Few studies have investigated the relationship between nutritional status and comprehensive assessment of oral hypofunction, especially protein intake-related sarcopenia. Thus, we explored these relationships in a large-scale cross-sectional cohort study using the seven-item evaluation for oral hypofunction and Diet History Questionnaire for nutritional assessment. We used the data from 1004 individuals who participated in the 2019 health survey of the residents of Tarumizu City, Kagoshima Prefecture, Japan for analysis. We found that individuals with oral hypofunction were significantly older with a lower skeletal muscle index. Although there were few foods that had a sig-nificant difference between the groups with and without oral hypofunction, the consumption of beans and meats was significantly lower in women and men in the oral hypofunction group, respectively. According to the lower limit of the tentative dietary goal defined in Japan, comprehensive evaluation of oral hypofunction was significantly and independently associated with protein intake in both men and women (odds ratio, 1.70; 95% confidence interval, 1.21–2.35). In conclusion, we found that oral hypofunction was associated with targeted protein intake for sarcopenia and frailty prevention in middle-aged and older community-dwelling adults. Comprehensive evaluation of oral function with intervention in cases of hypofunction could inform clinicians to better prevent sarcopenia.

61歳男。難治性口内炎を主訴とした。小児期に水痘罹患歴があり、50歳時に2型糖尿病と診断され、インスリン療法中であった。口腔外所見は、左右顎下部に小指頭大で可動性のリンパ節を各々1個ずつ触知した。口腔内所見では、右側舌縁周囲にびらんと白斑を伴った潰瘍形成を認めた。血液検査では、空腹時血糖283mg/dl、HbA1c8.5%、尿検査では尿糖4+を認めた。パノラマX線写真では、全顎的な水平的骨吸収を認めた。以上より右側舌癌を疑い、糖尿病コントロール及び舌生検目的に入院した。スライディングスケールを用いた強化インスリン療法を開始後、食前血糖値は150mg/dl以下を示すことが多くなった。眼科対診では、軽症非増殖糖尿病網膜症と診断された。全身精査後に舌扁平上皮癌(T2N0MO StageII)が判明し、舌部分切除を計画したが、血糖コントロールのため即手術が困難となった。手術待機中に潜在性微小転移の防止効果を期待する目的で、テガフール・ウラシル配合剤(UFT)にて術前補助化学療法を開始した。内服6日目に微熱、全身倦怠感、左頬部の疼痛、左外耳から頬部、オトガイ部にかけて淡い浮腫性紅斑と左側下唇を含めた水疱形成を認めたため、左三叉神経第2枝領域の帯状疱疹を疑った。UFTを中止し、アシクロビル点滴静注とした。翌日、左側舌縁部、左側舌顎歯肉にも水疱形成を認めた。また、左側耳痛および耳鳴り、耳閉・そう痒感もみられ、難聴は認められなかったことから、帯状疱疹に伴う耳痛と診断した。点滴静注3日目には、全身倦怠感やそう痒感は軽減したが、左側顔面神経麻痺を認めたため、左膝神経節領域のRamsay Hunt症候群と診断した。その後、右側絶縁部の腫瘍が増大傾向を認めたため、舌部分の切除術を施行した。術後は創治癒が得られ、病理組織所見にて高分化型扁平上皮癌(Grade0)と診断された。ウイルス抗体検査では、VZVIgM抗体が出現していた。最終的にはインスリン療法で血糖コントロールが可能となり、顔面麻痺や耳痛も改善し、退院となった。術後約5年経過したが、咀嚼・発音障害、舌癌の再発・転移、帯状疱疹後神経痛や左顔面皮膚の知覚低下、色素沈着など発現していない。

症例1は83歳女性で、骨粗鬆症、胸椎圧迫骨折、認知症の既往があり、アレンドロン酸ナトリウム水和物、パロキセチン塩酸塩水和物、レバミピド、ファモチジンを常用していた。初診の3日前に口腔粘膜潰瘍を指摘され、精査加療目的に介護士が付き添い当科へ紹介来院した。右側下顎臼歯部歯肉を中心に右側頬粘膜から口底部、下唇内側に至る50×40mm大の範囲で周囲に発赤を伴う一部偽膜に被覆された潰瘍形成を認め、アレンドロン酸錠を口腔内に滞留させることなく、確実に嚥下できたことを確認するよう介護士に指示した。症例2は74歳女性で、骨粗鬆症を含む既往があり、3ヶ所の医院からミノドロン酸水和物などを処方されていた。右側下顎歯肉の出血と疼痛を主訴に来院した。ミノドロン酸による口腔粘膜潰瘍の疑いにて、ペニシリン系経口抗菌薬と非ステロイド性解熱消炎鎮痛薬を5日分、ベンゼトニウム塩化物含嗽薬を処方した。初診から12日後の再診時には、右側下顎の潰瘍形成部はその範囲が縮小していた。

<p>The side effects of peroral bisphosphonate may cause mucous membrane disorders and oral exulceration if individuals dissolve the drug inside the oral cavity on oral administration. We experienced two cases in which we strongly suspected mucous ulcurs due to this drug. Finally, the oral exulceration disappeared after providing instructions on correct administration.</p><p>　An 83-year-old woman was referred because of exulceration of the right mandible. She suffered from osteoporosis, thoracic vertebra compression fracture, and dementia, while taking alendronate weekly. The exulceration appeared from the right lower lip to the right mandibular molar gingiva when we absorbed pooled saliva in the right oral cavity when the patient was lying down. We instructed the patient to confirm that the tablet was completely swallowed. The ulcers in the oral cavity completely disappeared 17 days after presentation.</p><p>　A 74-year-old woman with osteoporosis presented because of bleeding and pain in the gingiva of the right mandible. She was taking monthly minodronate while wearing incompatible dentures. Exulceration occurred with peripheral redness ranging from the right buccal mucosa to the right molar gingiva of the mandible and the tongue/floor of her mouth, along with a ventrolateral tongue. We taught her how to swallow minodoronate correctly without dentures, and the ulcers healed 4 weeks later.</p>
