia subscale. Regression model and crosstabulation table were used to investigate associations in detail. Results: A significant difference in financial management ability was observed among the four groups (p < 0.001), with the dual impairment group showed significantly lower than the robust and SCC groups (p < 0.001). Educational history, sex, and MDPQ-16 score were significantly associated with participants’ financial management ability (p ≤ 0.01). The proportion of participants who could use ATMs and electronic money independently was significantly lower among those with low proficiency in mobile devices (LPM), regardless of SCC (p < 0.05). Conclusion: The LPM group showed an impaired ability to manage their finances, particularly in situations where they would use information devices. Therefore, healthcare professionals should assess not only the SCC but also their proficiency with mobile devices to predict their impairment in activities of daily living.

    <p>【Objective】 The purpose of this study was to identify evacuation issues faced by psychiatric hospitals in Kagoshima Prefecture in the event of a disaster, to ensure patient safety, and to strengthen post-disaster response. 【Methods】 In cooperation with the Kagoshima Prefecture Psychiatric Hospital Association, an online survey was conducted to investigate the preparedness for and challenges of evacuation actions in hospitals. In addition, the location of hospitals was compared with hazard maps and nuclear disaster warning zones. 【Results】 A wide variation was found in the development and implementation of evacuation action plans. Notably, many hospitals were still considering or had not conducted evacuation drills. This finding highlights the lack of consistent planning and practices for ensuring safety and providing evacuation support during disasters. Furthermore, the need for expert support and cooperation was identified as a common need along with inherent challenges. 【Conclusion】 For psychiatric hospitals to improve their disaster response measures, they should support development of evacuation action plans, conduct evacuation drills, improve the specificity and consistency of evacuation support, and strengthen cooperation with specialists.</p>

    【目的】本研究の目的は、鹿児島県内の精神科病院が直面する災害時の避難課題を明らかにし、患者の安全確保および災害発生後の対応強化を図ることである。【方法】鹿児島県精神科病院協会の協力の下、WEBアンケートを通じて、病院における避難行動の準備状況とその課題を調査した。また、ハザードマップおよび原子力災害時の区域情報と病院の位置情報とを比較した。【結果】避難行動計画の整備や実施に大きなバラツキがあることが判明した。特に、避難訓練の未実施や検討段階である病院が多数を占め、災害時の安全確保と避難支援に関する一貫した計画や実践方法の不足が浮き彫りになった。さらに、共通した課題として専門家の支援や協力の必要性が明らかになった。【結論】精神科病院が災害対策を充実させるためには、避難行動計画の策定支援、避難訓練の実施、避難支援の具体性と一貫性の向上、専門家との連携強化が求められる。(著者抄録)

    東日本大震災の震災後に福島原発事故に被災し、入院中の病院から避難指示により避難した統合失調症患者に、避難行動、避難生活、予期せぬ退院からの社会復帰についてインタビュー調査を実施した。ビデオカメラに記録した面接内容から逐語録を作成し、オーラルヒストリー法を用いて分析を行った。データ収集は2023年2月11日に実施した。分析の結果、面接内容は救護・支援活動の変化を時系列に抽出し、カテゴリー化した。対象患者は若年期に統合失調症を発病し、約40年の長期入院となっていたが、事故によりケースワーカーの助言を受け、必要最低限の持ち物で避難した。その後複数の病院を転々とし、最終的にK病院で1年半を過ごし、状態が安定し、地域への移行が進められ、退院の機会が得られた。対象患者はサポートを受け、グループホームへの退院を決意した。その後アパート暮らしとなり、ピアサポーターとして活躍するようになった。

    奄美市、五島市の避難所に指定されている介護福祉施設に勤務する介護職者14名(奄美市7名、五島市7名)を対象に、防災・減災に対する認識を調査し、豪雨災害経験の有無による離島間の認識の違いについて検討した。調査期間は平成30年8月から9月であった。災害経験のある対象者は奄美市4名、五島市は0名であった。対象者にインタビューガイドに基づく半構造化面接調査を実施し、録音した面接内容から逐語録を作成した。逐語録についてコード化、カテゴリー化の分析を行った。対象者は災害経験の有無に関係なく、介護職者の役割として「平常心を保つこと」「生活環境調整」を介護職者の役割として必要なものとして認識していた。災害経験の有無に関係なく、両者ともに危機感の欠如があることが明らかになった。介護職者避難所での短期的支援だけでなく、退所後の長期的支援にも具体的なイメージを持つことや訓練・伝承の必要性が示唆された。

    Introduction: The Mishima Islands in Kagoshima Prefecture, Japan, are remote islands designated as medically underserved. Supporting the people living in such remote islands requires a multifaceted examination of the livelihoods of the residents, including their health, work, and living environment. Methods: Students from the faculties of nursing, veterinary science, agriculture, and engineering participated in a 3-day virtual fieldwork session held synchronously from the campus of Kagoshima University and on a remote island in the Mishima Mura island group in Kagoshima Prefecture, Japan. Coursework and post-course surveys were analyzed to determine student engagement and learning. In particular, the analysis considered learning in relation to interprofessional learning and community-based learning. Results and discussion: Students expressed being able to conduct fieldwork online, collaborate with students from multiple faculties and graduate schools, and apply their expertise. These outcomes were reflected in the five-point Likert scale evaluation responses, highlighting a sense of accomplishment and satisfaction. Virtual fieldwork can contribute to student learning by introducing new concepts and allowing them to consider development issues in remote areas from multiple perspectives.
